1.Clinical analysis on juvenile dermatomyositis combined with soft-tissue calcification

Objective To analyze the clinical features of juvenile dermatomyositis (JDM) combined with soft-tissue calcification. Methods Forty-seven patients with JDM combined with soft-tissue calcification (soft-tissue calcification group) were retrospectively analyzed, and they were contrasted with 89 patients with non-calcification (non-calcification group). Results The rates of Gotton signe, muscle contracture and joint dysfunction in soft-tissue calcification group were significantly higher than those in non-calcification group:87.23% (41/47) vs. 43.82% (39/89) and 68.09% (32/47) vs. 21.35% (19/89), and there were statistical differences (P<0.05). The dosage of glucocorticoid (conversion of prednisone measuring more than 1.5 mg/kg), rate of using immunodepressant, level of creatine kinase in soft-tissue calcification group were significantly lower than those in non-calcification group:17.02%(8/47) vs. 68.54%(61/89), 25.53%(12/47) vs. 88.76%(79/89), (566.45±240.41) U/L vs. (1 680.12±656.50) U/L, and there were statistical differences (P<0.05). Conclusions The patients with JDM combined with Gotton signe are more prone to soft-tissue calcification. The rate of muscle contracture and joint dysfunction in soft-tissue calcification patients is significantly higher than that in non-calcification patients. For the patients whose creatine kinase are not obviously elevated, they are more prone to soft-tissue calcification. Early active application of glucocorticoid and immunodepressant therapy can reduce or prevent the occurrence or development of late calcium deposition.
2.Effects of metformin on expression of AMP-activated protein kinase in rat glomerular mesangial cells

Objective To observe the effects of metformin on expression of Adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK),nuclear factor-κB (NF-κB) and transforming growth factor β1 (TGF-β1) in cultured rat glomerular mesangial cells (MCs),and explore its renoprotective mechanisms.Methods MCs were cultured in the medium with normal glucose (group NG,5.6 mmol/L),high glucose (group HG,25mmol/L) and different concentrations of metformin (group M1,M2,M3).After 48 h exposure,the supernatants and MCs were collected.The expression of NF-κB and TGF-β1 mRNA was analyzed by real time-PCR.Total-AMPK,phospho-Thr-172 AMPK (p-AMPK),NF -κB p65 and TGF-β1 were visualized by Western blot.Results The real time-PCR and Western blot result showed MCs could express AMPK,NF-κB and TGF-β1 mRNA and protein.After stimulated by HG,the levels of intracellular NF-κB and TGF-β1 expressions were significantly increased compared with group NG (P < 0.05); The levels of NF-κB and TGF-β1 were significantly decreased in group M1,M2 and group M3 compared with group HG in a dose-dependent manner.After stimulated by HG,the level of intracellular p-AMPK were down-regulated compared with group NG(all P < 0.05); The expression of p-AMPK increased with the rising of metformin concentration,presenting the opposite trend (P < 0.05),while the level of total-AMPK protein was unchanged with exposure to HG or different concentrations of mefformin(P > 0.05).Conclusion Metformin can suppress the expression of NF-κB and TGF-β1 of glomerular MCs induced by HG via AMPK activation,which may partly contribute to its reno-protection.
3.Effect of electroacupuncture at different time points on the recovery of gastrointestinal function after surgery for gastrointestinal malignant neoplasms

Objective: To observe the effect of electroacupuncture (EA) at different time points during the perioperative period on the recovery of gastrointestinal function after gastrointestinal malignant neoplasms surgery. Methods: Sixty-three patients who needed radical surgery for gastrointestinal tumors were randomized into a control group, treatment group 1 (postoperative EA group), and treatment group 2 (intraoperative and postoperative EA group). The control group received surgery and conventional Western medicine treatment, and treatment groups 1 and 2 received additional EA treatment at different time points. The initial flatus time after the surgery, visual analog scale (VAS) score at different time points after the surgery, the proportion of using patient-controlled analgesia (PCA) after the surgery, and the times of adding analgesics were observed in the three groups. Results: The initial flatus time after the surgery was earlier in treatment groups 1 and 2 than in the control group (P<0.05); the difference between treatment groups 1 and 2 was statistically insignificant (P>0.05). The VAS score was lower in treatment group 2 than in the control group at 6, 12, 24, and 72 h after the surgery (P<0.05); the VAS score was lower in treatment group 1 than in the control group only at 72 h after the surgery (P<0.05). There were no significant differences in the rate of using PCA among the three groups (P>0.05). Regarding the times of adding analgesics, it was less in treatment group 2 than in the control group at 12 h after the surgery (P<0.05). Conclusion: Either EA during and after the surgery or only after the surgery can hasten the initial flatus and boost the recovery of gastrointestinal function in patients after radical resection of gastrointestinal neoplasms. Successive EA during and after the surgery should be superior to postoperative EA regarding the analgesic effect after the surgery.
4.The relationship between hemoglobin adducts of trinitrotoluene and exposed level.

OBJECTIVETo explore the biomarkers for monitoring trinitrotoluene (TNT) exposure, the relationship between TNT hemoglobin adducts and TNT exposed level.
METHODHemoglobin adducts (4A-Hb and 2A-Hb) were determined by GC-MS in 25 TNT exposed workers. TNT exposed level was evaluated by determining skin contaminated and inhaled TNT levels. The correlation between hemoglobin adducts level and TNT exposed level was analyzed.
RESULTSThere was a correlation between total TNT exposure level, especially skin exposure level, and 4A-Hb or 2A-Hb content. No significant difference was found between the slopes and intercepts of lin ear equation of (4A-Hb) vs TNT exposed level and linear equation of (4A-Hb +2A-Hb) vs TNT exposed level (P > 0.05).
CONCLUSIONSkin contamination is the major role of TNT exposure. TNT exposed level can be evaluated by determining the content of both 4A-Hb and 2A-Hb, and 4A-Hb is more suitable for monitoring TNT exposure.

5. Mechanism of promoted neovascularization by SVF after fat grafting

Stromal vascular fraction(SVF)are the remaining cells after removing mature fat cells in the adipose tissue. Containing a certain amount of adipose derived stem cells(ADSCs), SVF also includes many other cells, which may have the potential of promoting angiogenesis. In this review, the role of SVF in angiogenesis after fat transplantation was summarized by intensive reading relative literature in recent years. The result is that angiogenesis and fat graft revascularization are regulated by various factors: SVF promotes secretion of a diverse array of cytokines and growth which are capable of stabilizing endothelium vascular network. ADSCs have the potential of differentiating into smooth muscle cells and endothelial cells which can coroperate to form new blood vessels.
6.Rapid identification and correction of the interference of cold agglutinins by using RET channel red blood cell parameters

Objective:To assess the role of Delta-RBC parameters in the automated hematocrit analyzer RET channel for the recognition of cold agglutinins (CA) and to explore the value of RET channel optical method parameters in correcting interference with CA.Methods:This is a retrospective study. The Cas group included 68 samples with Cas interference and the control group included 45 samples without CA interference. All specimens were collected from Zhongshan Hospital Fudan University Outpatient Department from January 2022 to January 2023. Specimens in both the CA and Control group were examined using the CBC+RET channel at room temperature. Recorded and calculated the Impedance method test parameters RBC-I, HGB, HCT-I, MCH-I, MCV-I, MCHC-I and the Optical method test parameters RBC-O, HCT-O, MCH-O, R-MFV, MCHC-O, Delta-RBC. Examined the specimens in the CA group using the CBC channel after prewarmed at 37 ℃ for 2 h, and Impedance method parameters RBC-I 37 ℃ 2 h, HGB 37 ℃ 2 h, HCT-I 37 ℃ 2 h, MCH-I 37 ℃ 2 h, MCV-I 37 ℃ 2 h, MCHC-I 37 ℃ 2 h were recorded. The ROC curves were used to analyze the discrimination efficacy of Delta-RBC in identifying CA interference, and the Bland-Altman method was used to analyze the consistency between the results of the optical method RBC parameters at room temperature and the results of the impedance method after prewarming. The correlation analysis was performed using Pearson and Spearman correlation analysis to analyze the results of the RBC parameters before and after prewarming in the CA group. Result:If Delta-RBC was used as diagnostic indicators for the identification of CA interference, the best cut-off value was 1.065, with AUCs of 0.998. In the CA group, the correlation coefficients between RBC-O, HCT-O, R-MFV, MCH-O, MCHC-O, and RBC-I 37 ℃ 2 h, HCT-I 37 ℃ 2 h, MCV-I 37 ℃ 2 h, MCH-I 37 ℃ 2 h, MCHC-I 37 ℃ 2 h were 0.985, 0.981, 0.729, 0.870, and 0.649, respectively. The percentages within the limits of agreement of the percentage differences between the results of the two methods were 95.6%, 92.6%, 95.6%, 94.1%, and 95.6%, respectively. Conclusions:The RBC parameter Delta-RBC from RET channel optical method can be used as an indicator to effectively assist in the clinical determination of the presence of CA. Reporting results using the optical method RBC parameters of the RET channel can correct the interference of CA without specimen pre-treatment and obtain more correct results of complete blood counts.
7.Metformin inhibits nuclear factor-κB activation and inflammatory cytokines expression induced by high glucose via adenosine monophosphate-activated protein kinase activation in rat glomerular mesangial cells in vitro.

BACKGROUNDThe renoprotective mechanisms of adenosine monophosphate (AMP)-activated protein kinase (AMPK) agonist - metformin have not been stated clearly. We hypothesized that metformin may ameliorate inflammation via AMPK interaction with critical inflammatory cytokines. The aim of this study was to observe the effects of metformin on expression of nuclear factor-κB (NF-κB), monocyte chemoattractant protein-1 (MCP-1), intercellular adhesion molecule-1 (ICAM-1) and transforming growth factor-beta 1 (TGF-β1) induced by high glucose (HG) in cultured rat glomerular mesangial cells (MCs).
METHODSMCs were cultured in the medium with normal concentration glucose (group NG, 5.6 mmol/L), high concentration glucose (group HG, 25 mmol/L) and different concentrations of metformin (group M1, M2, M3). After 48-hour exposure, the supernatants and MCs were collected. The expression of NF-κB, MCP-1, ICAM-1, and TGF-β1 mRNA was analyzed by real time polymerase chain reaction. Western blotting was used to detect the expression of AMPK, phospho-Thr-172 AMPK (p-AMPK), NF-κB p65, MCP-1, ICAM-1, and TGF-β1 protein.
RESULTSAfter stimulated by HG, the expression of NF-κB, MCP-1, ICAM-1, TGF-β1 mRNA and protein of MCs in group HG increased significantly compared with group NG (P < 0.05). Both genes and protein expression of NF-κB, MCP-1, ICAM-1, TGF-β1 of MCs induced by high glucose were markedly reduced after metformin treatment in a dose-dependent manner (P < 0.05). The expression of p-AMPK increased with the rising of metformin concentration, presenting the opposite trend, while the level of total-AMPK protein was unchanged with exposure to HG or metformin. Conlusion Metformin can suppress the expression of NF-κB, MCP-1, ICAM-1 and TGF-β1 of glomerular MCs induced by high glucose via AMPK activation, which may partly contribute to its reno-protection.

8.Study on the Effect and Its Mechanism of Bromophenylcurcumin on Apoptosis ,Migration and Invasion of Cho- langiocarcinoma Cells

OBJECTIVE:To study t he effects of bro mophenylcurcumin(GL63)on the apoptosis ,migration and invasion of human cholangiocarcinoma RBE cells ,and to investigate its mechanism based on JAK/STAT signaling pathway. METHODS :MTT assay was used to detect the effects of different concentrations of GL 63 [0(blank control ,similarly hereinafter ),1.25,2.5,5,10, 20,40 μ mol/L] on the proliferation of RBE cells after 48 h treatment ;the IC 50 was calculated. The effects of different concentrations of GL 63(0,5,10,20 μmol/L)on colony formation were detected by crystal violet staining after 48 h treatment. Flow cytometry ,Hoechst 33342 staining,cell scratch test and Transwell chamber invasion test were used to detect the effects of different concentrations of GL 63(0,5,10,20 μmol/L)on cell cycle distribution ,apoptosis,migration and invasion ability after 24 h treatment. Western blotting assay was adopted to detect the effects of different concentration of GL 63(0,5,10,20 μmol/L) on the expression of JAK 2/STAT3 signal pathway associated proteins. RESULTS :The proliferation inhibition rates of RBE cells in different concentrations of GL 63 groups(1.25-40 μmol/L)were significantly increase d,compared with blank control group (P< 0.01),and showed a dose-dependent trend ,with IC 50 of (8.46±1.30)μmol/L. Compared with blank control group, 85917439。E-mail:zhaoji-an-88@163.com inhibition rates of RBE cell colony formation were significantly decreased in different concentrations (5,10,20 μmol/L)of GL 63 groups(P<0.01). The percentage of RBE cells at G 0/G1 phase increased significantly ,while that at S phase decreased significantly (P<0.01). The apoptotic rate increased significantly(P<0.01),and the nucleus showed dense pyknosis and apoptotic bodies. The rate of cell migration and healing was significantly decreased (P<0.01),and the number of invasive cells through basement membrane was significantly decreased (P< 0.01). The protein expression of p-JAK 2, p-STAT3, Bcl-2, MMP-2, MMP-9, Pro-caspase-9 and P ro-caspase-3 were down-regulated significantly while the expression of Bax ,Cyt-c,Cleaved-caspase-9 and Cleaved-caspase- 3 were up-regulated significantly(P<0.01). CONCLUSIONS :GL63 may inhibit the proliferation ,migration and invasion of RBE cells and promote its apoptosis by inhibiting JAK 2/STAT3 signal pathway.
9. Comparison of curriculum system of biomedical science in different universities

Shanghai Jiao Tong University established the first biomedical science in China in 2016. Learning the successful experience from international universities can promote the development of this major. According to the comparison of cultivation program and curriculum system in different top universities at home and abroad, it was showed that Oxford University focused on the pluralism development of students, University of Technology Sydney paid attention to the combination of basis and clinical practice, and the University of Hong Kong stressed the integration of clinical practice, science and research, and transformation. Through the exploration of the details of educational system this university, we found that we mainly focused on five innovation ideas: building the basic knowledge, strengthening comprehensive basic knowledge teaching, stimulating students' interests towards biomedical science, developing their ability and promoting innovative education and training. Based on analysis of those cases, it is showed that the development of biomedical science in China should solve problems in curriculum integration and practical education of science and research, so as to achieve the goal to cultivate more talents in this major and boost the development and advances in the field of biomedical science.
10.Research progress on the study of the relationship between periodontitis and cancers of the digestive system

Periodontitis is closely related to many systemic diseases. Cancer of the digestive system is a common malignant tumor. Increasing evidence has shown that periodontitis is related to various digestive system cancers. This review summarizes the current research on the relationship between periodontitis and esophageal cancer, gastric cancer, and colorectal cancer and analyzes the possible mechanisms, including via microorganisms, immunity, inflammation, and genes. The content of periodontal pathogens and Helicobacter pylori in the mouth of patients with periodontitis is increased, with the secretion of many virulence factors and pathogenic enzymes and inhibition or evasion of the host’s non-specific immune function, making the digestive system organs connected to the oral cavity more vulnerable to cancer cell invasion. The plasma levels of interleukin-1β(IL-1β) , interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α) in patients with periodontitis and digestive system cancers are increased. These elevated factors promote the occurrence and development of cancer by activating endothelial cells, increasing the expression of adhesion molecules and inducing the production of matrix metalloproteinases. Additionally, formyl peptide receptors involved in the inflammatory response and NF-κB, as therapeutic targets of periodontitis, are associated with many cancers, but the mechanism is unclear. Periodontal health is considered a breakthrough point to provide a reference for the prevention and treatment of patients with these three common cancers of the digestive system.
