1.Treatment of distal clavicle type II fracture: suture anchor versus clavicular hook plate.

OBJECTIVETo compare the efficacy of suture anchor with clavicular hook plate in treatment of type II fractures of the distal clavicle.
METHODSForty patients with Neer type II fractures of distal clavicle were treated in our hospital from May 2009 to May 2010, including 21 cases treated with suture anchors and 19 cases treated with clavicular hook plate. The time of fracture union and Constant Murley scores were compared between two groups.
RESULTSAll incisions reached primary healing, and there was no infection or hematoma formation. Postoperative follow-up time was 12-24 months, mean 14.5 months. The mean time of fracture reunion in suture anchors group was 13.2±2.0 weeks (9-17 weeks) and that in clavicular hook plate group was 13.3±2.0 weeks (10-16 weeks, P>0.05). The mean postoperative Constant Murley scores in suture anchors and hook plate groups were (91.1±6.5)% and (89.2±7.6)%, respectively (P>0.05). But the pain score in suture anchor group was higher than that in hook plate group (12.6±1.9 vs 11.3±2.3, P<0.05).
CONCLUSIONCompared with claviculer hook plate, the suture anchor has good clinical results and less complications in treatment of type II fractures of distal clavicle.

2.Association between neutrophil-to-lymphocyte ratio and suicidal ideation in children and adolescents with first-episode depression

BackgroundSuicide is one of the serious public health problems worldwide. The relationship between suicide and neutrophil-to-lymphocyte ratio (NLR) may vary in different regions and different age. It is necessary to further investigate the relationship between NLR and suicidal ideation in Chinese children and adolescents with depression. ObjectiveTo explore the correlation between NLR and suicidal ideation in children and adolescents with depression, so as to provide clues for exploring the biomarkers of suicide. MethodsA retrospective analysis of 536 children and adolescents with depression who were hospitalized in the Third People's Hospital of Fuyang from January 2020 to December 2022 and met the diagnostic criteria of the International Classification of Diseases, tenth edition (ICD-10) was performed. Patients were divided into two groups according to whether they reported suicidal ideation. Demographic data, discharge diagnosis, Hamilton Depression Scale-17 item (HAMD-17) score and hematological test data (neutrophil counts, lymphocyte counts) on the second day were collected from medical records. Receiver operating characteristic (ROC) curve was used to determine the optimal cut-off point of NLR for predicting suicidal ideation in children and adolescents with depression, and binary Logistic regression was used to analyze the risk factors for suicidal ideation. ResultsAmong the 536 patients, 429 cases (80.04%) had no suicidal ideation, and 107 cases (19.96%) had suicidal ideation. Compared with patients without suicidal ideation, the HAMD-17 score [(25.28±8.86) vs. (21.21±8.46), F=19.400, P<0.01], neutrophil level [(3.85±1.68)×109/L vs. (3.15±1.14)×109/L, Z=4.073, P<0.01], and NLR level [(1.96±1.50) vs. (1.52±0.71), Z=3.532, P<0.01] in the suicidal ideation patients were significantly higher. The optimal critical NLR value determined by the ROC curve was 1.52 (59.80% sensitivity, 58.50% specificity), with an area under the curve of 0.610. Logistic regression analysis showed that the risk of suicidal ideation was 1.94 times higher in those with high NLR than in the low NLR after controlling for age, sex, age at onset, duration of illness, and HAMD-17 score (OR=1.940, 95% CI: 1.251~3.009, P=0.003). ConclusionNLR may be a risk factor and potential biomarker influencing suicidal ideation in the children and adolescents with first-episode depression. 3. Occupational exposure investigation and protective measures in a tertiary infectious disease hospital

Objective:
To investigate the cause of occupational exposure among 136 nurses in a tertiary infectious disease hospital, and puts forward the prevention strategy.
Methods:
A total of 136 nurses exposed to occupational exposure between 2014 and 2016 were included in the study. Analysis was conducted from the years of work of nurses, exposure routes, and the pathogens.
Results:
The nurses suffer from the highest risk of occupational exposures (73.91%) .Nurses working for less than 5 years and interns are most likely to suffer occupational exposure (45.59% and 35.29% respectively) . Occupational exposure was mainly caused by needle injuries, in which infusion was the main route of occupational exposure (36.76%) . The improper treatment of needle pulling after infusion is the main link of needle puncture (36.76%) . Occupational exposure pathogens were mainly HBV (63.24%) .
Conclusion
Nursing staff is the high-risk group of occupational exposure. Irregular operation, lack of awareness of protection, improper disposal after the needle withdrawal and poor safety assessment of the operating environment are the main causes of occupational exposure. It is suggested to strengthen the training of occupational safety and protection, enhance clinical nurses occupational safety protection consciousness, standardize medical operation, so as to prevent the occurrence of occupational exposure.
4.Improvement effect and mechanism of paeoniflorin on glucose metabolism,inflammation and oxidative stress in rats with gestational diabetes mellitus

OBJECTIVE To explore the effect of paeoniflorin on glucose metabolism, inflammation and oxidative stress in rats with gestational diabetes mellitus (GDM) and its potential mechanism based on nuclear factor-erythroid 2-related factor 2 (Nrf2)/ heme oxygenase-1 (HO-1)/nicotinamide adenine dinucleotide phosphate:quinone oxidoreductase 1 (NQO1) signaling pathway. METHODS The female rats fed with high fat and high sugar diet and the male rats fed with an ordinary diet were caged, the successfully conceived rats were collected, and streptozotocin was injected intraperitoneally once to induce the GDM model. The successfully modeled rats were randomly divided into the model group, metformin hydrochloride group (200 mg/kg metformin by gavage), paeoniflorin low-, high-dose groups (45, 90 mg/kg paeoniflorin by gavage, respectively), paeoniflorin+ML385 group (90 mg/kg paeoniflorin by gavage and intraperitoneal injection of 30 mg/kg Nrf2 inhibitor ML385), with 12 rats in each group; in addition, another 12 conceived rats fed with an ordinary diet were selected as the control group. The rats in each drug group were given the corresponding drug/normal saline, once a day, for 2 consecutive weeks. Glucose metabolism indexes [fasting blood glucose (FBG), fasting insulin (FINS), insulin resistance index (HOMA-IR)], serum inflammatory factors [interleukin-6 (IL-6), tumor necrosis factor- α (TNF- α)] and renal tissue oxidative stress indexes [superoxide dismutase (SOD), malondialdehyde (MDA), glutathione peroxidase (GSH-Px)] were detected; the pathological changes of renal tissue were observed, and the protein expressions of Nrf2, HO-1 and NQO1 in renal tissue were detected. RESULTS Compared with the control group, the renal tissue lesions of the model group were obvious, including glomerular atrophy, edema degeneration of renal tubular epithelial cells and a large number of inflammatory cell infiltration; the levels of FBG and FINS, HOMA-IR, the levels of IL-6 and TNF-α in serum, and the level of MDA in renal tissue were significantly increased (P<0.05), while the levels of SOD and GSH-Px and the protein expressions of Nrf2, HO-1 and NQO1 in renal tissue were significantly decreased (P<0.05). Compared with the model group, the renal tissue lesions of rats in paeoniflorin low-dose and high-dose groups were reduced, the above quantitative indexes were significantly improved, and the improvement effect was better in high-dose group (P<0.05), while ML385 could significantly reverse the improvement effect of paeoniflorin on the above indexes (P<0.05). CONCLUSIONS Paeoniflorin can improve the abnormal glucose metabolism, inflammation and oxidative stress damage of renal tissue in GDM rats, which may be related to the activation of Nrf2/HO-1/NOQ1 signaling pathway.
5.Comparative analysis of modified STER and endoscopic submucosal excavation in the treatment of paracardial submucosal tumors

Abstract
To investigate the curative effects of modified submucosal tunnel endoscopic resection(STER) and endoscopic submucosal resection(ESE) in the treatment of paracardial submucosal tumors. Eighty-four patients with paracardial submucosal tumors diagnosed through digestive endoscopy and endoscopic ultrasonography signed informed consent forms, and they were randomly divided into observation group(n=42) and control group(n=42) with the help of the table of random numbers. Patients in the control group received ESE treatment. Patients in the observation group received modified STER surgery.The operation time, average hospitalization time and treatment cost of patients in STER group were(61.32±32.01) min,(8.11±2.42) d and(21.7±3.4) thousand Chinese Yuan respectively, which were better than those in ESE group(87.63±34.09) min,(10.05±2.84) d and(25.9±3.9) thousand Chinese Yuan. The difference was statistically significant(P<0.05). The average number of titanium clips used in the observation group was(5.00±1.37), and in the control group the average number was(4.68±1.25). The difference was not statistically significant. In the STER group, there were 2 cases of intraoperative perforation and 1 case of delayed bleeding. In the ESE group, there were 4 cases of intraoperative perforation and 3 cases of intraoperative uncontrollable bleeding. The incidence of postoperative complications in the STER group was lower than that in the ESE group. The postoperative pathological examination revealed that in both groups the tumors were mostly stromal tumor and leiomyoma. A few of the patients were suffering from lipomas and schwannomas. There was no significant difference in terms of the pathological composition of the patients between the two groups.
6. Analysis of blood-borne occupational exposure of intern nurses in a tertiary infectious disease hospital

Objective:
To explore the risk factors of occupational exposure of intern nurses in an infectious disease hospital, and put forward prevention and control strategies.
Methods:
A retrospective analysis was made of 82 reported cases of occupational exposure of intern nurses in a tertiary infectious disease hospital from 2013 to 2017. The occupational exposure rate, pathogens, occurrence sites and exposure links of the intern nurses were analyzed.
Results:
The overall occupational exposure rate of internship nurses in 2013-2017 was 6.20% (82/1322) .The pathogens were mainly HBV, 52 cases, accounting for 63.4%. The site of occurrence was mainly finger, 75 cases, accounting for 91.5%. In the occurrence of the process, 35 cases were taken, accounting for 42.7%; 20 cases of arteriovenous puncture, accounting for 24.4%; 17 cases of separation needles and finishing materials, accounting for 20.7%.
Conclusion
The incidence of blood-borne occupation of intern nurses is higher, which needs special attention in infectious disease hospitals. To prevent blood-borne occupational exposure of intern nurses, occupational safety education should be strengthened, clinical operation should be standardized and humanized management should be provided.
7.Hemorrhagic transformation after mechanical thrombectomy for acute ischemic stroke and its effect on prognosis

Objective:
To explore the hemorrhagic transformation(HT) and its effect on prognosis in patients with acute ischemic stroke(AIS) after mechanical thrombectomy(MT).
Methods:
A total of 114 patients with AIS received MT were enrolled. The modified Rankin Scale was used to evaluate the clinical outcome at 90 days of onset(0-2 points were good prognosis; 3-6 points were poor prognosis). The patients were divided into HT group(n=25) and non-HT group(n=89) according to their HT conditions. Binomial Logistic regression analysis was performed to determine the vascular risk factors of HT after MT and the effect of HT on prognosis.
Results:
Among 114 patients, there were 25 cases of HT and 89 cases of non HT. The proportion of patients with diabetes in HT group was significantly higher than that in non-HT group. The NIHSS score of HT group at discharge was significantly higher than that in non-HT group. The proportion with good prognosis at 90 days in HT group was significantly lower than that in non-HT group(allP<0.05). Binomial Logistic regression analysis showed that diabetes, high levels of cholesterol and smoking were the major vascular risk factors for HT after thrombectomy(allP<0.05). HT was an important factor affecting the poor prognosis after arterial thrombectomy(P=0.026).
Conclusion
Diabetes, high levels of cholesterol and smoking are the main risk factors of HT after MT for AIS. HT is an independent risk factor for poor prognosis after MT.
8.Expression and clinical significance of vascular and vasculogenic mimicry generation factors VEGFA and MMP⁃14 in lung adenocarcinoma

Objective :
To investigate the expression and clinical significance of classic vascular endothelial growth factor A (VEGFA) and vasotropic mimicry factor matrix metalloproteinase 14 (MMP⁃14) in lung adenocarcinoma(LUAD) .
Methods :
The expression levels of MMP⁃14 and VEGFA genes and proteins in LUAD and their correlation with survival and prognosis were analyzed using TCGA and UALCAN databases . Serum of 69 patients with LUAD and 20 healthy subjects (control group) was collected , and the contents of MMP⁃14 and VEGFA were detected by ELISA and chemiluminescence , respectively , to analyze the correlation between the two and the clinicopathological features of tumors and their value in prediction and diagnosis of LUAD .
Results:
The levels of MMP⁃14 and VEGFA in LUAD tissues and serum were higher than those in control group . There were significant differences in serum VEGFA and MMP⁃14 expression levels among early stage group , advanced stage group and control group (P< 0. 001) . MMP⁃14 was higher in T3 /T4 stage than that in T1 /T2 stage (P = 0. 045) , higher in N2 /N3 stage than that in N0 /N1 stage (P = 0. 035) , and higher in the pleural metastasis group than that in the non⁃pleural metastasis group (P = 0. 034) . VEGFA level was higher in M 1 than M0 (P = 0. 025) . Elevated VEGFA level was a risk factor for LUAD (P = 0. 002) . The area under the curve (AUC) of MMP⁃14 , VEGFA and CEA alone was 0. 793 , 0. 849 and 0. 851 , respectively , and the AUC of the combined test was 0. 952 . The overall survival ( OS) and disease specific survival (DSS) of MMP⁃14 and VEGFA low expression group were longer than those of MMP⁃14 and VEGFA high expression group (P < 0. 05) .
Conclusion
High expression of MMP⁃14 and VEGFA in LUAD is associated with the growth , invasion and metastasis of LUAD , respectively , and has implications for survival prognosis determination .
9.Preparation of Chelerythrine Solid Dispersion and Study on Its Physicochemical Properties and Antioxidant Activity

OBJECTIVE:To prepare Cheler ythrine (CHE) solid dispe rsion (SD),optimize the formulation technology , characterize its preparation and investigate its in vitro antioxidant activity. METHODS :The content of CHE in SD was determined by UV spectrophotometry. Based on single factor tests ,using the product yield as index ,using preparation method ,carrier material type,carrier material proportion (drug-carrier material mass ratio )as factors ,the formulation technology of SD was optimized by L(9 34)orthogonal test and validated. Based on solubility and accumulative dissolution determination ,the product was characterized with thermal analyssis ,X-ray diffraction and scanning electron microscope. Using ascorbic acid as positive control ,in vitro antioxidant activity of the product was determined by DPPH method. RESULTS :The linear range of CHE was 2.4-5.6 μg/mL; quantitation limit and detection limit were 0.066 9,0.022 1 μg/mL;RSDs of precision ,stability and reproducibility tests were all lower than 2%;recoveries were 97.50%-99.25%(RSD<1%, n=3). The optimal preparation technology included using PEG 6000 as carrier material ,carrier material ratio of 1 ∶ 3, prepared by solvent method. Three batches of CHE-PEG-SD were prepared. Verification test results showed that the 话:0539-80311889。E-mail:zhenshengao@163.com accumulative dissolution of CHE-PEG-SD was (61.72 ± 0.67)% at 15 min,and the yield was (99.04±0.83)%. The results of characterization showed that after CHE-PEG-SD prepared , its solubility (3.725 mg/mL)and accumulative dissolution (61.25%,15 min)were higher than CHE raw material [ 0.098 mg/mL, 6.24%(180 min)]. The endothermic peak and crystal absorption peak moved or even disappeared compared with raw material and the carrier material ,and CHE was uniformly dispersed in the carrier material as an amorphous state. Results of in vitro antioxidation test showed that different concentration of CHE-PEG-SD showed certain ability of DPPH free radical scavenging ,and the IC 50 was 0.124 mg/mL,higher than 0.041 mg/mL of ascorbic acid. CONCLUSIONS :Established content determination method is simple and accurate. The optimal SD formulation technology is stable and feasible. The solubility of prepared CHE-PEG-SD increases,and the dissolution in vitro increases,showing certain in vitro oxidation resistance.
10.Inhibition of glutamatergic neurons in the dorsomedial periaqueductal gray alleviates excessive defensive behaviors of mice with post-traumatic stress disorder

Objective To investigate the role of glutamatergic neurons in the dorsomedial periaqueductal grey(dmPAG)in regulating excessive defensive behaviors in mice with post-traumatic stress disorder(PTSD).Methods Eight-week-old male C57BL/6 mice were subjected to stereotactic injections of different recombinant adeno-associated viral vectors(rAAV2/9-CaMKⅡ-mCherry,rAAV2/9-CaMKⅡ-hM3Dq-mCherry and rAAV2/9-CaMKⅡ-hM4Di-mCherry)into the bilateral dmPAG for chemogenetic activation or inhibition of the glutamatergic neurons,followed 2 weeks later by PTSD modeling by single prolonged stress.The looming test,response to whisker stimulation test and contextual fear conditioning(CFC)test were used to observe changes in defensive behaviors of the PTSD mice.The activity of glutamatergic neurons in the dmPAG were observed using immunofluorescence staining.Results Compared with the control mice,the mouse models of PTSD showed a shortened latency of flights with increased time spent in the nest,response scores of defensive behaviors and freezing time(all P<0.01).Immunofluorescence staining revealed significantly increased c-fos-positive glutamatergic neurons in the dmPAG of PTSD mice with defensive behaviors.Activation of the glutamatergic neurons in the dmPAG(in PTSD hM3Dq group)did not cause significant changes in the latency of flights or time in nest but obviously increased response scores of defensive behaviors and freezing time of the mice,whereas inhibiting the glutamatergic neurons in the dmPAG(in PTSD hM4Di group)caused the reverse changes and obviously alleviated defensive behaviors in the PTSD mice(P<0.05 or 0.01).Conclusion Inhibiting the activity of glutamatergic neurons in the dmPAG can alleviate defensive behaviors in mice with PTSD.
