1.The relationship between epithelial-mesenchymal transition and clinic pathology of IgA nephropathy

Objective To analyze the correlation between the epithelial‐mesenchymal transition and the clinicopathologic features of IgA nephropathy .Methods A total of 168 patients diagnosed as IgA nephropathy by renal biopsy in Xinqiao hospital from Janu‐ary 2011 to December 2013 were divided into high expression of dual‐positive Snail and a‐SMA group (Group A ,117 cases) and low expression of dual‐positive Snail and a‐SMA group (Group B ,51 cases) according to results of immunohistochemical method .The clinical parameters (age ,gender ,course of disease ,BMI and chemical indicators) and renal pathology grade were compared by statis‐tical analysis .Results There were difference between group A and group B in the course of disease and BMI(P<0 .05) .There were differences between group A and group B in the incidences of creatinine ,blood urea nitrogen ,serum triglycerides and 24‐hour urine protein amount (P<0 .05) .The percentage of Lee′s grade Ⅰ ,Ⅱ ,Ⅲ in group B was significant ,while percentage of Lee′s gradeⅣ + Ⅴin group A was significant .The expression of Snail and a‐SMA in grade Ⅳ + Ⅴ was more than that in grade Ⅰ + Ⅱ (40 .2%vs .9 .4% ) ,and the difference between two groups was statistically significant (P< 0 .05) .Conclusion The expression of Snail and‐SMA were related with 24‐hour urine protein amount and kidney function in IgA nephropathy ;and Lee′s grade was severe in patients with high expression of Snail and‐SMA .
2.Effect of hyperlipidemia and influence of simvastatin on endoplasmic reticulum stress in rat kidney

AIM: To investigate the role of endoplasmic reticulum stress in renal injury caused by hyperlipidemia and the influence effect of simvastatin. METHODS: Thirty male Wistar rats were randomly divided into three groups: rats in control group (n=10) were fed with normal diet; rats in high fat group (n=10) were fed with high fat diet; animals in simvastatin+high fat group (n=10) were fed with high fat diet and were received simvastatin 10 mg·kg~(-1)·d~(-1) by gastric irrigation. After 18 weeks, the quantitative urine protein in 24 h, the serum cholesterol and triglycerides levels were tested. The pathological changes of renal tissue were observed under optic microscope. The expressions of GRP78 and p-JNK in renal tissues were examined by immunohistochemistry. The apoptotic cells in the kidney were detected by TUNEL staining. The mRNA expressions of GRP78 and CHOP were examined by RT-PCR. RESULTS: The quantitative urine protein in 24 h, the serum lipid, the expressions of GRP78 and p-JNK proteins, the mRNA expressions of GRP78 and CHOP as well as the apoptotic cells in renal tissues were increased in high fat group (P<0.01).The quantitative urine protein in 24 h, the serum lipid, the expression of GRP78 and p-JNK proteins, the mRNA expressions of GRP78 and CHOP as well as the apoptotic cells in renal tissues were remarkably reduced in simvastatin+high fat group than those in high fat group (P<0.05). CONCLUSION: The endoplasmic reticulum stress is engaged in the renal injury caused by hyperlipidemia. The simvastatin play a role in renal protection by inhibiting the endoplasmic reticulum stress in the kidney.

4.Analysis of differences in cognition between doctors and patients and causes of medical disputes

Objective To identify the key factors triggering medical disputes induced by differences in cognition between doctors and patients in order to control the occurrence and development of medical disputes. Methods From 2002 to 2006,the outpatients,inpatients and medical staff of 3 hospitals in Shanghai(class 3 first level general hospital,n=2;class 3 first level special hospital,n=1) were investigated,and random sampling method was employed for the research analysis. Results There were significant differences between doctors and patients in cognition of influential factors triggering medical disputes such as health law,medical services,rights and obligations. Conclusion The differences in cognition between doctors and patients are important causes for medical disputes,and effective prevention and intervention measures must be taken.
5.Expression of VEGF-C and it's relationship with clinical pathologic characteristics in rectal cancer

Objective To study the expression of VEGF-C and it's relationship with pathologic characteristics in rectal cancer.Method In this study 82 patients of mid to low rectal cancer underwent radieal resection from December 2007 to August 2008.Tumor tissues and lymph nodes were studied by immunohistochemical staining for VEGF-C expression in the tumor tissue and CK20 expression in D3 lymph nodes.Results were compared with the pathological results of mesenterium lymph nodes to explain the relationships between VEGF-C expressions and clinical pathologic characteristics.Data were analyzed with Chi square test.Result As for the expression of VEGF-C.significant differences were found between tumor stages(X~2=8.529,P<0.05)and between positive and negative lymph node metastasis(X~2=4.712,P<0.05).but there was no difference between that of mesenterium lymph node metastasis and D3 lymph node micrometastasis(X~2=0.017,P>0.05). Conclmion In rectal cancer,VEGF-C expression is correlated with tumor stage,type,metastasis and micrometastasis of lymph node.
6.Surgical treatment for gyncomastia.

OBJECTIVETo introduce different surgical treatment for gyncomastia at different grades.
METHODS37 cases with gynecomastia were divided into three grades as: grade I with fat as main tissue, grade II with proliferated fibro-gland as main tissue, grade III with big and ptosis breasts and sagging skin. Different surgical methods were chosen according to the different grades of gyncomastia. These include liposuction, subareolar fibroglandular tissue removing, combined technique of the two methods, and breasts resection with free transplantation of nipple-areola complex.
RESULTSAll patients were satisfied for the appearance of post-operative flat male chest. Complications, such as scar, numbness of nipple and areola were acceptable for them.
CONCLUSIONSDifferent surgical methods should be chosen for the gynecomastia at different grades. It can improve both the physical and psychological problems for patients.

7.The inhibitive effect of tea polyphenols on oxidized low density lipoprotein intake in cultured rat mesangial cells.

OBJECTIVETo study the inhibitive effect of tea polyphenols (TP) on oxidized low density lipoprotein (OX-LDL) intake in cultured rat mesangial cells.
METHODRat mesangial cells incubated in 80 mg.L-1 OX-LDL were treated with TP at different concentrations. Total cholesterol (TC) content of the mesangial cells and proportion of mesangial cells containing lipid droplets were measured, and ultrastructure observation was performed.
RESULTFoam cells were observed in mesangial cells stimulated by OX-LDL, and both the TC content and the proportion of lipid-droplet-containing cell were higher in these cells than in mesangial cells cultured in lipid free conditions. There were concentration-dependent decreases both in TC content and lipid-droplet-containing cell proportions when treated with TP from 40 ng.L-1 to 40 mg.L-1. The number of lipid droplets was also decreased in TP treated mesangial cells.
CONCLUSIONTP has an inhibitive effect on OX-LDL intake in cultured rat mesangial cells, which is in a concentration-dependent manner from 40 ng.L-1 to 40 mg.L-1.

Utilizing micro-pump and under the control of SCM,the tourniquet can automatically control antimemorrhagic pressure,antimemorrhagic time and loosing time.It is suitable for the automatic hemostasis of limbs.There are two working modes for medical service staff to select including air automatic inflation & releasing mode and manual air releasing mode.A new design of the sleeve bandage enable the wounded arms and legs use the same tourniquet.It is easy and convenient to release and repressurizing,which is suitable for the wounded to self operated.
9.The curative effect and safety research of bisoprolol in the treament of the elderly COPD patients with chronic heart failure

Objective To assess the curative effect and security of bisoprolol in the elderly chronic obstructive pulmonary disease (COPD) patients combined with chronic heart failure (CHF) .Methods 61 cases of elderly COPD patients combined with CHF in our hospital were selected .on the basis of conventional treatment ,the patients were applicated bisoprolol 2 .5 mg ,1 time each day , the NT-proBNP ,LVEF% ,LEVDD and FEV1% were respectively observed before treatment ,4 weeks after treatment ,8 weeks af-ter treatment ,6 months after treatment .Results 4 weeks after treatment ,NT-proBNP ,LVEF% ,LEVDD showed no significant changes .8 weeks after treatment ,NT-proBNP and LEVDD decreased(P<0 .05) ,LVEF% showed no significant changes .6 months after treatment ,NT-proBNP and LEVDD decreased significantly ,meanwhile LVEF% increased (P< 0 .05) .before treatment ,4 weeks after treatment ,8 weeks after treatment ,6 months after treatment ,FEV1% were 47 .33 ± 5 .57 ,47 .08 ± 5 .50 ,48 .30 ± 6 .31 , 49 .64 ± 6 .24 ,All groups showed no changes in the pulmonary function(P>0 .05) .NT-proBNP of all groups were negatively corre-lation with the FEV1% (r= -0 .708 ,-0 .702 ,-0 .833 ,-0 .778) .Conclusion Small doses of bisoprolol is safe and effective in the treament of the elderly COPD patients with chronic heart failure .
10.Successful pregnancy following laser-assisted selection of viable but immotile spermatozoa for intracytoplasmic sperm injection: A report of 2 cases.

OBJECTIVETo investigate the feasibility and clinical application value of selecting viable spermatozoa by noncontact diode laser.
METHODSWe obtained immotile spermatozoa from 2 infertile men with obstructive azoospermia or severe asthenospermia and selected viable spermatozoa using a single laser shot at the sperm tail. Those that responded to the laser shot by a curling reaction of the tail were regarded as presumably viable and used for intracytoplasmic sperm injection (ICSI).
RESULTSThe mean fertilization rate was 88.89% after ICSI with the laser-selected viable spermatozoa. Both of the embryo transfers resulted in a single pregnancy.
CONCLUSIONNoncontact diode laser is a useful alternative for the assessment of sperm viability, which may help to achieve successful pregnancy.
