1.Minimally invasive cervical laminoplasty versus single-door laminoplasty for treatment of cervical spondylotic myelopathy

BACKGROUND:In recent years, a single-door laminoplasty for cervical spondylotic myelopathy was performed in the Department of Orthopedics, the First Affiliated Hospital of Zhengzhou University. This surgery achieved spinal decompression while remaining the important structure of cervical vertebra.
OBJECTIVE:To comparatively analyze the therapeutic effects of minimal y invasive cervical laminoplasty and single-door laminoplasty on cervical spondylotic myelopathy.
METHODS:A total of 78 cases of cervical spondylotic myelopathy were randomly divided into two groups, which respectively received minimal y invasive cervical laminoplasty and single-door laminoplasty.
RESULTS AND CONCLUSION:Al cases were fol owed-up for 3-36 months. No significant difference in excellent and good rate of Japanese Orthopaedic Association score was detected between the two groups in final fol ow-up (P>0.05). The apparent rate of axial symptoms was significantly lower in the minimal y invasive cervical laminoplasty group than that in the single-door laminoplasty group after treatment (P<0.05). Cervical curvature loss was significantly lower in the minimal y invasive cervical laminoplasty group than that in the single-door laminoplasty group (P<0.05). In final fol ow-up, in the minimal y invasive cervical laminoplasty group, two titanium screws of one patient were slightly loose, and no abnormal symptom was observed. In the single-door laminoplasty group, hinge breakage was visible in six patients. The broken vertebral plate was excised. Artificial spinal dura mater was coated on the surface of spinal cord for protection. Results suggested that the effect of two surgical methods in promoting functional recovery of spinal cord is identical. However, complications in the minimal y invasive cervical laminoplasty group are further fewer than those in the single-door laminoplasty group.
2.Clinical study of intravenous and intra-arterial thrombolysis with urokinase for acute cerebral infarction

ObjectiveTo evaluate the efficacy of intravenous and intra-arterial thrombolysis with urokinase for acute cerebral infarction.Methods Fifty patients with acute cerebral infarction occurred within 6 hours were divided into two groups by random digits table method with 25 cases each:intravenous and intra-arterial thrombolysis group and intravenous thrombolysis group.The patients in intravenous and intra-arterial thrombolysis group were given 200 000 U urokinase by intravenous infusion for 30 minutes immediately after being hospitalized,and arterial thrombolysis was prepared at the same time.With cerebrovascular angiography,the thrombolytic therapy was carried out in the target vessel blocking points through micro-catheter.Urokinase dissolved in 0.9% sodium chloride was infused at the rate of 10 000 U per minute,the total volume would not be more than 1 000 000 U.The patients in intravenous thrombolysis group were given 1 000 000 U urokinase in 100 ml 0.9% sodium chloride by intravenous infusion within 60 minutes.The clinical efficacy after thrombolysis was assessed according to the National Institutes of Health stroke scale (NIHSS) score,the quality of life was judged by Barthel index (BI) score and the prognosis was evaluated by modified Rankin scale (mRS) score of 90 days after thrombolysis.ResultsThere was no significant difference between two groups before thrombolysis according to the NIHSS score (P > 0.05).After thrombolysis,NIHSS scores in two groups showed a downward trend,but they were obviously lower in intravenous and intra-arterial thrombolysis group after 24 h,7 d and 14 d than those in intravenous thrombolysis group [(8.97±4.56) scores vs.(11.01±3.65) scores,(6.88±2.31) scores vs.(8.34±3.05) scores,( 4.06±3.02 ) scores vs.( 6.73±2.15 ) scores ] ( P < 0.05 or < 0.01 ).BI scores before thrombolysis between two groups had no significant difference(P >0.05),while BI score of 90 days after thrombolysis in intravenous and intra-arterial thrombolysis group [(79.55±19.64) scores] was higher than that in intravenous thrombolysis group [(69.31±21.35) scores](P=0.0162).The rate of mRS score 0-2 (good efficscy) in intravenous and intra-arterial thrombolysis group [72.0%(18/25) ] was obviously higher than that in intravenous thrombolysis group [ 52.0% ( 13/25 ) ] (P =0.0198 ).ConclusionsIt is significantly effective to treat acute cerebral infarction by superselective intravenous and intra-arterial thrombolysis.Therefore,it is supposed to be an optimal option for treating acute cerebral infarction in the future.

4.Investigation of current cognition of occupational exposure to HIV in healthcare workers in Liuzhou, China.

OBJECTIVETo investigate the current cognition of occupational exposure to human immunodeficiency virus (HIV) and the personal occupational protection awareness in healthcare workers in Liuzhou, China.
METHODSA total of 270 healthcare workers were selected from 10 hospitals in Liuzhou by stratified random sampling for a cross-sectional study. And a self-administered questionnaire of occupational exposure to HIV was designed to conduct a survey. The descriptive analysis of data was carried out by Excel. And a logistic regression analysis was done to analyze the effects of different factors on healthcare workers' cognition of occupational exposure to HIV using the statistical analysis software SPSS 19.0.
RESULTSA total of 260 usable questionnaires (96.3%) were returned. Among them, 220 healthcare workers (84.6%) had received the trainings on occupational exposure to HIV; 223 healthcare wofkers (85.8%) were aware of the rules and regulations on prevention of occupational exposure to HIV and the operation procedures in their hospitals. The healthcare workers who had not received the trainings or had not known the rules and regulations and the operation procedures were mainly from primary or secondary hospitals. A total of 106 healthcare workers (40.8%) had directly contacted patients' blood or body fluids; 154 healthcare workers (59.2%) were injured by sharp instruments, and most were hollow needle stick injuries (98/154, 63.6%). A total of 168 healthcare workers (68.08%) had better cognitive awareness of occupational exposure to HIV, and 76 healthcare workers (29.2%) had moderate cognitive awareness. Some healthcare workers had poor knowledge in the common sense of AIDS/HIV and occupational exposure to HIV, the personal protection awareness of occupational exposure, or the disposal measures after occupational exposure. The univariate analysis using chi-square test showed that occupation and professional title were significantly correlated with the cognition (P<0.05). The multivariate logistic regression analysis showed that the doctors (OR3.8; P<0.05), nurses (OR3.04, P<0.05), and laboratory technicians (OR=9.51, P<0.05) had better awareness compared with the others. The healthcare workers with a primary or lower professional title had poorer awareness compared with the healthcare workers with a higher professional title (OR=0.47, P<0.05).
CONCLUSIONHealthcare workers have the risk of occupational exposure to HIT. They do not have comprehensive and systematic knowledge related to occupational exposure to HIV, and they have a high demand for training.

5.Investigation into the advanced training status for emergency department nurses in Chongqing

ObjectiveTo study the advanced training status for emergency department nurses in Chongqing,and to improve the training program.MethodsThe basic and professional information of 208 emergency department nurses who obtained advanced training for emergency nursing in the previous 5 years was analyzed and the self-designed Satisfaction Survey on Teaching and Self-evaluation on Training Outcome were applied to learn about the feedback of trainees.ResultsThe average age of trainees was ( 29.24 ± 5.44 ),among whom 65.86% with junior college education experience,49.51% with 2-5 years of working experience,50.48% at primary professional rank,61.53% from second level hospitals.Their average score ( 77.91 ± 7.69 ),and teaching satisfaction score was ( 87.01 ± 7.63 ).The survey indicated that self-evaluation on training outcome was significant to enhance emergency nursing knowledge and skills,while weak to improve research and managing capability.ConclusionNurses attending advanced training for emergency nursing are characterized with short working time,low professional ranks,jagged education experiences and various levels of hospital. It is suggested to adopt leveled training in accordance with education experience and working hospital of trainees,and to strengthen nursing research and team managing competencies.
6.Clinical and CYP17A1 gene mutation analysis of 6 children with 17α-hydroxylase/17, 20-lyase deficiency

Objective:To analyze the clinical features and CYP17A1 gene mutation of 17α-hydroxylase/17, 20-lyase deficiency (17OHD). Methods:The clinical data, laboratory examination and genetic results of 6 children with 17OHD in the Department of Endocrinology, Children′s Hospital Affiliated to Capital Institute of Pediatrics from March 2014 to December 2019 were enrolled and analyzed retrospectively.Meanwhile, the clinical types of all congenital adrenocortical hyperplasia (CAH) patients were calculated and then the incidence of 17OHD was calculated.Results:The 6 cases were from 5 families, and the age at diagnosis was ranged from 1 year and 6 months to 15 years old, in which 2 cases were 46, XX and 4 cases were 46, XY.Their gender were all female.Three cases presented with hypertension (50.0%), 4 cases with hypokalemia (66.7%), and 1 case with labia mass (16.7%). The gonad developed into a testis in patients with 46, XY, and patients with 46, XX had ovarian hypoplasia.The laboratory tests revealed an decrease in the cortisol at 8 AM in all cases, ranging from 0.62 to 5.93 mg/L.Five cases displayed an increase in adrenocorticotropic hormone (ACTH) in the range of 84-271 ng/L, and 1 patient with normal ACTH (58 ng/L) had a peak cortisol of 1.75 mg/L after the ACTH challenge test.Elevated progesterone was detected in 6 patients with a normal 17 hydroxyprogesterone level.Further results proved low levels of testosterone and estradiol, and high levels of luteinizing hormone (LH), and follicle stimulating hormone (FSH). CT scan showed mild adrenal hyperplasia in all cases.Among 114 CAH patients during the same period, the incidence of 17OHD came second at 5.3%.The CYP17A1 gene mutation results indicated that 2 unrelated patients were homozygous mutation for p. Y329fs (c.985_987delTACinsAA), 2 siblings were compound heterozygous mutations for p. Y329fs and exon 1-7 deletion, 1 patient was compound heterozygous mutations of p. Y329fs and p. R416C (c.1246C>T), and 1 patient was homozygous mutations for p. L465P (c.1394T> C), which was first reported in China. Conclusions:17OHD is not rare in CAH.Female children with hypokalemia, hypertension, and hypogonadism can lead to diagnostic suspicion of 17OHD.The p. Y329fs mutation in Chinese 17OHD children is a hotspot.The p. L465P (c.1394T>C) mutation is a new mutation in China and it could enrich the mutant spectrum of CYP17A1 gene in China.
7.Film analysis algorithm of isocenter error based on Hough transform for the CyberKnife system

Objective:A new algorithm based on Hough transform (HT) was proposed to improve the accuracy and stability of the film image analysis of Automatic Quality Assurance (AQA) test, and to explore the influence of the resolution of film image on the test results.Methods:Nine pairs of films were obtained for AQA modules in this study. Firstly, the median filter was used to preprocess the grayed-out film image to remove noise interference. Then, a global threshold was utilized to binarize the image. The images were edge-detected and the film edge line was extracted by Hough transform. The film image was transformed to the correct position. Finally, the edge of the field shadow circle and the shadow circle of the tungsten ball were extracted by the edge detection method and Hough transform. The radial error was finally obtained by analyzing the concentricity.Results:There was no significant difference in the accuracy between the test results yielded by the HT method and the AQA software ( P>0.05). The difference in the standard deviation of the test results was statistically significant ( P=0.027), indicating that the algorithm increased the stability while ensuring the accuracy of film analysis. Increasing the resolution of film scanning failed to significantly improve the accuracy and stability of film analysis in both two methods. Conclusions:The algorithm used in this study can eliminate the human error caused by film scanning placement while ensuring the accuracy of film analysis, providing a more stable way for the AQA test of the CyberKnife system.
8.Analysis of clinical features and genetic variants among 12 children with Gitelman syndrome.

OBJECTIVE:
To summarize clinical manifestations and results of genetic testing in 12 children with Gitelman syndrome (GS).
METHODS:
Clinical data of the children was collected. Whole exome sequencing(WES) was carried out to screen potential variants of genomic DNA. Candidate variants were verified by Sanger sequencing.
RESULTS:
The patients have included 10 boys and 2 girls, whom were diagnosed at between 2.8 to 15.0 year old. Six patients were due to infections, 5 were due to short stature, and 1 was due to lower limb weakness. All patients were found to carry variants of SLC12A3 gene, which included 11 with compound heterozygous variants and 1 with homozygous variant. All of the 19 alleles of the SLC12A3 gene carried by the patients were delineated, which included 15 missense variants, 2 frameshift variants and 2 splice region variants. These variants were unreported previously, which included c.578_582dupCCACC (p.Asn195Profs*109), c.251C>T (p.Pro84Leu) and c.2843G>A (p.Trp948X).
CONCLUSION
The clinical symptoms of GS in children are atypical and often seen in older children. For children with occasional hypokalemia associated with growth failure, GS should be suspected. The majority of GS children carry two pathogenic variants of the SLC12A3 gene, mainly compound heterozygotes, among which p.Thr60Met is the most common one. The discovery of new variants has enriched the spectrum of SLC12A3 gene variants.

9.Construction and application of virtual simulation experimental teaching platform of nursing

Virtual simulation experimental teaching is a major component of nursing education reform in colleges and universities, which makes space-unlimited cyber-learning and self-learning into reality and is helpful in the development of students' critical thinking and clinical nursing decision-making ability. Based on the "5E learning cycle" model, we have set up an experimental teaching system, and constructed a virtual simulation experimental platform of nursing, which comprises basic experimental skills, clinical experimental skills, comprehensive virtual simulation projects and training system of clinical thinking. The platform combines online learning and offline learning, virtual simulation and physical simulation together, and narrowes the gap between theoretical teaching and clinical practice. This platform has promoted the development of nursing skills, clinical thinking and comprehensive abilities for nursing students and been also ready for the design and development of virtual simulation experimental teaching projects in the future.
10.Efficacy of Rehabilitation on Acalculia after Acquired Brain Injury

Objective To compare the efficacy of different rehabilitation models on acalculia after acquired brain injury. Methods 113 cases were randomly assigned to 3 groups: control group(n=37), computer-assisted training group(n=38) and face-to-face training group(n=38). The control group just received cognitive dysfunction evaluation. The training groups received cognitive rehabilitation training 5 days a week and 30 minutes a day which sustained for 6 weeks. And 33 patients were selected to prolong for 12 weeks. They were evaluated with Revised EC301Calculation and Number Processing Battery in Chinese version (EC301-CR) at the beginning, the 6th week point and the 12th week point respectively. Results 6-week after treatment, The performance of both the computer-assisted training group and face-to-face training group significantly improved(P<0.001); It showed that computer-assisted group>face-to-face group>control group(P<0.001) both 6 weeks and 12 weeks latter. Significant negative correlation was found between age and performance of EC301-CR(P<0.05).Conclusion The effect of computer-assisted training on acalculia is superior to face-to-face training; The first 6 weeks of training is the best period for rehabilitation; The younger the patient is, the better results are.
