1.Study on application of ultrasound in the diagnosis of gallbladder stone comparing with surgery

The ultrasound for early, right local diagnosis helps convenient operation, improves the treatment quality and reduces the mortality rate. Study on 97 patients with operation for the gallbladder stone in the central army hospital No 108 from Jan. 1993 to June 1994 received an operation by Kontrol. The corrects of ultrasound before operation for the gallbladder stone, OMC stone, inner hepatic stone and biliary ascariasis are 95%; 97.94%; 87.63%-93.84%; 92.78%-94.85%, respectively. The ultrasound only has determined dilation of bile-duct. The diagnosis of narrow of bile-duct, especially inner hepatic bile-duct remainly are limited by ultrasound.

2.The surgery for gallstone in the elderly.

In a consecutive series of 860 patients undergoing surgery for gallstones, 214 patients (24.9%) were aged over 65 years, of whom 93 (43.5%) were males and 121 (56.5%) were female. Choledochotomy and removes stones with T tube drainge: 54 (25%), choledochotomy and removed stones with cholecystectomy: 34 (15.9%), open cholecystectomy: 40 (18.7%), laparoscopic cholecystomy; 42 (19.6%), cholecystostomy: 12 (5.6%), open biliary surgery with other surgery: 32 (15%). Laparoscopic cholecystectomy offers advantages over the open technique by reducing wound infections as well as the other complications; a shorter hospital stay, a rapid return to full activities.

3.Clinical features and the surgical treatment of bile stone at hospital No 7 in HaiDuong

Study 759 patients who had gallstone surgery from May 1993 to May 2003. Result: gallstone is common. There is an increase of patients day by day. Female: 25,31% is more than male: 47,69%, the age from 21 to 83. Clinical signals are almost charcot symtom in 80%, relevant with the case of infected gall, OMC calculaus combining with calculaus in lung with high percentage (50,59%). Diagnostic ultrasonic results in high accuracy 89,33% which is convenient, simple, harmless, and quick. Treatment applied is surgery withdrawing kehr calculaus

4.Trans-umbilical three-dimensional single-incision laparoscopic cholecystectomy: report of two cases.

OBJECTIVETo explore the feasibility of single-incision laparoscopic cholecystectomy using a three-dimensional (3D) laparoscopic system.
METHODSTwo patients with benign gallbladder disease with a history of recurrent abdominal pain were selected to undergo the surgery. Gallstones were diagnosed by B ultrasound examination. All the operations were performed through the umbilical incision with the 3D laparoscopic system.
RESULTSThe 2 operations were completed successfully with a operative time of 35 min and 50 min. Both of the patients were ambulatory 8 h after the surgery, began to have a normal diet 1 day after operation, and were discharged 2 days postoperatively without any clinical symptoms.
CONCLUSIONS3D single-incision laparoscopic operation can well reveal the tissue anatomy in the operative field especially for some important structures such as the Calot's triangle with an improved safety compared to conventional laparoscopic operations.

6.Three-dimensional laparoscopic cholecystectomy: a case report and literature review.

We report a case of gallbladder stone receiving three-dimensional (3D) laparoscopic cholecystectomy, which allowed 3D visualization of the laparoscopic operative field and faithfully displayed the 3D anatomic structures of the abdominal organs and the gallbladder triangle. The operation was successfully completed in 32 min without intraoperative complications. 3D laparoscopic surgery allows more precise operation with reduced complications and helps to shorten the operative time, and is suitable for more complex laparoscopic surgery.

8.Consensus on the surgical management of benign gallbladder diseases(2021 edition).

Benign gallbladder diseases are common in surgery department,and the incidence rate is increasing in recent years.Currently,nonstandard treatment existed in the surgical management of benign gallbladder diseases in China.Based on relevant domestic and foreign literature,guidelines,and expert consensus,this consensus expounds on the diagnosis and surgical treatment of common benign gallbladder diseases such as gallstone,cholecystitis,gallbladder polypoid,gallbladder adenomyosis,gallbladder variation and deformity,complications after cholecystectomy.Further,recommendations related to diagnosis and treatment were presented to improve the standardization of surgical diagnosis and treatment of benign gallbladder diseases in China,so as to eliminate the clinical harm of gallbladder diseases and reduce the incidence rate of gallbladder cancer and prevent the occurrence.

9.Laparoscopic Common Bile Duct Exploration for Treatment of Common Bile Duct Stones:Clinical Analysis of 158 Cases.

Objective To evaluate the safety and effectiveness of laparoscopic common bile duct exploration in the treatment of common bile duct stones. Methods A retrospective analysis was conducted for 158 patients with cholecystolithiasis and choledocholithiasis admitted to the Number One Hospital of Zhangjiakou from January 2015 to December 2019.The patients were assigned into three groups according to the diameters of cystic duct and common bile duct,degrees of abdominal infection and tissue edema,and operation method.Group A(16 cases):laparoscopic cholecystectomy,transcystic choledochoscopic exploration for stone removal;Group B(94 cases):laparoscopic cholecystectomy,common bile duct incision exploration combined with choledochoscopy for stone removal,T tube drainage;Group C(48 cases):laparoscopic cholecystectomy,common bile duct incision exploration combined with choledochoscopy for stone removal,primary closure of the common bile duct.The operation time,residual rate of stones,and complication(bleeding,bile leakage,and wound infection) rate were compared between groups. Results The operation time of groups A,B,and C was(95.1±14.7),(102.2±18.1),(110.1±16.4) minutes,respectively,which showed no statistical difference between each other(F=0.020,P=0.887).One case in group A had residual stones,while no residual stone appeared in groups B and C.The overall stone clearance rate was 99.4% and the overall complication rate was 1.9%.There was no perioperative death. Conclusion It is generally safe and effective to carry out laparoscopic cholecystectomy and common bile duct exploration for stone removal in suitable populations.
