1.BMP-7 counteracts TGF0-?1-induced tubular epithelial-myofibroblast transdifferentiation of cultured renal tubular epithelial cells

Objective To study the possible role of bone morphogenetic protein 7(BMP 7) in the TGF ?1 induced tubular epithelial myofibroblast transdifferentiation (TEMT) of cultured renal tubular epithelial cells. Methods The normal rat kidney tubular epithelial cell line (HK 2) was cultured for three days on plastic plates in the presence or absence of recombinant TGF ?1 and BMP 7. The alterations in the phenotype were assessed by phase contrast microscopy. Transdifferentiation of tubular cells into myofibroblasts was assessed by immunofluorescence, with monoantibodies to alpha smooth muscle actin (? SMA), vimentin and cytokeratin respectively. The expression of ? SMA of HK 2 cells was measured by flowcytometry. The expression of ? SMA mRNA of HK 2 cells was assessed with reverse transcriptase polymerase chain reaction (RT PCR). Results Treatment of HK 2 cells with BMP 7(50 and 100 ng/ml) for 24~48 hours increased cellular proliferation. The culture of HK 2 cells in the presence of TGF ?1 induced a clear fibroblast like morphology, a loss of the epithelial marker cytokeratin and de novo expression of ? SMA and vimentin. Immunofluorescence staining showed the addition of various concentrations of BMP 7 to subconfluent cells for 24 and 48 hours, and the expression of ? SMA and vimentin was decreased. Objective To explore the molecular mechanisms involved in hypokalemic salt-losing tubulopathies ( SLTs) through genetic screening of WNK gene in patients with SLTs. Methods Forty-four kindreds of SLTs were diagnosed Batter's syndrome or Gitelman's syndrome after CLCNKB and SLC12A3 sequencing and analysis, 8 of whose phenotype can not be simply attributed to CLCNKB or SLC12A3 mutations. Primers for PCR-amplified exons of WNK4 and WNK1 gene in genomic DNA were designed, and direct sequencing was performed to analyse the PCR products. Results Two missense mutations of WNK1, Ile~(1172)→ Met (I1172M) and Ser~(2047) → Asn (S2047N), were identified. Both of these 2 mutations segregated with the disease in SLTs kindred. Conclusion Two heterozygote missense mutations of WNK1 gene (I1172 M and S2047N) were found in 8 SLTs kindreds, indicating that WNK1 might be another gene responsible for hypokalemic salt-losing tubulopathies.
3.Influence of information drug close-loop management mode on adverse events of nursing medication

Objective:To explore the effects of information drug close-loop management mode on adverse events of nursing medication and to analyze the 46 cases of adverse events after implementing the information drug close-loop management mode so as to understand the event characteristics and formulate the countermeasures with the aim to reduce the nursing medication errors.Methods:We retrospectively analyzed the 46 cases of adverse events of nursing medication reported by system in the First Affiliated Hospital of Xiamen University after implementing the information drug close-loop management mode (from January to December 2018) . Among the 46 cases of events, we analyzed the classifications, causes and links of error and compared those events with adverse events of nursing medication before implementing the information drug close-loop management mode in 2017.Results:After carrying out the drug close-loop management, the adverse events of medication declined compared with that in 2017, but there was no statistical difference ( P>0.05) . The adverse events included 17 (17/46, 36.96%) errors in identification, 8 (8/46, 17.39%) medication omissions, 7 (7/46, 15.22%) errors in dose and 5 (5/46, 10.87%) errors in route. The links of medication errors were mainly in drug dispensing (26 cases) and arranging (14 cases) . Conclusions:Adverse events of medication most often occur in drug dispensing. The most common errors were misidentification. Nursing managers should formulate the preventive intervention based on the classifications and characteristics of adverse events in nursing medication administering, and monitor as well as improve the rate of sweeping code in drug close-loop, strength training for nurses, and check the drug dispensing and arranging well.
4.The effect of 18β-glycyrrhetinic acid on tight junctions of the nasal mucosa epithelial cells in rat models with allergic rhinitis.

OBJECTIVE:
To observe 18β-glycyrrhetinic acid (GA) impact on ultrastructure of tight junctions (TJs) of nasal mucosa epithelial cells in rats models of allergic rhinitis (AR).
METHOD:
Ninety-six Wistar rats were randomly divided into control group, model group, loratadine group, and 18β-glycyrrhetinic acid group, and each group had 24 rats. Ovalbumin was used to establish a rat AR model. The behavioral changes and the tight junctions of nasal epithelial were observed and compared in different groups after 2,4,6 and 10 weeks intervention.
RESULT:
The length of TJs in allergic rhinitis model became shorter, electron-high-density plasma membrane became thicker, number of the integration loci reduced and gap of TJs widened or even ruptured. With the consistent effect of allergens,the changes of TJs in the model group aggravated gradually,and the changes of ultrastructure of TJs in 18β-glycyrrhetinic acid group was relieved apparently compared to model group and even were close to the control model with time.
CONCLUSION
18β-glycyrrhetinic acid can recover the ultrastructure of the tight junctions of AR rat nasal epithelial cells.

5.Mendelian randomization study on the association between telomere length and 10 common musculoskeletal diseases

BACKGROUND:Multiple observational studies have suggested a potential association between telomere length and musculoskeletal diseases.However,the underlying mechanisms remain unclear. OBJECTIVE:To investigate the genetic causal relationship between telomere length and musculoskeletal diseases using two-sample Mendelian randomization analysis. METHODS:Genome-wide association study summary data of telomere length were obtained from the UK Biobank.Genome-wide association study summary data of 10 common musculoskeletal diseases(osteonecrosis,osteomyelitis,osteoporosis,rheumatoid arthritis,low back pain,spinal stenosis,gout,scapulohumeral periarthritis,ankylosing spondylitis and deep venous thrombosis of lower limbs)were obtained from the FinnGen consortium.Inverse variance weighting,Mendelian randomization-Egger and weighted median methods were used to evaluate the causal relationship between telomere length and 10 musculoskeletal diseases.Inverse variance weighting was the primary Mendelian randomization analysis method,and sensitivity analysis was performed to explore the robustness of the results. RESULTS AND CONCLUSION:(1)Inverse variance-weighted results indicated a negative causal relationship between genetically predicted telomere length and rheumatoid arthritis(odds ratio=0.78,95%confidence interval:0.64-0.95,P=0.015)and osteonecrosis(odds ratio=0.56,95%confidence interval:0.36-0.90,P=0.016).No causal relationship was found between telomere length and the other eight musculoskeletal diseases(all P>0.05).(2)Sensitivity analysis affirmed the robustness of these causal relationships,and Mendelian randomization-Egger intercept analysis found no evidence of potential horizontal pleiotropy(all P>0.05).(3)This Mendelian randomized study supports that telomere length has protective effects against rheumatoid arthritis and osteonecrosis.However,more basic and clinical research will be needed to support our findings in the future.
6.Epidemiological characteristics of RSV infection in 4079 children in Hangzhou from 2017 to 2022

Objective To investigate the positive nucleic acid detection of respiratory syncytial virus(RSV)in children with acute respiratory tract infection in Hangzhou.Methods The information of RSV nucleic acid positive cases detected in Hangzhou Children's Hospital from January 2017 to December 2022 was collected and analyzed retrospectively by age,gender,detection time and other factors.Results A total of 22 755 samples were sent for examination,among which 4079 cases of RSV were reported,with a total positive detection rate of 17.93%,showing constant fluctuation of infection over the years.In terms of gender,the positive detection rate of RSV was 10.36%in boys and 7.57%in girls,and the ratio of male to female was 1.36:1,the difference was statistically significant(P<0.05).In terms of age,there were 2378 positive cases in infants<1 year old,accounting for 58.30%of all positive cases,and 94.93%in infants<3 years old.The positive detection rate of RSV in children of different age groups showed a decreasing trend with the increase of age.In terms of seasons,RSV occurred frequently in autumn and winter,and the detection rate in two seasons was 83.82%,while the detection rate in spring and summer was only 16.18%.The positive detection rate of RSV in autumn and winter was significantly higher than that in spring and summer(P<0.05).Conclusion RSV infection fluctuates in children with acute respiratory tract infection in Hangzhou over the years,and the incidence is high in autumn and winter.The positive detection rate is high in infants and boys.Attention should be paid to the protection of key populations in corresponding seasons.
7.Expert consensus on limb management of patients with transvenous temporary cardiac pacing

Objective To form the expert consensus on the limb management of patients with transvenous temporary cardiac pacing,standardize the limb management of patients with transvenous temporary cardiac pacing,and reduce complications related to the limb.Methods Using evidence-based methods,the evidence in this field was searched,evaluated and summarized,and relevant recommendations and research conclusions were extracted and classified by the level of evidence quality,and then the first draft of the consensus was formed.From December 2023 to January 2024,through 2 rounds of expert consultation and 4 rounds of expert meetings,the content was adjusted and the consensus was reached.Results Totally 16 experts participated in the consultation.The positive coefficient is 100%;the authoritative coefficient is 0.847 and 0.836;the average value of each index is more than>3.8;the coefficient of variation is less than 0.21.The Kendall's harmony coefficient of the 2 rounds of expert consultation is 0.372 and 0.314,respectively,which were statistically significant.The consensus covers the preoperative,intraoperative and postoperative on limb management of patients with transvenous temporary cardiac pacing.Totally 11 themes were involved,including the preoperative preparation,position and catheter fixation in operation,position and catheter fixation in postoperative,activity,turn and transfer,duty shift on limb,nursing care after withdrawal of the catheter,prevention of deep vein thrombosis of the operative limb and prevent infection.Conclusion The consensus is highly scientific,and it is helpful to standardize the limb management of patients with transvenous temporary cardiac pacing.

Objective To analyze theoretical frameworks, definitions, terminology and measurements of disability using International Classification of Functioning, Disability and Health (ICF), and United Nations Convention on the Rights of Persons with Disabilities (CRPD), to provide a scientific basis for developing definition, classification and measurements to meet international standards.Methods Content analysis had been conducted for the definitions and measurements of disability issued by international organizations using the ICF terminology and code and the definitions of disability and people with disability by CRPD.Results The core concepts and content had been reviewed using the ICF terminology and code, and the definitions of disability and people with disability by CRPD. The contents of definitions of disability proposed by international organizations cover all components of ICF. The contents of standardized disability measurements issued by United Nations or World Health Organization cover all components of ICF and map to CRPD definitions of disability and people with disability as the framework, classification, terminology and coding system had been adopted for the development.Conclusion The definition and measurements had been developed using ICF and CRPD framework, terminology, and coding system of disability. The framework and standard of disability had been constructed. The standard of disability data will implement in the fields of health, rehabilitation, education, employment, community and social services for people with disability to promote data exchange across sectors.
