1.Clinical observation of continuous jejunal interposition after total gastrectomy:a report of 60 cases

Objective To explore the application value of gastrointestinal reconstruction by continuous jejunum interposition after total gastrectomy.Methods A retrospective analysis in March 2006 to March 2012 was performed in 60 patients with gastric underwent continuous jejunal interposition after total gastrectomy.Results There were no operation death and anastomotic fistula.Follow-up for 12-24 months,some patients had mild reflux esophagitis,generation of gastric capacity of more than 300 ml,emptying time of 30-100 min.The quality of life was improved after operation.Conclusions Continuous jejunal interposition pouch is safe and reliable,with satisfactory effect of digestive tract reconstruction.
2.Recent Advance in Bacteriophage Therapy

At the time of phage’s discovery, phage therapy was regarded as a possible treatment method against bacterial infection. Although phage therapy was used to treat and prevent bacterial infection in the former Soviet Union and Eastern Europe, it was abandoned by the West in the 1940s with the arrival of the antibiotic era. However, the ongoing evolution of bacterial multidrug-resistance has recently motivated the Western scientific community to reevaluate phage therapy for bacterial infections that are incurable by conventional chemotherapy. With the indepth study of phages, it’s increasingly acknowledged that phages, as the medicine to cure bacterial infection, are convenient, safe and efficient therapeutics. This paper summarizes the recent years’ advanced researches in this area.
3.Clinical utility of combined administration of rapamycin and low dose of cyciosporin a in kidney transplantation recipients

Objective To investigate the feasibility and value of combined administration of rapamycin and low dose of cyclosporin A in kidney transplantation recipients. Methods Twenty-five recipients who received their kidney transplantations for 2 months to 10 years were transferred to a new immunosuppression regimen of combined administration of rapamycin,low dose of cyclosporin A and prednisone. During the clinical observation,the average serum trough level of cyclosporin A was maintained between 0.042 and 0.083 μmol/L,and the average serum trough level of rapamycin was controled to 4~8 μg/L. Meanwhile, the dosage of immunosuppressive drugs was adjusted properly according to the white blood cells counting,liver function,blood lipid level and clinical settings.Twenty-five kidney transplant recipients with usual immunosuppression regimen of cyclosporin A in combination with MMF and prednisone served as controls. Results Druing a follow-up period of 2 to 8 months,no acute rejection events were found in this group. All the recipients survived well with functional renal grafts. Two patients in the experimental group quitted during the follow-up. One was incurred to serious pulmonary infection diagnosed as cytomegalovirus infection,and another was sufferred from persistent unknown fever. Other main adverse effects included leucopenia, hyperlipemia and dental ulcer,similarly as control group. No significant abnormal liver function event was found in this group. Conclusions This new immunosuppression regimen of combined administration of rapamycin,low dose cyclosporin A and prednisone in kidney transplantation recipients could avoid or diminish the adverse and toxic effects of calcineurin inhibitors,mainlain well-functional allografts and not increase the incidence of severe untoward reactions. Meanwhile,the costs for taking medicine were also significantly reduced with this regimen. Therefore,it would be an ideal and optimal immunosuppressive maintainence regimen.

Breast cancer is by far the most common cancer for females.The recent researches focus on early diagnosis,optimal adjuvant therapy and curative effect surveillance.The paper summarizes the application of tissue and fluid tumor markers in the diagnosis,treatment,curative effect surveillance and prognostic prediction of breast cancer,which include widely-accepted estrogen receptors,HER2-neu and CA15-3,and novel ones such as Cyclin E,TIMP-1,RAR-?,hK10 and BRCA-1.
5.Postoperative tumor infiltrating lymphocytes treatment for patients with hepatocellular carcinoma

Objective To evaluate the effect of TILs treatment on postoperative patients with hepatocellular carcinoma (HCC) and the in vitro and in vivo hepatic targeting tendency of a compound of Galactosyl-Anti-CD_3McAb-TILs.Methods Thirty postoperative HCC patients were treated with TILs plus rIL-2. A hepatic targeting carrier, Galactosyl-Anti-mouse-CD_3-monoclonal antibody (Gal-Anti-CD_3-McAb), was synthesized.Result It was found that the peripheral blood level of IL-2 and T cell subset increased in all the 30 patients treated by TILs.24 HCC cases undergoing hepatectomy were treated by TILs therapy, and follow-up of 6 to 48 months found no recurrence. In an murine experiment a combination of TIL & Galactosyl-Anti-CD_3McAb-TIL tended to aggregate in lung while infused from peripheral vein. Its hepatic targeting tendency was unremarkable. Conclusion Postoperative use of TILs increased patients’ antitumor immunity, prolongs the survival, while galactosyl-anti-CD_3McAb-TIL does not show a satisfactory hepatic target tendency.
6.Clinical outcomes of tibial plateau fractures (SchatzkerⅠ,Ⅱ) treated with arthroscopy-assisted operative management and locked by LCP

[Objective]To discuss the clinical outcomes of low-energy tibial plateau fractures treated with arthroscopy-assisted operative management and locked by LCP.[Method]From January 2006 to March 2007,15 patients with tibial plateau fractures(SchatzkerⅠ,Ⅱ)were reduced with arthroscopy-assisted and treated by LCP and bone defects were filled with homogeneous allograft bone.[Result]With follow-up visits for 12~26 months,all cases were healed.According to creiteria of Rasmussen,excellent were in 10 cases,good in 4 cases,fair in 1 case,with the rate of being excellent and good added up to 93%.[Conclusion]LCP fixation is an effective technique for the low-energy tibial plateau fractures treatment with the advantage of less invasion,steadier fixation and lower complication rate.Arthroscopy-assisted operative management can help to reduce reduction loss from intra-articular more precisely to reach a higher union rate.It is also a preferable method for low-energy tibial plateau fracutures treatment(SchatzkerⅠ、Ⅱ).
8.Polyphenols from Terminalia chebula

AIM The purpose is to examine the chemical constituents in the fruits of Terminalia chebula. METHODS Using combined chromatographies over silica gel, Diaion HP-20, Toyopearl HW-40 and MCI gel CHP -20P to purify the constituents of Terminalia chebula, and identifying their structures on the basis of spectroscopic and chemical evidence were purified. RESULTS Twenty one hydrolyzable tannins and related polyphenols were characterized, here reports eight of them: 2,3-(S)-HHDP-D-glucose, 3,6-di-O-galloyl-D-glucose, 6-O-galloyl-D-glucose, (-)-shikimide 4-O-gallate, (-)-shikimic acid 3-O-gallate+(-)-shikimic acid 5-O-gallate, methyl gallate and 1,2,6-tri-O-galloyl-β-D-glucose were reported. CONCLUSION The above eight polyphenols were obtained from myrobalans for the first time.
9.Effects of hydrogen saline on oxidative stress damage in rats brain tissues after cardiopulmonary resuscitation

Objective To investigate the effect and mechanism of hydrogen saline on oxidative stress damage in rats brain tissues after cardiopulmonary resuscitation (CPR). Methods Eighteen adult male pathogen-free Sprague-Dawley (SD) rats were randomly divided into control group (Con group), conventional resuscitation group (ROSC group) and hydrogen saline treatment group (ROSC+HRS group), with 6 rats in each group. All rats were asphyxiated by tracheal clip method to establish cardiac arrest (CA) model, and received first aid with CPR, electric defibrillation and adrenaline until return of spontaneous circulation (ROSC). The rats in ROSC+HRS group were intraperitoneally injected with 2% hydrogen saline (5 mL/kg for the first time and 3 mL/kg every 2 hours). The rats in Con group were only tracheal intubated and mechanical ventilated. The rats were sacrificed after ROSC for 12 hours, and the brain tissue was harvested to determine the contents of malonaldehyde (MDA), superoxide dismutase (SOD), and catalase (CAT). The protein expression of heme oxygenase-1 (HO-1) was determined with Western Blot, and the mRNA expression of HO-1 was determined with reverse transcription-polymerase chain reaction (RT-PCR). Results Compared with the Con group, the MDA was significantly elevated in ROSC group (nmol/mg: 8.15±0.11 vs. 3.68±0.16, P < 0.05), the SOD and CAT were significantly decreased [SOD (U/mg): 69.30±2.39 vs. 94.65±2.75, CAT (U/mg): 74.38±1.65 vs. 95.68±1.88, both P < 0.05], HO-1 mRNA expression was significantly elevated (gray value: 1.383±0.194 vs. 1.117±0.083, P < 0.05), and HO-1 protein expression showed no significant change (gray value: 0.350±0.049 vs. 0.175±0.026, P > 0.05). Compared with the ROSC group, the MDA was significantly decreased in ROSC+HRS group (nmol/mg: 4.72±0.28 vs. 8.15±0.11, P < 0.05), the SOD and CAT were significantly elevated [SOD (U/mg): 83.02±1.10 vs. 69.30±2.39, CAT (U/mg): 85.07±1.94 vs. 74.38±1.65, both P < 0.05], HO-1 mRNA expression was significantly elevated (gray value: 3.200±0.200 vs. 1.383±0.194, P < 0.05), and the HO-1 protein expression was significantly elevated (gray value: 0.788±0.059 vs. 0.350±0.049, P < 0.05). Conclusions Oxidative stress damage is an important mechanism of CPR brain damage. Hydrogen saline can increase the expression of HO-1 in brain tissue, and decrease oxidative stress damage of brain after CPR.
10.Influence of intra-bone marrow injection of donor lymphocytes upon the development of graft-versus-host disease

Objective:To investigate the effect of donor lymphocyte infusion (DLI) by intra-bone marrow (IBM) or intravenous (IV) on the incidence of graft-versus-host disease(GVHD) after allogeneic peripheral hematopoietic stem cell transplantation (allo-PBSCT).Methods:Female C57BL/6 mice as recipients received total body irradiation (TBI) on day 0,followed by injection of peripheral hematopoietic stem cells from mobilized male BALB/c with granulocyte-colony stimulating factor (rhG-CSF),and DLI was performed via IV or IBM.The extent of GVHD was compared in recipients received allogeneic IBM-DLI with those received IV-DLI.The percentage of donor-derived cells and CD4~+CD25~+ regulatory T cells (Tregs) was detected by flow cytometry.14 days after DLI,the levels of IL-4 and interferon (IFN)-γ were tested by ELISA.Results:It was found that the frequency and severity of GVHD were reduced in IBM-DLI compared with that of IV-DLI (P<0.01).7 days after transplantation,the percentage of H-2~d-positive cells was over 95% in all surviving transplanted mice;and 14 days after transplantation,the percentage of Tregs detected as CD4~+CD25~+ was significantly higher in recipients treated with IBM-DLI than those treated with IV-DLI(P<0.01).Compared with that of the recipients in IBM-DLI group,the level of IL-4 was significantly decreased,while the level of IFN-γ were elevated in group IV-DLI (P<0.01).Conclusion:IBM-DLI could induce the proliferation of Tregs and the Th polarizing to Th2,resulting in decreasing the incidence and alleviating severity of GVHD after allo-PBSCT.
