2.Scalp psoriasis in China: a survey on epidemiological characteristics and influencing factors

Objective To describe epidemiological characteristics of scalp psoriasis in China,and to determine its influencing factors through a hospital-based,multicenter and large-scale survey.Methods An epidemiological survey was conducted on patients with scalp psoriasis in 42 hospitals in 11 provinces and 4 municipalities.A questionnaire was designed to collect general and clinical information of patients,including clinical type,lesion distribution,psoriasis scalp severity index (PSSI),aggravating factors,influence on quality of life,treatment course,degree of satisfaction with treatment,etc.Participants were asked to fill out the questionnaire with the help of well-trained dermatologists.Data were processed with the EpiData software.The WPS system and Epi Info 5.0 statistics software were used for descriptive analysis of the data.Results Totally,3 901 patients participated this study,and 3 586 valid questionaires were obtained.The mean age at onset of scalp lesions was (27.40 ± 13.34) years,and the male to female ratio was 1.6∶1 with a mean PSSI score of 13.61 ± 12.31.The hairline of the forehead was the most frequently affected area with the constituent ratio of forehead psoriasis being 74.3% (2 663/3 586).The duration of scalp psoriasis reached 3 to 6 months every year in 1 136 (31.7%) patients,and the area affected by scalp psoriasis was less than 25% of the total surface area of the scalp in 1 649 (46%) patients.The most frequent clinical presentation was scaling (92.1%),followed by erythema (80.6%),itching(76.7%),bunchy hairs (39.9%) and hair loss(18.0%).Disease aggravation was most frequently associated with seasons (76.7%),emotions (47.4%),stress (47.1%) and fatigue (46.6%).The quality of life (especially social,outdoor and entertainment activities) was reported to be negatively influenced by this entity to different degrees in 2 529 (70.5%) patients.Of these patients,583 (17.8%) received combination therapy (topical and systemic),with glucocorticoid ointment (27.5%),calcipotriol scalp solution (22.7%) and glucocorticoid-containing solution (11.9%) being the most commonly used topical drugs; only 38.6% complied with medication requirements and doctors' advice for 3 to 5 weeks,and 35.2% believed that the primary reason for poor compliance was unsatisfactory response to drugs.It was considered that an ideal drug for the treatment of scalp psoriasis should be highly effective (50.3%),safe (17.8%) and easy to apply (11%) with a rapid onset of action(11.9%).Conclusions Scalp psoriasis has a predilection for the hairline of the forehead with scaling as the main clinical presentation.Winter and spring seasons appear to be the most common aggravating factor of scalp psoriasis.The quality of life is negatively influenced to different degrees in many patients with scalp psoriasis.Topical corticosteroids plus non-hormonal drugs should be the first choice for the treatment of scalp psoriasis.Patients consider that the ideal drugs for the treatment of scalp psoriasis should be effective,safe,and easy to apply.
3.The use of multi-slice spiral CT angiography for hemoptysis

Objective To evaluate the nse of CT angiography in the diagnosis of hemoptysis and guiding the treatment of it with 64-slice spiral CT.Methods Twenty-two patients with repeated and massive hemoptysis underwent chest CT angiography.Results The blood supply of hemoptysis was nonbronchial systemic arteries in 3 patients,single or multiple bronchial arteries in 15 patients,mixed arteries of nonbronchial systemic and bronchial arteries in 3 patients and abnormal systemic arteries in 1 patient.Conclusion With 64-slice spiral CT,CT angiography provided useful information for the treatment of hemoptysis by guiding bronchial arterial embolization.
5.Therapy of Zhuifeng Tougu Pills on osteoarthritis with chronic kidney disease

AIM: To identify the effect and safety of Zhuifeng Tougu Pills(Radix aconiti praeparata;Radix aconiti kusnezoffii praeparata,Ramulus cinnamomi,Radix et Rhizoma asari,Radix angelieae dahuricae,Radix gentianae macrophyllae,Radix saposhnikoviae,Rhizoma et Radix notopterygii,Radix angelicae sinensis,Rhizoma chuanxiong, Rhizoma cyperi,Radix paeoniae rubra) for the osteoarthritis with chronic kidney disease. METHODS: Patients were divided into the treatment group and the control group randomly and paired with 1∶1 ratio by the involved joints.The treatment group was given Zhuifeng Tougu Pills,and the control group was treated with intermitted NSAIDs. RESULTS: The ratio of pain and stiffness of joint in treatment group was higher than that in control group.And those in treatment group had a less and later development of chronic kidney disease. CONCLUSION: Compared with intermitted NSAIDs,Zhuifeng Tougu Pills were more effective and safer to treat osteoarthritis with chronic kidney disease.
6.Correlation between Single Nucleotide of Polymorphisms of -238,-308 G/A in Tumour Necrosis Factor-? Gene Promoter and Juvenile Idiopathic Arthritis

Objective To study the correlation between single nucleotide polymorphisms of -238,-308 G/A in promoter region of tumor necrosis factor -?(TNF-?) gene and the type of juvenile idiopathic arthritis (JIA).Methods Clinical data and blood preparation of 127 children with JIA and 106 healthy children were evaluated.Subgroups of JIA were defined according to the Edmonton criteria.The -238 G/A and -308 G/A polymorphisms in DNA analysis in this study were extracted from the whole blood.The restricted fragment length polymorphisms were determined in the cases of all JIA children and control group.Results 1.The TNF-?-238 G/A allele frequencies of JIA group and control group:allele frequency of JIA group was 92.9% and 7.1%,and the control group was 95.3% and 4.7%.The distribution of allele frequencies was no significantly different between JIA group and control group(?2=1.149 P=0.284).But there were significant difference between polyarticular JIA (RF negative) and control group(?2=7.621 P=0.006).2.The TNF-?-308 G/A allele frequencies of JIA group and control group:allele frequency of JIA group was 94.1% and 5.9%,the control group was 95.3% and 4.7%.The distributions of allele frequencies was no significantly different between JIA group and control group(?2=0.322 P=0.571).There were significantly difference between polyarticular JIA (RF negative) and control group (?2=7.621 P=0.006).Conclusions The TNF-?-238,-308 polymorphisms of A in the-238 and-308 TNF-? gene are important to the joint destruction of JIA.The study will be beneficial to provide indirect support to the application of anti-TNF drugs to the treatment of JIA.
7.Effect of penetrating needling at head acupoints on perennial allergic rhinitis.

OBJECTIVETo evaluate the effects and safety of penetrating needling on head acupoints for perennial allergic rhinitis (PAR).
METHODSEighty-one cases of PAR were randomly divided into an acupuncture group (41 cases) and a medication group (40 cases). Penetrating needling at head acupoints was adopted from Baihui (GV 20) to Qianding (GV 21) and from Shangxing (GV 23) to Shenting (GV 24) in the acupuncture group. A to tal 4-week treatment was given to the patients with 3 treatments a week. Loratadine tablet and azelastine hydrochloride nasal spray were given to the medication group continuously for 12 days. A follow-up was carried out 3 months after the treatment. The efficacy, symptom score and physical sign score, and side accidents were observed in both groups.
RESULTSThe total effective rate was 95.1% (39/41) in the acupuncture group, which was better than 82.5% (33/40) in the medication group (P < 0.05). The total scores of clinical symptoms and each partial scores after the treatment, and total scores of clinical symptoms in follow-up were obviously decreased in both groups (all P < 0.01), the nasal obstruction score and the total scores of clinical symptoms in the acupuncture group were better than those in the medication group (P < 0.01, P < 0.05). Obvious side-effect had not been found during the treatment.
CONCLUSIONPenetrating needling at head acupoints is a safe therapy for patients with PAR, and favorable effects can be found in both short term and long term.

8.A clinical and mycological study of diaper dermatitis.

Diaper dermatitis is an infant dermatosis associated with the wearing of diapers, Despite of its high frequency in clinical pediatrics the clinical characteristics are not well described in the korean literatvre. We investigated the clinical and mycological characteritics of diaper dermatitis in 50 serial patients. Clinically diaper dermatitis was classified. into five categories,irritant contact derniatitis(ID), candidiasis(CD). combined rash of ID and CD(CR.), intertrigo, and miliari. Of 50 patients with diaper dermatitis, 18 had ID; 16, CI3; 10, CR; 5, intertrigo, and 1, miliaria, The mean age of patients with ID (11.8 months) was higher than that of patients with CD (4.0 months). Candida albicans(C. albicans) w is isolated from 25 patients with diaper dermatitis. The rate of positive cultures by clinical diagnosis was 130% in CD, 11.1% in ID, 60, 0% in CR, and 20.0% in intertrigo. C. albicons was isolated from 23 patients(82.1%) of patients with clinically suspected Ci") and 2 of 22 patients without the characteristic erythematous papulesThe presence of eryhematous papules was the most characteristic clinical feature of infection, Other candida species isolated from cut,aneous lesions were C. tropicalsis, C.'. parapsiolosis, and C. stellatoidea.

9.Construction and MRI of angiotensin Ⅱ transiently induced abdominal aortic dissection in ApoE-/-mouse model

Objective To establish abdominal aortic dissection model in ApoE-/-mice, and to evaluate the ability of 7.0T MR to detect the abdominal aortic artery aneurysms in ApoE-/-mice in vivo. Methods ApoE-/-mice aged 10 months were infused with angiotensin Ⅱ with 14 days Osmotic minipump after 10 weeks of high lipid diet. Two different doses of angiotensin Ⅱ were given to mice, i.e. 1000 ng/(kg·min) and 500 ng/(kg·min), respectively. The contrast group was infused with saline water. The abdominal aortic artery was observed in vivo with MR before and within 14 days infusion. At last, the pathological changes of the abdominal artery were compared with MRI findings. Results After 6 or 7 days higher dose of angiotensin Ⅱ infusion, aortic dissection was seen. MR T2WI showed crescent-shaped high signal in the vessel wall of one side,the pathological study identified the hematoma between media and adventitia. Abdominal aortic dissection aneurysms were also found in the mice 13 or 14 days after lower dose of angiotensin Ⅱ infusion, which were consistent with pathological studies. Besides, the signal of the vessel wall was significantly higher in both T2WI and PDWI sequences. There was excellent agreement between MR and histopathology. 〖WTHZ〗 Conclusion Abdominal aortic dissection aneurysms model can be successfully established with different doses (1000 ng/(kg·min) and 500 ng/(kg·min)) of angiotensin Ⅱ infusion into ApoE-/-mice fed with high lipid diet. High-resolution MR is able to visualize the abdominal aortic dissection aneurysm formation in vivo.
10.Clinical application of CT-guided ~(125)I seed interstitial implantation for recurrent rectum carcinoma

Objective to explore the safety and clinical efficacy of CT-guided radioactive seed implantation in treating recurrent rectum carcinoma. Methods CT-guided ~(125)I radioactive seed implantation was carried out in 20 patients with recurrent rectal carcinoma. Treatment planning system was used preoperatively to reconstruct three dimensional image of the tumor and to calculate the estimated seed number and distribution. The tumor matched peripheral dose (MPD) of the radioactive seeds was 80-130 Gy. The radioactivity of the seeds was 0.5-0.8 mCi/seed and the median implanted seeds was 48 (range 25-95) in number. CT scan was made immediately after the implantation to check the quality of the seeds. Change of pain score, tumor size and complications were recorded during the follow-up period. Results Twenty cases composed of 12 males and 8 females, aged 38 to 78 years (median age of 62 years). The follow-up period lasted 2-28 months. On an average, 3 to 7 days after the procedure patients experienced significant pain relief. CT scan performed 2 months after the procedure revealed that complete relief (CR) of the tumor was seen in 2 cases, partial relief (PR) in 13 cases, no change (NC) in 3 cases and progression (PD) in 2 cases. The total effective rate (CR + PR) was 75%. The median survival time was 18.8 months. The survival rate of 1 and 2 years was 75% and 25% respectively. Two cases died of tumor deterioration and 3 cases died of extensive metastases. No complications such as frequent micturation, pain on urination and hematuria occurred during the follow-up period. Conclusion CT-guided ~(125)I radioactive seed implantation is a safe and effective interventional treatment for recurrent rectal carcinoma with reliable short-term efficacy and excellent anti-pain effect.
