1.An animal experimental study of the loading time on anchorage implant stability.

OBJECTIVEThis study aimed to evaluate the influence of loading in different healing periods on implant stability and to establish an animal model of anchorage implant screws.
METHODSAnchorage implant screws were implanted in sheep alveolar bone to establish the implant screw animal model. The animals were randomly divided into four groups: group A, sheep without loading; group B, sheep loaded immediately after implant; group C, sheep loaded after two weeks; and group D, sheep loaded after four weeks. The maxillary and mandibular tissue specimens with implants were dissected. The maxillary tissue specimens were used to make undecalcified bone grinding slices, and the healing mode of the implant-bone interface was observed by light microscope. The maximum shear strength of loose mandibular specimens was measured by a material testing machine. The differences in group data were statistically analyzed. RESULTS 1) A general difference in shear strength exists among the four groups. The shear strengths in groups A and B were lower than those in groups C and D. 2) No significant difference in healing mode was found between the pressure side and the tension side of the longitudinal-grinded bone slices with implants. Both fiber combination and osseointegration of transverse-grinded slices could be found in the interface in all four groups. The major integration mode of the implant-bone interface of groups A and D was osseointegration, whereas that of the implant-bone interface of groups B and C was fibrous tissue transformation. The surrounding bone tissue regenerated toward the implants of group C.
CONCLUSIONResults show that loading time influences the stability of implant anchorage.

4.Improve clinical scientific research with a solid base in clinical work

5.Endovascular embolization treatment of intracranial aneurysms with Gugliemi detachable coils

Objective To report the experiences in the endovascular embolization treatment of intracranial aneurysm. Methods A total of 29 patients with intracranial aneurism were treated with Gugliemi detachable coils(GDC). Results Complete occlusion was found in 15 cases, 90% occlusion in 12 and 80% in 2. Three patients had different degrees of headache. The coil escaping from the sac of the aneurysm and suspending in the parent artery of the aneurysm was seen in 1 patient. Two patients died: one was due to disruption of aneurysm and another severe bleeding in digestive tube. Conclusion Endovascular embolization of intracranial aneurysm is a minimally traumatic, safe and effective method for the treatment of intracranial aneurysm. GDC system is of easy operation, high safety and low incidence of complications.

7.Intensity-modulated radiotherapy for sinonasal teratocarcinosarcoma.

Surgical excision and postoperative radiotherapy are widely accepted therapeutic modalities for sinonasal teratocarcinosarcoma (SNTCS). Previous studies have shown that intensity-modulated radiation therapy (IMRT) is safe and effective for the treatment of some sinonasal malignancies. We hypothesize that use of IMRT for SNTCS may result in clinical benefits. We report here two cases of SNTCS that were treated by IMRT. One patient was given extensive IMRT involving elective neck irradiation. Follow-up examinations revealed no recurrence and metastasis 3.5 years after IMRT. Another patient simultaneously suffered from multiple systematical diseases and was administered involved-field radiotherapy. He was found to have a marginal recurrence during the follow up and eventually died 8 months after disease diagnosis. It was suggested in this study that appropriate use of IMRT for the treatment of SNTCS may achieve excellent local control.

9.Risk factors of deep vein thrombosis in patients with acute traumatic spinal cord injury

Objective To investigate the incidence and risk factors of deep vein thrombosis (DVT) in a cohort of Chinese patients with traumatic spinal cord injury (TSCI),and in addition,to evaluate the effect of injury types and concomitant injuries of other parts of body.Method Records of 143 casualties admitted to our institute for TSCI were analyzed retrospectively.Both lower extremities of all casualties were examined routinely with color Doppler ultrasonography (CDUS) before major surgical treatment.None of these casualties received any thromboprophylaxis before CDUS.Logistic regression was used to analyze risk factors of DVT.Results Of the 143 patients,32.2% (n =46) suffered from DVT (proximal n =15,distal n =31).All casualties were diagnosed within one week after injury (mean 3.35 ± 1.65 days).Risk factors associated with DVT induced complete motor paralysis (odds ratio [OR] 5.05,95% CI 1.70-14.90),concomitant fracture of lower extremities (OR 4.30,95% CI 1.34-17.64),and lumbar vertebra injury (OR 4.29.95% CI 1.50-10.83).Conclusions Clinically detectable DVT in TSCI are not uncommon in the Chinese population.Casualties with multiple risk factors may be benefited from early administration of thromboprophylaxis.
