1.Effect evaluation of white acetate solution on cervical lesion screening

Objective To explore the value of cervical lesions screened with white acetate solution. Methods Four hundred and eighty-five patients who were suspected with cervical lesion during Janurary to March 2016 were enrolled in this study. The patients were asked about the medical history, filled the registration form, and performed routine gynecological examinations with white acetate solution, thinprep cytology test (TCT) and vaginal endoscopic biopsy. The value of white acetate solution in clinic was assessed. Results In 485 patients, 450 patients (92.78%) were pathologically diagnosed of inflammation (92.78%), 24 patients (4.95%) were diagnosed of low-grade squamous intraepithelial lesion (LSIL), 10 patients (2.06%)were diagnosed of high-grade squamous intraepithelial lesion (HSIL), and one patient (0.21%) was diagnosed of cervical cancer. The detection rate of white acetate solution and TCT were gradually increased with the increase of the degree of cervical lesions. The detection rate of white acetate solution in the inflammation was 1.33%, in LSIL was 91.67%, in HSIL was 9/10, and in cervical cancer was 1/1. The detection rate of TCT in the inflammation was 8.44%, in LSIL was 66.67%, in HSIL was 6/10, and in cervical cancer was 1/1. The sensitivity, specificity and coincidence rate of the diagnostic results with white acetate solution were 84.21%, 99.33%, 98.14%, and with TCT were 62.30%, 93.63%, 89.69%. There were significant differences (P<0.05). Conclusions Cervical lesions screened with white acetate solution is good in sensitivity and specificity, which is an effective way for screening cervical cancer.
2.Correlations between expectations and anxiety and depression of cervical cancer patients

Objective To explore the correlations of between expectations and anxiety and depression of cervical cancer patients. Method Herth hope index,self-rating anxiety scale(SAS)and self rating depression scale(SDS)and self-made general information questionnaire were used to investigate the levels of hope,anxiety and depression as well as the correlations between them.Results The overall level of hope in cervical cancer patients was(36.89±4.21)in a moderate to high level. The scores on hope level and keeping intimate relationships with other in the group of patients,aged above 50 years,married,with high school education or above and monthly income of 1,500 yuan were significantly higher than those of the other group patients.The incidences of anxiety and depression were 47.83%and 52.17%, respectively. The general hope level and its three dimensions was negatively related to the anxiety and depression(P<0.01).Conclusions The hope of cervical cancer patients are at moderate to high level and are negatively correlated between anxiety and depression.Improving the hope level of cervical cancer patients can effectively improve patients bad feelings.
3.Clinical study of the effect on reproductive potential of salpingectomy in child-bearing period women

Objective To analyze the effect on reproductive potential of salpingectomy in child-bearing period women.Methods Two hundred and forty-six tubal pregnancy patients were chosen,among whom 164 patients performed salpingectomy were classified as research group,and 82 patients retained ipsilateral tubal through conservative treatment were classified as control group.The levels of sex hormone,ovarian volume,sinus number of follicle,ovulation rate,reproductive outcome and reproductive status were compared between 2 groups.Seven patients were lost in research group,and 3 patients were lost in control group.Results One,three and six months after β-human chorionic gonadotrophin (β-hCG) put back,the serum levels of follicle stimulating hormone (FSH),luteinizing hormone (LH) in research group were significantly higher than those in control group,FSH:(10.00 ±4.82) U/L vs.(6.69 ± 2.71) U/L,(9.11 ± 3.77)U/L vs.(6.81 ± 2.80) U/L,(8.30 ± 3.49) U/L vs.(6.41 ± 2.41) U/L;LH:(8.74 ± 3.56) U/L vs.(6.10 ± 2.59)U/L,(9.42 ± 3.09) U/L vs.(5.79 ± 2.10) U/L,(7.96 ± 2.53) U/L vs.(6.50 ± 2.71) U/L,and there were statistical differences (P < 0.05).One and three months after β-hCG put back,the serum levels of estradiol (E2) and anti-Mullerian hormone (AMH) in research group were significantly lower than those in control group,E2:(111.44 ± 22.24) pmol/L vs.(128.22 ± 24.01) pmol/L,(108.74 ± 21.67) pmol/L vs.(126.30 ± 23.78) pmol/L;AMH:(3.22 ± 0.85) μ g/L vs.(3.64 ± 0.87) μ g/L,(3.67 ± 0.94) μ g/L vs.(3.83 ± 1.12) μ g/L,and there were statistical differences (P < 0.05).Six months after β-hCG put back,there was no statistical difference in the serum levels of E2 and AMH:(127.85 ± 24.42) pmol/L vs.(131.28 ± 25.54) pmol/L and (4.35 ± 1.34) μ g/L vs.(4.47 ± 1.76) μ g/L,P > 0.05.In salpingectomy patients,the ovarian volume and the sinus number of follicle at lesion side (salpingectomy side) after treatment of 6 months were significantly lower than those in no lesion side:(8.86 ± 2.36) cm3 vs.(9.74 ± 2.31) cm3 and (5.10 ± 1.14) pieces vs.(7.26 ± 2.52) pieces,and there were statistical differences (P < 0.05).After treatment of 2 years,the rate of ectopic pregnancy in control group was significantly higher than that in research group:12.66% (10/79) vs.5.10%(8/157),and there was statistical difference (P < 0.05).Especially,the patients who retained ipsilateral tubal conservative treatment had higher ectopic pregnancy rate:8.86% (7/79) vs.0,and there was statistical difference (P < 0.05).Conclusions Salpingectomy can affect the reproductive potential,but it can decrease the probability of ectopic pregnancy.Whether patients with tubal pregnancy need to be treated by salpingectomy,depends on the extent of tubal lesions and fertility desire of the patients.
4.Clinical observation of caffeic acid for the treatment of leukopenia and thrombocytopenia induced by imatinib in patients with chronic myeloid leukemia

Objective To investigate the curative effect and adverse reaction of caffeic acid (CFA) for leukopenia and thrombocytopenia caused by imatinib in chronic myeloid leukemia (CML) patients.Methods The effect and adverse reactions of CFA for leukopenia or thrombocytopenia induced by imatinib in 42 CML patients were observed.The CFA was admitted by 0.2 g/time,3 times/d,oral.Results 21 cases of 42 CML patients with thrombocytopenia achieved remarkable curative effect after CFA treatment,13 achieved good effect and 8 had no effect.The total effective rate was 81.0 % (34/42).While 15 of 28 leukopenia CML patients achieved remarkable curative effect,8 achieved good effect and 5 had no effect.The total effective rate was 82.1% (23/28).After CFA therapy for 2 weeks,both platelet and white blood cells count were elevated compared with before treatment,the differences were statistically significant (t =2.015,P =0.023,t =1.913,P =0.035).The average onset time of CFA for leukopenia and thrombocytopenia was 2 weeks and no treatment-related adverse reaction was found.Conclusion CFA is an effective and safe auxiliary drug for treatment of imatinib-induce leukopenia and thrombocytopenia in patients with CML.
6.Optimization of Radioligand Assays for Islet Autoantibodies

Objective To optimize the radioligand assays for glutamic acid decarboxylase antibody (GAD-Ab), tyrosine phosphatase antibody (IA-2A) and insulin autoantibody (IAA) and to facilitate their application in clinical practice. Methods The radioligand assays established in our laboratory were optimized in several aspects, including the mode and time of antigen-antibody incubation, pH value of TBST buffer, times of washing, and type of centrifuge. The sensitivity and specificity of the optimized assays were calculated by the Diabetes Antibody Standardization Program (DASP, 2003). And the results were compared to those measured with domestic radioimmune assay kits. Results The optimal condition for radiligand assay were as followed: incubating the antigens of GAD-Ab and IA-2A with serum by rotating slowly for 24 hours and IAA for 72 hours respectively; washing the precipitation complex of GAD-Ab and IA-2A with TBST buffer (pH value 7 2~7 4) 4 times and IAA 5 times respectively; and applying the horizontal type of centrifuge. The results from DASP demonstrated that sensitivity and specificity of the optimized assays were 82% and 98% respectively for GAD-Ab, and 64% and 100% for IA-2A. The proportion of consistency between our results for 82 IAA gradient index of optimized assay and those measured with domestic radioimmune kits was 89%. The extensive analysis showed that the consistency reached 100% for those with IAA index less than 0 04 or more than 1 00, and the inconsistency was for the bordline positive ones (index 0 04~1 0). Conclusion The optimized radioligand assays for islet autoantibodies are high sensitivity and specificity, and are applicable in clinical practice.

8.Correlative risk factor comparison of mild hemorrhage between patients of acute cerebral infarction and transient ischemia attack

Objective To compare the risk factors of mild hemorrhage and diiferences of TOAST type in patients with acute cerebral infarction (ACI) and transient ischemia attack (TIA).Methods Forty patients with mild hemorrhage were divided into ACI group(24 cases) and TIA group(16 cases) according to disease classification.Risk factors such as age,blood pressure,blood lipid and so on between 2 groups were compared,TOAST subtype differences of 2 groups were compared.Results The age [(73.3 ± 4.1) years],the ratio of hypertension [95.8%(23/24)],leukoaraiosis [70.8%(17/24)] in ACI group was significantly higher than that in TIA group [(65.5 ± 5.6) years,13/16,9/16],there was significant difference (P < 0.05).The ratio of large-artery atherosclerosis and cardioembolism in ACI group [75.0% (18/24) and 16.7% (4/24)] was significantly higher than that in TIA group (8/16 and 1/16),there was significant difference (P < 0.05).But the ratio of small artery occlusion in ACI group [8.3% (2/24)] was significantly lower than that in TIA group (7/16),there was significant difference (P < 0.05).Conclusion Correlative risk factors of milde hemorrhage in ACI and TIA patients have significant differences.
9.Investigation of Children with Cerebral Palsy in Rehabilitation and Their Families in Shenzhen

ObjectiveTo investigate the children with cerebral palsy (CP) in rehabilitation, their families and need of society support. Methods150 children with CP in rehabilitation participated in this study. A questionnaire was designed to investigate their basic situation, family status, society concern and requirements for society support, which completed by their parents. Results150 questionnaires were completed efficiently. 75 children were firstly treated before 1 year old. The rehabilitation treatments were effective to 130 children. 126 children appeared some disorder in emotion and personality. Home education was the main rehabilitation way in 133 children. The educational background of 59.4% mothers were senior high school or below. The income was less than 60 thousand yuan in 76.7% families, but spent more than 40 thousand yuan per year on treatment in 54.7% families. One or both parents in 119 families were forced to stop working. 93.3% of them suffered emotion pressure and 56.7% lacked the knowledge about CP. 81% families thought that more social support should be offered in 6 aspects including government welfare, social security, medical institutions, psychotherapy, community services and legal aid. ConclusionAll society should pay more attention on the children with CP and provide a good rehabilitation environment for them.
10.Normal lung dose-volume histogram varieties in the former and inter period of three dimensional conformal radiation therapy plans and their clinical significance in non-small cell lung cancer

Objective To analyze the normal lung dose-volume histogram(DVH) varieties in the former and later period(P1 and P2)of three dimensional conformal radiation therapy(3DCRT) plans and the compound (Pc) plan in non-small cell lung cancer(NSCLC),and to access the feasibility to modify the target volume during the treatment course.Methods Twenty-one NSCLC patients who had received accelerated hyper-frationation 3DCRT in P2 were included in the study.Both of the P1 and P2 plans were redesigned to a total dose of 70 Gy with V20 smaller than 35%.When the target volume was modified and P2 plan was rede signed using accelerated hyper-frationation 3DCRT of 30 Gy after P1 plan of 40 Gy,the Pc plan was compoun ded by transmitting the parameters(such as target volume,irradiation field and dose) of P1 plan into P2 plan. Total lung volume and target volumes(GTV,PTV) of P1 and P2 were evaluated.MLD,V5,V10,V20 and V30 of P1,P2 and Pc were calculated.Results The total lung volume in P1 and P2 plans was not significantly dif ferent(t = 0.19,P = 0.850).The volumes of GTV,PTV in P2 were obviously smaller than P1 (t = 2.88,P = 0.009 ; t = 4.01 ,P = 0.001) .When comparing P2 with P1 ,MLD were 16.5 Gy Vs 17.8 Gy (t = 2.60, DOI:10.3760/cma.j.issn.1004-4221.2009.01.057 P = 0.017),V30 was significantly decreased (t = 2.19,P = 0.041),but V5,V10 and V20 had no significant difference.Similar differences were found in MLD,V5 ,V10 ,V20 and V30 when comparing Po to P1.P2 plans had significantly smaller MLD,Vs,V10,V20 and V30 than Pc plans.Fourteen patients with decreased PTV were further analyzed.The V30 and MLD decreased significantly (t = 3.00,P = 0.0 I 0;t = 2.38,P = 0.033), but V5 ,V10,V20 had no difference when comparing P1 and P2 plans.Among these 14 patients,the V10 and V30 decreased significantly(t = 2.76,P = 0.033 ; t = 3.60,P = 0.011) when P2 plans were generated using the same field number and beam angles in P1 plans in 7 patients.The parameters were similar in P1and Pc plans,but increased significantly when comparing to P2.Various parameters were the same among P1,P2 and Pc plans when P2 plans were designed using 1-2 different fields and angles in the other 7 patients.The differ ences were not significant between P1 and P2 plans in 7 patients with the same or increased PTV.Expansion or contraction of PTV significantly influenced MLD and V2o (r =-0.62,P = 0.03 ; r = O.48,P = 0.029). Conclusions When the tumor regresses,the high dose volume of the lung decreases with modifying the tar get volume and replanning in the later period using accelerated hyper-frationation 3DCRT.The low dose vol ume of the lung may decrease if the field orientations are same throughout the treatment.It is rational to eval uate the normal lung DVH of the whole plan when the physical parameters of the later period plan are the same as the former one.
