1.Comparative study of analgesic effect of simple lidocaine and ropivacaine-plus in inguinal hernia tension-free repair in adults

Objective To investigate the analgesic effect of lidocaine plus ropivacaine in inguinal hernia tension-free repair under local anesthesia.Methods A total of 815 patients with unilateral inguinal hernia admitted to Chaoyang Hospital from July 2013 to Jane 2014 were enrolled in this retrospective study.The patients were divided into 2 groups:local anesthesia using a combination of 1% lidocaine (l0 ml) and 0.75% ropivacaine (10 ml) in observation group (n =412),and 1% lidocaine (20 ml) were administered in control group (n =403).Postoperatively heart rate (HR),mean arterial pressure (MAP),pain intensity (VAS),analgesic demand,adverse events and hospital duration were assessed.Results There was no significant difference between the 2 groups in postoperative hemodynamic monitoring (all P > 0.05).VAS score in observation group were lower than those in control group at 1,3,6 and 9 h.The former needed significantly less pain killer (all P < 0.05).Conclusion Ropivacaine decreases postoperative pain level after tension-free inguinal hernia repair without additional risk of perioperative events.
2.Progress in application of magnesium alloys in the implanted medical devices

Magnesium alloys have been a hotspot in the field of implanted medical devices due to their biodegradable absorbability, excellent mechanical properties and good biocompatibility.The reduction in their rapid corrosion rates becomes the key to the application of implant medical device materials.In this paper, the latest research progress and the existing problems of magnesium alloys as the material for implantation of medical devices in the fracture internal fixation, bone tissue porous scaffold, and cardiovascular stent are reviewed.Improving corrosion resistant of magnesium alloys by means of alloying, improving purity, surface modification,rapid solidification, deformation processing, non crystallization and preparation of nano alloy technology in body fluid are expounded, and research direction and application prospect of magnesium alloys in the field of implanted medical devices are also expected.
3.Relationship between human leukocyte antigen-DRB1 allele gene polymorphism and intrahepatic cholestasis of pregnancy

Objective To study the relationship between human leukocyte antigen-DRB1 (HLA-DRB1) allele genes polymorphism and intrahepatic cholestasis of pregnancy (ICP). Methods Forty-two patients with ICP were tested for HLA-DRB1 allele genes polymorphism with the polymerase chain reaction technique and sequence specific oligonucleotide (PCR-SSO) probes hybridization, 56 normal pregnant women as control group were also tested. In addition, the phenotype frequencies of HLA-DRB1 alleles were compared with it′s clinical character in patients with ICP. Results The higher frequencies were observed for alleles DR9, DR12 and DR4 in both groups. DR6 alleles were detected in 14 cases out of 42 patients. 4.Molecular characteristics of the hemagglutinin genes of avian influenza virus subtype H5N1 in the boundary region of Yunnan province

To elucidate the molecular characteristics of the hemagglutinin (HA) genes of H5N1 subtype of avian influenza viruses in the boundary region of Yunnan province. Of 420 samples were collected from the foreign poultry in boundary region of Yunnan province during 2003 to 2008 and these samples were subjected to screening by H5/N1 subtype-specific and multiplex RT-PCR. testing. The HA genes of H5N1 viruses from positive samples were amplified by RT-PCR and cloned into vector pMD18 T for subsequent sequencing. The alignment with sequences of the known reference strains and phylogenetic analysis were then performed. The genes from 21 representative positive samples with 4 different sequences at the cleavage site were obtained and all of them possessed the molecular characteristic of highly pathogenic avian influenza virus. The mutation of key amino acids had been found among receptor-binding sites, potential glycosylation sites and neutralizing epitopes.-Phylogenetic analysis showed those positive samples could be divided into 5 distinct clades, including clade 1, 2.4. 2.3.2, 2.3.4 and 7. It is evident that H5N1 viruses from the foreign boundary region of Yunnan province in 2003 to 2008 show genetic divergence and clade 2,3,4 is the dominant clade in this region.
5.Molecular characteristics of the full-length genome of dengue serotype 1 virus strains isolated from dengue fever cases in Sino-Myanmar border region in Yunnan Province, China

We investigated the molecular characteristics of the full-length genome of 14 dengue serotype 1 virus (DENV-1)strains isolated in Sino-Myanmar border region in Yunnan Province,China during 2013-2015.Isolation of dengue virus was using C6/36 cell culture method.Viral RNA was extracted from virus isolates,and then the full-length genome was amplified by RT-PCR.The homology and phylogenetic analysis was made on the nucleotide and deduced amino acid sequences by bioinformatics software including ClastalX1.83 and MEGA6 etc.Results showed that fourteen strains of DENV-1 isolated from dengue fever cases,of these,9 strains from Ruili City of Dehong Prefecture,3 from Lincang Prefecture,2 from Kunming City.RT-PCR and sequencing indicated that the full-length genes (10 735 nt) of 14 DENV-1 strains were obtained,and their open reading frame (95-10 271) were coded 3 392 amino acid residues.The genotypes of DENV-1 were revealed by homology and phylogenetic analysis based on structural and non-structural proteins.Thirteen were genotype Ⅰ (G-Ⅰ) (7 from indigenous cases in Ruili and Lincang and 6 from imported case from Myanmar to Ruili,Lincang and Kunming),and 1 G-Ⅲ from imported case from India to Kunming.The phylogenic analysis indicated that the 13 isolates from Yunnan divided into 2 phylogenic subgroups,and they had a closer genetic relationship with the strains isolated from Southeast Asia.The gene sequences of the 13 G-Ⅰ strains have been acquired,the rate of their nucleotide homology and amino acid homology were 97.02 %-100 % and 98.78 %100 % respectively.Compared with 6 strains from Southeast Asia,nucleotide homology and amino acid homology were 96.53%-99.53% and 97.33%-100% respectively.Compared with prototype strain (US_Hawaii) of DENV-1,nucleotide homology and amino acid homology were 93.76%-94.45 % and 95.86 %-96.91% respectively.Compared with US_Hawaii strain,there were 44 and 150 different sites in amino acid of structural and non-structural proteins,respectively.The G-1 of DENV-1 have been popular in Sino-Myanmar border region in Yunnan,2013-2015.They have genetic diversity but multiple transmission sources were from Myanmar,and should strengthen control cross-border spread of dengue fever in this region.It is necessary to further study that change of the amino acid sites of Yunnan strains of DENV-1 is related to its antigenicity and pathogenicity.
6.Blood oxygen saturation monitoring and nursing intervention of patients after artificial joint replacement

Objective To find out the law of occurrence, clinical characteristics and interventions of hypoxemia after artificial joint replacement.Methods From January to April 2013, totals of 100 patients underwent artificial joint replacement and were retrospectively analyzed.Perioperative pulse oximetry ( SpO2 ) was measured and recorded its curved line, and then we compared Perioperative changes of SpO2 curve for different types of surgery.Results During perioperative period, the curves of SpO2 were stable and nearly consistent.After operation, the lowest value of SpO2 usually happened in the first day, and patients usually performed hyoxemia in one to two days;but the value of it would steady increase to normal in three to five days. For unilateral joint replacement patients, the values of SpO2 were one to two days after operation and three to five days after operation(91.9 ±2.5)%,(93.7 ±2.0)% which were less than the unilateral hip replacement [(93.3 ±2.2)% and (93.7 ±2.0)%](t=-2.789,-2.231,respectively;P<0.05).Conclusions For artificial joint replacement in perioperative period, the change of SpO2 has certain law that hyoxemia normally occur in the first day after operation especially for knee replacement patients.Medical staff should enforce nursing intervention to prevent it.
7.Influencing factors and nursing of limb swelling among patients with total knee arthroplasty

Objective To investigate the nursing strategy and related factors for limb swelling after total knee arthroplasty ( TKA) for furtherly guiding and improving rehabilitation of patients in our clinical work. Methods A total of 183 patients, who underwent TKA during April to October 2013, were retrospective analysis and compared gender, ages, body mass index ( BMI ) , diagnosis before operation, the method of anesthesia, anticoagulation method, the using time of tourniquet, perioperative change of hemoglobin and type of prosthesis impacting on limb swelling after operation. All data were analyzed by SPSS 16. 0. Results Compared patients with BMI≤25 kg/m2 and those with BMI >25 kg/m2 , there was statistical difference on patients′circumference of 10 cm above knee (t= -0. 270,P<0. 05), but no significant differences among patients <60 years, 60 - 70 years and > 70 years ( P > 0. 05 ), the same as gender, the method of anesthesia, anticoagulation method having no impact on limb swelling (P>0. 05). The time of using tourniquet had no correlation with limb swelling by Pearson correlation analysis (P >0. 05). By two variable linear regression analysis, it represented that the effects of limb swelling including BMI, perioperative hemoglobin values and prosthesis type (P<0. 05). Conclusions Limbs swelling is easy to occur within the early postoperative period of TKA. Lower extremity swelling is related on BMI, perioperative hemoglobin values, and prosthesis type to some extent. Individualized nursing measures should be formulated to reduce postoperative limb swelling, and optimize patient rehabilitation training.
8.Detection and identification of a new bat bocavirus species in Yunnan

Objective To discover more novel bat viruses and molecularly characterize bat-borne bocavirus diversity in Yunnan.Methods Twenty-six Aselliscus stoliczkanus were sampled in Jinghong , Yunnan, and subjected to viral metagenomic analysis.Specific PCR was used to detect any bocavirus in these samples based on the metagenomic result , while full genome was amplified and compared with other bocaviruses .Results and Conclusion Totally, 3 of the 26 (11.5%) bats were positive for bocavirus, the full genome of which contained 5203 nucleotides and could encode NS1, NP and VP1/VP2 proteins.Phylogenetic analysis showed that this virus shared up to 58.7% and 53.3% amino acid identities with canine bocavirus 1 and canine minute virus .According to ICTV criteria (85%amino acid sequence identity ) on a new species of bocavirus , this virus could be a novel species within genus Bocaparvovirus .This study provides important data to better understand viral diversity in bats and to uncover the relationship between bocavirus and its hosts .
9.Relationship between the Levels of Serum AIM2 and Apolipoprotein J and Clinical Severity in Patients with Vascular Dementia and Their Prognosis

Objective To explore the relationship between the serum levels of absent in melanoma 2(AIM2)and apolipoprotein J(Apo J)and the severity and prognosis of vascular dementia(VD).Methods From October 2020 to September 2022,128 patients with vascular dementia were collected as the study group in the Second People's Hospital of Binzhou.Based on the mini-mental state examination(MMSE)score,the severity of the condition was evaluated and the study group was separated into mild group(n=43),moderate group(n=54)and severe group(n=31).After 90 days of treatment,the prognosis of patients was evaluated based on the ability of daily living(ADL)scale score and grouped into grade Ⅰ group(n=66),grade Ⅱ group(n=40)and grade Ⅲgroup(n=22).Another 96 healthy individuals who underwent physical examination were selected as the control group.The expression levels of serum AIM2 and Apo J were detected by qRT-PCR and ELISA methods,and the differences between the groups were compared.Spearman method was applied to analyze the correlation between serum AIM2 and ApoJ expression levels and MMSE scores,and the relationship between serum AIM2 and Apo J expression levels and prognosis in patients with vascular dementia.Logistic regression was applied to analyze the prognostic factors of vascular dementia patients.Results The expression level of serum AIM2 mRNA(3.11±0.57 vs 1.06±0.23)in the study group was higher than those in the control group,while the expression level of serum ApoJ(68.83±12.24 ng/L vs 81.07±13.15 ng/L)was lower than that in the control group,with significant differences(t=32.054,7.174,all P<0.001).The expression level of serum AIM2 mRNA in the severe group was higher than that in the mild and moderate groups(q=12.807,15.780),the expression level of serum Apo J and MMSE score in the severe group were lower than those in the mild and moderate groups(q=26.201,4.301;12.193,20.802),the expression level of serum AIM2 mRNA in the moderate group was higher than that in the mild group(q=14.688),and the expression level of serum Apo J and MMSE score in the moderate group were lower than those in the mild group(q=20.338,37.537),with significant differences(all P<0.001).The expression level of serum AIM2 mRNA(5.27±0.60)in the grade Ⅲgroup of vascular dementia was higher than that in the grade Ⅱ(3.36±0.58)and grade Ⅰ groups(2.23±0.55),while the expression level of serum Apo J(51.22±12.21 ng/L)was lower than that in the grade Ⅱ(64.15±12.23 ng/L)and grade Ⅰgroups(77.53±12.25 ng/L).The expression level of serum AIM2 mRNA in the grade Ⅱ group was higher than that in the gradeⅠ group,while the expression level of serum Apo J in the grade Ⅱ group was lower than that in the grade Ⅰ group,with significant differences(q=5.630~30.740,all P<0.001).The level of serum AIM2 in patients with vascular dementia was negatively correlated with MMSE score but positively correlated with prognosis(ADL grade)(r=-0.535,0.432,all P<0.001),while the level of serum Apo J in patients with vascular dementia was positively correlated with MMSE score but negatively correlated with prognosis(ADL grade)(r=0.467,-0.496,all P<0.001).Serum AIM2 mRNA[OR(95%CI):2.746(1.481~5.091)],Apo J[OR(95%CI):0.496(0.311~0.791)],and MMSE score[OR(95%CI):0.568(0.347~0.931)]were influencing factors for the prognosis of vascular dementia patients(all P<0.05).Conclusion The increase of serum AIM2 mRNA expression level and the decrease of serum Apo J expression level in patients with vascular dementia were closely related to the clinical severity and prognosis of patients,which may effectively evaluate the prognosis of patients.
10.A static magnetic field loading system for in vitro cultured cells.

Magnetic attachments have flux leakages, thus they will exert certain magnetic fields on the adjacent tissues when used in the patients' oral cavities. There are few research reports on the biological effects of the magnetic fields generated by magnetic attachments on human body. A cellular static magnetic field (SMF) loading system was developed in this study. By using this system, in vitro cultured human periodontal ligament cells (HPDLCs) were loaded with SMF simulating those of magnetic attachments. The cellular SMF loading system could produce constant SMF and the strength of the SMF is adjustable. The system is small and is able to exert SMF to cells cultured in different culture vessels such as culture dishes and culture plates, thus is suitable to researches in multiple biological items of cells. The results of the SMF loading experiment on HPDLCs showed that this cellular SMF loading system could effectively load cells with SMF of different strengths for different time in vitro. The development of this system has provided a useful tool for the researches on the cellular hiologioal effects of SMF.
