1.Design and Development of Drug Management System of Peacekeeping Medical Team

OBJECTIVE:To improve the drug comprehensive management level of peacekeeping medical team,achieve dynam-ic management of drug information and network. METHODS:The design and development of drug management system of peace-keeping medical team were introduced in terms of overall system architecture,technical routes,system function design,etc.,and the application effect was evaluated. RESULTS:The system was designed by referencing military hospital No.1 drug management system,adopting C/S two-tier architecture,which included system settings,drug dictionary management,inventory management and other 8 modules and numbers of sub-modules. It achieved drug getting out and entering warehouse,supplying maintenance man-agement,prescription dispensing,drug withdrawal and inquiries,expendable drug monitoring,rotation transfer export data,the whole English electronic medical instruments generation,data query statistics,timely query of drugs and disease diagnosis knowl-edge base and other functions. The application of system standardized drug management and significantly improved working efficien-cy of physicians and pharmacists. CONCLUSIONS:The application of the system promotes the transform of drug management from extensive management model to elaborating management model,and the data is safe and easy to operate.
2.Application of purse string suture stitched in gastric wall of anastomotic stoma for intrathoracic esophagogastric apparatus anastomosis

Objective To explore the safety and clinical efficacy of the purse string suture stitched in gastric wall of anastomotic stoma for intrathoracic esophagogastric apparatus anastomosis.Methods The clinical data of 238 patients with thoracic esophageal carcinoma and 24 patients with carcinoma of the esophagogastric junction received intrathoracic apparatus anastomosis at the Sichuan Provincial People's Hospital from January 2008 to December 2011 were retrospectively analyzed.There were 122 patients received conventional intrathoracic esophagogastric anastomosis (conventional group) and 140 patients received purse string suture stitched in gastric wall of anastomotic stoma before intrathoracic esophagogastric anastomosis (improvement group).The incidences of anastomotic fistula and stenosis of the 2 groups were compared.All data were anlayzed using the t test,chisquare test or Fisher exact probability.Results There were no significant differences in the operation time,intraoperative blood loss,volume of drainage of peritoneal effusion within 24 hours after operation,postoperative hospital stay and postoperative pTNM staging between the 2 groups (t =0.410,0.798,0.634,0.362,x2=0.605,P > 0.05).There were no significant differences in the anastomotic location,stapler type,the weight of stapler esophageal end tissue between the 2 groups (x2 =0.118,0.221,t =0.459,P > 0.05).There were no significant differences in the incidences of pulmonary complication,arrhythmia and mortality between the 2 groups (P > 0.05).The weight of stapler stomach end tissue in the improvement group was significantly greater than that of the conventional group,while the incidences of postoperative anastomotic fistula and stenosis of the improvement group were significantly lower than those of the conventional group (t =13.856,P < 0.05).Conclusion The purse string suture stitched in gastric wall of anastomotic stoma for intrathoracic esophagogastric apparatus anastomosis is simple and safe,and could effectively reduce the rate of anastomotic fistula and stenosis.
3.Effect of lipide diet on the pharmacokinetics of levofloxacin in Beagle dogs

Objective To study the effect of lipide diet on pharmacokinetics of levofloxacin (Lfox) in beagle dogs .Methods In randomized cross over experiment ,6 Beagle dogs were given Lfox 10 mg/kg at the condition of empty stomach or with lipide diet for 2 week .Plasma concentration of Lfox was determined by HPLC at different time (during 48 h) .The pharmacokinetic parameters were calculated by 3p97 software .Results The plasma concentration-time curves were two compartment modelat the condition of empty stomach or lipide diet .Main pharmacokinetic parameters were as follows :t1/2β:(11 .07 ± 8 .46)and(21 .79 ± 4 .59)h ;tmax :(3 .42 ± 0 .41) and(2 .00 ± 0 .32)h;Cmax :(2 .71 ± 0 .60)and(2 .52 ± 0 .38)μg/mL ;AUC0→ t :(36 .51 ± 8 .66) and(37 .19 ± 8 .21)μg · h-1 · mL -1 ,respectively .There were significant differences in pharmacokinetic parameters of tmax (P<0 .01)except for t1/2β、Cmax and AUC0→ t .Conclusion Lipide diet can quicken tmax ,has no effect on the absorption of Lfox .
4.Acupoint selection and medication rules analysis for allergic rhinitis treated with acupoint application-based on data mining technology.

To make a systematic study on acupoints selection and medication rules for allergic rhinitis treated with acupoint application, literature regarding clinical researches on acupoint application for allergic rhinitis since 2000 has been retrieved; medication and acupoints involved were preprocessed and cluster analysis technique was applied to classify the data. As a result, a total of 37 pieces of literature were included, involving 27 types of herbs and 33 acupoints. The results showed Baijiezi (Brassica alba Boiss), Xixin (Radix et Rhizoma Asari), Gansui (Radix Kansui), Yanhusuo (Corydalis), Jiangzhi (Ginger juice) and Baizhi (Radix angelicae) constituted the main herb prescription, and the layer was increased along with frequency of use. Feishu (BL 13), Dazhui (GV 14), Shenshu (BL 23), Pishu (BL 20), Gaohuang (BL 43), Fengmen (BL 12) constituted the main acupoint prescription, which were clustered into 4 classes; Feishu (BL 13) and Dazhui (GV 14) constituted the first class; Pishu (BL 20) and Shenshu (BL 23) constituted the second class; the first and second class merged into the third class, Including Feishu (BL 13), Dazhui (GV 14), Shenshu (BL 23) and Pishu (BL 20); Gaohuang (BL 43) and Fengmen (BL 12) constituted the fourth class; the fourth class and the third class constituted the whole acupoint prescription. It was showed that for the allergic rhinitis treated with acupoint application, the medication was mainly epispastic herbs with pungent nature that belonged to the lung, spleen and kidney meridian, which presented the principle of simultaneous treatment of principal and subordinate symptoms; also the medication can be selected from core layer to supporting layer based on clinical needs; for the selection of acupoints, acupoints in lung and bladder meridian were mainly selected to assist exterior and resist the pathogenic qi, which could treat allergic rhinitis fundamentally by joint use with acupoints in soleen and kidney meridian.

5.Acupoints selecting and medication rules analysis based on data mining technique for bronchial asthma treated with acupoint application.

Clinical literature of bronchial asthma treated with acupoints application from January 2000 to March 2014 in modern periodicals databases was retrieved through computer. With cluster analysis and frequency analysis methods of data mining, acupoints selecting and medication rules of bronchial asthma treated with acupoints application were analyzed. Total 38 articles were included eventually, including 25 acupoints and 42 medicines. The results indicate that on acupoints selecting, Feishu (BL 13) is used as the main acupoint and 3 groups of bladder meridian and conception vessel acupoints are applied alternately and on medicines, Baijiezi (Brassica alba Boiss), Xixin (Radix et Rhizoma Asari), Gansui (Radix Kansui), Yanhusuo (Corydalis) and Mahuang (Radix et Rhizonma Ephedrae) are primarily adopted, epispastic medicines being the main medicines; medicines mostly belong to lung meridian, main medicines being unchanged mostly with Shengjiang as guiding drug.

6.Influencing factors of arterial stiffness in the elderly patients with hypertension and type 2 diabetes mellitus

ObjectiveTo evaluate arterial stiffness and its influencing factors in the elderly patients with hypertension and (or) type 2 diabetes mellitus.MethodsTotally 320 elderly patients were divided into four groups: control,diabetes mellitus,hypertension,coexisting hypertension and diabetes mellitus.Arterial stiffness in ankle and brachial artery was determined by brachial-ankle pulse wave velocity (baPWV).Other factors affecting baPWV were also collected and measured,including age,body mass index (BMI),sex,smoking,blood pressure,pulse pressure,mean arterial pressure,serum fasting blood glucose,lipid profile,glycosylated hemoglobin and hypersensitive C-reactive protein (hsCRP).ResultsbaPWV in the four groups were ( 1667.2 ± 279.3 )cm/s,(2158.6 ±386.9)cm/s,(1881.2±383.8) cm/s and (2165.9± 479.9)cm/s,respectively.The baPWV value was higher in groups of hypertension,diabetes mellitus,and the coexisting diseases than in controls (F=8.473,P<0.05).Diabctic patients had a higher baPWV levels than those without diabetes.There was no difference in baPWV levels between diabetic patients with or without hypertension.Pulse pressure and hsCRP were higher in coexisting diseases group than in control group (P<0.05).Stepwise backward regression analysis showed that fasting blood glucose,pulse pressure,mean blood pressure,hsCRP and low density lipoprotein cholesterin (LDL-C) were positively related to baPWV P <0.05). ConclusionsHypertension and (or) type 2 diabetes mellitus are important influencing factors for higher arterial stiffness. Hyperglycemia might play more important role in the elderly arterial stiffness.Serum fasting blood glucose,pulse pressure,mean blood pressure,hsCRP,LDL-C may be the independent risk factors of arterial stiffness.
7.Immunogenicity of recombinant Lugurus zone pellucida-3 expressed in E.coli and efficacy of its antiserum to affect in vitro mouse sperm-egg binding in vitro

Objective:Present study was aimed to analyze the immunogenicity of recombinant Lugurus zona pellucida-3 protein (r-LZP3) expressed in E.coli,and to evaluate the efficacy of its antiserum to affect in vitro mouse sperm-egg binding.Methods:Female mouse were immunized using LZP3 DNA vaccine for priming,and boosted with r-LZP3 protein.The antibody response level of LZP3 antisera was determined by ELISA.The immunoreactivity and specificity of the anti-LZP3 antisera were tested by immunoblot, indirect immunofluorescence and immunohistochemistry using native mouse ZP.A sperm-zone pellucida binding assay was used to evaluate the efficacy of anti-LZP3 antisera to inhibit mouse sperm-egg binding in vitro.Results: The result of ELISA showed that LZP3 DNA vaccine was able to induce higher antibody titers in mouse.Antiserum could specifically recognize or bind to r-LZP3 protein expressed in E.coli and native mouse ZP in vitro.The LZP3 antisera and r-LZP3 protein also inhibited sperm-egg binding in vitro.Condusion:These results show that r-LZP3 protein is of strong immunogenicity.
8.The efficacy and safety of different simvastatin doses on hyperlipidemia in the elderly

Objective To study the efficacy and safety of different simvastatin doses on hyperlipidemia in the elderly .Methods 120 patients with hyperlipidemia were randomly divided into two groups ,60 cases in each group ,patients were given simvastatin 10 mg/d(10 mg/d group) or 20 mg/d(20 mg/d group) .Data were collected before and after treatment ,including total cholesterol (TC) ,three acyl glycerin (TG) ,low density lipoprotein cholesterol (LDL-C) ,high density lipoprotein cholesterol (HDL-C) ,treat-ment efficacy and adverse reactions events .Results 3 month after treatment ,TC ,TG ,LDL-C and HDL-C were better than before treatment in both groups(P<0 .05);and the curative effect in 20 mg/d group was better than 10 mg/d group(P<0 .05) .There was no significant difference in incidence of adverse reactions events between the two groups (P>0 .05) .Conclusion 20 mg/d simvasta-tin is better than 10 mg/d simvastatin in efficacy of elderly patients with hyperlipidemia ,and offers a safe and reliable treatment .
9.Glutamate Involved in Rats with Spinal Cord Injury Induced by Dynorphin A (1-17)

Intrathecal (I. T.)administration of K opioid dynorphin A (1-17) is used as a model of neurological dysfunction which lnvolved in spinal cord injury and secondary affection according to several previous reports. 5-amlno-2-phosphoveroleric acid (APV ), an NMDA receptor antagonist, can significantlly prevent the hindlimb paralysis in rats produced by I. T. dynorPhin A (1-17). To further investigate the mechanisms, we establis11 a nlodel of [3H]L,-Glu release in rats spinal slices influenced by dynorphin A (dynA ) (1-17). [3H]L-Glu release evoked by high [K+] (5Ommol/L,)is a Ca2+-dependent process. DynA (1-17) slgnificantly inhibited [3H]L,-Glu release at 1O-8mol/L,, but very significantly enhanced [3H]L-Glu release at 10-6 mol /L. The synthetic k agonist U50, 488H, which has no neurotoxic effect, inhibited [3H]L-Glu release at both high and low concentrations and did not have any enhancing effect. The results suggest that the analgesic effect of dynA (1-17) at physiological dosage may be rnediated by presynaptic K opioid receptor through the inhibition of Ca2+ influx and L-Glu release;but dynA (1-l7)enhanced L-Glu release through a non-opioid pathway and induced hindlimb paralysis at high neurotoxic dosage.
10.Normal light and fluorescence microscopy for authentication of Delphinii Brunoniani Herba of Tibet.

Dried herb of Delphinium brunonianum Royle (Ranunculaceae) has long been used under the herbal name "Xiaguobei" (Delphinii Brunoniani Herba) in traditional Tibetan medicine and prescribed for the treatment of influenza, itchy skin rash and snake bites. In order to find a useful and convenient method for the identification of microscopic features, the technique of fluorescence microscopy was applied to authenticate "Xiaguobei" of Tibet. The transverse sections of stem and leaf, as well as the powder of "Xiaguobei" were observed to seek for typical microscopic features by normal light and fluorescence microscopy. A style-like, single-cell glandular hair containing yellow secretions on the leaf, young stem and sepal of "Xiaguobei" was found. Under the fluorescence microscope, the xylem and pericycle fiber group emitted significant fluorescence. This work indicated that fluorescence microscopy could be an useful additional method for the authentication work. Without the traditional dyeing methods, the main microscopic features could be easily found by fluorescence microscopy. The results provided reliable references for the authentication of "Xiaguobei".
