1.Clinical study of chronic renal failure with syndrome of dampness-heat due to spleen deficiency treated with Jianpi Qinghua Recipe

OBJECTIVE: To evaluate the effects of Jianpi Qinghua Recipe (JPQHR) on chronic renal failure with syndrome of dampness-heat due to spleen deficiency and its mechanism. METHODS: One hundred and six patients were randomly divided into two groups: control group (53 patients treated with routine therapy) and JPQHR-treated group (53 patients treated with routine therapy and JPQHR). RESULTS: Routine therapy combined with JPQHR could improve symptoms. The serum creatinine, blood urea nitrogen, triglyceride and low density lipoprotein decreased significantly in the JPQHR-treated group as compared with those in the control group (P<0.05), while the number of erythrocyte rosette-forming cells increased significantly in the JPQHR-treated group as compared with those in the control group (P<0.05). CONCLUSION: Routine therapy combined with JPQHR can significantly decrease blood lipid level and strengthen cellular immune system, so it can lower the urine protein in the early and middle stages of chronic renal failure and improve renal function.

6.Extraction and purifiction process of total flavonoid compounds in Scutellaria baicalensis

AIM: To determine the suitable condition and the optimal parameters for the extraction and purifiction process of total flavonoid compounds in Scutellaria baicalensis. METHODS: It was investigated by orthogonal experiment design of form L_9(3~4) with the extraction percentage of the total flavonoid compounds and baicalin from Scutellaria baicalensis as the standard. RESULTS: The optimal condition of extraction was with water as much as ten times of the medicine amount and was boiled for 2 h and three times,and the optimal condition of purifiction was as follows: solution concentration 0.25 g(crude medicinal materials) /mL,adjusting pH to 2,80 ℃ heat preservation up to 60 min, 50% alcohol as precipitant and the precipitated adjusted pH to 4.0. CONCLUSIONS: The optimal process is rational,practical and adapted to industry production.
7.Diagnosis and treatment of mass-type chronic pancreatitis

Objective To summarize the experience of the diagnosis and treatment of mass-type chronic pancreatitis(MTCP) to improve the knowledge about MTCP. Methods The clinical data of tweent-five patients with MTCP undergoing operation in recent 8 years were analyzed retrospectively. The positive discovery rates with computed tomography(CT),ultrasonography(US),endoscopic retrograde cholangiopancreatography(ERCP),and endoscopic ultrasonography(EUS) were 95.7%,85.7%,83.3%,and 100%,respectively. All patients received operation,including pancreatoduodenectomy(15 cases),pancreatoduodenectomy with reservation of pylorus(2 cases),Beger′s procedure(1 case),Frey′s procedure(3 cases),and resection of pancreatic body and tail(4 cases). Results Upper quadrant abdominal pain is the main symptom of MTCP. Pancreatic masses were located in the head of pancreas in 21 cases(84.0%),in the body or tail in four cases(16.0%). 16 patients were diagnosed as MTCP and 9 diagnosed as pancreatic adenocarcinoma before operation. 22 patients were diagnosed as MTCP and 3 were diagnosed as chronic pancreatitis complicated with adenocarcinoma by pathology after operation.Preoperative misdiagnosis rate was 24.0%. Pain relieved immediately after operation in 92.0% of patients. One patients developed pancreatic leakage and two had anastomotic hemorrhage postoperatively,morbidity rate was 12.0%. 23 patients were followed-up for 1-5 years (mean 3.4 years). The therapeutic outcome was satisfactory in 82.6% of patients. Conclusions Operation should be performed as early as possible when the MTCP is diagnosed. It is effective to delay the progress of the disease.

Male breast cancer (MBC)is one of rare malignant carcinomas,of which the pathogenesis and biological characteristics remain elusive.Recently,it has been reported that breast cancer susceptibility gene (BRCA)1 ,BRCA2,checkpoint kinase 2 (CHEK2)and partner and localizer of BRCA2 (PALB2)play important roles in the pathogenesis of MBC.Whereas the relationship between BRCA1 -interacting protein 1 (BRIP1 )and MBC is still controversial.
9.Peri-operative nursing of infants with cavernous transformation of the portal vein and gastrointestinal bleeding undergoing distal splenorenal shunt

Objective To summarize the nursing of infants with cavernous transformation of the portal vein (CTPV) and gastrointestinal bleeding undergoing distal splenorenal shunt.Methods 3 cases of infants with CTPV and gastrointestinal bleeding were given with preoperative nursing for upper gastrointestinal bleeding,strengthening nutritional support; strengthening prevention measures of thrombosis and bleeding complications,monitoring of urine and urine gravity,completing the artery piezometer tube care,paying attention to the discharge guidance and follow-up work.Results The platelet increased in 3 cases post-operation.1 cases occurred abdominal bleeding,but improved after symptomatic treatment.1 case occurred leakage on the site of venous indwelling needle during intravenous infusion of high nutrient solution.3 cases of children received 3 months to 3 years follow-up after discharge,no complications such as liver,kidney and blood clots occurred.Conclusions Infants with CTPV and gastrointestinal bleeding undergoing distal splenorenal shunt is very rare in clinic.Good perioperative care and strengthening the observation of the postoperative complications,attaching great importance to the discharge guidance and follow-up work,are the assurances of successful treatment.
