1.Application of evidence-based laboratory medicine in quality control of biochemical tests in Chinese peacekeeping secondary hospital of United Nations Interim Force in Lebanon

Objective To seek a operation method of quality control of biochemical tests in peacekeeping mission via perspective of evidence-based laboratory medicine .Methods Literatures related to quality control of biochemical tests in international peace-keeping were referred ,compared and discriminated .Combined with 5-year peacekeeping laboratory experience ,a set of feasible ap-proaches were worked out and a practice evaluation was performed in a task period .Results Under the supervision of the quality control system ,providing accurate and reliable test results to the clinical ,developing and implementing quality control of biochemical tests were economical ,practical and reliable ,and could guarantee the effectiveness of reports of biochemical test in peacekeeping medical institutions .Conclusion Evidence-based laboratory medicine is important for quality control of biochemical tests in peace-keeping medical institutions .
2.Progress in evidence-based medicine for the treatment of optic neuritis

Optic neuritis is a common optic neuropathy,which typically presents with sudden loss of vision.Visual deficit of optic neuritis varies in severity and generally can be improved spontaneously over several-month duration.The most convincing eight evidences from level Ⅰ and Ⅱ evidence of evidence-based medicine( EBM ) about the treatment of all the optic neuritis were collected by searching theCochrane Controlled TrialsRegister (CENTRAL),MEDLINE,EMBASE and reference lists of identified trial reports since the last decade.At present,the latest findings showed that there is no conclusive evidence of benefit in terms of recovery to normal visual acuity,visual field or contrast sensitivity with either intravenous or oral corticosteroids at the doses evaluated in trials included in this review.Either no treatment or intravenous corticosteroid therapy followed by oral corticosteroids is appropriate,and intravenous corticosteroid may benefit the patient in terms of faster recovery to normal vision.There may be of different treatment of optic neuritis in China compared to Western countries due to geographical,racial and genetic background that may cause different etiology and clinical features of optic neuritis.Therefore,in China,it is imperative to carry out a large randomized control trial(RCT) of optic neuritis as soon as possible to get the best evidence for the management of optic neuritis patients.
3. Adjuvant electrical stimulation for patients with dysphagia during the acute phase of ischemic stroke

Objective: To evaluate the efficacy of adjuvant electrical stimulation for patients with dysphagia during acute phase of ischemic stroke. Methods: Twenty-three patients with acute ischemic stroke complicated with dysphagia were divided into a control group (n = 10) and a treatment group (n = 13). The control group only received conventional treatment, and the treatment group received adjuvant electrical stimulation on the basis of conventional treatment. The efficacy evaluation was conducted by using Watian water swallowing test, visual assessing scale (VAS) score, functional oral intake scale (FOIS), National Institute of Health Stroke Scale (NIHSS) score, and modified Rankin scale (mRS) score. Results: Circled digit oneWatian water swallowing test, VAS score, and NIHSS score were all decreased after the treatment in the two groups. There were statistical significances (P <0.05) ; FOIS score were increased after the treatment in both groups. There was no statistical significance (P <0.05); although the FOIS score was increased in the control group, there was no statistical significance (P >0.05). Circled digit twoAfter comparing the different values of each score before and after the treatment in the treatment group with those in the control group, there were no statistical significances. Circled digit threeThe patients were followed up for mean 8.7 months, the swallowing function almost returned to normal in both groups. There was no statistical significance in FOIS and mRS scores between the treatment group and the control group. Conclusion: The swallowing function of the patients was improved significantly after being treated with the two treatment modalities, however the adjuvant electrical stimulation did not have more benefits than the conventional treatment.
5.Dynamic hip screw hole filled by bone cement: Anti-flexion and anti-torsion strength

OBJECTIVE: To evaluate the clinical outcomes of medical supporting bone graft following posterior approach and bone cement implantation in the hip joint in treating intertrochanteric fracture.METHODS: A computer-based online search of Science Direct, Ei databases was performed for English articles published between January 1960 and October 2009, with the key words "bone cement, intertrochanteric fracture". In addition, CNKI and CBM were searched for related Chinese articles published between January 1994 and October 2009, with the key words "intertrochanteric fracture, coxa vara, posterior approach of hip joint, bone cement implantation in major screw hole". Moreover,related books were manually searched. Treatment of intertrochanteric fracture, basic and clinical experiment of intertrochanteric fracture treated by bone cement was included.RESULTS: Intertrochanteric fracture treatment includes lateral or anterior approach for dynamic hip plate system, dynamic hip screw, and femoral proximal intramedullary screw internal fixation. Dynamic hip screw is standard internal fixation to treat intertrochanteric fracture, with strong anti-rotation function, and meets the biomechanical requirements. However, screw loosening,extraction and breakage frequently occur. Cancellous bone screw track enhanced by bone cement increases screw retention force,benefits screw compression, enhances bone-screw interface to transfer the stress to screw-bone regions, which significantly increases the anti-flexion and anti-torsion strength of dynamic hip screw and improves fracture stability. The mechanism involves the integral formation of cancellous bone, bone cement and screw by the micro-interlocking of bone trabecula and surrounding cancellous bone.CONCLUSION: Dynamic hip screw filled by bone cement significantly enhances the anti-flexion and anti-torsion strength of internal fixation and improves fracture stability.

7.Clinical observation on the effect of resection of eyelid tumor and stageⅠ recovery

AIM: To investigate clinical effects of local resection of eyelid tumors and pathological examination or frozen section examination as well as the stage I recovery.
METHODS: For all 92 cases 92 eyes of eyelid tumor patients who underwent local resection, reconstruction in stage Ⅰ and routine pathological examination ( 37 cases of suspected malignant tumors using frozen section examination. If it was malignant, excision by Moths surgery) . Using free palate mucosa autograft in stage I or reconstruction by Hughes surgery combined with eye flaps repaired eyelid defects.
RESULTS: The postoperative pathological diagnosis:52 cases were benign tumors, and 40 cases malignant tumors. All cases of eye flaps and hard palate mucosa grafts were survived. Followed up for more than 6mo, no tumor were recurred. The appearance and function of eyelid were satisfactory.
CONCLUSION: In this group of cases, nearly half of eyelid tumors are malignant tumors. It should be treated and operated as early as possible. For suspected malignant tumors, we use frozen section examination to confirm its properties in order to excision cleanly in stageⅠ, reducing the recurrence and metastasis. According to the eyelid defect after resection, we could do repair operation in stage Ⅰ. The inner larger defect is repaired by using hard palate mucosa transplantation or Hughes surgery. The outer layer of the eyelid defect is repaired by using eye flap glide, the free flap, the kite flap, the simple suture, etc. It should be careful when using the hard palate mucosa transplantation in the upper eyelid defect.
8.Efficacy of alpha-2 adrenergic agonists for prevention of postoperative cardiac adverse events in patients with coronary disease: a meta-analysis

Objective To evaluate the efficacy of α2 adrenergic agonists for the prevention of postoperative adverse cardiac events in patients with coronary disease.Methods MEDLINE,EMBASE,CINHAL,Web of Science,CBM and CNKI were searched to identify all randomized controlled trials on the efficacy of α2 adrenergic agonists for the prevention of postoperative cardiac complications following operation performed under general anesthesia in patients with coronary artery disease.The incidence of myocardial ischemia,myocardial infarction,bradycardia and hypotension were evaluated.Meta-analysis was conducted using the RevMan 5.1 software.Results Fifteen trials included 9 high-quality trials,4 medium-quality trials and 2 low-quality trials.A total of 3422 patients were included in this meta-analysis.1790 patients received α2 adrenergic agonists (clonidine and dexmedetomidine) and 1632 patients received placebo; 724 patients underwent non-cardiac surgery and 2698 patients underwent cardiac surgery.Meta-analysis indicated that α2 adrenergic agonists reduced incidence of myocardial infarction following non-cardiac surgery and myocardial ischemia following non-cardiac surgery and cardiac surgery.Alpha-2 adrenergic agonists significantly increased the incidence of postoperative bradycardia.Conclusion Alpha-2 adrenergic agonists can decrease postoperative cardiac adverse events in patients with coronary disease,but can induce bradycardia,and the efficacy of clonidine is consistent with that of dexmedetomidine.
9.Surgical treatment for fibrous dysplasia of proximal femur-a partition method

[Objective]To retrospectively analyze 39 cases of surgical treatment of fibrous dysplasia of proximal femur,in order to propose a partition method which can be used to guide the surgical treatment,and probe the surgical treatment strategy according to the partition method of fibrous dysplasia of proximal femur. [Methods]The data on 39 cases of fibrous dysplasia of proximal femoral from 1998 to 2009 were retrospectively analyzed.The study determined extent of the lesion according to preoperative X-ray film and proposed partitioning method in accordance with the scope of proximal femoral involvement,analyzed the relation of partition type and surgical treatment,and evaluated the effect of surgical treatment through the postoperative localized lesions control and functional score(MSTS 93).[Results]According to the scope of 39 cases of lesions,combined with characteristics of proximal femoral anatomy,proximal femur was divided into four zones: area I,femoral shaft(below small protuberance);area II,intertrochanteric(small rotor to the neck base division);area III,femoral neck;area IV,the femoral head.Based on the above partition,lesions were divided into 7 types,type I,type II,type III,type I + II,type II + III,type II + III + IV,type I + II + III,type I + II + III + IV.Surgical treatment was performed mainly with curettage of bone lesions,bone grafting and fixation.Internal fixation included DHS,intramedullary nails,and artificial joint replacement,DHS was most common in area II involvement(57.58%),followed by area III involvement(24.24%).Intramedullary nail was most common in area I involvement(64.70%) and area II involvement(35.30%).For lesions involving ≥3 zones,joint replacement was carried out.The patients were all followed up,with a median time of 6.3 years(0.5-11 years).Local recurrence rate was12.82%(5 / 39),local deformity was not found deterious.The followed up results were satisfactory.The selected treatment was reasonable.[Conclusion]When making a surgical treatment regimen for fibrous dysplasia of the proximal femur,Surgeons should consider lesion characteristics,and choose a reasonable internal fixation to achieve better surgical results.Partition method proposed in this paper can be formulated as one of preoperative surgical treatment options.
10.Effect of Yangfei Huoxue Prescription on ERK1/2 and NF ?-B of Bleomycin-induced Pulmonary Fibrosis in Rats

Objective To study the effect of Yangfei Huoxue Prescription on ERK1/2 and NFK-B of bleomycin-induced pulmonary fibrosis in rats.
