1.The Observasion of Heparin-induced Thrombocytopenia During Blood Purification

Objective To observe the occurring time of heparin-induced thrombocytopenia,resuming time and effects of treatment during continuous hemofiltration.Methods To select 9 cases of patients with continuous veno-venous hemofiltration(CVVH)hospitalized in Luohe centre hospital,70 cases of patients with hemoperfusion and 20 hemodiaglysis cases of patients with acute and chronic renal failure during 2005,1 to 2006,1,blood samples were collected from the patients,platelet was tested before and after the treatment,the bleeding symptoms and effects of treatment were observed.Results There was 1 case during the 27th hour of CVVH,the lowest platelet was 10?10~9/L,resumed 48h hours after stoping heparin;there was 1 case during the first hour of hemoperfusion,the lowest platelet was 67?10~9/L,resumed 60h hours after stoping heparin.Conclusions It is essential to observe the changes of disease condition closely,to test platelet and whenever necessary to give cortex hormone,hemostatics and interrupted using platelet transfusion.
2.Use of laryngeal mask airway ProSeal for airway management during open heart surgery performed under CPB in children

Objective To investigate the use of laryngeal mask airway (LMA) ProSeal for airway management during open heart surgery performed under CPB in children. Methods Seventy-six ASA Ⅱ and NYHA class Ⅰ or Ⅱ patients aged 3 months-8 yr, weighing 3.3-34.5 kg undergoing open heart surgery under CPB were randomly divided into 2 groups ( n = 38 each): tracheal intubation group (group T) and ProSeal LMA group (group P1). Tracheal tube and LMA were inserted after induction of anesthesia with 8% sevoflurane. The rate of successfultracheal intubation and LMA placement, placement time, peak airway pressure and side effects during and after surgery including hypoxemia, tachycardia, bradycardia, hypotension and hypertension, laryngesl edema, dysphagia, bucking, dyspnea and hoarseness were recorded. Results There were no significant differences in the rate of successftl tracheal intubation and LMA placement, peak airway pressure, bucking, dyspnea and hoarse voice between the two groups (P> 0.05). The LMA placement time was significantly shorter than tracheal intubation time and the incidence of laryngeal edema and dysphagia lower in group P than in group T ( P < 0.05). Conclusion The LMA ProSeal can provide adequate ventilation during operation with less complications and can be used effectively for cardiac surgery performed under CPB in children.
3.Evaluation of Crohn's disease treatment efficacy with CT enterography

Objective To explore the value of CT enterography(CTE)in the clinical treatment efficacy evaluation of Crohn's disease(CD).Methods A total of 55 patients with CD confirmed by clinical and pathology were selected.All patients both underwent CTE and enteroscopy before and after treatment.After treatment,the patients were divided into the good curative effect group and the poor curative effect group according to Crohn's disease activity index(CDAI).The CT findings were both analyzed before and after treatment.Results There were significant differences in bowel wall thickness,bowel wall attenuation,bowel wall thickness type and"comb sign"of 55 CD patients before and after treatment(P<0.05).After treatment,phlegmon,abscess,swollen lymph node enlargement,and intestinal fistula all decreased from 18%,7%,27%,and 2%to 2%,2%,9%,and 0%,respectively.Also,there were significant differences in bowel wall thickness,bowel wall attenuation,bowel wall thickness type and"comb sign"between the good curative effect group and the poor curative effect group(P<0.05).Logistic regression analysis and receiver operating characteristic(ROC)curve analysis showed bowel wall thickness,bowel wall attenuation and bowel wall thickness type were the most valuable parameters for the treatment efficacy evaluation of CD patients.Conclusion CTE can fully show intestinal lesions and extrenteral complications,and is of great value in evaluating clinical treatment efficacy of patients.
