1.Tracking quality survey and analysis on junior college graduates of clinical medicine tailored to rural areas

Objective To evaluate the training quality of junior college graduates of clinical medicine based on the feedback from the graduates and their employers.Methods Totally 112 junior college graduates tailored to rural areas among all the three years' graduates were selected.These graduates and their employers were asked to do the questionnaire and discussion.Results The overall evaluation from the graduates on the school curriculum and teachers was good,with satisfactory rate being above 95%.The employers had nice impression on the graduates and spoke highly of them,with satisfactory rate reaching 97.4%.Of all the three years' graduates,one-time passing rate in the qualification examination for assistant practitioners came to 41.0%,much higher than the average passing rate of 20.12% in the same area and at the same time.Conclusion The teaching effect for junior college graduates of clinical medicine tailored to rural areas is satisfactory and the reform on teaching is worka ble and effective,however,it needs further practice.
2.Clinical analysis of 5 cases of duodenal gastrointestinal stromal tumors

Objective To analyze the features of clinical diagnosis and treatment of duodenal gastrointestinal stromal tumors(GIST).Methods Retrospective analysis was performed on clinical data of 5 cases of duodenal GIST treated in our hospital between April 2002 and May 2009.Results The duodenal GIST was mainly located in the 2nd(3/5)and 3rd portion of duodenum(2/5).Clinical diagnosis of the disease mainly depended on barium meal examination, gastrointestinal endoscopy, and CT scanning.Two patients were treated with pancreaticoduodenectomy, 1 with segmental duodenectomy and 2 with local resection.After operation, 2 patients had recurrence and 1 of them underwent adjuvant therapy with Gleevec.Conclusion Surgical resection is the only effective therapeutic method for duodenal GIST.Various surgical procedures are mainly determined by the location and size of the tumors.For patients with a high degree of pathologic grade, adjuvant therapy with Gleevec is necessary.
3.Changes in relative pituitary hormones following severe traumatic brain injury in children and mild hypothermia treatment

Objective To observe changes in relative pituitary hormones after severe traumatic brain injury (STBI) in children and to investigate the short-and long-term effects of mild hypothermia on pituitary function.Methods Included for this study were 81 children who had been admitted to the Brain Hospital of Liaocheng People's Hospital from June 2011 to January 2015 for STBI and abnormal secretion of pituitary-related hormones.They were divided by the wishes of patients and their families into 2 groups,subjected to mild hypothermia treatment (n=42) or conventional treatment (n=41).The levels were examined of growth hormone (GH),prolactin (PRL),adrenocorticotropic hormone (ACTH),free triiodothyronine (FT3),free thyroxine (FT4) and thyroid stimulating hormone (TSH) for both groups unpon admission and in the moming of 1,3,5,7 and 14 days after injury.At 6 months after injury the patients were followed up for Glasgow Outcome Score (GOS) and levels of pituitary-related hormones.Results Of the 83 patients,disturbance of one single hormone was found in 31 (30.10%) and disturbance of 2 or more hormones in 52 (50.48%).The top 3 hormones disturbed were PRL,ACTH and GH.On days 3,5 and 7 after injury,the levels of PRL and ACTH in the mild hypothermia treatment group were significantly lower than in the conventional treatment group (P<0.05).On day 3 after injury,the level of FT3 in the mild hypothermia treatment group was significantly lower than in the conventional treatment group (P<0.05).At 6 months after injury,the GOS score for the conventional treatment group was 3.04±0.38,significantly lower than that for the mild hypothermia treatment group (4.56±0.62) (P<0.05).The incidence of hypopituitarism in the hypothermia treatment group (16.67%) was significantly lower than in the conventional treatment group (46.88%) (P<0.05).Conclusions Incidence of pituitary dysfunction is high in children following severe traumatic brain injury.Mild hypothermia treatment can effectively alleviate early-stage parasecretion of pituitary hormones,and decrease the incidence of later pituitary dysfunction.
4.Effects of FOCUS couple intervention on depression and psychological resilience of young and middle-aged stroke patients and their spouses

Objective:To explore the effect of family involvement, optimistic attitude, coping effectiveness, uncertainty reduction and symptom management (FOCUS) couple intervention on depression and psychological resilience of young and middle-aged stroke patients and their spouses.Methods:From June 2018 to January 2020, convenience sampling was used to select 76 young and middle-aged stroke patients and their spouses in a Class Ⅲ Grade A hospital in Zhengzhou as the research object. According to the order of hospitalization, patients were randomly divided into control group and experimental group, with 38 cases in each group. Control group implemented routine post-stroke nursing, and experimental group conducted the FOCUS couple intervention on the basis of control group. Depression and psychological resilience of patients and their spouses were evaluated with the Hamilton Depression Scale and Connor-Davidson Resilience Scale before and after intervention.Results:Finally, 37 cases in experimental group and 38 cases in control group completed the study. After intervention, the depression scores of patients and their spouses in experimental group were lower than those in control group, and the psychological resilience score and all dimension scores of patients and their spouses in experimental group were higher than those in control group, and the differences were statistically significant ( P<0.05) . Conclusions:The FOCUS couple intervention can reduce the degree of depression of stroke patients and their spouses, and improve the patients' psychological resilience, which has strong clinical significance.

Odontogenic maxillary sinusitis(OMS)is a subtype of maxillary sinusitis(MS).It is actually inflammation of the maxillary sinus that secondary to adjacent infectious maxillary dental lesion.Due to the lack of unique clinical features,OMS is difficult to distinguish from other types of rhinosinusitis.Besides,the characteristic infectious pathogeny of OMS makes it is resistant to conventional therapies of rhinosinusitis.Its current diagnosis and treatment are thus facing great difficulties.The multi-disciplinary cooperation between otolaryngologists and dentists is absolutely urgent to settle these questions and to acquire standardized diagnostic and treatment regimen for OMS.However,this disease has actually received little attention and has been underrepresented by relatively low publication volume and quality.Based on systematically reviewed literature and practical experiences of expert members,our consensus focuses on characteristics,symptoms,classification and diagnosis of OMS,and further put forward multi-disciplinary treatment decisions for OMS,as well as the common treatment complications and relative managements.This consensus aims to increase attention to OMS,and optimize the clinical diagnosis and decision-making of OMS,which finally provides evidence-based options for OMS clinical management.

Endo-periodontal lesions(EPLs)involve both the periodontium and pulp tissue and have complicated etiologies and pathogenic mechanisms,including unique anatomical and microbiological characteristics and multiple contributing factors.This etiological complexity leads to difficulties in determining patient prognosis,posing great challenges in clinical practice.Furthermore,EPL-affected teeth require multidisciplinary therapy,including periodontal therapy,endodontic therapy and others,but there is still much debate about the appropriate timing of periodontal therapy and root canal therapy.By compiling the most recent findings on the etiology,pathogenesis,clinical characteristics,diagnosis,therapy,and prognosis of EPL-affected teeth,this consensus sought to support clinicians in making the best possible treatment decisions based on both biological and clinical evidence.
7.Research Progress on Sensory Feedback Technology in Prostheses.

As an important auxiliary tool for amputees to gain abled limb functions, prosthetic limbs with decoration or feedforward control channel could not meet the needs. In order to enable the prosthesis to deliver the information, includes temperature, pressure, position, shape and so on, a variety of sensory feedback methods have been integrated into the prosthesis. According to the position of the feedback terminal on the human body, the perceptual feedback systems include invasive and noninvasive sensory feedback. This review presents the research progress of these perceptual feedback techniques, and summarizes the problems in the application in artificial limbs. Finally, the development trend of sensory feedback technology in prostheses is prospected.

8.Systematic comparison of two kinds of Bufonis Venenum derived from different Bufo gargarizans subspecies based on metabolomics and antitumor activity.

This paper compared the differences between two kinds of Bufonis Venenum produced by Bufo gargarizans gargarizans and B. gararizans andrewsi, and verified the rationality of the market value orientation of Bufonis Venenum based on the zebrafish mo-del. Twenty batches of Bufonis Venenum from Jiangsu province, Hebei province, Liaoning province, Jilin province, and Liangshan, Sichuan province, including B. gargarizans gargarizans and B. gararizans andrewsi, were collected. The UHPLC-LTQ-Orbitrap-MS combined with principal component analysis was used to compare the differences between two kinds of Bufonis Venenum. According to the limiting conditions of VIP>1, FC<0.5 or FC>2.0, and peak total area ratio>1%, 9 differential markers were determined, which were cinobufagin, cinobufotalin, arenobufagin, resibufogenin, scillaredin A, resibufagin, 3-(N-suberoylargininyl)-arenobufagin, 3-(N-suberoylargininyl)-marinobufagin, and 3-(N-suberoylargininyl)-resibufogenin. The content of 20 batches of Bufonis Venenum was determined according to the Chinese Pharmacopoeia(2020 edition) by high-performance liquid chromatography, and the 2 batches of Bufonis Venenum, CS7(8.99% of total content) and CS9(5.03% of total content), with the largest difference in the total content of the three quality control indexes of the Chinese Pharmacopoeia(bufalin, cinobufagin, and resibufogenin) were selected to evaluate their anti-liver tumor activity based on the zebrafish model. The tumor inhibition rates of the 2 batches were 38.06% and 45.29%, respectively, proving that only using the quality control indexes of the Chinese Pharmacopoeia as the value orientation of Bufonis Venenum market circulation was unreasonable. This research provides data support for the effective utilization of Bufonis Venenum resources and the establishment of a rational quality evaluation system of Bufonis Venenum.

9.Effects of the components of traditional Chinese medicine for invigorating qi and activating blood circulation on cognitive function of chronic cerebral ischemia model mice and its mechanism

OBJECTIVE To investigate the effects of the compo nents of traditional Chinese medicine (TCM)(total ginsenoside,total ketone ester of Ginkgo biloba ,total glucoside of Crocus sativus )for invigorating qi and activating blood circulation on cognitive function of mice with chronic cerebral ischemia (CCI)and its related mechanisms . METHODS CCI model was established by stimulating bilateral common carotid arteries of C 57BL/6J mice with temperature controlled current . Model mice were randomly divided into model group ,aspirin group (positive control 10 mg/kg)and TCM group (the components of TCM for invigorating qi and activating blood circulation 33 mg/kg),sham operation group was set up additionally ,with 16 mice in each group. Except that the mice in model group and sham operation group were given water intragastrically ,the mice in the other groups were given corresponding medicine liquid intragastrically,once a day ,for 8 weeks. The latency of step down and the number of step -down errors of mice in each group were detected ;the ultrastructure of neurovascular units(NVU)in cerebral cortex and hippocampus was observed ;the protein and mRNA expressions of vascular endothelial growth factor(VEGF)and α7 nicotinic acetylcholine receptors (α7nAChRs) in cerebral tissue were detected ;the contents of VEGF,angiopoietin 1 (Ang1) and basic fibroblast growth factor(bFGF)were detected . RESULTS Compared with model group ,the latency of step -down was significantly prolonged (P< 0.05),and the number of step -down errors was significantly reduced (P<0.05)in TCM group . The ultrastructural abnormalities of NVU in cerebral cortex and hippocampus were significantly improved ,and the protein and mRNA expressions of VEGF and α7 nAChRs and the contents of VEGF ,Ang1 and bFGF in cerebral tissue were significantly increased (P<0.05). CONCLUSIONS The components of TCM for invigorating qi and activating blood circulation can improve CCI -induced cognitive dysfunction in mice,the mechanism of which may be associated with protecting NVU ,recovering ultrastructural abnormalities of injured NVU , and regulating the expression of proteins closely related to NVU and memory function .
10.Single-shot AAV-vectored vaccine against SARS-CoV-2 with fast and long-lasting immunity.

Due to the insufficient long-term protection and significant efficacy reduction to new variants of current COVID-19 vaccines, the epidemic prevention and control are still challenging. Here, we employ a capsid and antigen structure engineering (CASE) strategy to manufacture an adeno-associated viral serotype 6-based vaccine (S663V-RBD), which expresses trimeric receptor binding domain (RBD) of spike protein fused with a biological adjuvant RS09. Impressively, the engineered S663V-RBD could rapidly induce a satisfactory RBD-specific IgG titer within 2 weeks and maintain the titer for more than 4 months. Compared to the licensed BBIBP-CorV (Sinopharm, China), a single-dose S663V-RBD induced more endurable and robust immune responses in mice and elicited superior neutralizing antibodies against three typical SARS-CoV-2 pseudoviruses including wild type, C.37 (Lambda) and B.1.617.2 (Delta). More interestingly, the intramuscular injection of S663V-RBD could overcome pre-existing immunity against the capsid. Given its effectiveness, the CASE-based S663V-RBD may provide a new solution for the current and next pandemic.
