1.Nutrition support therapy for patients with hepatic encephalopathy

Hepatic encephalopathy is one of the most common complications of liver failure and has a high incidence in hospitalized patients. Most patients with hepatic encephalopathy have varying degrees of malnutrition.In recent years,more and more clinical trials have confirmed that the nutritional support therapy is beneficial to patients with hepatic encephalopathy,and more attention has been paid to nutritional ther-apy.This paper introduces the pathogenesis of malnutrition in hepatic encephalopathy,and reviews the methods for nutrition evaluation and nutritional therapy in patients with hepatic encephalopathy.
2.Study on preparation of borneol-?-cyclodextrin inclusion complexes in Zhnyu Hewei Zhitong Capsules

Objective:To optimize the preparation of borneol-?-cyclodextrin inclusion complexes in Zhuyu Hewei Zhitong Capsules,and to establish the metered method of borneol in inclusion complexes.Methods:Orthogonal design was used and inclusion complex yield and inclusion rate of berneol were used as evaluation index to optimize the best preparation craft. The chromatographic conditions were:PEG-20M with 10%,3m??3mm of application concentration was used as stationary liquid phase,FID detector was used at 140℃,betula oil was used as internal standard substance and the sample was dissolved in Colonial spirit by ultrasounding.Results:The best preparation craft were:the weigh ratio of borneol and?-CD was 1:6,the ratio of?-CD and moisture addition was 1:10,reaction time was 30min,the temperature was 30℃.A good linearity was obtained when the concentration of borneol was between 0.7146-4.2878mg/ml(r=0.9999),the average recovery was 101.97%,RSD% was 1.77%.Conclusion:Three batches of tests indicated that the preparation of borneol inclusion complexes was stable and feasible. This assay method of borneol was accurate,sensitive and reproducible.
3.Clinical and endoscopic analysis of hepatogenic ulcer hemorrhage

Objective To summarize the clinical symptoms and endoscopic features of hepatogenic ulcer hemorrhage so as to strengthen the recognition of this disease.Methods The clinical data of 42 patients with hepatogenic ulcer hemorrhage from January 2000 to May 2005 were analyzed retrospectively.Results The clinical symptoms were multiple,including melena in 42 cases(100%),hematemesis in 30 cases(71.4%),abdominal distention in 35 cases(83.3%),abdominal pain in 9 cases(21.4%) and acid regurgitation in 18 cases(42.9%).Emergency endoscopy could find active hemorrhage of stomach or duodenum and ulcer lesions with blood scab;The ulcer shape was round or ellipse.The stage under endoscopy all was A_1,17 patients combined with portal hypertensive gastropathy.35 patients was homeostasis after medical conservative treatment,the effective rate was 83.3%,7 patients needed surgical operation.Conclusion Hepatogenic ulcer hemorrhage is not rare,it is an important factor of upper gastrointestinal bleeding in liver cirrhosis.Emergency endoscopy can make a correct diagnosis in time and guide to choose the best treatment planning.
4.Research on clinical application of navigation bronchoscopy system

Objective:To analyze the clinical application of navigation bronchoscopy system, and improve the accuracy during routine use.Methods:Through discussing ENB, System composition, clinical application and environment, the paper presents the clinical application standard and working environment standard of the navigation bronchoscopy system.Results:Electromagnetic navigation bronchoscopy(ENB) combined with the advantages of electromagnetic technology, virtual bronchoscopy and 3D CT reconstruction, which can perform the accurate localization of peripheral pulmonary lesion and mediastinal lymph node to acquire tissue samples for pathological diagnosis. ENB-guided localization can be used in interventional therapy(local injection or radioactive seed implantation).Conclusion:Navigation bronchoscopy system shows the merit of minimally invasive technique and medical visualization. It is a great progress in interventional pulmonology history.
5.Chinese medicinal compound Yinzhiku significantly inhibits hepatitis virus

Objective: To evaluate the anti-hepatitis effect of Yinzhiku Compound and obtain pharmacodynamic evidence for its clinical application.Methods: Using the model of HepG2.2.15 cells,we determined the inhibitory effects of the drug extract with water and that with alcohol on HbsAg and HbeAg from the HepG2.2.15 cells by serum pharmacological methods.Results: The inhibition of the two Abstracts on HbsAg reached the peak on the sixth day and gradually dropped afterwards,but still at a high level on the twelfth day.Their inhibitory effect on HbeAg,too,attained the highest level on the sixth day,but did not fall obviously afterwards and remained high even on the twelfth day.Conclusion: Yinzhiku Compound has a high inhibitory effect on both HbsAg and HbeAg,with no evident difference between the drug extract with water and that with alcohol.
6.The inhibitory effect of CCR5Delta32 protein on cell surface expression of the HIV-1 coreceptor CCR5 and CXCR4

Objective: To demonstrate that expression of the CCRSDelta32 protein in PBMCs able to down-regulate surface expression of the HIV-1 coreceptor CCR5 and CXCR4.Methods:CCR5Delta32 gene was amplified from human peripheral blood mononuclear cells(PBMCs)genomic DNA by using PCR, and then cloned into lentiviral vector pLenti6/V5-D-TOPO.Recombinant lentiviral particles were produced by packaging using 293T cells.Human PBMCs were transfected with the constructed recombinant lentiviral particles and the expression of CCR5Delta32 was detected by Western blot.The level of CCR5 and CXCR4 expression on transfected PBMCs was detected by FACS analysis.Results: The recombinant lentiviral vector pLenti-CCR5Delta32 was constructed successfully, and the target protein was expressed in PBMCs.FACS analysis showed that CCR5Delta32 protein expressed in PBMCs was able to down-regulate cell surface expression of CCR5 and CXCR4.Conclusion: This study is expected to be used for the gene therapy on AIDS, which deserves further study.

10.Ion-sensitive nanoemulsion-in situ gel system for ophthalmic delivery of flurbiprofen axetil.

The aim of the study is to prepare flurbiprofen axetil nanoemulsion-in situ gel system (FBA/NE-ISG) and observe its ocular pharmacokinetics, rheological behavior, TEM images, irritation and cornea retention. Production of nanoemulsion was based on high-speed shear and homogenization process, and then mixed with gellan gum to prepare FBA/NE-ISG. Rheological study showed that FBA/NE-ISG possesses strong gelation capacity and its viscosity and elastic modulus increases by 2 Pa*s and 5 Pa respectively when mixed with artificial tear at the ratio of 40 : 7. TEM images suggested no significant changes in particle morphology of the pre and post gelation. Good ocular compatibility of FBA/NE-ISG was testified by the irritation test based on histological examination. In vivo fluorescence imaging system was applied to investigate the characteristics of cornea retention, and the results indicated that the nanoemulsion-in situ gel (NE-ISG) prolonged the cornea retention time significantly since K(NE-ISG) (0.008 5 min(-1) was much lower compared with flurbiprofen sodium eye drops (FB-Na, 0.03% w/v) of which the K(Eye drops) was 0.105 2 min(-1), indicated that the cornea retention time of NE-ISG was prolonged significantly. Pharmacokinetics of FBA/NE-ISG in rabbit aqueous humor was studied by cornea puncture, the MRT (12.3 h) and AUC(0-12h) (126.8 microg x min x mL(-1)) of FBA/NE-ISG was 2.7 and 2.9 times higher than that of the flurbiprofen sodium eye drops respectively, which meant that the ocular bioavailability was improved greatly by the novel preparation. Therefore, FBA/NE-ISG can enhance the ocular bioavailability by prolonging drug corneal retention significantly. What's more, encapsulated by emulsion droplets prodrug flurbiprofen (FBA) instead of flurbiprofen (FB) can reduce the ocular irritation.
