1.Clinical features of infectious mononucleosis in 30 adult patients

Objective To summarize the clinical features of infectious mononucleosis(IM)in aduIt patients.Methods Thirty cases of adult patients with IM were analyzed retrospectively being inDatient in Huashan Hospital from 2002 to 2007.Results Of these 30 patients with IM,male was slightly more than female with average age of 31.8 years old.Liver function of 77 percent patients were abnormal,while 70%splenomegaly,33%skin rash,20%EB virus-IgM positive and 10%hepatomegaly.Two cases were diagnosed with chronic active EB virus infection(CAEBV).Treatment strategies of IM were antiviral and symptomatically therapy.Conclusions IM diagnosis is difficult due to the variety of clinical features.CAEBV is rare in adult patients.The clinical prognosis of IM is good if treated properly.
2.Hyperbaric oxygenation attenuates hippocampal neuron apoptosis and cognitive impairment in newborn rats with bilirubin encephalopathy

Objective To evaluate the effect of the hyperbaric oxygenation therapy on the apoptosis of the hippocampal neuron and the impairment of cognitive function of the newborn rats suffering from bilirubin encephalopathy. Methods One hundred and forty-five five-day-old SD rats of both sex were randomly divided into 3 groups,control group,bilirubin encephalopathy group and hyperbaric oxygenation treated group. Bilirubin ( 10 mg/ml) was injected into the cerebellomedullary cistern to establish experiment model,and normal sodium was injected for control. The expression of cytochrome C and Caspase-3 in the hippocampus neurons were detected by immunohistochemistry and immunofluorescence,respectively,at the time of 6 h,1,3,5 d and 7 d after intra-cisternal injection. Neuronal apoptosis was checked by TUNEL method. The cognitive function was tested by Morris experiment. Results The expressions of the cytochrome C and Caspase-3 in hyperbaric oxygenation treated group were lower than those in bilirubin encephalopathy group,but higher than control group at abovementioned time points ( P
3.Expression of fragile histidine triad gene in endometrial carcinoma and its significance

0.05).Conclusion The reduced expression of FHIT may be associated with endometrium canceration and may be not associated with surgical pathologic staging,lymph node metastatic and depth of myometrial invasion in endometrial carcinoma.The reduced expression of FHIT indicates the pathogenesis of endometrial carcinoma.

Objective To evaluate the efficacy and safety of interventional occlsu ion operait no by analyizn g thes urgical data of 23 csa es of infants with patent ductusa rteriosus complicated with middle and severe pulmo an ry arterial hypertension.Methods Teh 23 cases of infants wiht patne t ductus arteriosus com-plicated with middlea nd severe pulmonary arterial hypertensionw ere collected in the hospital from January 2011 to December2014 .These infatn s rce eived transcateh ter occlusion with intravenuo s anesthesia after the preoperative examination.The operation procse s included:right ventriculography and pulmonary atr ery pressure tested,then lateral angiogar phy of descending aorta was performed to observe the type and size of patent ud ctus atr eriosus and measure ascending aorta,descending aortic pressure,and recorded the pressure re-spectively.1 ml blood sample of ascending aorta,pulmonary artery and inef rior vena vein respectively was used for gas analysis.All these data was used to calculate pulmonary vascular resistance.After tried to plug-ging effectiveyl we can release the occluder.In the postoperative 24 h,1 month,3 months,the infants should be measured with Doppler echocardiography,chest X ray and electrocardiogram examination.Results The clinical symptoms disappeared and the short-term follow-up was not associated with the complications of interventional therapy.Th e comparison of the pressure changes before and after the operation were performed as following, aortic per ssure decreased [ preoperation ( 68.3 ±17.5 )/( 21.4 ±3.7 ) mmHg, postoperation (52.4 ±8.7)/(15.6 ±3.5) mmHg,1 mmHg=0.133 kPa],ascending aorta pressure increased(preoperation (83.5 ±5.9)/(51.3 ±3.6) mmHg,postoperation(88.2 ±5.1)/(52.4 ±2.7) mmHg),and descending aorta pressure increased ( preoperation ( 81.4 ±3.3 )/( 48.2 ±2.7 ) mmHg, postoperation ( 86.5 ±4.7 )/(51.5 ±3.2) mmHg), the differences were statistically significant before and after surgery ( t =5.455/3.945 ,P<0.01;t=-2.696/-1.193 , P<0.05; t=-4.167/-3.745 , P<0.01 ) .Conclusion Under conditions of mastering the appropriate operation time and strengthening the management of the perioperative management,transcatheter measurement is safe and effective for infants with patent ductus arteriosus compli-cated with middle and severe pulmonary arterial hypertension.
6.Impact of systematic retroperitoneal lymphadenectomy during primary debulking surgery on prognosis in patients with advanced ovarian cancer

Objective To explore the impact of systematic retroperitoneal lymphadenectomy (SL) in patients with advanced ovarian cancer. Methods The data on 188 patients with FIGO stageⅢC epithelial ovarian cancer were retrospectively analyzed. All the patients underwent primary debulking surgery and then received platinum-based chemotherapy. The patients were divided into SL group and non-systematic lymphadenectomy (USL) group. Progression free survival (PFS) and overall survival (OS) times were analyzed. Results PFS and OS were higher in SL group than in USL group (36 versus 17 months and P = 0.022; 58 versus 52 months and P = 0.041). Univariate analysis showed that both SL and optimal debulking surgery were the independent prognostic factors , but multivariate analysis indicated that optimal debulking surgery was the only independent prognostic factor. However , for 143 patients without enlarged retroperitoneal lymph nodes , multivariate analysis showed that either SL or optimal debulking surgery were the independent prognostic factor (P = 0.040 and P = 0.014). Conclusions Optimal debulking surgery is helpful in improvement of the survival in patients with advanced ovarian cancer. For patients without enlarged retroperitoneal lymph nodes, SL still has a positive effect on the prognosis.
7.The effects of passive and active movements of foot and ankle on the venous return in the lower limb

Objective To explore the effects of different types of passive and active movements of foot and ankle on the venous return in the lower limb in order to provide evidence for prevention of venous stasis and deep- vein thrombosis(DVT)after joint replacement. Methods Peak and average velocities of the femoral vein were detected and measured by using color ultrasound Doppler on 30 limbs in 15 healthy volunteers under the circumstance of resting and different types of passive and active movements of foot and ankle. Results During the passive movements of the flexion and extension, inversion and eversion, and the circumduction of foot and ankle, the flexion and extension as well as the inversion and eversion only slightly increased the velocities of venous blood flow. The circumduction produced a higher increase in venous blood flow than the former two movements, increasing the peak velocity by 31.3% and average velocity by 33.9% . In the same 3 types of the active movements of foot and ankle, the active flexion and extension, inversion and eversion resulted in increases in peak velocity of 41.3% and 32.9% and in average velocity of 40.1% and 32.4% , respectively. The active circumduction of foot and ankle produced the strongest influence on the promotion of venous blood flow with the peak velocity increased by 69.3% and average velocity by 69.1% . Conclusion The active circumduction, flexion and extension, inversion and eversion, and the passive circumduction of foot and ankle can be utilized to prevent DVT after joint replacement. The active circumduction of foot and ankle is the optimal exercise to prevent DVT.
8.Methylprednisolone's influence on the plasma plasminogen activator inhibitor-1 in lupus nephritis patients

Objective:To investigate the effect of methylprednisolone that decrease plasminogen activator inhibitor-1(PAI-1) in plasma through mediating the PA/plasmin system.Methods:Employed immunohistochemical technique-ELISA to determine PAI-1 level in plasma.Results:①PAI-1 in LN patients is significantly higher than that in the control group (P0.05).Conclusion:It is thus proved that methylprednisolone is capable of decreasing PAI-1 in plasma through mediating the PA/plasmin system and two shocks is the minimum for effective cure. 9.Clinical Study on Shuanghuanglian Injection in Treating Uterine Tube Obstruct

Objective To investigate the effect of Shuanghuanglian injection parafornix-injecting in treating uterine tube obstruct (UTO) and explore its mechanism. Methods Clinic study included 386 uterine tubes of 200 patients, which were randomly divided into treatment group (50 cases, treated with Shuanghuanglian injection parafornix-injecting) and three control groups - group one (50 cases, treated with Lincocin vial parafornix-injecting), group two (50 cases, treated with hydrotubation) and group three (50 cases, treated with transvaginal catheter dilation). The changes of UTO condition and blood rheology, IL-2, IgA, IgG and IgM were observed before and after treatment. Results The total effective rate of Shuanghuanglian injection in treating UTO was 97.76%. It was characterized by regulating the blood stasis and hyperviscosity, improving the immune function of human body. Conclusions Shuanghuanglian injection parafornix-injecting has obvious therapeutic effect on UTO. The effect may be related with rectifying the body pyrotoxin and blood stasis, improving pelvic blood circulation and immune function.
10.Off-pump total arterial revasculization by bilateral IMA

Objective To evaluate the early outcome of off-pump total arterial revascularization using bilateral IMA with "Y" manner. Methods Between Oct.2002 to Aug.2004, 52 patients underwent off-pump coronary artery bypass grafting (OPCABG) using bi-IMA. The mean age was 56 (33-78) years. Triple vessel disease was in 42 (80%) patients. The free right IMA was anastomosed to the in-sitr left IMA, composed “Y” graft. LIMA anastomosed sequentially to the diagonal branch and LAD and RIMA were sequentially to the ramus, obtuse marginal and PDA. Results There was no 30-day death. 171 grafts were constructed and the mean number of grafts per patient was 3.3. Peri-operation MI occurred in 2 patients, temporary IABP in one. There were no sternal wound infection and strode. Conclusion Off-pump total arterial revascularization using bilateral IMA with "Y" manner is secure and feasible.
