1.Epidemiological investigation on the prevalence of bladder hyperactivity in Huzhou,Zhejiang province

Objective To investigate the prevalence of bladder hyperactivity in Huzhou city,Zhejiang prov-ince.Methods 1 872 patients aged over 50 years old male residents were selected as research subjects in Huzhou. To investigate the prevalence of bladder hyperactivity in the elderly male population in the local area.The correlation between age,IPSS score,body mass index,diabetes and the bladder was investigated.Results In 1 872 cases,the data were complete and in accordance with the requirements of this study were 1 863 cases.1 863 cases,505 patients with bladder hyperactivity,bladder disease prevalence was 27.1%.Bladder hyperactivity patients with IPSS score (17.9 ±8.1 )points,prostate volume (36.6 ±18.5 )mL,QOL score (3.6 ±1.1 )points,residual urine volume (26.9 ±3.5 )mL in the bladder,were higher than those with non bladder hyperactivity,IPSS score (3.2 ± 16.8)points,prostate volume (28.1 ±1.3)mL,QOL score (2.3 ±1.8)points,the differences were statistically sig-nificant (t =7.277,5.910,19.814,2.406,P <0.01 -0.05).Qmax(12.6 ±6.3)mL/s in the patients with bladder hyperactivity was significantly lower than Qmax(17.1 ±7.4)mL/s of non bladder hyperactivity,and the difference was statistically significant (t =3.577,P <0.01).With the increasing age of male population,the prevalence of blad-der diseases was increased.With the increase of IPSS score,the incidence rate of bladder excessive activity increased. Diabetes,higher body mass index,the incidence of bladder excessive activity was higher.Conclusion Men with blad-der hyperactivity disorder has higher prevalence in Huzhou city,Zhejiang province.Age,lower urinary tract symptoms, diabetes,obesity are the risk factors in the incidence of bladder disease.
2.Effect of mediastinal drainage on treatment of intra-thoracic anastomotic leak in post operation of e-sophageal cancer patients

Objective To explore the effect of mediastinal drainage on the treatment for intra -thoracic anastomotic leak in esophageal cancer patients after esophagectomy .Methods One hundred and thirty -four e-sophageal cancer patients underwent esophagectomy and a mediastinal drainage tube was routinely placed intro -operatively ( observation group ) .Other 150 esophageal cancer patients underwent esophagectomy without mediasti -nal drainage were retrospectively set as control group .The following factors in the two groups were compared:in-cidence of anastomotic leak,and mortality rate,incidence of respiratory failure,incisional infection,rate and dura-tion of moderate or high fever ( T≥38℃) , duration of antibiotic use , duration of anastomotic leak healing and length of hospital stay of anastomotic leak patients .Results There was no significant difference in incidence rate of anastomotic leak between observation and control groups .There was no death ,no respiratory failure in observa-tion group;rate of chest incisional infection in observation group was 18.2%(2/11).Mortality rate,respiratory failure rate and chest incisional infection rate in control group were 33.3%(3/9)、44.4%(4/9)and 77.8%(7/9)respectively.Incidence and duration of moderate or high fever (36.4%and 2.3 ±1.2d respectively)in obser-vation group were significantly lower than those in control group (100 % and 8.6 ±2.3 d respectively)(P<0.05).Duration of antibiotic use,duration of leak healing and length of hospitalization (9.6 ±3.2 d,23.6 ±5.5 d and 22.6 ±5.7 d respectively)were significantly shorter than those in control group (21.3 ±6.8 d,38.3 ±8.4 d and 38.5 ±9.6 d,P<0.05)respectively).Conclusion Although mediastinal drainage could not prevent anasto-motic leak in patients underwent esophagectomy ,it could definitely decrease death and respiratory failure resulted from anastomotic leak .Mediastinal drainage could also decrease severity of intrathoracic infection caused by anas -tomotic leak and shorten the duration of leak healing .
3.Relationship Between Syndrome Patterns of Traditional Chinese Medicine and Apoptosis- Related Oncogene mRNA Expressions in Precancerous Lesions of Gastric Cancer

[Objective] To study the relationship between syndrome patterns of traditional Chinese medicine and bcl- 2 oncogene, p53 suppression gene mRNA expressions in precancerous lesions of gastric cancer (PIGC). [Methods] Forty PLGC cases were endoscopically and pathologically confinned, including 24 cases of moderate dysplasia of gastric mucosa, 9 cases of severe dysplasia, 7 cases of incomplete colonic intestinal metaplasia. Of the 40 cases, 10 were complicated with Qi stagnation, 12 with stomach-heat, and 18 with blood stasis, mRNA expression of bcl- 2 oncogene and p53 suppression gene were detected in situ by molecular hybridization method. [Results] The mRNA overexpression of bcl- 2 oncogene and p53 suppression gene were found in PLGC, and the expression was gradually increased with the progress of lesions. In the complicated cases, the mRNA expression of bcl- 2 oncogene and p53 suppression gene were the least in the cases with Qi stagnation and less in the cases with stomach- heat than in the cases with blood stasis. [Conclusion] Abnormal mRNA expression of bcl- 2 oncogene and p53 suppression gene were found in PLGC cases and related with different complicated cases, indicating the specificity of different syndrome patterns.
4.Comparison between Gamma nail and DHS/Richard in treatment of proximal femoral fracture: a meta analysis

Objective To asses the effects of Gamma nail and DHS/Richard (dynamic hip screw) in the treatment of proximal femoral fractures. Methods A meta analysis of all the relevant randomized controlled trials (RCTs) was performed. We included randomized and quasi randomized controlled trials in patients with proximal femoral fracture to compare Gamma nail and DHS/Richard. Results First we identified 88 papers on comparison of Gamma nail and DHS/Richard in the treatment of proximal femoral fractures published from 1969 to 2003. 7 trials involving 1256 patients were identified as meeting all the eligibility criteria. 3 investigators independently graded study quality and abstracted relevant data, including information on mortality rates, wound infection, function, revision in patients with a proximal femoral fracture. 4 trials, which included a total of 621 patients, provided detailed information on mortality rates over the first 6 postoperative months. We found there was no significant difference in the relative risk of death in the first 6 months postoperative between treatments of Gamma nail and those of compression hip screw (relative risk 1.17;P=0.51). 6 trials that included a total of 1083 patients provided data on operative complications. The risk of operative complications from Gamma nail fixation appeared to be higher than that from compression screw and side plate fixation but not higher than that from compression hip screw (relative risk 1.41; P=0.02). We also found an obvious increase in the relative risk of fracture of femoral shaft between Gamma nail and compression hip screw (relative risk 6.99; P=0.00). Patients treated with Gamma nail had a higher rate of revision compared with those with compression hip screw, but there was no significant difference between the two groups (relative risk 1.85; P=0.20). In addition, wound infection, operative blood loss and functional recovery were similar between the tow groups(relative risk 0.98 for wound infection and 1.02 for function). 5.Coronary artery stenosis: comparison study of CT angiography with 16-detector rows scanner and convention angiography

Objective To evaluate the accuracy of 16-detector row computed tomography coronary angiography in detection of hemodynamically relevant stenosis(≥50%) of the coronary artery. Methods Twenty-six patients undergoing both multi-detector row CT and conventional coronary angiography were included in this retrospective study. CT scan were obtained with collimation of 16 and rotation time of 500 ms. Retrospective electrocardiographic(ECG) gating was used for image reconstrution. The stenosis degree was detected with axial scanning and three-dimensional reformation. Results With the results of conventional coronary angiography as golden standard, the sensitivity, specificity, accuracy, positive predicted value and negative predicted value of the CT angiography in detection of hemodynamically relevant stenosis(≥50%) were 90.0%, 95.5%, 94.7%, 77.6%, and 98.2% respectively. Conclusion Sixteen-detector row computed tomography coronary angiography has high clinical value with evaluating hemodynamically relevant stenosis(≥50%) of coronary artery,and could be a significant guide for diagnosis and therapy of coronary heart disease.
6.Synthesis and Pharmacological Activities of a Quercetin Derivative

Objective:To preliminarily explore the synthesis of a quercetin derivative 3′,4′,5,7-four-(O-methoxy carbonyl meth-yl) quercetin and its pharmacological activities. Methods:Quercetin as the reactant and N,N-dimethyl-formamide ( DMF) as the sol-vent, the target product 3′,4′,5,7-four-(O-methoxy carbonyl methyl) quercetin was obtained by the slow addition of methyl chloroace-tate in the presence of anhydrous K2 CO3 to introduce ether bond at 3′,4′,5,7- bit. The structure was characterized by LC-MS, 1 H-NMR and element analysis. The nanoemulsion of the product was prepared using a film dispersion method, and with intraperitoneal in-jection, the effect on pituitrin-induced myocardial ischemia cardiovascular system in rats was observed. Results:3′,4′,5,7-Four-(O-methoxy carbonyl methyl) quercetin was successfully synthesized, and could be metastasized to a demethylation product containing dis-tal free carboxyl with increased polarity proved by metabolic tests in vitro. The results of electrocardiogram and animal experiments showed that the compound had improving effects on pituitrin-induced myocardial ischemia in rats. Conclusion: The nanoemulsion of 3′,4′,5,7-four-(O-methoxy carbonyl methyl) quercetin with intraperitoneal injection shows significant antagonism against pituitrin-in-duced myocardial ischemia in rats.
7.Isolation, identification and anticancer activity of an endophytic fungi from Juglans mandshurica.

The endophytic fungus named FSN006 was isolated from the inner bark of Juglans mandshurica. It grew quickly and formed circular colony on PDA plate. The upper side of the colony was white, while the lower side of the colony and the conditioned medium were light yellow as a result of significant yellow pigment substances were produced and secreted by the fungi. Green elliptic conidia appeared when cultured on CMX plate. Based on the morphology identification and ITS sequence, it was clear that this fungus belonged to the Deuteromycotina, HyPhomycetes, Moniliales, Trichoderma longibrachiatum. The conditioned medium of FSN006 showed a high anti-tumor ability against liver cancer cell-HepG2, and reached its IC50 concentration after being diluted 20 times, while the IC50 concentration of curcumine was(11.49 +/- 0.12) mg x L(-1). In addition, there was preeminent selective inhibiting effect against the normal liver cell strain HL-7702 and its caner counter strain HepG2. The inhibiting effect against strain HL-7702 was only one quarter of that against HepG2 at the concentration of IC50. Therefore, the fermentation of FSN006 may provide a possible way to produce anticancer drug with higher efficiency and lower toxicity.

8.Predictive value of OCT parameters to prognosis of anti-VEGF therapy for macular edema secondary to retinal vein occlusion

Macular edema secondary to retinal vein occlusion (RVO) is the leading cause of visual impairment in patients with RVO.Intravitreal injection of anti-vascular endothelial growth factor (VEGF) agents is the first-line treatment for RVO and most of patients can obtain improvements of macular morphology and visual function.Optical coherence tomography (OCT) can clearly characterize the macular microstructure.Qualitative or quantitative analysis of histological retinal layers of the macula, before and after anti-VEGF treatment, can guide the clinical treatment and predict visual outcome of patients with RVO.OCT predictors of visual prognosis in patients with RVO following anti-VEGF treatment, including central retinal thickness, disruption of external limiting membrane and ellipsoid zone, hyperreflective foci, disorganization of the retinal inner layers, serous retinal detachment or intraretinal cystoid spaces, subretinal hemorrhage, vitreomacular adhesion and retinal pigment epithelium (RPE) integrity are focused on in this review, aiming to enhance patient counseling and improve risk assessment and advance clinical management.
9. miR-34a inhibits proliferation of prostate cancer LNCaP cells by regulating androgen receptor gene

AIM: To explore the effect of microRNA-34a (miR-34a) on androgen receptor gene (AR) targeting regulation and proliferation and apoptosis of prostate cancer (PCa) LNCaP cells. METHODS: Thirty-six patients with PCa confirmed by prostate biopsy in urology department of our hospital were collected from October 2016 to September 2019, and 41 cases of benign prostatic hyperplasia (BPH) tissue samples were taken and operated at the same time. LNCaP cells were cultured in vitro and transfected with miR-34a mimics (mimics group), miR-34a mimic NC (NC group) respectively, and normal growth cells were set as blank control group (BC group). In addition, on the basis of mimics group, the AR over-expression vector (AR over-expression group) and its control group (AR control group) were transfected. The cell activity was detected by CCK-8 kit, the apoptosis rate was detected by Annexin V-FITC/PI double staining flow cytometry test kit, the expression of miR-34a and AR mRNA was detected by real-time quantitative PCR (RT-qPCR), the expression of AR protein, CyclinD1, c-Myc and apoptosis-related proteins (Bcl-2, Bax, capase-3) was detected by Western blotting. RESULTS: Compared with BPH tissues, the expression of miR-34a in PCa tissues decreased significantly (P<0.05), and the expression of AR mRNA and protein increased significantly (P<0.05). Compared with BC and NC group, miR-34a expression level, apoptosis rate and Bax protein expression level of LNCaP cells in mimics group increased significantly (P<0.05), the expression levels of AR, Cyclin D1, c-Myc and Bcl-2 decreased significantly (P<0.05), the activity of LNCaP cells decreased significantly at the same time (P<0.05). The dual-luciferase assay showed that miR-34a may have a direct targeting relationship with AR. Over-expression of AR gene could reverse the inhibition of miR-34a mimics on LNCaP cell proliferation and promote apoptosis. CONCLUSION: miR-34a may inhibit the proliferation and promote the apoptosis of prostate cancer LNCaP cells by regulating AR gene.

Objective:To investigate the diagnostic accuracy of serological indicators and evaluate the diagnostic value of a new established combined serological model on identifying the minimal hepatic encephalopathy (MHE) in patients with compensated cirrhosis.Methods:This prospective multicenter study enrolled 263 compensated cirrhotic patients from 23 hospitals in 15 provinces, autonomous regions and municipalities of China between October 2021 and August 2022. Clinical data and laboratory test results were collected, and the model for end-stage liver disease (MELD) score was calculated. Ammonia level was corrected to the upper limit of normal (AMM-ULN) by the baseline blood ammonia measurements/upper limit of the normal reference value. MHE was diagnosed by combined abnormal number connection test-A and abnormal digit symbol test as suggested by Guidelines on the management of hepatic encephalopathy in cirrhosis. The patients were randomly divided (7∶3) into training set ( n=185) and validation set ( n=78) based on caret package of R language. Logistic regression was used to establish a combined model of MHE diagnosis. The diagnostic performance was evaluated by the area under the curve (AUC) of receiver operating characteristic curve, Hosmer-Lemeshow test and calibration curve. The internal verification was carried out by the Bootstrap method ( n=200). AUC comparisons were achieved using the Delong test. Results:In the training set, prevalence of MHE was 37.8% (70/185). There were statistically significant differences in AMM-ULN, albumin, platelet, alkaline phosphatase, international normalized ratio, MELD score and education between non-MHE group and MHE group (all P<0.05). Multivariate Logistic regression analysis showed that AMM-ULN [odds ratio ( OR)=1.78, 95% confidence interval ( CI) 1.05-3.14, P=0.038] and MELD score ( OR=1.11, 95% CI 1.04-1.20, P=0.002) were independent risk factors for MHE, and the AUC for predicting MHE were 0.663, 0.625, respectively. Compared with the use of blood AMM-ULN and MELD score alone, the AUC of the combined model of AMM-ULN, MELD score and education exhibited better predictive performance in determining the presence of MHE was 0.755, the specificity and sensitivity was 85.2% and 55.7%, respectively. Hosmer-Lemeshow test and calibration curve showed that the model had good calibration ( P=0.733). The AUC for internal validation of the combined model for diagnosing MHE was 0.752. In the validation set, the AUC of the combined model for diagnosing MHE was 0.794, and Hosmer-Lemeshow test showed good calibration ( P=0.841). Conclusion:Use of the combined model including AMM-ULN, MELD score and education could improve the predictive efficiency of MHE among patients with compensated cirrhosis.
