1.Application of“Cask Theory”in the Management of Automatic Medicine Packaging Machine in Our Hos-pital

OBJECTIVE:To improve the management of automatic medicine packaging machine(ATC),so as to improve drug dispensing management. METHODS:The short-plate effect of“cask theory”was used to analyze the deficiency of ATC in soft-ware,hardware and management;pertinent improvement was conducted and its effects were evaluated. RESULTS & CONCLU-SIONS:Considering the deficiency of ATC software function,automatic form export function had been set up additionally,involv-ing form of adding and dismounting drug information,form of repeated medical order. Some improvement were carried out for fre-quent machine halt in hardware,such as adjusting the height of drug box and using spare drug groove;some carried out for incon-sistency between the accounts and drugs due to management defects,such as fixing post,improving adding drug information input. Compared with before improvement,daily times of machine halt during packaging had cut from 13.8 times to 2.3 times(decreased by 83.3%);daily total time of machine halt had shortened from 97.4 minutes to 9.6 minutes(shortened by 90.14%);the rate of packaging had increased from 14.3 packages/min to 23.33 packages/min(increased by 62.9%);the management of drug stock be-came more standard. The application of“cask theory”in the improvement of ATC can promote the quality of drug dispensing man-agement.
2.Features of clinical and EEG changes during follow-up in patients with benign epilepsy of childhood with central-temporal spikes

Objective To investigate the features of clinical and EEG changes during follow up in patients with BECCT. Correlative analysis between the disease duration and some factors were also performed.Methods The clinical and EEG data of 87 recovered patients with BECCT were retrospectively analyzed.Results The EEG and clinical manifestations are mostly characteristic. Clinical seizures disappeared early before the rolandic spikes of EEG in most patients,showing a longest duration of 6 years. There was only the onset age being correlative with the duration of disease in both seizures and abnormal EEG. Medication was withdrawn after there appeared seizures free for more than two years and a recovery of EEG. However medication can also be stopped even at patient′s EEG still remaining abnormal.Conclusion BECCT is an age dependent, idiopathic epilepsy syndrome in childhood, comprehensively recognizing the clinical and EEG features should be helpful in clinical practice.
3.Ictal video-electroencephalography monitoring in presurgical evaluation for intractable temporal lobe epilepsy in 15 cases

Objective To assess the value of ictal video-electroencephalography monitoring (IVEEG) in presurgical evaluation for medically intractable nonlesional temporal lobe epilepsy(TLE). Methods The data of presurgical evaluation and postsurgical follow-up from 15 patients with intractable TLE were retrospectively analyzed.Results In this selected patients group, all 15 patients manifested clinically seizures during long term VEEG monitoring, with initial rhythmic activity from one side of temporal lobe in 13 cases. Of these 13 patients, the side of initial ictal focal pattern was concordant with the side of anterior temporal lobectomy or selective amygdalohippocampectomy in all cases and excellent postsurgical outcome were attained in 11 patients.Conclusion IVEEG monitoring may not only help to confirm the diagnosis and seizure type(s),but also more important to correctly localize the side of epileptogenic region in most patients with intractable TLE.
4.The sensitivity of cytopathic effect caused by influenza viruses in cultured Madin-Darby canine kidney cells

Objective To understand the sensitivity of cytopathogenic effect (CPE) in MadinDarby canine kidney cells(MDCK) that cultured influenza A pharyngeal swab specimens of patients for one,two and three passages. Methods Influenza A pharyngeal swab specimens of patients were inoculated in MDCK for three blind passages. The presence of CPE of every passage was observed by inverted microscope. Results Of the 279 influenza A pharyngeal swab specimens of patients tested by colloidal gold, the presence of CPE in MDCK for one,two and three passages was 65.9%(184/279),91.4%(255/279) and 96.4%(269/279), respectively. Two hundred and seventy-one of 279specimens were identified as influenza A by multiplex reverse transcription-polymerase chain reaction (RT-PCR). Conclusion The positive separation rate can reach more than 95% by inoculating influenza A pharyngeal swab specimens of patients in MDCK for three blind passages.
5.Lateralizing value of ictal head deviation in patients with mesial temporal lobe epilepsy

Objective To investigate the lateralizing value of head deviation(HD) during complex partial seizures (CPS) in patients with refractory mesial temporal lobe epilepsy (mTLE).Methods Presurgical videotypes of 43 patients who were seizure-free for at least one year after temporal lobectomy were retrospectively reviewed.Attention was paid to the relationship between time and type of HD and the side of epileptogenic zone.Results HD was seen in 88 CPS from 43 patients who had total 206 CPS with or without secondary generalization.Both versive and non-versive HD displayed high positive predictive value (83% (33/40) and 88% (22/25)) for localization of an ipsilateral and contralateral seizure onset,respectively.Conclusion Both non-versive HD and versive HID during CPS in patients with mTLE are reliable lateralizing signs that can complement other diagnostic modalities in presurgical evaluation.
6.Assessment of patients discomfort and nursing care with the application of vector system in periodontal treatment

Objective The study aimed to compare the vector system with the hand instrument in the pain severity during scaling and root planning.Methods 60 periodontal patients were randomly divided into two groups for supportive periodontal therapy (SPT):the experimental group (using vector system) and the control group (using gracey instruments),with 30 cases in each group.And the painfulness after SPT was evaluated by Visual Analogue Scale (VAS).Results 66% patients felt mild pain during the supportive periodontal therapy with the application of vector system,while 36% patients felt mild pain in the control group.24 patients in the experimental group accepted vector system for SPT and 23 patients in the experimental group felt less fear of the treatment.Conclusions Patients will feel less discomfort with the application of the vector system,therefore better compliance will be reached.And the cooperation of the doctors and nurses has great impact on the treatment effect.
7.Antimicrobial Resistance Report of 321 Strains of Nonfermenters Gram-negative Bacilli

OBJECTIVE To investigate the distribution and the antimicrobial resistance of nonfermenters Gram-negative bacilli causing nosocomial infection.METHODS Routine method was used to isolate and identify,antibiotics susceptibility was tested by the Kirby-Bauer on the NCCLS criterion of 2001.RESULTS Among 321 strains nonfermenters Gram-negative bacilli,Pseudomonas aeruginoa was 53.9%,Acinetobacter were 17.4%,and Stenotrophomonas maltophilia was 12.5%,Three kinds added up to 83.8%.CONCLUSIONS P.aeruginosea,Acinetobacter and S.maltophilia are important nonfermenters Gram-negative bacilli causing nosocomial infection and their drug resistant rate is very high.The matter is horrible in clinics,we should enforce monitoring.
8.Mixed culture of Madin-Darby canine kidney cells, human epidermoid cancer cells and African green monkey kidney cells for detection of common respiratory viruses and enteroviruses

Objective To establish a clinical test assay for detecting common respiratory viruses and enteroviruses (EV) by using mixed cultured Madin-Darby canine kidney cells (MDCK), human epidermoid cancer cells (Hep-2) and African green monkey kidney cells (Vero) to isolate common respiratory viruses and enteroviruses. Methods Throat swabs with influenza A and B viruses,adenovirus and EV71 were incubated with mixed cultured MDCK, Hep-2 and Vero in a single vial to observe the presence of cytopathic effects. Polymerase chain reaction (PCR), reverse transcription (RT)-PCR and monoclonal antibody-based immunofluorescene assay were also used for confirmatory test. Results The sensitive cell lines developed obvious cytopathic effects to the corresponding viruses, which were confirmed by the specific green particles observed by immunofluorescence assay and specific target PCR segments. Conclusions The shell-vial of mixed cells can simultaneously isolate different common respiratory viruses and EV. The isolated pathogens can be further confirmed by antigen test and PCR. This assay may improve the diagnosis of clinical viral diseases.
9.Evaluation of the short-term efficacy of cardiac resynchronization therapy by three-dimensional speckle tracking imaging

Objective To evaluate the short-term efficacy of cardiac resynchronization therapy(CRT)by three-dimensional speckle tracking imaging (3D-STI).Methods 27 chronic heart failure patients who underwent CRT were enrolled.All following indices were measured respectively:the left ventricular systolic function indices in the 16 segments of 3D-STI including the mean area peak strain,the mean longitudinal peak strain,the mean circumferential peak strain,and the mean radial peak strain(MAS,MLS,MCS,MRS) ; the strain-derived dyssynchrony indices in the 16 segments of 3D-STI including the standard deviation for time to peak area strain,the standard deviation for time to peak longitudinal strain,the standard deviation for time to peak circumferential strain,and the standard deviation for time to peak radial strain (TAS-SD,TCS-SD,TLS-SD,TRS-SD) ;the standard deviation for time to peak area strain,the standard deviation for time to peak circumferential strain,the standard deviation for time to peak longitudinal strain,and the standard deviation for time to peak radial strain(TAS-dif,TCS-dif,TLS-dif,TRS-dif) ;the rotation and twist index of 3D-STI including peak rotation on apical plane,peak rotation on basal plane,peak twist angle(RotA,RotB,Twist),time to peak of the rotation on apical,time to peak of the rotation on bastal,time to peak of the twist (PTRotA,PTRotB,PTtw).The differences of all the indices between before CRT and 3 months after CRT were calculated and the correlation between the changes of all those indices and the changes of left ventricular ejection fraction (△LVEF) were analyzed.Results MAS,MLS,MCS,MRS,RotA and Twist were increased 3 months after CRT (P <0.01,P <0.05,P <0.01,P <0.01,P< 0.01,P <0.01);TRS-SD and PTtw was decreased 3 months after CRT (P <0.01,P <0.05) ; |△MAS|,| △MLS|,△TRSSD,△RotA and △Twist were correlated with △LVEF 3 months after CRT (r =0.521,P =0.005; r =0.389,P =0.045; r =-0.560,P =0.002; r =0.513,P =0.006; r =0.610,P =0.001).Conclusions The myocardial systolic strain function,dyssynchrony and twist function of left ventricle were improved significantly 3 months after CRT.3D-STI offered a new way for the evaluation of short-term efficacy of CRT.
10.Studying and practicing of ultrasound anatomy teaching sample of fetus heart with complex abnormity

ObjectiveTo make ultrasound anatomy teaching samples of odynopoeia fetus heart with complex abnormity consistent with echocardiographic view,and utilize them in echo teaching.MethodsTen odynopoeia foetus hearts were cutted comparing to different echocardiographic imaging respectively.Results Dissections of fetus heart were obtained including 5 cases of single cardiac ventricle(2 case with interruption of aortic arch),4 cases of double outlet of right ventricular,and 1 case of hypoplastic left heart syndrome with serious coarctation of the aorta.The apical long axis view,the three blood vessel view,as well as the long axis view of the aortic arch were typical views of ultrasound diagnosis and teaching.ConclusionsIt is profit to raise and accurate rate of fetus echocardiography diagnosis by studying dissection of fetus heart anatomic structure with complex abnormity,which would make ultrasound imaging visualization.
