1.Mediating effect of blood glucose in the association between myosteatosis and complications in non-diabetic postoperative gastric cancer patients on supplemental parenteral nutrition

Objective:To explore the effects of myosteatosis and blood glucose (BG) on postoperative complications in non-diabetic gastric cancer patients receiving supplemental parenteral nutrition (SPN) after gastrectomy.Methods:Patients who underwent radical gastrectomy between March 2017 and June 2021 in the Department of Surgical Oncology, the First Hospital of Lanzhou University were included in this study. Various preoperative inflammatory and nutritional indicators, including skeletal muscle metrics at the third lumbar level on CT, were collected retrospectively. Postoperative BG within 3 days and complications within 30 days were monitored. Patients were divided into two groups according to the presence or absence of myosteatosis (assessed via skeletal muscle density [SMD]) and the differences in postoperative BG and complication incidence were compared. Mediation model was used to analyze the mediating effect of BG in the association between SMD and postoperative complications.Results:A total of 357 patients were included in the study. Compared with the 299(83.8%) patients without myosteatosis , the incidence of hyperglycemia, mean BG, maximal BG, and BG fluctuation while on SPN in the 58(16.2%) patients with myosteatosis were higher, and the comprehensive complication index (CCI) and the incidence of complication were higher ( P<0.05). More importantly, BG showed the mediation effect of -0.0892 in the effect of SMD on CCI ( P<0.05), with the effect size of 19.3%. Conclusion:Myosteatosis and postoperative hyperglycemia are associated with higher incidence of complications, and BG plays an intermediary role in the association between myosteatosis and CCI.
2.Investigation on the knowledge and behavior of breast milk collection and storage in nurses in Neonatal Intensive Care Unit

Objective:To evaluate the current status of knowledge and behavior in breast milk collection and storage among nurses in the Neonatal Intensive Care Unit (NICU) , and to explore the factors influencing knowledge and behavior of nurses.Methods:By adopting the convenient sampling method, a total of 175 NICU nurses from ClassⅢ Grade A hospitals in Henan province were selected as the research objects in April 2019. A cross-sectional survey of NICU nurses was conducted using the knowledge and behavior questionnaire on breast milk collection and storage of NICU nurses. A total of 175 questionnaires were issued and 175 were recovered, with a recovery rate of 100%.Results:The total score of knowledge and behavior standards of breast milk collection and storage of 175 NICU nurses was (59.69±8.24) , which was at a low level. Among which, the score of knowledge dimension standard was (43.11±9.74) , and the score of behavior dimension standard was (33.58±5.14) . 85.1% (149/175) NICU nurses had not received special training in breast milk collection and storage, and there were great individual differences in the perception of the working environment of breast milk collection and storage among NICU nurses. The results of multivariate analysis showed that age, working environmentperception score of breast milk collection and storage and training of breast milk collection and storage were the main influencing factors of breast milk collection and storage knowledge behavior of NICU nurses ( P<0.05) . Conclusions:The knowledge and behavior of NICU nurses in breast milk collection and storage need to be improved, especially in breast milk thawing, storage temperature and other issues. Nursing managers should formulate objective and unified breast milk collection and storage systems and procedures and strengthen relevant training for nurses to improve nurses' perception of the working environment of breast milk collection and storage, strengthen relevant knowledge and improve breast milk collection and storage behavior.
3.Current status and influencing factors of family resilience in children with acute leukemia

Objective:To investigate the current status of family resilience of children with acute leukemia and explore its influencing factors.Methods:Using the convenient sampling method, the families of children with acute leukemia who were admitted to 2 wards of Department of Hematology and Oncology of a Children's Hospital in Zhengzhou from April to August 2019 were selected as the research objects. The survey was conducted using the general information questionnaire, the Chinese-Family Resilience Assessment Scale (C-FRAS) and Chinese version of Posttraumatic Growth Inventory (PTGI) . A total of 220 questionnaires were issued, 207 of which were valid, with an effective recovery rate of 94.1%.Results:The total score of C-FRAS of main caregivers of 207 children with acute leukemia was (122.86±13.97) , the total score of Chinese version of PTGI was (65.31±14.82) . The total score of C-FRAS was positively correlated with the total score of Chinese version of PTGI ( P<0.05) . Multiple linear regression analysis showed that treatment time, the relationship with the children, whether there was social assistance, whether there was only one child, and the posttraumatic growth of the main caregiver were the influencing factors of the family resilience of children with acute leukemia ( P<0.05) . Conclusions:The family resilience of children with acute leukemia is at a moderate level, and the posttraumatic growth of the main caregiver of the children is at a moderate to upper level. The treatment time, the relationship with the children, whether there is social assistance, whether there is only one childand the post-traumatic growth of the main caregiver are the influencing factors of the family resilience of children with acute leukemia. In the process of clinical nursing, the family advantage can be promoted by improving the post-traumatic growth level of the main caregivers and help the families of the children to adapt well.

Objective To explore characteristics and significance of the indexes of peripheral white blood cell (WBC) in patient with human brucellosis.Methods People checked by brucellosis physical checkup and routine physical checkup at Qiqihar Center for Disease Control and Prevention from December 2014 to December 2015,including 40 acute brucellosis patients (acute group),35 chronic brucellosis patients (chronic group) and 72 healthy people (control group),were selected.Automatic blood analyzer was used to determine the indexes of WBC,lymphocyte count (LY),lymphocyte percentage (LY%),monocytes count (MONO),monocytes percentage (MONO%),eosinophil count (EO),eosinophil percentage (EO%),basophilic granulocyte count (BASO),basophilic granulocyte percentage (BASO%),neutrophils count (NEUT) and neutrophils percentage (NEUT%).The receiver operating characteristic (ROC) curve was used to evaluate the diagnostic value of WBC parameters in acute and chronic groups.Results Compared to control group,the levels of WBC,EO,EO%,BASO,BASO%,NEUT and NEUT% were decreased in acute group [(5.222 0-± 2.551 2) × 109/L vs (6.352 5 ± 1.905 8) × 109/L,(0.030 0 ± 0.006 8) × 109/[,vs (0.083 9 ± 0.039 3) × 109/L,(0.54 ± 0.12)% vs (2.31 ± 0.14)%,(0.009 0 ± 0.001 1) × 109/L vs (0.019 0 ± 0.002 4) × 109/L,(0.17 ± 0.09)% vs (0.32 ± 0.20)%,(2.698 7 ± 1.948 4) × 109/L vs (4.012 9 ± 1.579 0) × 109/L,(48.13 ± 14.38)% vs (62.13 ± 9.00)%,all P < 0.05],and the levels of LY,LY% and MONO% were increased in acute group [(2.125 3 ± 0.949 9) × 109/L vs (1.794 4 ± 0.606 6) × 109/L,(43.37 ± 14.52)% vs (29.10 ± 7.97)%,(7.84 ± 2.23)% vs (6.55 ± 2.04)%,all P < 0.05].Compared to control group,the level of NEUT% [(54.63 ± 9.26)%] was decreased in chronic group (P < 0.05),and the levels of LY,LY% and EO [(2.212 0 ± 0.633 2) × 109/L,(36.41 ± 8.51)%,(0.153 9 ± 0.028 8) × 109/L] were increased in chronic group (all P < 0.05).The levels of LY% and MONO% [(6.45 ± 1.58)%] in chronic group were lower than those in acute group (all P < 0.05),and the levels of WBC [(6.175 7 ± 1.469 5) × 109/L],EO,EO% [(2.32 ± 1.21)%],BASO [(0.021 8 ± 0.001 9) × 109/L],BASO% [(0.37 ± 0.21)%] and NEUT% were higher than those in acute group (all P < 0.05).The areas under ROC curve (AUCs) of LY and MONO in acute group were 0.681 and 0.529,they were in 0.5-0.7,and the diagnostic value was low;the AUCs of EO,EO%,LY%,NEUT%,NEUT,BASO,BASO%,MONO% and WBC in acute group were 0.816,0.816,0.806,0.790,0.766,0.760, 0.721,0.715 and 0.710,they were in > 0.7-0.9,and the diagnostic value was medium.The AUCs of LY,NEUT,BASO,EO,BASO%,EO%,MONO%,MONO and WBC in chronic group were 0.693,0.617,0.586,0.584,0.581,0.541,0.500,0.513 and 0.510,they were in 0.5-0.7,and the diagnostic value was low;the AUCs of LY% and NEUT% in chronic group were 0.725 and 0.717,they were in > 0.7-0.9,and the diagnostic value was medium.Conclusion The indexes of peripheral WBC in patient with acute and chronic human brucellosis are changed abnormally,which has a certain reference value in diagnosis of human brucellosis.

Nowadays,acute pancreatitis remains a clinical challenge with considerable morbidity and mortality.Acute pancreatitis is associated with systemic and metabolic derangements which may promote hypermetabolism and negative nitrogen balance due to negative energy balance.Therefore,optimal nutritional support intreatmentof acute pancreatitis has been a subject for decades.Originally,the concept of pancreatic rest by fasting was thought to improve outcome as early enteral nutrition was believed to aggravate inflammation or infection through pancreatic stimulation.At present,an increasing number of studieswere performed showing numerousbenefits of enteral nutrition.In this review,intestinal dysfunctionandroute,timing,type and amount of enteralnutritional support in acute pancreatitis will be discussed.
6.Let-7 miRNA silencing promotes Kaposi′s sarcoma-associated herpesvirus lytic replication via activating mitogen-activated protein kinase kinase kinase kinase 4 and its downstream factors

Objective To explore the effect of let?7 miRNA silencing on Kaposi′s sarcoma? associated herpesvirus ( KSHV) lytic replication and the underling mechanism. Methods The pEGFP?C2?let?7 sponge vector was transfected into BCBL?1 and 293T cells with Lipofectamine 2000 to silence the expression of let?7 miRNAs. Quantitative real?time PCR ( qRT?PCR) was used to quantify the expression of let?7 miRNAs, the transcriptional levels of mitogen?activated protein kinase kinase kinase kinase 4 ( MAP4K4) , cyclooxygenase?2 ( COX?2) and matrix metalloproteinase 13 ( MMP?13) , and the DNA copy numbers of KSHV open reading frame 50 ( ORF50) and open reading frame 72 ( ORF72) . Western blot was used to detect the total and phosphorylated protein levels of MAP4K4, COX?2, extracellular regulated protein kinases (ERK1/2), c?Jun N?terminal kinase (JNK) and p38 MAPK. Results The expression of let?7 miRNAs was dramatically decreased in the let?7 sponge transfected BCBL?1 and 293T cells compared with that in the vector?transfected cells ( P<0.05 for all) . The gene copy number and mRNA transcriptional level of KSHV ORF50 were significantly increased in the let?7 sponge transfected BCBL?1 cells compared with that in the vector?transfected cells ( 1. 00 ± 0. 10 vs. 2. 33 ± 0. 18 and 1. 08 ± 0. 48 vs 3. 22 ± 0. 27, respectively, P<0.001 for both) . The gene copy number and mRNA transcriptional level of KSHV ORF72 were also significantly increased in let?7 sponge transfected BCBL?1 cells compared with those in the vector?transfected cells(1.07±0.49 vs 1.67±0.45 and 1.01±0.19 vs 1.54±0.11, respectively, P<0.05 for both). 9.Changes of serum brain derived neurotrophic factor and heat shock protein 70 and their significance in children with complicated febrile convulsions

Objective To investigate the serum levels of brain derived neurotrophic factor (BDNF) and heat shock protein 70 (HSP70),and evaluate their clinical significance in brain injury of children with complicated febrile convulsions (CFC).Methods Forty-three patients with CFC,41 children with simple febrile convulsions and 36 healthy controls were recruited in this study; their serum levels of BDNF and HSP70 were detected by enzyme linked immunosorbent assay (ELISA); correlation between levels of BDNF and HSP70 and both time of twitch duration and twitch frequency were analyzed.Results The mean serum concentrations of BDNF (4.96±1.21 ng/mL) and HSP70 (2.86± 0.93 ng/mL) in patients of CFC group were significantly higher than those in patients of simple febrile convulsions and healthy controls (P<0.05); while no significant difference was noted between patients of simple febrile convulsions and healthy controls (P>0.05).These changes were closely related to the duration time and frequency of convulsions(r=0.553,P=0.015; r=0.587,P=0.012; r=0.526,P=0.024; r=0.562,P=0.018).Conclusions It is sure that some brain injury exists in children with CFC; BDNF and HSP-70 might be one of the molecular mechanisms for the brain injury and protective effect.
10.The mechanism of melamine-induced renal damage in rats

Objective To investigate the mechanism of melamine-induced renal damage in rats.Methods 48 male SD rats were randomly divided into 4 groups with 12 in each group and feed for 3 months.Group A were the control group,feed with standard granule feedstuff and drinking tap water.Group B were stone-induced group,feed with granule feedstuff containing 3% Mel and drinking tap water.Group C were feed with granule feedstuff containing 3% Mel and drinking water containing 2% taurine.Group D were feed with standard granule feedstuff and drinking water containing 2% taurine.Every week 24 h urine was collected to test PH,SCr,uric acid,protein,8-IP,H2O2 and Mel level.All rats were sacrificed at the end of 3 months.Blood creatinine detection,renal pathology analysis ( HE and Oil ep-red O dyeing,immunohistochemical) and mitochondria separation and detection were undertaken. ResultsMel was not detedted in urine of Group A and Group D.The urine concentration of Mel in Group B and Group C in 1 week,2 weeks,3 weeks,4 weeks were 3.16 ±0.45,4.39 ±0.213,5.40 ±0.28,5.50 ±3.26 and 3.52 ±0.49,4.32 ± 0.135,5.34 ± 0.40,5.46 ± 2.99 mg/ml,respectively.Compared with Group A,the Mel concentration in urine of Group B and C were drug exposure time dependent.In Group A,the urine protein,urine creatinine clearance,serum creatinine,and renal/weight ratio were 6.45 ± 1.45 mg/24 h,28.0 ± 7.4mmol/l,0.56 ±0.03 ml · min-1 · 100g-1,2.29 ±0.89 mg/g,while in Group B and C,the urinary protein urine,serum creatinine,creatinine clearance,kidney/weight ratio were 14.56 ± 7.69,56.8 ± 5.2,0.29 ±0.05,4.16 ±0.27 and 16.44 ±6.29,55.8 ±7.4,0.30 ±0.07,4.40 ±0.56,respectively.Compared with group A,in Group B and C,the urinary protein increased significantly,urine creatinine clearance reduced,serum creatinine reduced,and renal/weight ratio increased.Compared with Group B,the improvement of renal function in Group C was not significant,and the decrease of serum creatinine and urinary protein were not obvious (P > 0.05).In Group B and C,the urine H2O2,8-IP and mitochondrial oxidatie detection reagent SOD,GSH-PX numerical were 28.5 ± 5.2 mmol/1,3.26 ± 1.6 pg/ml,21.1 ± 7.8 U/mg prot,19.0 ±2.5 energy unit and 26.7 ±4.8 mmol/l,2.99 ±8.5 pg/ml,20.3 ±6.9 U/mg prot,17.9 ±4.8 energy unit,respectively.The difference between Group B and C was not statistically significant (P >0.05).Pathological analysis showed Mel was mainly concentrated in crystal tubular lumen (Group B and C),kidney interstitial damage was apparent,and kidney inflammation and fibrosis progressive developed with the increase of the drug exposure time. Conclusions Mel can induce kidney damage and stone formation in rats,and stone was mainly in tubular location in inner medullary zone.It is not the oxidative stress way that Mel leads to kidney damage.
