1.Person-centered Care and Nursing Service Quality of Nurses in Long-term Care Hospitals.

PURPOSE: This study investigated the correlation between person-centered care (PCC) and nursing service quality of nurses in long-term care hospitals. METHODS: The subjects were 114 nurses working in 8 long-term care hospitals. Instruments for evaluating PCC and nursing service quality were used. The data were analyzed by descriptive statistics, two samples-test, one-way ANOVA, Pearson's correlation and Multiple regression. RESULTS: The mean of PCC was 3.25±0.45 out of 5 and the nursing service quality was 3.87±0.40. There were significant differences in PCC in terms of age and income satisfaction, the application of their opinions, the satisfaction of hospital managers, administrators and nurse managers. There were significant differences in nursing service quality according to age, position, the satisfaction of hospital managers, administrators and nurse managers. Nurses' PCC showed a significant positive correlation with nursing service quality. Factors influencing nursing service quality included PCC, their position and age and the most influencing one was PCC. CONCLUSION: This study suggests that the PCC is the strongest affecting element to the quality of nursing service in long-term care hospitals. Therefore, the strategies to improve the practice of person-centered care should be carried out to enhance the quality of nursing service.

2.Perception and Practice of Hospital Infection Control of Nursing Staff depending on the Supplementation of Nurses in Long-term Care Hospitals.

PURPOSE: The purpose of this study was to describe the perception and practice of hospital infection control of nursing staff in long-term care hospitals by the level of supplementation of nurses. METHODS: The participants were 212 nurses and nurse assistants in 13 long-term care hospitals in a metropolitan city and the data were gathered by self-reported questionnaires during August 2011 and analyzed by SPSS/WIN program. RESULTS: The beds per a nurse were 15, and the proportion of nurses among nursing staff in long-term care hospitals was about 33%. In general, the level of infection control in practice was lower than that of perception. The highest perception and practice domain was 'Management of disinfection/contamination', and the lower level domains were 'Personal hygiene' and 'Hand washing' There were statistically significant differences in the hospital infection control of perception and practice depending on age, education, career in long-term care hospital, job position, the quantity of beds, nurse, and nurse assistant, beds per a nurse and proportion of nurses in hospitals. CONCLUSION: According to these results, the systematic and continual education on hospital infection control of the nursing staff in long-term hospitals should be carried out. In addition, the policy to add more nurses into long-term care hospitals must be implemented.

3.A survey of the prevalence and classification of cerebrovascular diseases in Ganzi Tibetan Autonomous Prefecture,Sichuan Province

Objective To investigate the stroke classification of different age groups in Ganzi Tibetan Autonomous Prefecture,Sichuan Province in order to provide a basis for targeted prevention and treatment of stroke in the area. Methods From October 2011 to June 2014,a cross-sectional survey was used for cluster sampling and stratified sampling in 10000 residents,farmers,and herdsmen in Dege,Ganzi,Litang, Batang,and Kangding counties,Ganzi Tibetan Autonomous Prefecture,Sichuan Province. The questionnaires were used to collect demographic characteristics,stroke,hypertension,and other information. A total of 9186 effective questionnaire responders were enrolled. According to their ages,they were divided into five groups (18-29,-39,-49,-59,and ≥60 years). The included respondents in each group were 2492,2356, 2161,943,and 1234,respectively. Strokes were classified as transient ischemic attack,cerebral infarction, cerebral hemorrhage, and subarachnoid hemorrhage groups. The differences in the prevalence among the groups used chi-square test. Results In 9186 subjects, 4584 ( 49. 9%) were females and 4602(50. 1%) were males. There were 174 patients with stroke,the prevalence was 1. 89%. The incidences of stroke in 5 age groups from small to large were 0. 08%% (2/2492),0. 68% (16/2356),1. 39% (30/2161),2. 65% (25/943),and 8. 18% (101/1234),respectively. There were significant differences in the incidence of cerebrovascular diseases in different age groups (χ2 =693. 72,P<0. 01). The proportions of cerebral infarction,intracerebral hemorrhage,transient ischemic attack,and subarachnoid hemorrhage in the classification of stroke were 44.2% (n=77),43.7% (n=76),7.5% (n=13),and 4.6% (n=8),respectively. The prevalence of hypertension in the above stroke types were 77. 9% (60/77),94. 7%(72/76),7/13 and 5/8,respectively. Conclusions The prevalence of cerebrovascular diseases is high in Ganzi Tibetan Autonomous Prefecture,Sichuan Province,and hemorrhagic stroke is significantly higher, the incidence of hypertension is the highest,which suggests that hypertension is an important risk factor for high incidence of hemorrhagic stroke in the local area. It is necessary to do a good job of health education on stroke according to the characteristics of local stroke
4.Pneumomediastinum secondary to diabetes ketoacidosis in high altitude: a case report

Diabetic ketoacidosis (DKA), a serious acute complication of diabetes mellitus, mainly manifests as hyperglycemia, ketosis, and acidosis. It is a metabolic syndrome resulting from insulin deficiency and increased insulin-antagonistic hormone levels. While type 2 diabetes mellitus complicated by DKA is relatively uncommon, secondary pneumomediastinum in DKA is extremely rare. Following alveolar rupture, air can travel through various routes to reach the hilum, causing anterior, middle, or posterior pneumomediastinum or even leading to intracranial epidural pneumatosis. The diagnosis of pneumomediastinum is mainly dependent on chest computed tomography findings. After the successful treatment of DKA, pneumomediastinum usually resolves spontaneously within 5-10 days with a good prognosis. One DKA patient admitted to Dege County People's Hospital developed Kussmaul respirations, followed by an increase in intra-alveolar pressure, an elevation in intra and extra-alveolar pressure difference, and protein decomposition in the alveolus wall, which promoted alveolar rupture and induced mediastinal emphysema. After rapid fluid replacement, blood glucose control with insulin, and maintenance of acid-base balance (correction DKA), the mediastinal emphysema was spontaneously absorbed. Through the analysis of the clinical data of this case, the purpose is to improve the clinicians' internal understanding of the relationship between mediastinal emphysema and DKA, avoid over-examination and over-treatment, and provide strategies for correct diagnosis and treatment.
5.Mapping of pathogenic genes in a pedigree with autosomal dominant ichthyosis vulgaris.

OBJECTIVETo elucidate the pathogenic genes in a pedigree with autosomal dominant ichthyosis vulgaris (IV).
METHODSLinkage analysis was performed by using STR markers in chromosome 1, and mutation detection was used to screen for FLG gene mutation.
RESULTSA maximum two-point Lod score of 3.46 (theta=0) was obtained at D1S2696. Haplotype analysis placed the critical region in a 15-CM interval defined by D1S2726 and D1S305, but no mutation of FLG was found in our IV patients.
CONCLUSIONThe pathologic gene of the IV family locates near D1S2696, and the FLG gene may not ruled out from the pathologic genes.

6.Establishment and clinical application value of an automatic diagnosis platform for rectal cancer T-staging based on a deep neural network.

BACKGROUND:
Colorectal cancer is harmful to the patient's life. The treatment of patients is determined by accurate preoperative staging. Magnetic resonance imaging (MRI) played an important role in the preoperative examination of patients with rectal cancer, and artificial intelligence (AI) in the learning of images made significant achievements in recent years. Introducing AI into MRI recognition, a stable platform for image recognition and judgment can be established in a short period. This study aimed to establish an automatic diagnostic platform for predicting preoperative T staging of rectal cancer through a deep neural network.
METHODS:
A total of 183 rectal cancer patients' data were collected retrospectively as research objects. Faster region-based convolutional neural networks (Faster R-CNN) were used to build the platform. And the platform was evaluated according to the receiver operating characteristic (ROC) curve.
RESULTS:
An automatic diagnosis platform for T staging of rectal cancer was established through the study of MRI. The areas under the ROC curve (AUC) were 0.99 in the horizontal plane, 0.97 in the sagittal plane, and 0.98 in the coronal plane. In the horizontal plane, the AUC of T1 stage was 1, AUC of T2 stage was 1, AUC of T3 stage was 1, AUC of T4 stage was 1. In the coronal plane, AUC of T1 stage was 0.96, AUC of T2 stage was 0.97, AUC of T3 stage was 0.97, AUC of T4 stage was 0.97. In the sagittal plane, AUC of T1 stage was 0.95, AUC of T2 stage was 0.99, AUC of T3 stage was 0.96, and AUC of T4 stage was 1.00.
CONCLUSION:
Faster R-CNN AI might be an effective and objective method to build the platform for predicting rectal cancer T-staging.

7.Interferon-γ and interleukin-4 detected in serum and saliva from patients with oral lichen planus.

Our previous salivary study had demonstrated an apparent T helper 2 (Th2)-predominance in saliva of oral lichen planus (OLP) patients and suggested a potential of salivary interleukin-4 (IL-4) as a biomarker for monitoring disease severity. To further determine the consistency of Th1/Th2 bias of OLP, this study investigated the expression profile of interferon-γ (IFN-γ) and IL-4 in serum and the relationship of the serum levels of these cytokines with their saliva partners. Sixty ethnic Chinese patients with OLP (40 of the erythematous/ulcerative form and 20 of the reticular form) were recruited for this study, with 40 age-sex-matched healthy volunteers as control group. IFN-γ and IL-4 levels in serum and paired saliva samples were screened by enzyme-linked immunosorbent assay. OLP patient showed a low-level IFN-γ but high-level IL-4 expression profile in both serum and saliva, with a lower IFN-γ/IL-4 ratio. Serum IL-4 level in the erythematous/ulcerative group was significantly higher than that in the reticular group. Serum levels of IFN-γ and IL-4 were significantly and positively correlated with their saliva partners. These results provided more evidence for Th2 cytokine-predominant immune imbalance in OLP, as well as the potential of IL-4 as the biomarker for monitoring severity of OLP.

9.Quality evaluation of Huocao based on UPLC fingerprint and multi-component content determination.

Huocao(a traditional Chinese herbal medicine) moxibustion is a characteristic technology in Yi medicine suitable for cold-dampness diseases. Huocao, as the moxibustion material, is confusedly used in clinical practice and little is known about its quality control. In this study, UPLC method was used to establish the chemical fingerprint of non-volatile components in Huocao, and the contents of eight phenolic acids such as chlorogenic acid were determined. Multivariate statistical analysis was performed to obtain the indicator components of Huocao for quality evaluation, and thus a comprehensive evaluation system for the quality of Huocao was built. The UPLC fingerprints of 49 batches of Huocao were established, and there were 20 common peaks, of which eight phenolic acids including neochlorogenic acid and chlorogenic acid were identified. Except for three batches of Huocao, the similarity of the other 46 batches was higher than 0.89, suggesting that the established fingerprint method could be used for quality control of the medicinal herb. The correlation coefficient between entropy weight score of the eight phenolic acids and comprehensive fingerprint score in Huocao was 0.875(P<0.01), which indicated that the eight phenolic acids could be used as indicator components for the quality evaluation of Huocao. Furthermore, in multivariate statistical analysis on the common peaks of fingerprint and the contents of the eight phenolic acids, chlorogenic acid, isochlorogenic acid A and isochlorogenic acid C were screened to be the indicator components. The results revealed that the proposed method achieved a simple and accurate quality control of Huocao based on UPLC fingerprint and multi-component content determination, which provided useful data for establishing the quality standard of Huocao.
