1.Clinical observation on the treatment of breast cancer with traditional Chinese and western medicine

Objective To explore the influence of breast cancer treated with chemo-therapy and Shengxue decoction.Methods 91 Cases of breast cancer were randomly recurited into two groups:The western medicine treatment group (40 patients) was given anastrozole tablets and chemo-therapy; The Chinese and western medicine treatment group (51 patients) was given chemo-therapy and Shengxue decoction.Results The total effective rate of was 82% and 53% in the Chinese and western medicine treatment group and western medicine treatment group respectively,the difference was statistically significant (x2=19.74,P<0.05).The peripheral blood's CEA and MMP-2 mRNA expression of western medicine treatment were [ (7.50 ±4.32)ng/ml、(116.32±67.40)ng/ml respectively],higher than the Chinese and western medicine treatment group [ (7.20 ± 3.22) ng/ml、(105.08 ± 37.82) ng/ml,respectively,P< 0.05 ],but the TIMP-2/MMP-2 (26.65 ±19.75) was significantly lower compared with the Chinese and western medicine treatment group [ (39.48±21.21) (P<0.05) ].Conclusion The combination of traditional Chinese and western medicine was effective in treatment of breast cancer and well worth for popularization.
2.Correlation between peripheral nerve injury and evoked electroneurographic changes in early stage

BACKGROUND: The injury of peripheral nerve may cause electroneurographic changes. However, what is the correlation between electroneurographic changes and the extent of nerve injury?OBJECTIVE: To probe into the changes of evoked electroneurography in early stage after nerve injury for clinical application and reference.DESIGN: Paired t-test in three groups of samples.SETTING: Department of Traumatic Orthopaedics People' s Hospital of Beijing University.MATERIALS:The trial was conducted in the Animal Center of the People's Hospital of Beijing University from August 2003 to December 2003. The subjects were 8 adult New Zealand rabbits of SPF grade(weighing 1.5 kg) and either gender from the Animal Center of Medical College of Beijing University.METHODS:The model of nerve injury was made in the 16 tibial nerves of the 8 rabbits, in which one third or two thirds of each nerve was ligated at the same transverse part. The samples were thus divided into three groups: non-injury (normal) group, 1/3 nerve injury group and 2/3 nerve injury group.MAIN OUTCOME MEASURES: The indices of evoked electroneurography, i. e. motor nerve condution velocity, negative M wave amplitude, negative M wave area, maximal M wave amplitude and maximal M wave area were measured in the three groups. The correlation between the extent of nerve injury and the 5 indices was analyzed.RESULTS: All the 5 indices of the 3 groups were significantly different( P< 0.05). The mean values of the 5 indices were 1.00 ±0.0,0.97 ±0.18,1.01 ±0.22;0.98 ±0.16 and 0.98 ±0.19 in normal group, 0.76 ±0.12,0.41 ± 0.24, 0.47 ± 0.30, 0.48 ± 0.27 and 0.50 ± 0.29 in 1/3 nerve injury group, and 0. 62±0. 10, 0. 11 ±0. 11, 0. 11 ±0. 10, 0. 12 ±0. 14 and 0.13± 0. 12 in 2/3 nerve injury group. The coefficient of correlation between the 5 indices and the injury were 0. 902, 0. 938, 0. 936, 0. 907, and 0. 914, respectively.CONCLUSION: In the early stage after peripheral nerve injury, the electroneurographic indices negatively correlate with the extent of nerve injury. The negative M wave amplitude and area are relatively more closely correlated with the extent of injury.
3.Hypoxia can induce EMT and promote invasion of breast cancer cells

Objective To study the effect of hypoxia on EMT molecule E-cadherin, Vimentin and invasion of human breast cancer MCF-7 cells, reveal the mechanism of breast cancer invasion and metastasis and provide experimental and theoretical basis. Methods Western blot was used to observe the change of HIF-1α, E-cadherin and Vimeutin during hypoxia on MCF-7. MTT was used to study the effects of viability.Transwell chamber was used to detect the ability of invasion and metastasis. Results The expression of E-cadherin was significantly lower (0.09±0.02)(t=30.98,P=0.0007) and the expression of Vimentin was significantly higher (0.69±0.04) (t=915,P=0.0000) with the extension of hypoxia.The capability of adhesion (81.23±0.74) (t=82.05,P=0.000),invasion(120±6) (t=22.78,P=0.0009) and migration(190±6) (t=23.49,P=0.000)was significantly increased after 72 h hypoxia(P<0.05).Conclusion Hypoxia can downregulate E-cadherin and upregulate Vimentin and enhance the adhesion,invasion and migration of MCF-7.
4.Clinical value and safety of antiarrhythmic drugs in the treatment of atrial fibrillation

Objective To investigate the clinical value and safety of cedilanid, esmolol in the treatment of atrial fibrillation. Methods From August 2014 to August 2016 our Hospital from 117 patients with atrial fibrillation clinical data, according to the random number distribution principle, the patients were divided into observation group 59 cases and control group of 58 cases, all patients were given the treatment of primary disease, the clinical symptoms, the patients in the observation group were given oxygen, has given furosemide, cedilanid, after micro injection pump intravenous nitroglycerin, 5-20 g/min. Start after the injection of nitroglycerin, establish another vein channel, every 30 min to 0.2 mg patients, the treatment group were treated with intravenous injection of small dose esmolol. The clinical efficacy, ventricular rate, systolic blood pressure, diastolic blood pressure and adverse reactions were observed in two groups. Results The early and late effective rates of the two groups were not significantly different. The observation showed that after treatment, the ventricular rate, systolic pressure and diastolic pressure in the observation group were significantly higher than those in the control group (P<0.05), and the incidence of adverse reactions in the two groups was significantly different (P<0.05). Conclusion High dose cedilanid combined with small dose esmolol in treatment of atrial fibrillation, obvious curative effect, high safety, can choose the appropriate application.

Objective:
To evaluate the relevance of diabetes mellitus and hepatitis B virus(HBV) infection in people by Meta-analysis.
Methods:
Databases we searched included CNKI, VIP China Science and Technology Journal Database, Wanfang Data Knowledge Service Platform, PubMed, Cochrane Library and Web of Science Core Collection database. Objective To evaluate the feasibility and clinical effect of stage Ⅰ posterior vertebral corpectomy filled with titanium mesh cages plus long-segmental instrumentation for treatment of Denis type D and E severe unstable thoracolumbar burst fracture.Methods A retrospectively review was made on 14 patients with Denis type D and E severe unstable thoracolumbar burst fracture,having had posterior vertebral corpectomy,titanium mesh bone grafting and long-segmental pedicle screw fixation.Parameters recorded were operation time,amount of bleeding,bone fusion and reduction,postoperative complications,low back pain,neurological performance,restoration and loss of anterior vertebral height,correction and loss of kyphosis,and intraspinal space occupying lesion.Results Mean operation time was 207.1 min (range,148-306 min) and blood loss was 585 ml (range,300-1,500 ml).Intraoperative fracture reduction was satisfactory.Follow-up ranged from 18 to 54 months (mean,28 months),which showed no complications of infection,screw-rod breakage,loosening or shifting of the internal fixation device,titanium mesh subsidence and pseudarthrosis,and no signs of adjacent segment degeneration.Six months after operation,CT scan showed bony fusion of the bone-implant interface.At the final follow-up,the Denis pain scale improved significantly including 12 patients with completely relieve of pain (P1) and 2 ache slightly but no need of taking painkiller (P2).Neurological functions were improved by 1-3 degree in all patients.Percentage of anterior vertebral height was (41.2 ± 8.9)% before operation,significantly reduced to (8.3 ± 4.8) % one week after operation and to (8.9 ± 5.1) % at the final follow-up (P <0.01).Mean loss of anterior vertebral height was 0.6%.Sagittal kyphotic angle was (36.9 ± 4.9) °before operation,significantly reduced to (8.1 ± 3.4) ° one week after operation and (8.5 ± 3.8) °at the final follow-up (P <0.01).Mean loss of kyphotic angle was 0.9°.Ratio of intraspinal space occupancy was (74.9 ± 11.3) % before operation but recovered to (4.1 ± 1.6) % one week after operation and (1.8 ± 1.4) % at the final follow-up,with significant differences in pair comparison (P < 0.01).Conclusion The use of posterior vertebral corpectomy and fusion with titanium mesh cage fusion in conjunction with long-segment instrumentation are effective for spinal canal decompression,fracture reduction,three-column reconstruction as well as rigid fusion at one stage,lumbar pain relief,neurological function recovery,prevention of correction loss and vertebral height restoration.
7.Recombined adenovirus expressing P53 in the treatment of malignant pleural effusion with lung cancer

Objective To assess the therapeutic efficacy of a recombined adenovirus expressing p53 (rAd-p53) via intrapleural injec-tion in the treatment of lung cancer with malignant pleural effusion. Methods Thirty-six cases with lung cancer and malignant pleural effu-sion were randomly divided into two groups,which were given intravenous injection of Nedaplatin with (observation group,n=20) or without (control group,n=16) intrapleural injection of rAd-p53,respectively. Between the two groups,the efficacy in treatment of pleural effusion, the amelioration of maximal ventilatory volume ( MVV) ,Kamofsky scoring ( KPS) and quality of life were compared. Results The efficacy in treatment of pleural effusion in observation group are significantly higher than that in control group(17/20 vs. 50%,P<0. 05). The cases with KPS≥80 in observation group were significantly increased following treatment (5/20 vs. 11/20,P <0. 05). However,there was no difference with the cases in control group. Conclusion Intrapleural injection of recombinant adenovirus expressing p53 (rAd-p53) is effec-tive to reduce the occurrence of malignant pleural effusion and increase the quality of life remarkably.
8.High-frequency ultrasonic screening method and characters of plantar metatarsal levelin normal adults

Objective To investigate high-frequency ultrasonic characters and screening method of plantar metatarsal level in normal adults.Methods Plantar metatarsal level were examined with high frequency ultrasound in 40 normal adults.The base,body and head of the first to the fifth metatarsal were used as anatomical location markers.The ultrasonic characters and screening method were described and recorded.Results The plantar skin,plantar fat pad,plantar aponeurosis and deep muscular structures,including the flexor digitorum brevis,abductor hallucis,abductor digiti minimi,lumbricalis,felxor hallucis brevis,flexor digiti minimi brevis,abductor hallucis and plantar interossei were illuminated in detail with high-frequency ultrasound.Conclusion High frequency ultrasound can be used to depict anatomical structures of plantar metatarsal level.
9.Pathogenic mechanisms of congenital abnormalities of kidney and urinary tract

Congenital abnormalities of the kidney and urinary tract (CAKUT) are common congenital malformations identified by prenatal ultrasound. This review summarizes the fetal renal development, the mechanism of CAKUT, and the influence of the fetal environment on CAKUT. CAKUT can manifest as different degrees of renal disease, from transient hydronephrosis to severe bilateral renal dysplasia, a major risk factor for chronic and end-stage renal disease in childhood. Genetic factors and abnormal fetal environment can both contribute to CAKUT. Ultrasound screening is conducive to detect CAKUT but may miss some defects. Monogenic mutations identified in CAKUT can help us gain more insight into the molecular mechanisms of renal development.
10.Validity and reliability evaluation of Chinese version of Glasgow antipscyhotics side-effect scale based on the experience of schizophrenia patients

Objective:To evaluate the reliability and validity of Chinese version of Glasgow antipsychotics side-effect scale (GASS).Methods:Totally 200 patients diagnosed as schizophrenia following the criteria of DSM-IV were enrolled in the study.Cronbach’s α was used to investigate the internal consistency of GASS.Exploring factor analysis was used to examine the construct validity.Pearson correlation analysis between GASS and TESS and CGI sub-scale of side effect were conducted to evaluate the criterion validity.Based on clinical criteria, the sensitivity and specificity and ROC curve was calculated so as to establish the cut-off point of the scale and test discrimination validity.Results:For reliability, Cronbach’s α ws 0.926, indicating that GASS had a good internal consistency.Four main factors were extracted by exploratory factor analysis which were extrapyramidal side effect factor, genitourinary side effect factor, anticholinergic side effect factor, over sedation and cardiovascular side effect factor.For validity, exploring analysis demonstrated that nineteen item of the scale has relatively higher factor load on the principal factor (0.531-0.972), suggesting that the scale had a good construct validity.The total score of the GASS was positively correlated with that of TESS and CGI subscale of side effects( r=0.90, P<0.05; r=0.87, P<0.05). The cut-off point of experimental validity of GASS for mild antipsychotics side effects was determined as ≥1.With this cut-off point, GASS had both high sensitivity (96.3%) and high specificity (70.7%), area under curve (AUC) were 0.78.The cut-off point of GASS for moderate antipsychotics side effects was determined as ≥20, with the sensitivity and specificity of GASS were 86.3% and 84.7%, AUC were 0.92.When the cut-off point of GASS for severe antipsychotics side effects was determined as ≥41, GASS had both high sensitivity (92.1 %) and high specificity (96.3 %), AUC were 0.96. Conclusion:GASS has good reliability and validity and can reflect the side effects of antipsychotics, and it can be adopted as a useful instrument to evaluate the severity of side effects of antipsychotics.
