1.The efficacy and safety of xiaojin wan plus nimesulide in patients with subacute granulomatous thyroiditis

Objective To assess the effect of xiaojin wan plus nimesulide on patients with subacute granu-lomatoas thyroiditis(SAT). Methods 70 SAT cases were divided into nimesulide group(nimesulide 0.1 g,2/d) and combined group(nimesulide 0.1 g,2/d and xiaojin wan 1.2 g,2/d). When the patients' conditions were re-lieved after 3-4 weeks treatment,half doses of the medicines were given in two groups for 8 weeks. The efficacy,safe-ty and relapse rate after withdrawing medicines for 12 weeks were observed. Results The effective rates were 82.90% (29/35) in nimosulide group and 85.7% (30/35)in combined group, but both groups' effective rate was 50.0% (3/6) in patients with high temperature (higher than 39 ℃). The time for fever relieved in two groups were similar (P>0.05), but the thyroid pain smoothing time, normalized rates of blood sedimentation after 1 week therapy, enlarged thyroid or thyroid nodule after 3 week therapy, relapsed rate were significantly more efficacious in combined group than in nimesulide group (P<0.05). There was no severe side effects in the two groups. Conclu-sious There is a synergic effect of xiaojin wan plus nimesulide on patients with SAT, that is an effective and safe therapeutic regimen for SAT patients without high temperature.
2.Oxidative stress and expression of heme oxygenase-1 mRNA in the myocardial tissue of rats after limb ischemia/ reperfusion injury

Objective To study the mechanism of oxidative damage in myocardial tissue after limb ischemia reperfusion (IR), and the protective effects of heme oxygenase-1 on myocardial injury in experimental rats. Method The models of bilateral hind limbs ischemia and reperfusion in rats were established by using tourniquets applied to the roots of both hind limbs until palm blanched and pulseless for 4 hours. A total of 56 SD rats were randomly (random number) divided into 7 groups, namely one normal control group ( n = 8) and 6 ischemia-reperfusion groups as per different lengths of reperfusion time, e. g. 2 hrs, 4 hrs, 8 hrs, 16 h rs and 24 hr ( n = 8 each). The experimental rats were sacrificed after different lengths of reperfusion time. Specimens of myocardium and blood were taken for assays of malonaldehyde (MDA), superoxide dismutase (SOD) and myeloperoxidase (MPO), and pathological changes of myocardium were observed, and the expressions of HO-1 mRNA in myocardium were detected. Data were analyzed with ANOVA. Results (1) Compared with the control group, the levels of serum MDA and myocardial MDA of rats were increased in all IR groups and were higher (P < 0.05), and the levels of MDA reached the peak after reperfusion for 4 hours. The levels of serum SOD and myocardial SOD in rats of all IR groups were decreased and lower than those in rats of the control group ( P < 0.05), and the levels of serum SOD dropped away to the lowest point after reperfusion for 4 hours, and the levels of myocardial SOD fell off to the bottom after reperfusion for 8 hours. The levels of serum MPO and myocardial MPO were significantly increased in rats of all IR groups compared with the control group (P < 0.05). The levels of serum MPO reached peak after reperfusion for 4 hours, and the levels of myocardial MPO were increased to the highest spot after reperfusion for 6 hours. (2) The pathological changes in myocardium showed the most severe damage after reperfusionfor 4-6 hours.(3) After reperfusion for 2 hours, there were no significant differences in the expression of HO-1 mRNA between IR groups and control group (P >0.05), and after reperfusion for 4 hours and over, the expressions of HO-1 mRNA were markedly increased in IR groups and reached peak after reperfusion for 16 hours in comparison with the control group (P < 0.05). Conclusions The activation of neutrophils and free radicals may play a primarily adverse role in myocardial injury after limb IR, and the increase in the expression of HO-1 mRNA lessens the harm effects of IR on myocardium.
3.Stem cells and congenital essence of the basic theory of traditional Chinese medicine

BACKGROUND: To discuss the basic theory of traditional Chinese medicine (TCM) related to stem cell for a try to offer a new combining site +for the modernization research of the basic theory of TCM.METHODS: Comparatively analyzing the characteristic of stem cells and the essence theory in TCM.RESULTS: Stem cells have the capacity for both self-renewal and differentiation and can produce the daughter cells with the same phenotype and genotype as themselves. Stem cells have the plasticity that a stem cell may differentiate into cells other than the lineages to which it is committed, and therefore, is also termed transdifferentiation. So it is of great value for stem cell application in tissue engineering, curing tissue-damaged diseases and as the vector for transgene therapy. We compared the character of stem cells with essence theory, an elementary and important part of the basic theory of TCM, and found that stem cells are related greatly to essence,according to its two origin, congenital essence and acquired essence, and four kinds of functions of essence, which are controlling reproduction,controlling growth and development, manufacturing marrow and transforming blood; nourishing and moistening the zangfu-organs. Furthermore, stem cells especially have direct connected with the congenital essence which is stored in the kidneys. Congenital essence is the reproductive essence accepted from parents. Sperm and ovum merge to be oosperm, that is just the totipotent stem cell. The connotative meanings of congenital essence include all germ plasm and the special developing message of a genus in the oosperm. Genital stem cells are in charge of the function of controlling reproduction of congenital essence. Growth and development function is related to the proliferation and differentiation of adult stem cells controlled by gene. Manufacturing marrow function has relation with bone marrow stem cells and neural stem cells (NSC) in medullary cavity in which contains bone marrow, spinal cord and brain. Dominating bones function of the kidneys is related to mesenchymal stem cells (MSC), and hematopoietic stem cells (HSC) execute the function of producing blood completely.CONCLUSION: We consider that stem cells have a close relation with the congenital essence, and we put forward a new viewpoint in this research that stem cells have the same properties of congenital essence, and they are existence formation of congenital essence in cell level.
4.Study on the Feasibility of Evaluating Drug in vitro Sustained Release Performance by Overflow Dissolu-tion Method

OBJECTIVE:To study the feasibility of overflow dissolution method for evaluating the drug in vitro sustained re-lease performance. METHODS:Overflow dissolution method was established by simulating the drugs elimination in vivo. Using Nifedipine sustained-elease tablets(Ⅰ)from 2 different manufacturers as model drug A,B,concentration-time curve,cumulative release rate- time curve,release velocity-time curve of model drugs in release pool at 3 different overflow speed (0,1.50,3.00 mL/min)were investigated. RESULTS:When overflow speed was 0,the cumulative dissolution was consistent with that of the con-ventional dissolution method. As the overflow speed increased,cmax of drug A,B was decreased [A:(8.89±0.20),(5.21±0.04), (3.51±0.03)μg/mL;B:(7.62±0.05),(4.80±0.09),(2.89±0.04)μg/mL];cumulative release rate was increased [A:(85.47± 2.45)%,(94.29 ± 2.44)%,(96.04 ± 2.56)%;B:(73.28 ± 1.13)%,(78.46 ± 1.94)%,(82.50 ± 1.69)%] ;tmax was ahead (A:1.5,1.0,0.5 h;B:2.0,1.0,0.5 h). CONCLUSIONS:Overflow dissolution method has avoided the inhibition of too large drug concentration on drug release,making complete drug release and more accurate evaluation of in vivo sustained release performance of the preparation.
5.Nosocomial Infection Rate:An Investigation Analysis

OBJECTIVE To understand the status of the hospital infection and risk factors,as well as the existing problems of infection management,to improve the surveillance method of incidence rate of hospital infection and strengthen the infectious precaution and control measures.METHODS By efforts of the professional staff of hospital,with cooperateion of the clinic personnel infection management group,all the patients in hospital were investigated when sampled from zero hour of the day before investigation to the zero hour of the day of investigation,according to the requirement of nosocomial infection rate to design the questionnaire.The data were analyzed.RESULTS The patients investigated in hospital were 638,from them 38 cases with hospital infections were found,the incidence and case-time rates of hospital infection were 6.0% and 7.4%,respectively.Respiratory infection was the most common.CONCLUSIONS Compared with the investigation method used previously,the current approach is easy and standardized.It also provides the credible reference to improve infection surveillance and management.
6.Effect of different doses of atorvastatin on early neurological deterioration and short-term outcomes in patients with acute ischemic stroke

Objective To intestate the effect of different doses of atorvastatin on early neurological deterioration and short-term outcomes in patients with acute ischemic stroke.Methods The patients with acute ischemic stroke were enrolled prospectively.They were randomly assigned to either a standard therapy group (atorvastatin 20 mg/d) or an intensive treatment group (atorvastatin 40 mg/d).The primary outcomes were early neurological deterioration within 1 week of treatment and the good outcome of evaluation at 1 month after treatment (the modified Rankin Scale score 0-2); the secondary outcomes were the National Institutes of Health Stroke Scale (NIHSS) score and adverse events at 1 month.Results A total of 125 patients with acute ischemic stroke were enrolled,including 62 in the standard therapy group and 63 in the intensive treatment group.The incidence of early neurological deterioration at 1 week after treatment in the standard therapy group was significantly higher than that in the intensive treatment group (16.13% vs.4.76%;x2=4.333,P=0.038); the proportion of good outcome in the standard therapy group was significantly lower than that in the intensive treatment group at 1 month after treatment (53.23% vs.71.43% ;x2 =4.413,P=0.036).During the treatment,no significant liver damage,muscle toxicity and other adverse events of causing atorvastatin reduction or withdrawal occurred in the patients of both groups.Conclusions Using high-dose atorvastatin in the acute phase of ischemic stroke may decrease the incidence of early neurological deterioration compared with the conventional dose,and improve short-term clinical outcomes.
7.Listeria monocytogenes meningitis in systemic lupus erythematosus patients

Objective To summarize the clinical characteristics of SLE patients complicated with Listeria monocytogenes meningitis (LMM).Methods The clinical manifestations,systemic involvements,laboratory tests,cerebrospinal fluid (CSF) examinations,treatment and prognosis of SLE inpatients combined with LMM in Peking Union Medical College Hospital during Janurary 2000 to December 2015 were retrospectively analyzed.Results Eight SLE patients with LMM accounted for 0.14% of the 5 675 hospitalized SLE patients.Seven were female and 1 was male,with the mean age of (35±16) year.Headache (7/8) was the most common neurological symptom followed by positive meningeal irritation sign (5/8),unconsciousness (3/8) and nausea or vomiting (2/8).Increased intracranial pressure was found in 7 cases and 4 of them were even higher than 330 mmH2O.CSF protein elevated in 6 cases and CSF glucose decreased in 6 cases.LMM often occurred in inactive or mild active SLE with the average SLE disease activity index (SLEDAI) score of 4.7±1.5.Systemic manifestations included hematologic changes (6/8),rashes (4/8),renal involvement (4/8) and arthritis (3/8),etc.Treated by autibiotics such as penicillin and medications for SLE,7 patients achieved clinical improvements,1 patient died.Conclusion LMM is one of the opportunistic infections in SLE patients and tends to occur in inactive lupus cases.There are unspecific clinical and laboratory features and CSF changes which maybe overlap and mix up with systemic manifestations of SLE.That's why it is necessary to take continuous cultures of CSF.Early identification and appropriate treatment lead to prognostic improvement of these patients.
8.Temperature-switched high-efficiency D-lactate production from glycerol.

Glycerol from oil hydrolysis industry is being considered as one of the abundent raw materials for fermentation industry. In present study, the aerobic and anaerobic metabolism and growth properties on glycerol by Esherichia coli CICIM B0013-070, a D-lactate over-producing strain constructed previously, at different temperatures were investigated, followed by a novel fermentation process, named temperature-switched process, was established for D-lactate production from glycerol. Under the optimal condition, lactate yield was increased from 64.0% to 82.6%. Subsequently, the yield of D-lactate from glycerol was reached up to 88.9% while a thermo-inducible promoter was used to regulate D-lactate dehydrogenase transcription.

9.Establishment of a sandwich ELISA method for detection of reporter chloramphenicol acetyltransferase gene.

BACKGROUNDTo establish a sandwich ELISA method for detection of reporter chloramphenicol acetyltransferase (CAT) gene.
METHODSThe full length sequence of CAT gene was amplified with PCR using plasmid pBLCAT6 as template, and inserted into the prokaryotic expression plasmid Pgex-2T. The purified fusion protein was emulsified with complete or incomplete Freund adjuvant and injected subcutaneously into rabbits. The antibody was labeled with biotin, and a sandwich ELISA technique with biotin streptavidin amplify system was established. Several CAT reporter plasmids containing different HPV 16 LCR sequences were generated and transfected transiently to monolayer cells in vitro. The cytoplasm proteins were extracted and the expressions of CAT were evaluated with the newly established ELISA assay.
RESULTSSDS-PAGE displayed that the molecular weight of the expressed fusion protein was about 54,000. The prepared antiserum was able to recognize the CAT protein expressed by mammalian cells or prokaryote cells. Under the control of different promoters and their regulate sequences,two to eight folds CAT expression increased were evaluated in transiently transfected mammalian cells by the newly established sandwich ELISA method.
CONCLUSIONSThe established method could sensitively reflect the activities of the upstream promoters, as well as the influence of exchanges of nucleotides within the regulate region on the promoter activities. Therefore, it proposes a convenient assay for the studies using CAT as the reporter gene.

10.Lunch satisfaction and leftovers among students of Changsha compulsory education schools

Objective:
To understand lunch satisfaction and leftovers of Changsha compulsory education schools under different supply modes served by school canteens and specialized enterprises,and to provide scientific basis for the government to formulate school lunch improvement policies.
Methods:
Multistage stratified cluster sampling method was used to random select 2 203 students from 8 primary and secondary schools in Changsha who were administered with questionnair survey in April to May of 2018.
Results:
For school lunch, the overall rate of satisfaction was 78.8%.The overall lunch satisfaction of female and primany school students were higher than that of male and junior high school students respectively,and the difference was statistically significant(P<0.05). Totally 76.0% of students had the highest satisfaction in food hygiene and the lowest satisfaction rate was 60.0% in food taste. By comparing different supply modes, the overall satisfaction rate of meal quantity was higher (47.8%) under school canteens, and that of dining environment was relatively high (43.7%) under specialized enterprises. "Don-t like some food" (63.8%) was the main reason for students leftovers under the two supply modes. The lunch leftover rate of specialized enterprises (37.8%) was significantly higher than that of school cafeteria (30.6%)(χ 2=12.81,P<0.05).
Conclusion
The satisfaction of food taste and the rate of lunch leftovers need to be further improved. School canteens environment and management should be strengthened, as well as the flexibility and communication under specialized enterprises and family-school communication. The joint efforts by school and family are needed for healthy eating education.
