Objective: To provide a scientific method for the reasonable use of the drugs,and to use it as guidelines for clinical chemotherapy. Methods: Samples of test group were obtained from 38 cases of bladder cancer.which were cultivated with different medicines and then examined the inhibiting rate with MTT.The medicine with the highest inhibiting rate was selected to treat bladder cancer.30 cases of bladder cancer were treated with HCPT routinely as control group.The rate of tumor recurrence between two groups was compared. Results:The inhibiting rate on tumour cells was positively related to the concentration of drugs,the combined chemotherapy was superior to single drug.Statistic results showed significant difference between the test group and control group(P
2.Long-term efficacy observation of Topiramate in treatment of epilepsy

Objective To evaluate long-term efficacy and safety of Topiramate in patients with various types of epilepsy.Methods 115 patients with epilepsy were underwent open-label experience with Topiramate as monotherapy or add-on therapy. The efficacy was assessed after 6 months. The rates of seizure-free and Topiramate retention were estimated after 1 and 2 years, and side effect were also evaluated during open-label treatment period.Results (1) After 6 months, total effective rate was 83.9% in monotherapy group and 66.1% in add-on therapy group. Topiramate was proved to be effective on various types of epilepsy, but there were not significant differences in seizure-free rate and total effective rate between the different types. (2) The optimal dosage was ( 105.72?48.28) mg/d in children and ( 176.26?62.81) mg/d in adults. (3) The seizure-free rate was 40.0% at 1 year and 28.7% at 2 years, and retention rate was 67.8% at 1 year and 46.1% at 2 years. (4) Side effects were observed in 34 patients (29.6%), including mild to moderate events related to central nervous system, anorexia and weight decrease.Conclusion Topiramate is a widespread antiepileptic drug with high efficacy and safety for long-term administration.

Objective To study the changes of plasma renin activity(PRA) and angiotensin Ⅱ(Ang Ⅱ) concentration in patients with acute cerebral infarction(ACI) as well as their clinical significance.Methods Radioimmunoassay was used to detect PRA and Ang Ⅱ concentration in 55 patients with ACI in the acute,convalescent phrase,and carried out correlative analysis with focal size and the degree of neurological deficit.Results (1) PRA and Ang Ⅱ in 3d of the acute phase of ACI were significantly higher than those in the controls ( P 3.0 cm 2) and the focus of middle infarct group(1.5 to 3.0 cm 2),but PRA and Ang Ⅱ in both groups were significantly higher than those in lacunar infarction group(
4.Clinical analysis of 30 patients with sporadic Creutzfeldt-Jakob disease

Objective To investigate the features of clinical,electroencephalogram(EEG)and imaging in the patients with sporadic Creutzfeldt-Jakob disease(sCJD).Methods The clinical data of 30 patients with sCJD were analyzed retrospectively.Results 6 patients were acute onset,18 patients were subacute onset,which followed by conscious disturbance,myoclonus and decorticate rigidity in 1~3 months.6 patients were chronic onset,which followed by above described typical symptoms in 1~2 years.All the patients showed generally persistent slow waves on EEG in the early stage.18 patients showed typical triphasic wave on EEG and 8 patients presented atypical triphasic wave,which were almost in the intermediate-advandced stage.There were 10 patients in bilateral basal ganglia,and 1 patient in right lenticular nucleus showed hypersignal in MRI T2WI.17 patients carryed out MR diffusion-weighted images(DWI).All the images showed the hypersignal in frontal,temporal,occipital cortex,and 8 patients of them accompanied symmetric hypersignal in bilateral basal ganglia.Conclusions Almost of sCJDs are subacute onset.DWI show typical hypersignal lesions in occipital,frontal,temporal cortex and /or basal ganglia in their early course of disease,which are useful in early diagnosis of CJD.All the patients in the intermediate-advandced stage shows conscious disturbance,myoclonus and decorticate rigidity.The character of EEG is typical triphasic wave.

Objective To explore the effects of Irbesartan on the plasma concentrations of lysophosphatidic acid(LPA), endothelin-1(ET-1),serum concentration of nitric oxcide (NO) in the patients with acute cerebral infarction(CI).Methods All of the 53 patients were randomly assigned to Irbesartan therapy group(n=25) treated with Irbesartan 150 mg/d and Aspirin 100 mg/d and conventional therapy group(n=28) treated with Aspirin 100 mg/d.Both groups were treated for 14 days and the other measures of symptomatic therepy were the same. And another 23 patients without cerebrovascular diseases and 7 health volunteers were taken as control group.The venous blood samples were taken for measureing the levels of LPA,ET-1,NO while the scorces of neurological deficit (NDS)were evaluated before and after treatment. 6.Inhibitory effect of EP4 antagonist on bone metastasis of prostate cancer

Objective The purpose of this study was to examine the inhibitory effect of ONO-AE3-208, an EP4 antagonist, on prostate cancer with bone metastasis in an animal model . Methods A PC3/LUC cell line was constructed by stably transfecting luciferin to prostate cancer PC 3 cells and inoculated into the left ventricle of nude mice to establish an animal model of prostate cancer with bone metastasis .After modeling , the animals in the experimental group and control groups were intraperitoneally given ONO -AE3-208 and double-distilled water, respectively, followed by examination of the metastasis loci and tumor burden by bioluminescence ima -ging and statistical analysis with survival curves . Results At 60 days after modeling , the animals in the control group exhibited sig-nificantly increased metastases and fluorescence burdens as compared with the experimental group (P<0.01), and the increase was in a time-dependent manner (P<0.01).At 60 days, the controls began to die while the experimental animals remained well alive , and at 180 days, the mice of the control group all died .The survival rate of the animals was significantly higher in the experimental group than in the control ( 13.3% vs 0%, P <0.01 ) and the median survival time remarkably longer in the former than in the latter group (162 d vs 116 d, P <0.01). Conclusion The EP4 antagonist ONO-AE3-208 inhibited the bone metastasis of prostate cancer and prolonged the survival time in the model mice .
7.Efficacy of double balloon catheter in full-term women for cervical ripening and labor induction

Objective To determine the efficacy of the double balloon catheter in ripening and dilatation of the unfavorable cervix for induction of labor .Methods A total of 332 full-term women who were underwent induction of labor with the double balloon devices were collected for retrospective analysis .Cervix was assessed before the double balloon insertion and after it was removed 12 hours later, then amniotomy alone or combined with oxytocin was performed .Cord blood gases were analyzed .Results The double balloon catheter increased Bishop score in all subgroups with a mean improvement of 2.5 ±0.7 ( P <0.05 ) .The mean insertion-deliv-ery interval was (19.07 ±4.31)h.Cesarean section was performed in 80/332 (24.1%) patients.Cord arterial pH value was 7.3 ± 0.1 .Conclusions The double balloon catheter induced significant ripening and dilatation of the unfavorable cervix .It was effective and safe.

As with all neurologic reflex mechanisms,efferent activity is brought about by afferent input. Furthermore, the superimposed voluntary control, which is a critical feature of social continence, is largely determined by sensations of bladder fullness. In recent years, interest in the pathophysiology of urinary incontinence has shifted from a focus on the detrusor muscle to the afferent innervation of the bladder. In this respect, it is notable that investigations of detrusor muscle function are often performed in the absence of an intact urothelium and lamina propria, along with their sensory innervation. This review will summarize the results from recent animal and human experiments.

10.Effects of combined siRNA-TR and -TERT on telomerase activity and growth of bladder transitional cell cancer BIU-87 cells.

The effects of combined RNA interference (RNAi) of human telomerase RNA (hTR) and human telomerase reverse transcriptase (hTERT) genes on telomerase activity in a bladder cancer cell line (BIU-87 cells) were investigated by using gene chip technology in vitro with an attempt to evaluate the role of RNAi in the gene therapy of bladder transitional cell cancer (BTCC). Three TR-specific double-stranded small interfering RNAs (siRNAs) and three TERT-specific double-stranded siRNAs were designed to target different regions of TR and TERT mRNA. The phTR-siRNA, phTERT-siRNA, and the combination of both plasmids phTR+phTERT-siRNA were transfected into BIU-87 cells. The expression of hTR and hTERT mRNA was detected by quantitative fluorescent reverse transcription-polymerase chain reaction, and a telomeric repeat amplification protocol was applied to detect telomerase activity. Growth inhibition of BIU-87 cells was measured by MTT assay. Gene chip analysis was performed to evaluate the effects of the combined RNAi of hTR+hTERT genes on telomerase activity and growth of BIU-87 cells in vitro. The results showed that the expression of hTERT and hTR mRNA was inhibited by pRNAT-hTERT-III, pRNAT-hTR-III, and pRNAT-hTR-III+hTERT-III in BIU-87 cells. The inhibition efficiency of pRNAT-hTERT-III, pRNAT-hTR-III, pRNAT-hTERT-III+pRNAT-hTR-III was 67% for TERT mRNA, 41% for TR mRNA, 57% for TR mRNA and 70% for TERT mRNA in BIU-87 cells respectively. The growth of BIU-87 cells was inhibited and telomerase activity was considerably decreased, especially in the cells treated with combined RNAi-hTR and -hTERT. Gene chip analysis revealed that 21 genes were down-regulated (ATM, BAX, BCL2, BCL2L1, BIRC5, CD44, CTNNB1, E2F1, JUN, MCAM, MTA1, MYC, NFKB1, NFKBIA, NME4, PNN, PNN, SERPINE1, THBS1, TNFRSF1A, and UCC1). The results indicated that hTR-siRNA and hTERT-siRNA, especially their combination, siRNA hTR+hTERT, specifically and effectively suppressed the expression of both hTR and hTERT mRNA and telomerase activity. Molecular biological mechanism by which combined siRNA-TR and -TERT inhibited telomerase activity and growth of BIU-87 cells in vitro may involve the down-regulation of the 21 genes.
