Objective To investigate the safety of laparoscopic cholecystectomy(LC) under the use of cardiac pacemaker for the patients with cholelithiasis and serious cardiac arrhythmia.Methods The clinical data of 8 cases undergoing LC under the use of vvI cardiac pacemaker were analysed.Results All the 8 cases undergoing LC by cardiac pacemaker had successful results. There was no cardiac accident or complication of the pacemaker in perioperative period in this series, and the postoperative course was smooth . Conclusions LC is safe under the use of cardiac pacemaker, if the decompensation of heart is controlled and the anaesthesia and operation are estimated correctly.

Objective Preparation of specific antibody of neuron protein 14 3 3 and application for the diagnosis of prion diseases. Methods The rabbits were immunized with prokaryotic expressed GST 14 3 3 fusion protein and the collected antiserum was evaluated by ELISA and Western blot assays. Results ELISA revealed that the reactive titer of the prepared antibody against purified 14 3 3 protein was up to 1:4 096 000; Western blot assays confirmed that both prokaryotic expressed,and the natively presenting 14 3 3 proteins in the brain extracts were recognized by the prepared antibody. Two cases of clinically diagnosed Creutzfeldt Jakob disease (CJD)with typical EEG changes showed the positive 14 3 3 protein in cerebral spinal fluid (CSF). Four patients with dementia and narcoma but without typical EEG change were 14 3 3 negative in CSF,three of them being even further tested by brain biopsies without typical histological change of CJD and presentation of protease resistant PrP Sc protein. 35 samples CFS collected from patients without dementia were 14 3 3 negative in CSF. All the results were the same as that of standard 14 3 3 polyclonal antibody.Conclusion Using purified prokaryotic expressed GST 14 3 3 fusion protein as antigen,the specific antibody was elicited in the immunized animals. The prepared antibody can be used in identification of 14 3 3 protein in CSF for the diagnosis of CJD.

BACKGROUND: To discuss the basic theory of traditional Chinese medicine (TCM) related to stem cell for a try to offer a new combining site +for the modernization research of the basic theory of TCM.METHODS: Comparatively analyzing the characteristic of stem cells and the essence theory in TCM.RESULTS: Stem cells have the capacity for both self-renewal and differentiation and can produce the daughter cells with the same phenotype and genotype as themselves. Stem cells have the plasticity that a stem cell may differentiate into cells other than the lineages to which it is committed, and therefore, is also termed transdifferentiation. So it is of great value for stem cell application in tissue engineering, curing tissue-damaged diseases and as the vector for transgene therapy. We compared the character of stem cells with essence theory, an elementary and important part of the basic theory of TCM, and found that stem cells are related greatly to essence,according to its two origin, congenital essence and acquired essence, and four kinds of functions of essence, which are controlling reproduction,controlling growth and development, manufacturing marrow and transforming blood; nourishing and moistening the zangfu-organs. Furthermore, stem cells especially have direct connected with the congenital essence which is stored in the kidneys. Congenital essence is the reproductive essence accepted from parents. Sperm and ovum merge to be oosperm, that is just the totipotent stem cell. The connotative meanings of congenital essence include all germ plasm and the special developing message of a genus in the oosperm. Genital stem cells are in charge of the function of controlling reproduction of congenital essence. Growth and development function is related to the proliferation and differentiation of adult stem cells controlled by gene. Manufacturing marrow function has relation with bone marrow stem cells and neural stem cells (NSC) in medullary cavity in which contains bone marrow, spinal cord and brain. Dominating bones function of the kidneys is related to mesenchymal stem cells (MSC), and hematopoietic stem cells (HSC) execute the function of producing blood completely.CONCLUSION: We consider that stem cells have a close relation with the congenital essence, and we put forward a new viewpoint in this research that stem cells have the same properties of congenital essence, and they are existence formation of congenital essence in cell level.

Objective To investigate the relationship between the C825T polymorphism of G protein β3 subunit (GNB3) and essential hypertension and cerebral infarction patients with a history of hypertension. Methods The C825T polymorphism of GNB3 was determined by polymerase chain reaction-restriction fragment length polymorphism in 110 healthy subjects aged over 40 years, 92 patients with essential hypertension, and 80 cerebral infarction patients with a history of hypertension. Sex, age, family histories of diabetics, smoking and alcoholic consumption were documented, and body mass index, waist-hip ratio, total cholesterol (TC),triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and fasting blood glucose concentration were measured. A multivariate logistic regression analysis was used to screen the factors associated with the occurrence of cerebral infarction in patients with hypertension.Results The distribution of the C825T polymorphism of GNB3 in the 3 groups all met the Hardy-Weinberg Law of genetic equilibrium. The genotype frequencies of CC, CT and TT in cerebral infarction patients with a history of hypertension were 33％, 57％, and 20％, respectively; those in patients with essential hypertension were 33％, 42％, and 25％ ; respectively;and those in the control group were 26％, 54％, and 20％, respectively. There were no significant differences (x2 =4. 030, P =0. 402). The allele frequencies of 825T in the 3 groups were 39％, 40％, and 47％, respectively. There were no significant differences (x2 =0. 832). Conclusions The C825T polymorphism of GNB3 may not be associated with cerebral infarction and essential hypertension.

Objective To investigate the early diagnostic value of cornel confocal microscopy for the screening of small neuropathy in elderly patients with type 2 diabetic mellitus.Methods In the prospective study,96 elderly patients with diabetes as study group and 46 patients with non-diabetes as the control group were continuously collected from our hospital endocrinology and ophthalmology out patients during May 2014 to February 2016.The 96 cases of type 2 diabetes were subdivided into 47 patients with diabetic peripheral neuropathy (DPN)and 47 patients with nowdiabetic peripheral neuropathy(non DPN).Results The diabetes course was shorter in non-DPN group than in DPN group(P=0.000).The levels of glycosylated hemoglobin and urine albumin were lower in the nonDPN than in the DPN(P =0.072,0.007,respectively).The corneal nerve fiber density was lower in the DPN group than in NDPN group (P =0.000).Corneal nerve fiber density was higher in control group than in DPN and NDPN group.The differences in number of corneal nerve fibers showed no statistical significance between DPN and NDPN group (x2 =2.391,P =0.314).But the number of corneal nerve fibers was significant less in DPN and NDPN group than in control group(x2 =16.014,P =0.000).The negative correlation was found between the course of disease and corneal fibrous density by using single factor linear regression analysis.The number of corneal nerve fibers was lower in smoking group than in non-smoking group(P=0.003).The multiple linear regression analysis showed that duration of diabetes was a risk factor for diabetic neuropathy.Conclusions In some elderly diabetic patients with non-neuropathy,corneal nerve fiber density and number have been significantly decreased before nerve conductive velocity is reduced.Therefore,corneal confocal microscopy can be used to detect and diagnose small diabetic neuropathy in elderly patients with diabetes mellitus.

Objective To intestate the effect of different doses of atorvastatin on early neurological deterioration and short-term outcomes in patients with acute ischemic stroke.Methods The patients with acute ischemic stroke were enrolled prospectively.They were randomly assigned to either a standard therapy group (atorvastatin 20 mg/d) or an intensive treatment group (atorvastatin 40 mg/d).The primary outcomes were early neurological deterioration within 1 week of treatment and the good outcome of evaluation at 1 month after treatment (the modified Rankin Scale score 0-2); the secondary outcomes were the National Institutes of Health Stroke Scale (NIHSS) score and adverse events at 1 month.Results A total of 125 patients with acute ischemic stroke were enrolled,including 62 in the standard therapy group and 63 in the intensive treatment group.The incidence of early neurological deterioration at 1 week after treatment in the standard therapy group was significantly higher than that in the intensive treatment group (16.13％ vs.4.76％;x2=4.333,P=0.038); the proportion of good outcome in the standard therapy group was significantly lower than that in the intensive treatment group at 1 month after treatment (53.23％ vs.71.43％ ;x2 =4.413,P=0.036).During the treatment,no significant liver damage,muscle toxicity and other adverse events of causing atorvastatin reduction or withdrawal occurred in the patients of both groups.Conclusions Using high-dose atorvastatin in the acute phase of ischemic stroke may decrease the incidence of early neurological deterioration compared with the conventional dose,and improve short-term clinical outcomes.
