1.Clinical effect of exercise therapy on the patients with osteoporosis

Objective To evaluate the clinical effect of exercise therapy on patients with osteoporosis according to osteoporosis quality of life scale(OQOLS)and the changes in biochemical markers of bone metabolism.Methods Totally 94 patients with primary osteoporosis were randomly divided into 2 groups:the intervention group [exercise therapy (Wu Xing Jian Gu Cao) with calcium/vitamin D supplementation for 90 days] and the control group (only calcium/vitamin D supplementation for 90 days].OQOLS and the changes in biochemical markers of bone metabolism were observed before and after treatment.Results Compared with before treatment,25-hydroxy vitamin D[25-(OH) D]levels were increased in control group (33 cases) and intervention group (29 cases) after treatment [(61.2± 11.1) mmol/L vs.(48.1±26.2)mmol/L,both P<0.001],and the enhanced level was higher in the intervention group than in the control group (P<0.05).Compared with before treatment,bone alkaline phosphatase (BALP) was decreased in control and intervention group after treatment (both P<0.05).There were significant differences in OQOLS in both groups between before and after treatment except for the function of physical activities in intervention group (P<0.05).Conclusions The basic and exercise therapy can both increase [25-(OH) D] level,reduce BALP,and have clinical effects on bone metabolism,while exercise therapy has an improvement in osteoporosis quality of life in patients with osteoporosis.
2.Predictive value of serum PTX3 and Sema4d for postoperative fracture recurrence in elderly patients with osteoporotic fracture

Objective To investigate the predictive value of pentraxin 3(PTX3)and semaphorin 4D(Sema4d)in serum for postoperative fracture recurrence in elderly patients with osteoporotic fracture(OPF).Methods A total of 168 elderly patients with OPF who were treated in this hospital from January 2022 to January 2023 were selected.According to the occurrence of postoperative fracture recurrence,the patients were divided into non-recurrence group(116 cases)and recurrence group(52 cases).The levels of PTX3 and Se-ma4d in serum were detected by enzyme-linked immunosorbent assay.The influencing factors of fracture re-currence were analyzed by multivariate Logistic regression,and the correlation between PTX3 level and Se-ma4d was analyzed by Pearson correlation.The receiver operating characteristic(ROC)curve was used to ana-lyze the predictive value of PTX3 and Sema4d levels for postoperative fracture recurrence,and Z test was used to compare the area under the curve(AUC).Results There were significant differences in nutritional defi-ciency,puncture site,bone mineral density T value,fracture degree and serum PTX3,Sema4d levels between recurrence group and non-recurrence group(P<0.05).Bone mineral density T value was an independent pro-tective factor for postoperative fracture recurrence,and fracture degree,serum PTX3,Sema4d levels were in-dependent risk factors for postoperative fracture recurrence(P<0.05).There was a positive correlation be-tween serum PTX3 and Sema4d in recurrence group(r=0.415,P<0.001).The AUC of the combination of PTX3 and Sema4d in the diagnosis of postoperative fracture recurrence was 0.910(95%CI 0.857-0.949),which was better than that of serum PTX3 and Sema4d alone(Zcombination-PTX3=3.129,Zcombination-Sema4D=3.123,both P=0.002).Conclusion The serum levels of PTX3 and Sema4d are increased in elderly OPF patients with postoperative fracture recurrence.The combination of PTX3 and Sema4d has a high value in the diagnosis of postoperative fracture recurrence in elderly OPF patients,and can be used as an early diagnostic index.
3.Clinical curative effect of ShuganJianpi acupuncture manipulation combined with magnetic stimulation in Yintang acupoint in the treatment of depression with liver-qi stagnation and spleen deficiency type

Objective To evaluate the clinical efficacy of different combinations with "Shugan Jianpi" acupuncture and Yintang acupoint magnetic stimulation in the treatment of depression with liver depression and spleen deficiency type.Methods 160 patients with depression treated in hospital from December 2013 to June 2017 were divided into 4 groups according to the random number table,with 40 cases in each group.Group A received basic drug therapy," Shugan Jianpi" acupuncture combined with Yintang acupoint magnetic stimulation,group B received basic drug therapy combined with Yintang acupoint magnetic stimulation,group C received basic drug therapy combined with " Shugan Jianpi" acupuncture treatment,group D was treated with basic medicine only for 4 weeks.Hamilton Depression Scale (HAMD) was used to assess the short-term and long-term efficacy for 4 groups before treatment,after 2 weeks of treatment,after 4 weeks and 2 months after treatment and the safety of these 4 types of therapy was evaluated with the side effects scale (TESS).2 months after the treatment,the quality of life universal scale (SF-36) was evaluated for patients in 4 groups.Results After 2 weeks,4 weeks and 2 months of treatment,HAMD scores in the 4 groups were significantly lower than those before treatment (P<0.05).After 2 weeks of treatment,HAMD score of A group was (15.28±3.45),and HAMD score of B,C,D group were ((18.30±3.57),(22.50±3.71),(27.30±4.82)) respectively.HAMD score of A group was significantly lower than those of B,C,D group (P<0.05).After 4 weeks of treatment,HAMD scores in A,B group were significantly lower than those in C,D group (P <0.05).After 2 months of treatment,HAMD scores in all the 4 groups were further declined compared with those after 4 weeks of treatment (P< 0.05),however,there was no significant difference between the four groups (P>0.05).The scores of each dimension of SF-36 in group A were significantly higher than those of group B,C and D (P<0.05),and which in group B and C were significantly higher than those in D group after 2 months of treatment.(P<0.05).There was no statistical difference among the 4 groups in TESS scores (P >0.05).Conclusion The results suggest that there is a synergistic effect in combination of the " ShuganJianpi" acupuncture combined with Yintang acupoint magnetic stimulation in the treatment of depression of liver qi stagnation and spleen deficiency type,and it can effectively reduce the degree and improve their quality of life in patients with depression.
