1.Recurrent anterior Dislocation of the Shoulder-Analysis of the Bristow Bristow and the Combined Bakert and Putti: Platt Operation

Numerous operations have been described to treat the recurrent anterior dislocation of the shoulder, but no one procedure was accepted as the choice of operation. We perforemed 6 Bristow procedures and 5 combined Bankart and Putti-Platt procedures and examined six shoulders by arthroscope from sep. 1989 to Sep. 1992, and we analysed the pathologic feature and the results of them. The average follow-up period was 2.2 years. 1. There were 8 males and 3 females and their average age was 33.7 years. The mean age in initial dislocation was 28 years. 2. The number of recurrent dislocation before operation was 17.9 times and the average duration of the disease was 5.7 years. 3. The pathologic lesions observed in intraoperative field were Hill-Sachs lesion (72.7%), Bankart lesion (90.9%), and the erosion of the glenoid rim (54.5%). On arthroscopic examination of 6 cases, Hill-Sachs lesion was found in 4 cases, Bankart lesion was seen in all of 6 cases, and the erosion of the glenoid rim was observed in 4 cases. 4. The average range of the limitation of external rotation was 17.3 and its significant difference was not observed between 2 operative groups. 5. By Rowes rating sheet, 4 cases of 5 combined Bankart and Putti-Platt operations were excellent and 1 case was good. 6. In conclusion, combined Bankart and Putt-Platt operation was considered as a good surgical procedure in the treatment of recurrent anterior dislocation of the shoulder.

2.Basic Study on the Effect of Korean Ginseng upon Fracture Healing of the Bone

This study was planned by the necessities of basic information about the effects of Korean Ginseng upon fracture healing and biomechanical properties of bone. Three hundred white mice were used for this study, and we could have tested the biomechanical properties in one hundred and nintyfour. The bones of the white mice were evaluated by testing gross stability, radiological appearance, and biomechanical properties. It appears that Korean Ginseng has an excellent effect upon early phase of fracture healing of the bone. The summary of this study are as follows: 1. With near maximal dose, Korean Ginseng showed very excellent effect upon early phase of the fracture healing. 2. There was no apparent effect on remodeling phase of the fracture healing with Korean Ginseng of near maximal dose. 3. Korean Ginseng appeared to have no effect on fracture healing with minimal dose. 4. Upon biomechanical properties of the bone, Korean Ginseng was seemed to be effective with near maximal dose. But, there was no definite evidence for that.

3.A Case ef Orbital Cellulitis Subdural Subdural Abscess.

Acute orbital cellulitis is most commonly caused by sinusitis. This complication is based on the anatomy of sinuses, orbit, and their venous connections. We have experienced a case of acute orbital cellulitis combined Subdural abscess due to maxillay sinusitis in a 14-year-old girl. Rapid Surgical drainage in the both upper lid and forehead and craniectomy resulted in complete recovery.

4.Posterior Tarsal Tunnel Syndrome Caused by a Ganglion.

Tarsal tunnel syndrome is relatively an uncommon and usually under-recognized or misdiagnosed condition. We report a case of posterior tarsal tunnel syndrome caused by a ganglion . A 58-year-old woman complained of burning and tingling sensation along the left sole and presented weakness, muscle atrophy, and Tinel's sign in the left foot. Electrodiagnostic study revealed prolonged terminal latency, reduced compound muscle action potential, absent sensory nelve potential, and denervation potentials. After removal of the ganglion, patient's symptoms improved and the electrodiagnostic study performed after two months showed significant improvement.

5.Treatment of Fibrous Dysplasia

Authors have reviewed a series of 41 cases of fibrous dysplasia which were treated surgically at the Department of Orthopedic Surgery, College of Medicine, Seoul National University. After mean follow-up of 25 months, we summarized the results as follows; 1. Among the 41 patients, 20 were male patients(M:F= 1:1.05). The average age was 20.2 years old. It was the most common in the 2nd decade(44%). The most common site was femur (53.6%) followed by tibia (28.6%) in monostotic type. Each polyototic patient had 3.8 locations in average. 2. The results were better in skeletally mature patients (86%) than in immature patients (42%). 3. Satisfactory results seemed to be expected in monostotic type (71%) than polyostotic type(33 %) 4. Satisfactory results seemed to be expected in sclerotic type(78%) than cystic type(50%) or ground glass appearance type (62%). 5. Autograft(71%) was superior to xenograft(40%) in our study. However, xenograft only or xenograft with autograft can be a recommendable method of treatment for a very large lesion or for a patient with insufficient graft donor site.

6.Two Cases of Pilomatricoma Occurring on Upper Eyelid.

Pilomatricoma(calcifying epithelioma of Malherbe) is a rare benign turner of the skin which develops from immature hair matrix cells. The diagnosis of the calcifying epithelioma of Malherbe is only rarely made on clinical ground but is, as a rule, easily made by histologic examination. We briefly report 2 cases of pilomatricoma occurring on upper eyelid with review of the literature.

7.The Effect of Diffusion Blur on Central and Peripheral Vision in Normal Adults.

Diffusion blur, caused by liquid crystal windows, reduces contrast sensitivity and luminance, so visual acuity is decreased. Moreover, the windows' optical characteristics are similar to those of the cataractous lens. We measured the effect of liquid crystal window on central and peripheral visual acuity using Snellen and grating visual acuity charts. The subjects were 10 normal adults, therefore 20 eyes. We measured the voltage of the liquid crystal window at which the subjects could read the visual acuity chart through the liquid crystal window. In central vision, visual acuity was overestimated according to the Snellen chart, when compared to the results measured by those grating chart. But in peripheral vision, there were no differences between the two visual acuity charts. These results suggested that grating acuity has some limitations when used as a visual acuity test for cataractous patient. The ability of liquid crystal windows to cause diffusion blur suggests they could be used for foUowup observation in treatment of amblyopia or macular diseases.

8.A Case of Halo Congenital Nevus Undergoing Spontaneous Regression.

Halo congenital nevus is a condition in which halo formation is associated with congenital nevocellular nevus. Both humoral and cell-mediated immunity have been implicated to be involved in halo formation. We described a 8-year-old boy with halo congenital nevus characterized by the unique histologic location of inflammatory cells. He has no personal and familial history of vitiligo, dysplastic nevus, melanoma or autoimmune disease. Histologically, the present case of halo congenital nevus undergoing spontaneous regression showed a marked inflammatory infiltrate with remnants of original nevus cell nests in the upper dermis, whereas no inflammatory infiltrate was present in the deep dermis. Most of inflammatory cells were T-cells.

9.Bacteriologic Study of Upper Urinary Tract Infection with Ureteral Stone.

In clinical practice, we have occasionally experienced the difficulty to identify the pathologic changes of the non or faint-visualized kidney induced by ureteral stone on I.V.P. and especially to decide whether upper urinary tract infection of ipsilateral side was in accordance with lower urinary tract infection or not. We have checked routinely the bacteriological study of the urine collected from the ureterolithotomized site on operation during last two years. Following results were obtained: 1) Among the cultured urine of 42 patients, organism detected in 20 patients and no growth in remainder. Colifrom bacilli 11(26.2%) Staphylococci 5(11.9%) Streptococci 1(2.4%) Pseudomonas 1(2.4%) TB bacilli 2(4.7%) 2) Sensitive drug pattern of 18 patients, Coliform bacilli (11): Kanamycin, Streptomycin, Chloramphenicol Staphylococcus (5): Penicillin, Kanamycin Streptococcus (1): Penicillin Pseudomonas (1): Geopen From above results, some advantages has been provided in the post-operative management of upper urinary tract infection after removal of the ureteral stone.

10.Squamous Cell Carcinoma of the Renal Pelvis: A Case Report.

Squamous cell carcinoma of the renal pelvis is a rare disease which is occupied about 17% of all tumors of the renal pelvis. Through many experimental and clinical practices, the stone and the infection as its etiological factors have been suggested, however, squamous cell carcinoma of the renal pelvis not associated with these etiological factors were found occasionally. We have experienced a case of squamous cell carcinoma developed in the renal pelvis without stone or infection.
