1.Application of the permeability surfaces of CT perfusion in early period treatment with chemotherapy in advanced non-small cell lung cancer

Objective To investigate the change ofpermeability surfaces (PS) of CTperfusion(CT-p) before chemotherapy and early period treatment with chemotherapy (the first cycle of chemotherapy) in advanced non-small cell lung cancer and explore the correlation with the efficacy of chemotherapy.Methods Eighty patients with advanced non-small cell lung cancer which confirmed by pathology were taken standardized treatment by first-line drug.All patients performed with the CT-p technique before chemotherapy and one cycle after chemotherapy.The efficacy was evaluated by RECIST after 4 cycles of chemotherapy,the patients were divided into complete remission (CR) + partial response (PR),stable (SD),development (PD) according to the results and the change of PS was compared among these groups before chemotherapy and after 1 cycle of chemotherapy.Results Seventy-four patients' values of PS were useful.The remission rate and the stability rate was 32.4% (24/74) and 78.4% (58/74) respectively after 4 cycles of chemotherapy.PS after 1 cycle of chemotherapy in CR +PR (24 cases) and SD (34 cases) was lower than that before chemotherapy (12.38 ± 1.86 vs.18.13 ± 2.45,13.24 ± 1.97 vs.17.93 ± 2.38),and there was significant difference (P <0.05).There was no obvious change in PS before chemotherapy and after 1 cycle of chemotherapy in PD (16 cases),and there was no significant difference(P> 0.05).Conclusions The value of PS which reflects the change of perfusion in tumor vessel at early chemotherapy and the prognosis of the non-small cell lung cancer.Use the change of PS to predict the prognosis of patients with chemotherapy.It could be the norm of calculating the curative effect of chemotherapy in non-small cell lung cancer.
2.Research advances in diagnosis and treatment of hepatocellular carcinoma complicated by arterioportal shunt

Hepatocellular carcinoma (HCC) complicated by arterioportal shunt (APS) is commonly seen in clinical practice,with an incidence rate of 28.8%-63.2%.It is manifested as abdominal pain,diarrhea,and ascites and can also lead to serious complications of portal hypertension including gastrointestinal bleeding.Although there are various therapeutic methods,they tend to have poor clinical effects.APS is one of the most important causes of death in patients with HCC.This article introduces the etiology,typing,clinical manifestation,and therapies of HCC complicated by APS and points out that although there are various therapeutic methods for HCC complicated by APS,interventional treatment remains the most important method.The exploration of interventional treatment helps to improve patients' prognosis.
3.Mutual promotion, mutual assistance and mutual inhibition in the compatibility of acupoints.

Mutual promotion, mutual assistance and mutual inhibition are commonly for the compatibility of Chinese herbs, but they are also existed among the acupoints. In the paper, the relevant literature on acupoint, compatibility was collected and the mutual promotion, mutual assistance and mutual inhibition relationships of acupoints were described. The laws of the above mentioned combinations, the theoretic evidences and modern researches were summarized briefly. The mutual promotion and mutual assistance refer to the coordination of acupoints, presenting the relationship of the primary and the secondary about the mutual assistance. Hereby, the acupoints could be divided into the simple assistant acupoints and the acupoints from the affected meridians. The mutual inhibition is used to describe the antagonism effect in acupoint combination. There are more researches on the mutual promotion of acupoint combination but less ones on the mutual inhibition and its mechanism is just hypothetic. It is very important to have the deep study on those compatibility relationships for the improvement of acupuncture efficacy and to elaborate the effect mechanism.

4.Assessment of relationship among color Doppler flow pattern,vascular distribution and lymph nodes metastasis in breast carcinomas

Objective To study the association among the color Doppler flow pattern, vascular distribution and the lymph nodes metastasis in breast carcinomas. Methods Sixty-eight patients with infiltrating mammary gland duct carcinoma were divided into the no-lymph-nodes-metastasis group (group I , 37 cases) and the lymph-nodes-metastasis group (group Ⅱ , 31 cases), group Ⅱ were subdivided into group Ⅱa (19 cases): lateral lymph nodes positive of pectoralis minor and group Ⅱ b (12 cases): rear of pectoralis minor and subclavian lymph node positive. Blood flow parameters [peak systoltc velocity (PSV),resistent index ( RI)] and vascular Alder grades were observed and measured with color Doppler ultrasound.The results were compared with the incidence of lymph node metastasis. Results PSV and RI were not significantly different between group Ⅰ and group Ⅱ (t = -0.19,-0.63,P> 0.05),and also between group Ⅱ a and group Ⅱ b (t =-1.12,-013,P >0.05). By using vascular Alder grades,there were no significant differences between group Ⅰ and group Ⅱ (x2= 0.46, P> 0.05 ), but the significant differences were observed between group Ⅱ a [10.5%(2/19) ,63.2%( 12/19) ,26.3%(5/19) in Alder Ⅰ , Ⅱ , Ⅲ grade] and group Ⅱ b [8.3%( 1/12), 16.7%(2/12),75.0%(9/12)in Alder Ⅰ , Ⅱ ,Ⅲ grade](x2 = 7.41,P<0.05). Conclusion The pattern of blood flow has no correlation with lymph nodes metastasis, vascular Alder grades of carcinoma shows some relevance with the lymph nodes metastasis in the patients with infiltrating mammary gland duct carcinoma.
5.Intraoral approach for surgical treatment of mandibular cyst in the angle and ramus

Objective:To compare the effect of intraoral and extraoral approach for the sugical treatment of mandibular cysts in the angle and ramus. Methods:Cysts in the mandibular angle and ramus were treated by intraoral approach in 57 cases and extraoral in 18 cases. The following data were collected: operation time, peroperative blood loss, postoperative infection, mouth opening, recurrence rate, injury of the submandibular branch of the facial nerve and sensory function of the inferior alveolar nerve. The data of the two groups were compared statistically. Results:Average operation time (min) of intraoral and extraoral appoarch was 84.65?5.16 and 122.78?10.84(P0.05). Conclusion:Intraoral approach is effective in the treatment of cyst in mandibular angle and ramus.
6.Attention should be paid to hereditary ovarian cancer

The occurrence of 20 to 25 percent of ovarian cancer is associated with genetic factors.Hereditary ovarian cancer includes genetic ovarian cancer and familial ovarian cancer.The genetic characteristics of tumors and the development of gene testing technology make it possible to detect tumor-related mutational genes by testing the gene of the proband and thus identify the other family members at risk and intervene early so as to realize early prevention and detection of the cancer.The paper reviews the genetic basis and pathogenesis of hereditary ovarian cancer as well as methods of detecting mutational genes, management of the proband, problems at present, and disease prevention for high-risk individuals.It stresses that we should pay attention to hereditary ovarian cancer in clinical work, identify the proband, and make comprehensive evaluation of risks of the patients` relatives so as to provide individualized guidance and carry out the concept of precision medicine.
7.TCM Name Combing and Specification Suggestions in Rumex

OBJECTIVE:To provide reference for standardizing the names of TCM from Rumex. METHODS:According to lit-erature research,the most frequently used names (such as Jinbuhuan,Tudahuang,Yangti) of TCM in Rumex were combed,and suggestions for existing problems were put forward. RESULTS & CONCLUSIONS:The TCM names were used confusedly,main-ly showing that inconsistent names of TCM from the same plant in different areas,same name of TCM from different plants in the same area,same name of TCM from different plants in different areas. The key to solving the confused use of names of TCM in Rumex is standardization and unification of medicinal plant source and names. It can be developed and solved in order by resource investigation and identification,establishing quality standard and excavating medicinal value,in which,conducting the resource in-vestigation and specie identification,determining the medicinal names,stipulating the official names and establishing the quality standard are the important work in future.
8.Analysis and thinking of the test papers of the rehabilitation medicine of for five-year program clinical medical students

The score of the final test papers of rehabilitation medicine among the fiveyear prgram clinical students of Grade 2005 in our university were analyzed,which showed that the test paper had the degree of difficulty and discrimination was good,which reflected the students’real level.And at the same time we found that students could not to make good use of learning to analyze and work out a solution. Therefore,we should pay attention to the combination between teaching and clinical practice,increase student’chance of learning on the job, and properly append such subjects as application, synthetic and evaluation in the test papers.
9.Analysis of predisposing factors of pharyngo-cutaneous fistulas after laryngeal cancer surgery

OBJECTIVE To explore the predisposing factors of pharyngo-cutaneous fistulas after surgical treatment of laryngeal cancer. METHODS A retrospective study of 757 patients who underwent laryngectomy during a 10-year period was carried out. RESULTS The occurrence rate of post-laryngectomy pharyngo-cutaneous fistula was 4.2 %. Single-factor analysis showed that parameters related to the occurrence rate of post-laryngectomy pharyngo-cutaneous fistula included clinical stage of tumor, appearance of post-operative infection, undergoing of pre-operative radiotherapy, and pre-operative nutrition states. CONCLUSION Pharyngo-cutaneous fistulas appears more frequently in patients with T3 and T4 tumor and who underwent pre-operative radiotherapy. Skillful operative technique and effective prevention of post-operative infection may contribute toward reducing the incidence of pharyngo-cutaneous fistulas after surgical treatment of laryngeal cancer.
10.Clinical Efficacy of Laparoscopy Combined with Colonoscopy in the Treatment of Small Colorectal Space Occupying Lesions Cao

Objective To evaluate the clinical efficacy of laparoscopy combined with colonoscopy for small colorectal space occupying lesions (equal or less than 3 cm). Methods From January 2010 to January 2015,a total of 41 cases of small colorectal space occupying lesions (equal or less than 3 cm)were treated under general anesthesia with laparoscopic surgery combined with colonoscopy.The patients were placed at the lithotomy position.After the establishment of pneumoperitoneum,injection of methylene blue for staining was carried out under colonoscopy. Laparoscopic titanium clipping positioning was conducted, and then the colonoscope was withdrawn.According to the intraoperative frozen pathological results,bowel resection surgery or colorectal surgical resection was selected. Results All the 41 cases of laparoscopic combined with colonoscopic surgery were successfully completed, with no conversion to laparotomy.There were 9 cases of preoperative diagnosis of precancerous lesions,6 of which were found neoplasia in the colon epithelium with intraoperative pathological diagnosis,with 3 cases of Tis adenocarcinoma.Among 32 cases of preoperative diagnosis of stage 0 -Ⅰ colorectal cancer,there were 29 cases of intraoperative and postoperative diagnosis of stage Ⅰ (23 cases of T1 N 0 M0 adenocarcinoma and 6 cases of T2 N 0 M0 adenocarcinoma)and 3 cases of stage Ⅲ (T2 N 1 M0 adenocarcinoma).Two cases were multiple lesions.Bowel resection was performed in colon intraepithelial neoplasia and Tis lesions of colorectal cancer,while resection of colorectal cancer was conducted in stage T1 -T2 colorectal cancer.Two patients with colonic epithelial neoplasia were followed up for 9 and 12 months,without recurrence.The remaining 39 cases were followed up for 24 -49 months,with a median of 38.6 months. Among the 35 cases of colorectal cancer,colonoscopy detected local recurrence in 1 case of T2 N 1 M0 adenocarcinoma (stage Ⅲ)after 34 postoperative months.No metastasis was found in all the patients. Conclusion Laparoscopy combined with colonoscopy for small diameter (equal or less than 3 cm)benign tumor or stage Tis -T2 malignant colorectal cancer has advantages of both endoscopy,especially suitable for difficultly located or removed lesions,which can improve the positioning precision and surgical safety.
