1.Change of the hilar periportal space in the patients with chronic hepatitis B and liver cirrhosis after hepatitis B

Objective To investigate thickness of the hilar periportal space and caudate-right lobe ratio in the patients with chro-nic hepatitis B and liver cirrhosis after hepatitis B .Methods Eighty-four patients who were clinically and histologically diagnosed with chronic hepatitis B or cirrhosis and 18 healthy subjects without history of liver disease underwent abdominal MRI .The rela-tionship among liver fibrosis degree ,hilar periportal space and caudate-right lobe ratio were observed .Results There was signifi-cant correlation between the hilar periportal space and hepatic fibrosis for chronic hepatitis and cirrhosis (rs = 0 .546 ,P< 0 .01) . There was significant difference between S2 and S3 for thicknesses of the hilar periportal space(P<0 .01) ,and no significant differ-ence among S3 ,S4 and cirrhosis(P=0 .188) .A cutoff value of 9 mm for the hilar periportal space had a sensitivity of 85 .37% and a specificity of 76 .79% for a diagnosis of hepatic fibrosis with S3 or higher .There was no significant correlation between the caudate-right lobe ratio and hepatic fibrosis(rs = -0 .155 ,P=0 .119) .Conclusion Thicknesses of the hilar periportal space increase gradu-ally with hepatic fibrosis in patients with chronic hepatitis B and cirrhosis ,with a high sensitivity and specificity for a diagnosis of hepatic fibrosis with S3 or higher .
2.Preliminary study of three-dimensional speckle tracking echocardiography in evaluation of left ventricular systolic longitudinal strain in patients with uremia

Objective To evaluate the characteristics of left ventricular systolic longitudinal strain in uremic patients and its relationship with dialysis time using three-dimensional speckle tracking echocardiography (3D-STE).Methods Forty-six uremic patients were divided into three groups according dialysis time (ranged 0-5 years):group A (≤1 year),group B (1 year>time ≤3 years),group C (3 years >time≤5 years),group D was control group.The dynamic images of apical four chamber view were displayed and apical full-volume images were obtained and saved in all subjects using a full volume matrix probe.The original data was recorded and was analyzed by using off-line 3DT analysis software (TomTec 4D MV-Assessment).The longitudinal peak systolic strain and 16 segment systolic strains and strain mean value in 16 segment of left ventricular were acquired and compared.Results The peak strain of apical segment was lower than that of the basal segment and the middle segment and it had an increasing trend from the apical segment to the basal segment in three uremia groups.Instead,it had a decreasing trend from the apical segment to the basal segment in control group.But the differences were not statistically significant.The longitudinal peak systolic strain value,16 segment strain value and mean value of 16 segment strain all were lower in three uremic groups than those of control group.Every segment of 16 segment stain value in group A was higher than those of group B and C.There was no difference in every segment between B and C group in addition to basal and middle segment of anterior septum.Conclusions 3D-STE could evaluate the left ventricular systolic function in uremic patients and the cardiac function could be partly protected by regular dialysis treatment.
3. Optimization of ion selective electrode method in detection of urinary fluorine

Objective:
To optimize the ion selective electrode method of measuring urinary fluorine in WS/T 30-1996.
Methods:
The volume of 5 mol/L NaOH solution was fixed to confect the TISAB buffer solution. The samples were prepared by mixing 5 ml urine and equal volume of TISAB solution. Fluorine electrode and calomel electrode was used to test. When the potential value changed less than ±0.5 mV in one minute, it could be recorded. The standard curve method was selected as a quantitive method.

4. Determination of fluorine-ion in human urine by ion chromatography method

Objective:
To establish a method to determine fluorine ion in human urine by ion chromatography method.
Methods:
The samples were centrifuged, diluted and processed by C18 solid-phase column, potassium hydroxide as mobile phase, gradient elution, separated by AS15, and determined by anion conductivity detector.
Results:
The linear correlation coefficient of fluoride ion in the range of 0.02-10.00 mg/L was more than 0.999. The lowest detection limit of urinary fluoride was 0.06 mg/L (in 3 ml urine) . The recoveries of the method were 96.9%-99.5%, the inter batch precision range was 0.40%-2.89%. The intra batch precision was from 1.01 to 7.23%. Fluoride samples could be stored for 7 days at room temperature and 4 ℃, and be stored for 14 days at -20 ℃.
Conclusion
Determination of fluoride in urine by ion chromatography, the method could meet the requirement in GBZ/T 210.5-2008.
5. Analysis on occupational health laboratories' proficiency testing program for determination of bovine blood lead and urinary cadmium

Objective:
To analyze the results obtained from the proficiency testing program for determination of bovine blood lead and urinary cadmium, so as to evaluate the proficiency of the laboratories, and improve the detection and quality control capability of occapational health laboratory.
Methods:
Prepared 3 levels of lead samples with bovine blood as base material, taking Technical Norm of Primary Reference Material as Operating standard, 60, 80, 180 μg/L were used as the standard concentration. All samples were made for 200 bottles. Normal urine were used to prepared 3 levels of cadmium samples with 5.0, 7.0, 10.0 μg/L as standard concentration, then 3 level of samples were made for 200 bottles respectively. Blood lead and urinary cadmium samples were sent to each laboratory in the form of sample pairs. One-way ANOVA was used to investigate the homogeneity. 6.Group rehabilitation training can improve the speech and life quality of persons with post-stroke dysarthria

Objective:To observe the effect of comprehensive rehabilitation training in a group on the speech and life quality of stroke survivors with dysarthria.Methods:Stroke survivors with dysarthria were randomly divided into an observation group (of 43) and a control group (of 41). In addition to routine medication, the control group was given 30 minutes of one-to-one rehabilitation training twice a day, 6 days a week for 4 weeks while the observation group was provided with 30 minutes of group training. Before and after the intervention, both groups were evaluated using the comprehensive dysarthria assessment scale and the WHOQOL-100 life quality assessment.Results:After the treatment, both groups′ average scores on the comprehensive assessment scale had improved significantly. Except for jaw positioning, all of the observation group′s other scores were significantly better than those of the control group, on average. The observation group also performed significantly better on the WHOQOL-100 assessment, on average.Conclusions:Group rehabilitation training can improve the speech function and life quality of stroke survivors with dysarthria significantly better than one-to-one training.
7.Evaluating the role of quantitative flow ratio in percutaneous coronary intervention using regadenoson stress D-SPECT

Objective To evaluate the role of quantitative flow ratio(QFR)in percutaneous coronary intervention(PCI)by using regadenoson stress dynamic single-photon emission computed tomography(D-SPECT).Methods We selected 200 patients with unstable angina admitted to Department of Cardiology,Hebei Medical University First Hospital,from June 31,2021 to June 31,2023 for elective PCI.The patients were aged 57.56±8.23 years and were randomly divided into a conventional group(n=100)and a QFR group(n=100)according to a random number table.The trial was conducted using a double-blind method.The conventional group received PCI treatment based on the experience of the physician,while the QFR group received PCI treatment based on the QFR measurement results.All enrolled patients underwent pre-operative and 7-day post-operative D-SPECT stress imaging using regadenoson stress D-SPECT,and their images were acquired from short axis,vertical long axis,and horizontal long axis to calculate the total myocardial perfusion score and the total myocardial ischemia segment number under the distribution of 17 myocardial segments.Results There was no significant difference in the number of myocardial ischemia segments(7.59±3.14 vs.7.48±3.36,P=0.811)or the total myocardial perfusion score(15.87±7.61 vs.15.63±5.97,P=0.860)between the two groups before PCI.The number of myocardial ischemia segments(5.58±3.36 vs.6.51±2.14,P=0.020)and the total myocardial perfusion score(10.55±4.41 vs.12.96±6.50,P=0.002)in the QFR group were significantly better than those in the conventional group 7 days after PCI(P<0.05).Conclusion Applying QFR guidance for PCI can better improve the degree of myocardial ischemia in patients.

Objective:To develop a reference material for the analysis of mercury in freeze-dried bovine blood.Methods:The whole blood of the bovine was collected, mercury standard solution was added, After mixing, packaging and freeze-drying. Two concentration levels of reference materials were prepared. The homogeneity and stability were evaluated by t-test and F-test methods respectively. The uncertainty in preparation, long-term stability, short-term stability and fixed value were evaluated Evaluation and assignment of reference materials. The research products were fixed value and were analyzed by two methods and fixed value in 9 laboratories and the uncertainty during development was analyzed.Results:The reference material has good uniformity and can be kept stably for 12 months at -20 ℃; When transported at ≤35 ℃, the quantity value is stable within 7 days; After re dissolution, it can be stored at 4 ℃ for 5 days; The values are 9.4 respectively μg/L and 29.2 μg/L, and the uncertainty is ±1.1 respectively μg/L and ±2.3 μg/L ( k=2) . Conclusion:The indexes of reference materials for mercury composition analysis in freeze-dried bovine blood meet the requirements of technical specifications and have certain application value.

10.Evaluation of the effect of remote ischemic preconditioning combined with adenosine injection in patients undergoing PCI using D-SPECT

Objective This study utilized regadenoson stress D-SPECT to evaluate the impact of remote ischemic precondi-tioning combined with adenosine injection in patients undergoing percutaneous coronary intervention(PCI)using D-SPECT imaging.Methods Three hundred patients requiring coronary angiography and stent implantation were randomly assigned into two groups utilizing the random number table method.The conventional treatment group(150 patients)and the experimental group(150 patients)received standard treatment for coronary heart disease PCI.The experimental group underwent two sessions of remote ischemic precon-ditioning on the day of PCI,wherein the left and right upper arms were inflated to 200 mmHg with continuous pressure for 5 minutes using a blood pressure cuff.Additionally,during the PCI procedure,adenosine was administered to the experimental group at a rate of 50 μg/(kg·min)until the end of the procedure,with other treatments aligning with the conventional treatment group.Each patient received a coronary SYNTAXⅡscore.Resting D-SPECT and regadenoson stress D-SPECT were employed to evaluate total myocardial perfusion score,total myocardial ischemia segments,and left ventricular ejection fraction across 17 segment distributions before and 7 days after PCI.Results Before PCI,the differences in the number of myocardial ischemic segments,total myocardial perfusion score,and left ven-tricular ejection fraction between the two patient groups were not statistically significant(P>0.05).However,7 days post-PCI,compared with the conventional treatment group,the experimental group exhibited significantly improved numbers of myocardial ischemic segments,total myocardial perfusion score,and left ventricular ejection fraction(P<0.05).Furthermore,the differences in adverse drug reactions between the two patient groups were not statistically significant(P>0.05).Conclusion The combined application of remote ischemic preconditioning and adenosine injection in patients undergoing PCI is deemed safe and effective.
