1.Research in reason analysis and prevention of phlebitis caused by PICC in cancer patients undergoing chemotherapy

Objective To investigate the related factors of phlebitis caused by PICC and support effective preventive nursing measures. Methods 163 patients diagnosed as malignant tumor who first used peripherally inserted central catheters (PICC) were recruited. Patients were observed for 3 months. According to the diagnostic criterion of infection, 34 patients with phlebitis were divided into the phlebitis group and others were divided into the control group. All patients were investigated for general information survey :sex, age, tumor type, allergic history,location of inserted vein, one-time success rate,activity after catheter, elbow bending,phlebitis occurred time, grade and type. Laboratory markers:WBC,neutrophils,immune parameters, fasting plasma glucose. Trait anxiety inventory (T-AI)was used for assessment of emotional state. Case-control study between groups was carried out. Results The results of univariate analysis showed that location of inserted vein,times of inserting vein,activity of inserted limb,elbow bending,blood sugar level and emotion were associated with phlebitis. Sex, age, tumor type, allergic history, immune function,WBC,neutrophils were not involved with phlebitis. The results of multivariate logistic regression analysis showed that PICC through basilica vein was the protective factor. Multiple insertions, more activity of inserted limb,durative anxious emotion were the risk factors of phlebitis. Conclusions Different location of vein,times of inserting vein,activity of inserted limb,elbow bending,high blood glucose level and persistent anxious emotion are the risk factors of phlebitis. Taking effective preventive measures before,during, and after PICC can reduce the incidence of phlebitis.
2.Clinical Observations on Acupuncture at Heel Vessels for the Treatment of Cervical Vertigo

ObjectiveTo investigate the clinical efficacy of points Shenmai(BL62)and Zhaohai(KI6)in treating cervical vertigo. MethodSixty-eight patients meeting the diagnostic criteria were randomly allocated to treatment and controlgroups, 34 cases each. The treatment group received conventional acupuncture at heel vessel points Shenmai and Zhaohai plus cervical Huatuo jiaji points and the control group, conventional acupuncture at cervical Huatuo jiaji points alone. In the two groups, the vertigo symptom score was recorded, and the clinical therapeutic effects and pre-/post-treatment change in the score were observed.ResultThe total efficacy rate was 94.1% in the treatment group and 79.4% in the control group. The therapeutic effect was significantly better in the treatment group than in the control group (P<0.05). The vertigo symptom score decreased significantly in the two groups after the course of treatment compared with before (P<0.01). Vertigo improved significantly more inthe treatment group than in the control group (P<0.05).ConclusionAcupuncture at points Shenmai and Zhaohai is significantly effective by comparison with conventional acupuncture in clinicaltreatment of cervical vertigo.
3.Desmoplastic fibroma of the bone: a clinicopathological analysis of seven cases

Purpose To investigate the clinicopathologic features of desmoplastic fibroma of bone (DF).Methods The clinical and pathologic profiles of 7 cases were retrospectively analyzed.Results Desmoplastic fibroma of bone was a rare neoplasm of bone,which typically occurred in young children and adolescents.The tumor showed infiltrative and locally aggressive nature,the most common sites were long tubular bones and mandible.Microscopically,the tumor was composed of sparse fibroblasts in a rich background of collagen fibers.Immunohistochemically,the tumor cells were positive for vimentin and SMA.Conclusion Desmoplastic fibroma is a rare,locally aggressive fibroblasticlesion of bone.It can make an accurate diagnosis by integrating the clinical location with histopathological features.Trauma,hormone and horomosome aberration may be related with its pathogenesis.
4.Study on Quality Control of Fushu Granules

OBJECTIVE:To establish a method for the quality control of fushu granules.METHODS:Five main chinese crude drug angelica,chuanxiong,flos lonicerae,glycyrrhizae and astragali in the fushu granules were identificated by TCL;the content of ferulic acid was determined by HPLC.RESULTS:Ferulic acid showed a good linearity in a range of 0.02~0.10?g/ml(r=0.9 998),the average recovery rate was 96.9%,RSD=1.4%.CONCLUSION:This method has a good recurrence and could be used to control the quality of fushu granules.

Objective To explore the influence of hyperbaric oxygen therapy (HBOT) on the learning and memory ability using a rat model of vascular dementia (VD).Methods Sixty Sprague-Dawley rats were selected and divided into a normal group,a sham operation group,a VD group and an HBOT group,each of 15 rats using a random number table.The improved four-vessel method was used to establish a VD model in all except the rats in the normal group,but for the sham operation group,the vertebral artery was not subjected to firing and the arteria carotis communis was not clipped.The HBOT group was then given HBOT for 7 days.The Morris water maze test was used to evaluate their learning and memory ability of all 4 groups.Results Compared to the normal group,the mean escape latency of the sham operation group,the VD group and the HBOT group was significantly longer in each case (P<0.05).Their platform quadrant crossing times were also significantly less,and their quadrant swimming distance as a proportion of their total swimming distance was significantly smaller (P<0.05).Compared with the sham operation group,the mean escape latency of the VD and HBOT groups was significantly longer (P<0.05),the times of crossing platform quadrant were less and their quadrant swimming distance was a smaller proportion of their total swimming distance.As compared with the VD group,the mean escape latency of the HBOT group was significantly shorter [(25.73±6.20) s],the average platform quadrant crossing times were significantly more [(9.51±2.25) times] and the platform quadrant swimming distance of the HBOT group was significantly longer [(494.72±208.26)],representing a significantly larger proportion of the total swimming distance [(28.71 ± 5.32) %] (P<0.05).Conclusion HBOT can effectively improve learning and memory in vascular dementia,at least in rats.
7.Effects of intravenous infusion of bilirubin on the expression of phospho-p38 mitogen-activated protein kinase and the apoptosis in splenocytes of neonatal rats

Objective To explore the effects of bilirubin on myeloid differentiation factor phospho-p38 mitogen-activated protein kinase (p-p38MAPK) and apoptosis in splenocytes of neonatal rats.Methods Seven-day-old Sprague Dawley rats (clean grade),male or female,weighting 12.0-15.0 g,were randomly assigned to 6 groups.There were blank control group (Ⅰ),lipopolysaccharide (LPS) control group (Ⅱ),15 mg/kg bilirubin control (free-LPS) group (Ⅲ),15 mg/kg group (Ⅳa),30 mg/kg group (Ⅳb) and 50 mg/kg group (Ⅳc),and then subsequently divided into 2 h,5 h and 24 h subgroups in each groups.Some of the 200 newborn rats died amid the experiment,tinally,a total of 144 cases were involved in the analysis of results,and 8 rats in each subgroups.Newborn Sprague Dawley rats were administered at various doses of bilirubin (15 mg/kg,30 mg/kg and 50 mg/kg,respectively) intravenously; 1 h after injection,the rats were administered LPS intraperitoneally at a dose of 1 mg/kg;p-p38MAPK were detected by immunohistochemistry;Apoptosis in splenocytes was detected by terminal deoxynucleotidyl transferase mediated dUTP nick end labeling methods at 2 h 5 h and 24 h after the injection of bilirubin.Results 1.Expression of p-p38MAPK in each group:bilirubin in low-mid concentrations of range inhibited LPS-induced p38MAPK activation (qⅣa =20.93,10.37,respectively at 2 h,and 5 h,all P < 0.01 ;qⅣ b =79.97,14.79,all P < 0.01).The inhibition strengthened with increasing concentration of bilirubin.The effect was observed at 2 h,strengthened at 5 h,disappeared at 24 h.Bilirubin in the high concentrations of range stimulated the expression of p-p38MAPK (qⅣc =32.55,19.23,27.72,respectively at 2 h,5 h and 24 h,all P <0.01),observed at 5 h,reduced at 24 h.2.Effects of bilirubin on apoptosis in splenocytes:LPS could increased the apoptosis index (AI) of splenocytes(q =54.62,P < 0.01);The AI of splenocytes had no significant change in low concentrations of range of bilirubin (q =43.92,P > 0.05).Low-mid concentration of bilirubin with LPS reduced the AI of splenocytes (q Ⅳ a =4.48,P < 0.01 ;q Ⅳ b =2.07,P < 0.05),while high concentration of bilirubin with LPS increased the AI of splenocytes (q =5.08,P < 0.01).Conclusions Bilirubin in low-mid concentrations of range could inhibit the expression of LPS-induced p38MAPK,while bilirubin in high concentrations of range stimulated the expression.As the concentration of bilirubin elevated,its inhibition was prolonged.Bilirubin in high concentrations of range bilirubin could induce apoptosis in splenocytes.The immune dysfunction in neonatal hyperbilirubinemia may have something to do with the regulation of phosphorylation of p38MAPK and activation of apoptotic pathways.
8.Symptom clusters in patients with lung cancer patients during chemotherapy:a scoping review

Objective To identify the characteristics and research status of symptom clusters in patients with lung cancer patients during chemotherapy.Methods This scoping review was conducted under the Joanna Briggs Institute guidelines.A literature search was performed in the PubMed,Embase,CINAHL,Web of Science,Scopus,CNKI,Wanfang,and CBM databases to identify relevant studies about symptom clusters in patients with lung cancer from inception until December 31,2023.The included studies were summarized and analyzed.Results 19 papers were included and 22 symptom clusters were extracted,mainly including psychological,gastrointestinal,lung cancer-specific,fatigue-related,and respiratory symptom clusters.There are 6 symptom cluster assessment tools,of which 4 were M.D Anderson Symptom Inventory in different version;the symptom clusters were dynamically changing during chemotherapy in patients with lung cancer,with variations in core symptom clusters at different stages.Conclusion Multiple symptom clusters exist during chemotherapy in lung cancer patients,and the naming of symptom clusters and attribution of symptoms need to be further standardized.The assessment tools for symptom clusters in patients undergoing chemotherapy for lung cancer are mainly based on the Anderson Symptom Assessment Scale and other language versions.Symptom clusters in patients with lung cancer during chemotherapy show dynamic changes,and there are differences in core symptom groups at different stages.
9.Construction of evaluation index system of core competence of specialized gynecological nurses

Objective:To construct the evaluation index system of core competence of specialized gynecological nurses, and to provide reference for the curriculum setting and effect evaluation of standardized training for specialized gynecological nurses.Methods:The draft evaluation index system was prepared through literature research and group discussion. From May to June 2021, two rounds of Delphi letter consultation was used among 18 experts, to complete the selection and modification of evaluation index, and to determine the index weight.Results:The effective response rate of the two rounds of Delphi expert consultation was 100%, the expert authority coefficient was 0.938, and the Kendall harmony coefficient of the two rounds of expert letter consultation was 0.117 and 0.304, respectively (both P<0.01). Finally, the evaluation index system of core competence of specialized gynecological nurses was formed, including 4 first-level indexes, 11 second-level indexes and 48 third-level indexes. Conclusions:The evaluation index system of core competence of specialized gynecological nurses established in this study is scientific and reliable, which can provide reference for the curriculum setting and effect evaluation of standardized training for specialized gynecological nurses.
10.School bullying and its influencing factors in higher vocational college students in western Hunan province

Objective:
To understand current situation and influencing factors of school bullying among left-behind and non left-behind students in vocational colleges in western Hunan, and to provide reference for the prevention and control of campus bullying in vocational colleges in poverty-stricken areas.
Methods:
A multi-stage stratified random cluster sampling method was used to select 1 241 students from higher vocational colleges in Xiangxi for questionnaire survey. The survey included basic demographic characteristics, health-related behaviors and school bullying. Associcted factors were compared among students with and without school bullying by using the χ2 test and Logistic regression.
Results:
The reported rate of school bullying in vocational colleges was 41.82%. The reported rate of left-behind students (45.05%) was higher than that of non-left-behind students (39.21%). The difference was statistically significant (χ2=4.29,P<0.05). Multivariate unconditional Logistic regression showed that male students and good parental relationship negatively associated with school bullying among left-behind students(OR=0.55,0.47). While rural students, 5 years of senior vocational, smoking, drinking, and game addiction were positively associated with school bullying among left-behind students(OR=1.93,2.57,2.51,3.95,4.73); good relationship with mothers was associated with less school bullying(OR=0.40), while smoke, drinking and game addiction was associated with more school bullying among non left-behind students(OR=1.86,2.32,3.81)(P<0.05).
Conclusion
School bullying among students in higher vocational colleges in western Hunan is highly prevalent. The reporting rate among left-behind students is higher than that of non-left-behind students. Due to different factors, joint intervention measures by government, schools and families should be carried out as soon as possible.
