1.Versatile Applications of Anterolateral Thigh Perforator Flap in the Reconstruction of Upper Extremity Defects: Retrospective Analysis of 119 Cases.

PURPOSE: The perforator flaps have established their role in the reconstruction of various soft tissue defects. For the last five years, we have extensively used anterolateral thigh (ALT) flap for the reconstruction of the complex tissue defects of the hand and upper extremity and report the clinical results and our experiences with the versatile applications of this flap. MATERIALS AND METHODS: From March 2003 through May 2008, 119 free ALT perforator flaps were transferred for reconstruction of the complex tissue defects of the elbow, forearm, wrist and hand after crushing or degloving injuries as well as severe scar contractures. There were 95 females and 24 males. The mean age of the patients was 37 years and mean size of the flap was 170 cm(2). In 20 cases, the flap was vascularized by septocutaneous and in 99 cases by musculocutaneous perforators. Intra-muscular dissection length averaged 3.4 cm. The total length of pedicle averaged 8.4 cm and the average arterial diameter was 0.84 mm. End-to-end arterial anastomosis was performed in 103 cases and end-to-side in 16 cases. RESULTS: Flap survival rate was 98.3%(117/119) and there were 6 cases of partial necrosis. Donor site was closed primarily in 41 cases and skin grafts were applied in 78 cases. CONCLUSION: The reliability and versatility of ALT flap makes it one of the foremost choices for the reconstruction of complex tissue defects of the upper extremity.

2.Diagnostic Modality in Spine Disease: A Review

Spine diseases are common and exhibit several causes, including degeneration, trauma, congenital issues, and other specific factors. Most people experience a variety of symptoms of spine diseases during their lifetime that are occasionally managed with conservative or surgical treatments. Accurate diagnosis of the spine pathology is essential for the appropriate management of spine disease, and various imaging modalities can be used for the diagnosis, including radiography, computed tomography (CT), magnetic resonance imaging (MRI), and other studies such as EOS, bone scan, single photon emission CT/CT, and electrophysiologic test. Patient (or case)-specific selection of the diagnostic modality is crucial; thus, we should be aware of basic information and approaches of the diagnostic modalities. In this review, we discuss in detail, about diagnostic modalities (radiography, CT, MRI, electrophysiologic study, and others) that are widely used for spine disease.
3.A Case of Autoimmune Hemolytic Anemia in a Renal Transplant Recipient due to Anti-A of Donor Origin.

Autoimmune hemolytic anemia due to antibody formation against the A or B antigen in renal transplant recipients has been reported on rare occasions. We experienced a case of autoimmune hemolytic anemia which developed 11 days after renal transplantation during CsA and prednisolone administration as immunosuppressive agents. The patient was a 46 year old male, blood group was Rh(+) A, who had received a kidney from his Rh(+) O, HLA haploty'pe identical elder brother. He was transfused with three units of Rh(+) A RBCs preoperatively and his hemoglobin level was 9.2g/dl 1 day after transplantation. After 11 to 12 days posttransplantation, the hemoglobin level dropped to 3.8g/dl. A peripheral blood smear showed marked spherocytosis and polychromatophilia. The reticulocyte count was increased to 4.2%, and total bilirubin was increased to 2.91mg/dl. The LDH was raised to 561 IU/L and the plasma Hb level was 6.Smg/dl. Blood bank tests confirmed that the autoantibody cause hemolytic anemia was anti-A. If transplant recipients of blood groups A, B, or AB, who recieve organs from blood group 0 donors, have hemolytic anemia and ABO discrepancy, the possibility of AIHA due to anti-A or anti-B should be considered.

4.Recurrent onycholysis in a patient with Behcet's disease.

Onycholysis is defined as a distal or distal lateral separation of the nail plate from the underlying or lateral supporting structures including nail bed, hyponychium, and lateral nail fold. Trauma, infection, psoriasis, thyrotoxicosis, and numerous drugs are common causes of onycholysis. However, there are few specific data on nail findings in Behcet's disease (BD). In this paper, we report on a 60-year-old man with BD, with no past history except BD, who developed recurrent onycholysis. The symptoms of onycholysis are considered to be recurrent depending on the activity of BD. The nail lesion showed improvement after classic treatment of BD and topical steroid ointment.

5.A Case of Primary Biliary Cirrhosis Mimicking Acute Hepatitis B in the Clinic, Republic of Korea.

Primary biliary cirrhosis (PBC) is a slowly progressive cholestatic autoimmune liver disease characterized by progressive bile duct injury. The most common symptoms of this disease include fatigue and pruritus. The diagnosis of PBC is based on cholestatic biochemical liver tests, presence of antimitochondrial antibodies, and characteristic histological biopsy findings. We report a case of a patient with PBS, who was initially suspected to be in the window period of hepatitis B by a private doctor in a local clinic based on the detection of isolated immunoglobulin M antibody against hepatitis B core antigen. The presence of this antibody is the most useful index in diagnosing acute hepatitis B (+) by immunoserological test. The final diagnosis of the patient in Good Gang-An Hospital was PBC through additional tests. The patient is receiving outpatient treatment.

6.Lead Levels in Maternal and Umbilical Cord Blood.

This study was conducted to evaluate the blood lead levels in 141 pregnant women who were not occupationally exposed to lead and their umbilical cord, arid to identify some differences in blood lead levels by living area. ; The subjects-were all delivered at St. Benedict Hospital in Pusan and the blood lead levels were measured by graphite furance atomic absorption spectrophotometer. The summarized results were as follows. 1. The geometric mean lead levels in maternal and cord blood were 8.0+/-1.8microgram/dl, 5.0+/-0.6lmicrogram/dl respectively. 2. Lead levels in maternal (8.7+/-1.7microgram/dl) and cord blood(5.4+/-0.7microgram/dl) of industrial area(n=24) were higher than residential area(n= 124) which were 7.9+/-1.7microgram/dl and 5.0+/-0.6microgram/dl respectively (p<0.05). 3. Maternal blood lead concentration was significantly correlated to cord blood lead concentration (r=0.6090, p<0.001), but other variables (maternal age, paternal smoking, birth weight, etc) were not correlated.

7.Correction of the Trichiasis and the Cicatricial Entropion by Transposition of Skin Flaps.

Trichiasis is usually associated with congenital entropion or conjunctival destruction from disease or injuries. Cicatricial entropion follows scarring of the palpebral conjunctiva, which may be caused by chemical injuries, surgical procedures, trauma and infections. Numerous surgical procedures have been described for correction of trichiasis and cicatricial entropion in accordance with the severity and location of the condition. We have treated partial trichiasis in 2 cases(4 lids),(one involving nasal one forth of the upper lid with corrected congenital ptosis and the other in temporal one forth of the upper lid with narrow palpebral fissure) and cicatricial entropion on the middle one forth of the upper lid in 3 cases(3 lids) with transposition of skin flaps. The resuits were satisfactory.

8.Effect of School-Based Social Skills Training Program on Peer Relationships: Preliminary Study

OBJECTIVES: The aim of this study was to evaluate the effect of a school-based social skills training program on peer relationships in children and adolescents and to assess the plan for effective school-based mental health services. METHODS: The Child and Adolescent Mental Health Promotion Team of Bugok National Hospital conducted 7-sessioned school-based social skills training for elementary and middle school students (n=90). Changes in peer relationships were evaluated before and after application of the program using a name generator question. RESULTS: The social skills training program increased peer relations, indicating significant changes in social network indices. CONCLUSION: The social skills training program positively influenced peer relationships. The school-based social skills training program can be expected to have positive effects on school-based mental health services. Future investigation is needed to validate the long term effects of this program.

9.A Case of Conjunctival Choristoma.

Choristoma are tissue masses normally not present at the involved site and can be classified into 4 groups, epidermoid cyst, dermoid cyst, teratoma and ectopic lacrimal gland. Ectopic lacrimal gland is an unusual congenital and developmental abnormality and may be associated with other choristomatous tissues as muscle, nerve, cartilage or various dermal appendages. We have recently experienced a case of ectopic lacrimal gland associated with muscle, fat, nerve, tissue in the lateral side of bulbar conjunctiva in the right eye of a 23 year-old korean girl.

10.Apoptosis of Podocyte and Role of TGF-beta1 LAP in Unilateral Ureteral Obstruction in Mice.

PURPOSE: It was well known that transforming growth factor (TGF)-beta1 plays a pivotal role in interstitial fibrosis and loss of podocyte. We explored the effects of exogenous administration of TGF-beta1 latency-associated peptide (LAP) in a model of renal fibrosis induced by unilateral ureteral obstruction (UUO) and examined whether TGF-beta1 LAP can inhibit apoptosis of podocyte. METHODS: Twenty four male BALB/c mice were unilaterally obstructed of proximal ureters by ligation. Half of the mice with operation and half of 8 control were administered recombinant human LAP intraperitoneally. One to three mice per group were euthanized on days 3, 7, 14, and 21 after operation for observation of renal fibrosis and apoptosis of podocyte. RESULTS: Interstitial fibrosis was less severe in LAP-treated group. Obstructed kidneys from LAP- untreated mice had more glomerular apoptotic podocytes (TUNEL assay) compared to LAP-treated mice at day 7, 14, and 21 after operation. CONCLUSION: Intraperitoneal administration of TGF-beta1 LAP prevents the loss of podocyte & renal damage partially up to day 14 after operation.
