1.Impact of Anger Expression Style and Occupational Stress on the Risk of Cardiovascular Disease in Middle-aged Male Workers

Purpose:
This study aimed to investigate the factors affecting cardiovascular disease in middle-aged male workers.
Methods:
The participants were 137 male office workers over 40 years old and under 60 years old working in small- and medium-sized workplaces from three southern provinces of Korea. Data were collected through self-reported questionnaires and health screening reports from the 2014 National Health Examination. The influencing factors included general characteristics of participants, anger expression style, and occupational stress. Data were analyzed using t-test, ANOVA, and regression analysis with SPSS 22.0.
Results:
The mean score of risk of cardiovascular disease was 6.73±4.69 and there were significant differences in exercise (t=2.13, p=.035) working time (t=-2.15, p=.034). Logistic regression analysis showed that, when adjusted for exercise and working time, the probability of becoming under a cardiovascular disease risk was 21% higher for those who anger-in (OR=1.21, 95% CI=1.02~1.44, p=.027) and 12% higher for those perceived occupational stress (OR= 1.12, 95% CI=1.01~1.24, p=.030).
Conclusion
The results suggest developing the strategies for middle-aged male workers to encourage exercise and to decrease occupational stress, as well as an appropriate anger expression style to improve holistic aspect of health considering their demographic characteristics.
2.Factors Affecting Fall-Prevention Behavior of Long-Term Care Nurses

Purpose:
The purpose of this study was to identify factors influencing fall-prevention behaviors of nurses working in long-term care hospitals.
Methods:
Participants included 147 nurses working in 10 long-term care hospitals in B city. Data were collected from September 20-October 12, 2016. SPSS/WIN 21.0 was used for analysis with t-test, ANOVA, Scheffé ́ test, Pearson correlation coefficients, and multiple regression.
Results:
It was found that attitude toward fall (r=.29, p<.001) and patient safety culture (r=.25, p=.002) had a significant positive correlation with fall-prevention behaviors of nurses working in long-term care hospitals. The factors influencing fall-prevention behaviors in participants were clinical career and patient safety culture (β=.21, p=.012), contributing to 19% of the total variance in fall- prevention behaviors.
Conclusion
The findings showed that systematic delivery of differentiated fall prevention education is preferred to nurse's clinical career as a private factor to improve fall-prevention behaviors of nurses in long term care hospital. Particularly, it is imperative to conduct periodical and practical fall-prevention education for nurses to prevent career discontinuity. An independent report system and open communication system as well as a scheme that can disseminate patient safety culture in individual departments to implement patient direct nursing are required to encourage patient safety culture in organizations.
3.Clinical and Phylogenetic Characteristics of Escherichia coli Urinary Tract Infections.

PURPOSE: We aimed to investigate the clinical and phylogenetic characteristics of Escherichia coli Urinary Tract Infections (E. coli UTI). METHODS: We enrolled patients with culture-proven E. coli UTI, who were admitted at the study hospital from September 2008 to August 2009. We investigated clinical data of patients with E. coli UTI and characteristics of isolated E. coli strains. The phylogenetic groups were classified using triplex polymerase chain reaction (PCR), and the distribution of nine virulent genes was determined by multiplex PCR. RESULTS: A total of 47 patients have participated in this study. Thirty (63.8%) were under 6 months; eight (17.0%) were between 6-12 months; and nine (19.1%) were over 12 months. We compared two age groups between under 6-month and over 6-month. In the age group under 6-month, higher proportion of male (P=0.002) and group B2 strains (P=0.020) were observed. In contrast, higher proportion of female and group non-B2 strains were observed in age group over 6-month. Frequencies of papC, papGII, papGIII, sfa/foc, hlyC, cnf1, fyuA, iroN and iucC were estimated as 68.1%, 57.4%, 42.6%, 46.8%, 46.8%, 31.9%, 87.2%, 48.9% and 63.8%, respectively. In the comparison of phylogenetic groups, group B2 showed higher distribution of virulent genes, while group D included more strains resistant to trimethoprim/sulfamethoxazole (TMP/SMZ) than other groups. CONCLUSION: We showed the age group-specific difference in the distribution of sex ratios and phylogenetic groups; more male and group B2 strains in age group under 6-month, while more female and group non-B2 in age group over 6-month. However, further evaluation including larger number of patients will be necessary to confirm above thesis in future molecular epidemiological studies.

4.Association of ADRβ2 Haplotypes with Coronary Artery Disease in Korean Patients.

BACKGROUND: Beta₂-adrenergic receptor (ADRβ2) gene variations seem to be correlated with disease progression, prognosis, and drug response to β-blockers in cardiovascular events. In this study, we investigated the genotypes and haplotypes of ADRβ2 in Korean patients and analysed their association with coronary artery disease (CAD). METHODS: One hundred five patients diagnosed with stable angina (SA), 109 patients with acute coronary syndrome (ACS), and 88 controls were enrolled. Five single nucleotide polymorphisms (SNPs) were determined at positions 46, 79, 252, 491, and 523 nucleotides, using the polymerase chain reaction and direct sequencing analysis. The haplotype reconstruction was carried out using genotype data, and analyses of the association between the genetic variation and CAD were performed. RESULTS: There were significant differences in the allele frequencies for the 79CG SNPs among the three groups. Relative to the control group, the distribution of 79CG genotypes was significantly different in both the SA group (P=0.0003) and the ACS group (P=0.0056). Compared with the CC genotype of 79CG, subjects with CG or GG had a higher risk of CAD (adjusted odds ratio [OR], 12.851; P=0.014). The frequencies of specific ACGCA, GCACC, and GGGCC haplotypes were 6.4% vs. 0%, 8.3% vs. 0%, and 6.9% vs. 0.6%, respectively, in the ACS group and controls. The GGGCC haplotype was significantly associated with CAD (adjusted OR, 12.266; P=0.016). CONCLUSIONS: Although there are large ethnic differences in the distribution of ADRβ2 SNPs and their association with CAD, the 79G polymorphism and GGGCC haplotype in ADRβ2 might specifically contribute to CAD pathogenesis in Korean patients.

It shows variable clinical findings ranging from a solitary papule or nodule to multiple lesions withlinear or diffuse distribution. Typically, it occurs as a solitary nodule on the extremities of elderly people. Histopathologic findings usually show proliferation of anastomosing cords and strands of cuboidal epithelial cells embedded in fibrovascular stroma. We report an uncommon case of solitary eccrine syringofibroadenoma with plasma cell infiltration on the dorsum of the foot of a 53-year-old man.

7.Scleral Attachment of Extraocular Muscle using a Stitch Combined with Tissue Adhesive in Rabbits.

In experimental sutureless muscle surgery with tissue adhesives, current materials have inadequate adhesive strength in the early postoperative period.We performed a modified muscle surgery using a stitch combined with Tisseel (Immuno AG, Vienna, Austria)in order to evaluate the adhesive strengh as a replacement for conventional suture techniques in muscle surgery. Thirty eyes of 15 rabbits were used.We recessed superior and inferior rectus muscles by three different methods, Tisseel only, a stitch with Tisseel, and a stitch only, in each group of 5 rabbits.Conjunctival incisions were closed with the remaining Tisseel in 7 rabbits, and with vicryl in 8 rabbits. Tensile strength of the scleral reattachment site was measured and conjunctival closure was examined by slit-lamp biomicroscopy, at intervals of 1 hour, 12 hours, 1 day, 3 days and 5 days. The strengths of a stitch with Tisseel at each interval were 83.25 +/-1 4 .0 6 gm, 137.50 +/-22.88 gm, 169.75 +/-23.95 gm, 151.50 +/-41.99 gm, and 265.50 +/- 25.01 gm, respectively.The strengths of Tisseel alone were 61.50 +/-2 0 .2 1 gm, 101.50 +/-11.00 gm, 113.25 +/-28.69 gm, 120.50 +/-18.36 gm, and 222.75 +/- 57.67 gm.They were 52.25 +/-24.85 gm, 89.50 +/-16.05 gm, 130.75 +/-21.98 gm, 1 5 3 .7 5 +/-30.35 gm, and 261.50 +/-60.47 gm at each interval in rabbits with a stitch alone. These results showed that the tensile strengths of a stitch with Tisseel were stronger than those of the other two methods up to 1 day after surgery.Even at postoperative 12 hours, the strength was more than 130 gm which was strong enough for scleral attachment.All of the conjunctival closures with Tisseel were well maintained without any complication up to 5 days, the same as vicryl sutures. In conclusion, this study revealed that muscle attachment to the sclera by a stitch with Tisseel was simpler and safer than classical sutures with reduced risk of sclera perforation.Its strong tensile strength at early postoperative days suggests that the method might be considered as an alternative method to classical reattachment using suture material and also that conjunctiva could be closed with Tisseel remnant.

8.A Case of Atypical Lymphocytic Lobular Panniculitis.

We report here on a 63-year-old woman who presented with recurrent bruise-like infiltrative plaques on the left leg without systemic symptoms or laboratory abnormalities. The histopathologic findings showed an infiltration of panniculus by small to medium-sized atypical lymphocytes. But the prominent lymphoid atypia, fat necrosis, vascular thrombosis, erythrophagocytosis and striking dominance of CD8+ lymphocytes seen in subcutaneous panniculitis-like T-cell lymphoma (SPTCL) were not detected. The diagnosis of atypical lymphocytic lobular panniculitis (ALLP) was made based on these histopathologic and clinical features. The lesions responded well to systemic steroid treatment. But the CD4/CD8 ratio in the later lesion was more decreased than that of the initial lesion, and the patient is under clinical follow up.

9.The Expressions of TGF-beta1 and IL-10 in Cultured Fibroblasts after ALA-IPL Photodynamic Treatment.

BACKGROUND: Topical photodynamic therapy (PDT) with 5-aminolevulinic acid (ALA) was originally used for treating superficial skin tumors. The application of PDT to other inflammatory dermatoses like acne vulgaris, psoriasis, granuloma annulare, localized scleroderma and lichen sclerosus has recently been introduced. However, the underlying mechanisms are not well understood. We've previously reported the induction of tumor growth factor (TGF)-beta1 and interleukin (IL)-10 after PDT with ALA and intense pulsed light (IPL) in cultured HaCaT cells. OBJECTIVE: The purpose of this study was to investigate the expressions of TGF-beta1 and IL-10 in cultured fibroblasts after PDT with using ALA and IPL. METHODS: Cultured fibroblasts were treated with ALA-IPL PDT (1micromol/L of ALA; 0, 4, 8 and 12 J/cm2 of IPL). The expressions of TGF-beta1 and IL-10 were investigated by reverse transcription-polymerase chain reaction and enzyme linked immunosorbent assay. RESULTS: TGF-beta1 mRNA and protein were reduced down to 0.52- and 0.63-fold, respectively, after PDT and the IL-10 protein was increased up to 2.74-fold after PDT. CONCLUSION: The reduction of TGF-beta1 was prominent after PDT and so an antisclerotic effect can be expected after PDT. The induction of IL-10 may contribute to the anti-inflammatory effect, which explains the therapeutic benefit of PDT for inflammatory dermatoses.

10.A Case of Familial Anterior Cervical Hypertrichosis.

Anterior cervical hypertrichosis (ACH) refers to a patch of hair that is presented at the lower anterior aspect of the neck just above the sternal notch along the cervical midline. The condition occurs as an isolated defect or in association with other abnormalities. Eighteen out of the 28 ACH patients who were previously reported on in the English literature showed a familial inheritance pattern. Some of the reported cases were associated with other clinical symptoms such as peripheral sensory and motor neuropathy, developmental delay, retinal change and bony abnormality. Herein, we report on a case of 2 sisters and 1 brother who had familial anterior cervical hypertrichosis without any other abnormality.
