2.Study for Addictive Effect of Glycopyrrolate (Robinul) and Neostigmine .

The purpose of this study was to observe the additive effect of glycopyrrolate and neostigmine on the heart rate in healthy young volunteer. The authors administered glycopyrrolate and neostigmine intravenously, and compared its effect on the heart rate in both drugs. The results were as follows: 1) A small dose of glycopyrrolate(0.1mg) showed a slight decrease in the heart rate. 2) Neostigmine(0.5mg) showed a significant decrease in the heart rate. 3)glycopyrrolate(0.1mg) 10minutes after glycopyrrolate(0.1mg), showed a marked increase in the heart rate. 4) Neostigmine(0.5mg), 10 min. after glycopyrrolate(0.1mg), showed a significant decrease in the heart rate. 5) glycopyrrolate(0.1mg), 10 min. after neostigmine(0.5mg), showed a decrease in the heart rate. 6) The above results suggest that the site of action of glycopyrrolate is at variance with that of neostigmine.
3.Analysis of the Clinicopathologic Characteristics of Men with Prostate Cancer Undergoing Radical Prostatectomy in the Prostate-Specific Antigen Range of Less than 4 ng/ml.

PURPOSE: We analyzed the clinicopathologic characteristics of men undergoing radical prostatectomy in the prostate-specific antigen (PSA) range of less than 4 ng/ml and compared this with the results for men who had a PSA range of 4 to 9.9 ng/ml. MATERIALS AND METHODS: The study population consisted of 447 men treated for prostate cancer with radical prostatectomy between 1990 and 2006 at our institute who had a prebiopsy PSA of less than 10 ng/ml. The average follow-up period was 37 months. Clinicopathologic characteristics were compared between men with a PSA value of less than 4 ng/ml (low-PSA group) and men with a value in the range of 4 to 9.9 ng/ml (intermediate-PSA group). Survival analysis was performed by the Kaplan-Meier method and Cox proportional hazard regression analysis. RESULTS: Of these 447 patients, 60 (13.4%) and 387 (86.6%) had a low or an intermediate prebiopsy PSA level, respectively. The pathologic findings of the prostatectomy specimens showed no significant differences between the 2 groups, including Gleason score and pathologic stage. The 5-year biochemical recurrence-free survival in the low- and intermediate-PSA groups was 82.8% and 79.3%, respectively, and there was no significant difference between the 2 groups (p=0.946). Multivariate analysis showed that, in the entire cohort, pathologic Gleason score and lymph node involvement were independent predictors of biochemical recurrence. CONCLUSIONS: No statistically significant differences were found in clinicopathologic characteristics or clinical outcome between the low- and intermediate-PSA groups. These results suggest that a lower PSA cutoff should be considered as an indication for prostate biopsy in the Korean population.

Out of all oral malignant tumor, malignant lymphoma occurs in only 3.5%. Especially, most of the primary malignant lymphomas, which occur in the head & neck region are high-grade diffuse large B-cell lymphoma and mucosa-associated lymphoid tissue (MALT) marginal zone B-cell lymphoma is very rare. In the head & neck region, malignant lymphoma is reported to occur in the thyroid, salivary gland, trachea, larynx, orbital lobe and the Waldeyer's ring. Among the Waldeyer's ring, palatal tonsil is reported to be the most common region, but, only 1 case report was published in Korea. Until now, there were no case reports of MALT lymphoma that occurred in the tongue. The purpose of this case report is to report and discuss on a case of MALT lymphoma of the tongue.

Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a side effect of Bisphosphonates (BPs) use. These days, oral bisphosphonates are normally prescribed to treat osteoporosis. Intravenous BPs are used extensively to treat osteolytic bone lesions related to multiplemyeloma and bone metastasis of solid cancers, breast cancer or prostate cancer. As the prescription of BPs is universalized and the number of people treated with BPs is increasing, an accurate understanding and proper management of BRONJ are required. The aim of this study was to improve the clinicians'understanding of BRONJ by reviewing the literature. To achieve this, this paper introduces case reports as well as the current concept of BRONJ based on the 2009 updates by American Association of Oral and Maxillofacial (AAOMS) including the definition, epidemiology, etiology, diagnosis, treatment and prevention of BRONJ.

Cemento-osseous dysplasias are a group of disorders known to originate from periodontal ligament tissues and involve, essentially, the same pathological process. They are usually classified, depending on their extent and radiographic appearances, into three main groups. Periapical cemental dysplasia predominantly involves the periapical region of the anterior mandible. Florid cemento-osseous dysplasia presents with multifocal lesions in the tooth bearing or edentulous areas of the maxilla and mandible, often occurring bilaterally with symmetric involvement. Focal cemento-osseous dysplasia exhibits a single site of involvement in any tooth bearing or edentulous area of the mandible or maxilla, with the posterior mandible representing the most common site. We report the clinical, radiographic and histological findings of 3 types of cemento-osseous dysplasia with a review of literatures.

9.Prevalence and clinical significance of incidental 18F-fluoro-2-deoxyglucose uptake in prostate.

PURPOSE: To investigate the prevalence and clinical significance of incidental prostate fluoro-2-deoxyglucose (FDG) uptake and to evaluate its impact on patient management. MATERIALS AND METHODS: Of 47,109 men who underwent FDG positron emission tomography between 2004 and 2014, 1,335 (2.83%) demonstrated incidental FDG uptake in the prostate, with 99 of the latter undergoing prostate biopsy. The primary end point was the histological presence of prostate adenocarcinoma in the biopsy specimen. Outcomes, including treatment methods, survival, and causes of death, were also assessed. Factors associated with the diagnosis of prostate cancer were evaluated by using logistic regression analysis. RESULTS: Patients with prostate cancer were more likely to have higher serum prostate-specific antigen (PSA) (p=0.001) and focal FDG uptake (p=0.036) than were those without. Prostate cancer occurred in 1 of 26 patients (3.8%) with serum PSA<2.5 ng/mL, compared with 40 of 67 patients (59.7%) with serum PSA> or =2.5 ng/mL. Multivariable analysis showed that focal lesions (odds ratio [OR], 5.50; p=0.038), age (OR, 1.06; p=0.031), and serum PSA (OR, 1.28; p=0.001) were independent predictors of prostate cancer diagnosis. Most patients with prostate cancer had organ-confined tumors. Of these, 12 (29.3%) underwent radical prostatectomy and 25 (60.9%) received hormone therapy. Of the 11 patients who died, 9 died of primary cancer progression, with only 1 patient dying from prostate cancer. CONCLUSIONS: The prevalence of incidental FDG uptake in the prostate was not high, although patients with elevated serum PSA had a higher incidence of prostate cancer. Patients with FDG uptake in the prostate should be secondarily evaluated by measuring serum PSA, with those having high serum PSA undergoing prostate biopsy.

10.Mechanisms of Motility Change on Trinitrobenzenesulfonic Acid-Induced Colonic Inflammation in Mice.

Ulcerative colitis is an inflammatory bowel disease (IBD) characterized by recurrent episodes of colonic inflammation and tissue degeneration in human or animal models. The contractile force generated by the smooth muscle is significantly attenuated, resulting in altered motility leading to diarrhea or constipation in IBD. The aim of this study is to clarify the altered contractility of circular and longitudinal smooth muscle layers in proximal colon of trinitrobenzen sulfonic acid (TNBS)-induced colitis mouse. Colitis was induced by direct injection of TNBS (120 mg/kg, 50% ethanol) in proximal colon of ICR mouse using a 30 G needle anesthetized with ketamin (50 mg/kg), whereas animals in the control group were injected of 50% ethanol alone. In TNBS-induced colitis, the wall of the proximal colon is diffusely thickened with loss of haustration, and showed mucosal and mucular edema with inflammatory infiltration. The colonic inflammation is significantly induced the reduction of colonic contractile activity including spontaneous contractile activity, depolarization-induced contractility, and muscarinic acetylcholine receptor-mediated contractile response in circular muscle layer compared to the longitudinal muscle layer. The inward rectification of currents, especially, important to Ca2+ and Na+ influx-induced depolarization and contraction, was markedly reduced in the TNBS-induced colitis compared to the control. The muscarinic acetylcholine-mediated contractile responses were significantly attenuated in the circular and longitudinal smooth muscle strips induced by the reduction of membrane expression of canonical transient receptor potential (TRPC) channel isoforms from the proximal colon of the TNBS-induced colitis mouse than the control.
