1.Research on diffusion tensor imaging and magnetic resonance spectroscopy imaging of brain white matter damage in hypoxic-ischemic encephalopathy of prematurity

76 premature babies stayed in NICU of Taian City Central Hospital from January, 2012 to December, 2012 whose gestational age is from 31 week to 35 week. The babies were scanned with MRI, DTI and MRS. According to the statistics of Brain White Matter Damage Based on MRI those babies are divided into case group and control group which has 38 babies respectively. 38 full-term infants, who don’t have history of perinatal asphyxia, whose MRI is normal, are taken as control group. The ADC value and FA value in different ROIs and different metabolite peak ratios in two groups are tested with DTI and MRS respectively.Results: 1.With the growth of gestational age the ADC value decreases while the FA value increase gradually. 2.With the deterioration of white matter damage the values of Cho, Lac, Cho/Cr, Cho/NAA rise while the values of NAA, NAA/Cho, NAA/Cr fall.Conclusion: Cerebral white matter damage can severely influence the changes of brain inner water molecular diffusion and anisotropy and metabolin. By uniting the DTI and MRS these two technologies together, people can diagnose cerebral white matter damage and white matter damage in the early period.

3.Effects of Single High Dose Intake of Ethanol on the Liver of Mice

Objective To investigate the liver damage caused by single high-dose intake of ethanol in mice. Methods Twenty-four male Kunming mice were randomized into 2 groups. The mice in ethanol group were treated with single dose of ethanol (50%,V/V,12 ml/kg),while the control mice received distilled water. The serum biochemical indices were determined 16 h after ethanol exposure. The hepatic triglyceride (TG) and free fatty acid (FFA) levels and the histological changes were examined to evaluate the fat accumulation,while the Ca2+-induced mitochondrial permeability transition (MPT) was detected for the mitochondrial function assay. The liver and mitochondrial malondialdehyde (MDA) and glutathione (GSH) levels were determined to assess the lipid peroxidation level. The activities and the protein expression of the CYP2E1,1A2,3A were determined. Results Slow response,ataxia and righting reflex disappearance were gradually emerged,which disappeared at about 3 h later. Compared with the control group,the liver index,the serum ALT and AST activities,the hepatic TG,FFA,and MDA levels,and the activity and protein level of the CYP2E1 significantly increased,while the liver GSH level significantly decreased (P0.05). 4.Three-dimension conformal radiation therapy for 42 rectal cancer patients

Objective To evaluate the effects of 3-dimensional conformal radiation therapy(3DCRT) in form of local control and survival of rectal cancer patients. Methods Forty-two patients with rectal cancer were irradiation by 3DCRT. They first received 40 Gy with larger field, at 1.8-2.0 Gy/f, 1 fraction qd, then followed by a boost of 24-27 Gy with reduced field, at 3.0-4.0 Gy/f, 1 fraction qod, to a total dose of 0,64-67 Gy. Results The 1-,2-,3-year survival rates were 83.3% ,64.3% and 45.2% .The 1-,2-,3-year local recurrence rates were 2.4%,11.9% and 23.9%. Conclusion Three-dimensional conformal radiotherapy is able to prolong the survival and improve the life quality of patients with rectal cancer.
5.Therapeutic effect of 138 imported falciparum malaria patients

Objectives To summarize the clinical characteristics of imported falciparum malaria patients and the treatment so as to provide the evidences for improving the diagnosis and treatment of the disease. Methods A total of 138 imported falci?parum malaria patients who received the treatment in Shandong Institute of Parasitic Diseases from January 2007 to February 2013 were adopted as the observation subjects and their clinical data were collected and analyzed. Results All the 138 pa?tients were back from African countries. The main manifestations were fever headache asthenia and hepatosplenomegaly and most of them were with decreased RBC PLT levels and increased LDH levels and 36.96%of them were misdiagnosed as respiratory diseases nephritis hepatitis and so on. Through antimalarial treatment of artemether or artesunate or dihydroartemis?inin and primaquine or dihydroartemisinin and piperaquine and symptomatic treatment the short?term and long?term cure rates were 98.55%and 94.93%respectively with 1 case unrecovered and 1 died. Conclusions Artemisinins are still the most effective antimalarial drugs for falciparum malaria. However some patients recrudesce as the Plasmodium in their body is resis?tant or insensitive to these drugs. We should pay more attention to the antimalarial and symptomatic treatments in the early stage of severe malaria so as to improve the cure rate.
6.Clinical Research on Near-term Quality of Life among Unstable Angina Patients by Shen-Yuan Yi-Qi Huo-Xue Capsule Combined with Early Percutaneous Coronary Intervention

This study was aimed to investigate the effects of Shen-Y uan Y i-Qi Huo-Xue (SYYQHX) capsule com-bined with early percutaneous coronary intervention (PCI) on near-term quality of life (QOL) in unstable angina (UA) patients. Seventy-eight patients diagnosed with UA were selected and randomly divided into the treatment group and control group, with 39 patients in each group. Early PCI was undergone after coronary angiography. Before PCI, the control group was given routine western medication. The treatment group was given routine western medication plus SYYQHX capsule, three pills once, three times daily. The treatment lasted for 30 days. The QOL scores were evalu-ated among patients from both groups before and after treatment in order to determine the efficacy on angina, electro-cardiogram (ECG) and traditional Chinese medicine (TCM) main symptom. The results showed that compared to pre-treatment, the scores of physical limitation (PL), angina stability (AS), angina frequency (AF), and treatment satisfac-tion ( TS ) were significantly increased after treatment ( P < 0 . 05 ) . Compared with the control group , after treat-ment with SYYQHX capsule, the AF, duration time and symptoms of palpitation, fatigue and shortness of breath were obviously improved (P< 0.05). The scores of AS, AF and TS in the treatment group were significantly increased (P < 0.05). It was concluded that SYYQHX capsule combined with early PCI can improve the near-term QOL and TCM main symptoms among UA patients.
7.Significance of peritoneal effusion detection by emergency ultrasound in closed abdominal injury

Objective To discuss the diagnostic value of peritoneal effusion detection by emergency-ultrasound in patients with closed abdominal injury. Method From August 2006 to June 2009,212 patients with closed abdominal injury were studied to evaluate peritoneal effusion detection by emergency ultrasound. Results of 212 patients,peritoneal effusion frequency rate was 78.8%( 167/212), meanwhile,abdominal paracentesis confirmation ratio was only 46.2%(98/212). In the follow-up, 13 patients with injuried hollow viscera and 1 patient with rupture of kidney showed peritoneal effusion. The volume of abdominal fluid was increasing in 17 patients,which needed to be managed by surgery. The accuracy rates were respectively 78.3%( 112/143) and 36.1%(13/36) in the solid organs and the hollow organs. Conclusion During the course of diagnosis and treatment in closed abdominal injury,peritoneal effusion monitoring by ultrasound should be used routinely, which can help to decrease the rate of misdiagnosis and avoid delayed treatment.
8.Effects of Compound Danshen Dripping Pills or Tablets on Chronic Hepatitis B Complicated with Hepatic Fibrosis: A Systematic Review

OBJECTIVE:To systematically review the efficacy of Compound danshen dripping pill or Compound danshen tablets for chronic hepatitis B complicated with hepatic fibrosis,and to provide evidence-based reference for clinical treatment.METHODS:Retrieved from PubMed,Cochrane Library,EMBase,Medline,Wanfang database,VIP,CJFD and CBM database,randomized controlled trials (RCTs) about Compound danshen dripping pills or tablets combined with entecavir(trial group) vs.entecavir alone (control group) for chronic hepatitis B complicated with hepatic fibrosis were collected.Meta-analysis was performed by using Rev Man 5.3 statistical software after data extraction and quality evaluation according to Cochrane systematic evaluation manual 5.1.0.RESULTS:A total of 9 RCTs were enrolled,including 714 patients.The results of Meta-analysis showed that ALT [SMD=-0.44,95%CI(-0.86,-0.01),P=0.04],HA [SMD=-1.53,95%CI(-2.28,-0.79),P<0.001],LN [SMD=-0.93,95%CI(-1.41,-0.46),P<0.001],CⅣ [SMD=-1.38,95%CI(-2.13,-0.63),P<0.001] and PCⅢ [SMD=-1.42,95% CI (-1.60,-1.23),P<0.001] of trial group were significantly lower than those of control group;HBV-DNA negative rate [OR=3.73,95 % CI (2.14,6.49),P < 0.001] was significantly higher than control group,with statistical significance.CONCLUSIONS:Compound danshen dripping pills or tablets can not only improve liver function and liver fibrosis in chronic hepatitis B patients with hepatic fibrosis,but also improve the antiviral effect.
9.Percutaneous Kirschner wire leverage plus plaster fixation versus elastic intramedullary nailing for radial neck fractures in children

Objective:To compare the clinical outcomes between percutaneous Kirschner wire leverage plus plaster fixation and elastic intramedullary nailing in the treatment of radial neck fractures in children.Methods:A retrospective study was conducted to analyze the 60 children with radial neck fracture who had been treated by percutaneous Kirschner wire leverage plus plaster fixation at Department of Pediatric Orthopedics, Children's Hospital of Soochow University from January 2016 to July 2023 (set as an internal fixation-free group). They were 30 males and 30 females (34 left and 26 right sides) with an age of (7.7±3.0) years. At the same time, another cohort of 60 patients were chosen as an intramedullary nailing group who had been treated by percutaneous Kirschner wire leverage plus elastic intramedullary nailing and matched in age and gender with those in the internal fixation-free group. The preoperative fracture angulation, operative time, hospitalization time, fracture angulation on the first postoperative day, fracture angulation at 1 month postoperatively, rate of angulation loss after reduction, Mayo elbow performance score (MEPS) at the last follow-up and complications were compared between the 2 groups.Results:There was no significant difference between the 2 groups in their preoperative general data, showing comparability ( P<0.05). The 120 pediatric patients were followed up for (7.5±3.2) months. The operative time [(27.4±15.0) min] and hospitalization time [(3.4±1.9) d] in the internal fixation-free group were significantly shorter than those in the intramedullary nailing group [(45.4±13.5) min and (4.4±1.3) d] ( P<0.05). The preoperative fracture angulation (50.8°±1.9°), fracture angulation on the first postoperative day (11.3°±1.2°), fracture angulation at 1 month postoperatively (12.1°±1.3°), rate of angulation loss after reduction (2.9%±0.5%), and MEPS at the last follow-up [(90.4±2.0) points] in the internal fixation-free group showed no significant differences from those in the intramedullary nailing group [49.5°±1.7°, 11.1°±1.2°, 13.3°±1.5°, 3.9%±1.4%, and (90.2±2.3) points] ( P>0.05). None of the patients in the internal fixation-free group developed pin-tail irritation sign or premature epiphyseal closure after surgery, whereas 3 patients in the intramedullary nailing group developed pin-tail irritation sign and 2 ones premature epiphyseal closure after surgery, showing a significant difference in the complication rate between the 2 groups [0 (0/60) versus 8.3% (5/60)] ( P<0.05). Conclusions:Percutaneous Kirschner wire leverage plus plaster fixation and close elastic intramedullary nailing can both achieve satisfactory outcomes in the treatment of radial neck fractures in children. However, percutaneous Kirschner wire leverage plus plaster fixation needs shorter operative time and hospitalization time, leads to fewer complications, and requires no reoperation to remove internal fixation.
