1.MRI Appearances of Liver Carcinoma after Gamma Knife Therapy

Objective To study the value of unenhanced and dynamic contrast-enhanced MR imaging in follow-up of liver carcinomas after gamma knife therapy.Methods 57 patients examined by MR imaging after gamma knife therapy were analyzed retrospectively,and compared with outcome of AFP.Results Lesions treated with gamma knife therapy on precontrast imaging showed 4 kinds of appearances:(1) Slightly low signal intensity on T1WI and slightly high signal intensity on T2WI.(2)Slightly low signal intensity on both T1WI and T2WI.(3)Slightly high signal intensity on T1WI,isointensity and slightly high signal heterogeneous intensity on T2WI.(4)Slightly high signal intensity on both T1WI and T2WI.The necrotic lesions showed sustained ring-shaped or honeycomb enhancement or patching enhancement in hepatic tissue around the lesions with lesions unenhancing.Recurrent lesions or new lesions were enhanced in arterial phase,followed by rapid wash-out in the delay phase.The rate of completely necrosis was 93.5% in small liver carcinoma group(
2.CT-guided transthoracic cutting needle lung biopsy: its clinical application in diagnosing diffuse lung diseases

Objective To evaluate the clinical application and safety of CT-guided transthoracic cutting needle lung biopsy in diagnosing diffuse lung diseases. Methods By using automatic biopsy gun (14 & 16 gauge), CT-guided transthoracic cutting needle lung biopsy was performed in 29 cases with diffuse lung diseases. The samples obtained were sent for pathological and immunohistochemical examination. The sampling successful rate, the diagnostic accuracy and the occurrence of complications were analyzed. Results Technical success rate was 100%, and large size of sample enough for pathological and immunohistochemical examination was obtained in all 29 cases. Definite pathological diagnosis could be made in 25 cases, with the positive diagnostic rate of 82.8%. The main complications included pneumothorax and pulmonary hemorrhage. The symptoms in most cases were not severe and disappeared within one week after the treatment. Conclusion For the diagnosis of diffuse lung diseases, CT-guided transthoracic cutting needle lung biopsy is a safe, easy, effective and reliable method with high successful rate, high diagnostic value and fewer complications, in these respects this technique is superior to transbronchial lung biopsy, open lung biopsy and video-assisted thoracoscopic lung biopsy. Therefore, this technique should be popularized in clinical practice.

4.Efficacy of amlodipine combined irbesartan in treatment of essential hy-pertension

Objective To investigate the efficacy of irbesartan combined amlodipine in treatment of essential hyperten-sion. Methods A total of 100 cases essential hypertension in our hospital were randomly divided into study group and the control group, 50 cases in each group, the study group were given irbesartan(Upway) 150mg/d orally, amlodipine (Norvasc)5 mg/d orally; control group were given 150 mg/d orally. The total treatment course were 8 weeks. Results (1)The study group and the control group of patients with SBP, DBP after treatment was significantly lower than before treatment, after treatment, SBP and DBP of study group was improved than the control group more significant, the dif-ference was statistically significant. (2)The total efficiency of study group and the control group respectively was 95.0%, and 80.0%, the difference was statistically significant between the two groups (χ2=4.923, P<0.05) after treatment. (3)Ad-verse reactions: During the two groups of patients showed no abnormal kidney function. Conclusion Amlodipine com-bined with irbesartan in treatment of essential hypertension has significant effect and safety, should be popularized and applied.
5.A cross-sectional study of 4 mental disorders in Chifeng City of Inner Mongolia Autonomous Region

Objective:To describe the epidemiological characteristics of mental disorders in community resi-dents aged 18 years and over in the Chifeng City of Inner Mongolia Autonomous Region.Methods:Six thousand three hundred and seventy six individuals aged 18 years and over were sampled using stratified Probability-Propor-tional-to-Size Sampling in Chifeng City in 2010.All respondents were investigated by face-to-face interview.The Composite International Diagnostic Interview-3.0 Computer Assisted Personal Interview (CIDI-3.0-CAPI)was used to make diagnoses based on the definition and criteria of the Diagnostic and Statistical Manual of Mental Dis-orders,Fourth Edition (DMS-IV).Results:A total of 4528 subjects completed the CIDI-3.0-CAPI.Regarding anxi-ety disorder,mood disorder,substance use disorder,and impulse control disorder,the 30-day adjusted prevalence rates (95%CI)were 2.5%(2.08% -2.99%,0.9%(0.67% -1.23%),0.5%(0.34% -0.76%),and 0.6%(0.41% -0.86%)respectively,and the 12 -month prevalence rates were 4.5% (3.93% -5.15%),2.4%(2.0% -2.90%),1.1% (0.84% -1.45%),and 1.0% (0.74% -1.32%)respectively.The lifetime adjusted prevalence rates were 6.6% (5.92% -7.36%),6.5% (5.81% -7.24%),2.7% (2.26% -3.21%),and 1.4%(1.23% -1.95%)respectively.Conclusion:According to morbidity,anxiety disorders,mood disorder,substance use disorders and impulse control disorders are common in Chifeng City of Inner Mongolia Autonomous Region se-quencely,being a prominent public health problem.

7.Epidemiological characteristics of thyroid nodules and risk factors for malignant nodules: a retrospective study from 6,304 surgical cases.

BACKGROUNDThe prevalence of thyroid nodules (TN) is increasing rapidly. This study analyzed the epidemiological and clinical characteristics of TN in surgically treated patients and identified the risk factors for malignant nodules (MN) to provide more understanding of the differential diagnosis of TN.
METHODSA total of 6 304 TN cases who underwent thyroid surgery were included in this retrospective study. The clinical data were collected to evaluate the clinical and epidemiological characteristics and related risk factors for MN. The nature of TN (benign nodules (BN) or MN), medical records, laboratory data, and imaging data were analyzed. The risk factors for MN were screened using Spearman's rank correlation analysis and nonconditional binary Logistic regression analysis.
RESULTSThe number of surgically treated TN cases increased yearly. A total of 34.33% of cases were MN and 65.67% were BN. Up to 56.74% of these cases underwent unnecessary surgery. Among the MN cases, papillary thyroid carcinoma accounted for 94%, in which 46.71% coexisted with benign thyroid disease and 32.28% with multiple foci. Single-related factor analysis showed that age, employment, disease duration, history of breast nodules and/or hypertension, the levels of serum thyroid-stimulating hormone (TSH), thyroglobulin antibody (TgAb), and thyroid peroxidase antibody (TPoAb), and ultrasound features of TN were related to MN. Stepwise nonconditional binary Logistic regression analysis showed that 13 factors may be the independent risk factors for MN, including <40 years old, previous history of breast nodules and/or hypertension, disease duration <1 month, employment, hypoechoic nodule, irregular nodules, nodule calcification, solid echo nodule, fuzzy boundary, rich blood flow within nodules, abnormal lymph nodes around the neck, nodule diameter <1 cm, and abnormally high TgAb.
CONCLUSIONSOur results demonstrate a rapid increase in surgically treated TN cases and ratio of MN and indicate unnecessary surgeries in some cases. This study also suggest that age, duration of thyroid disease, history of breast disease and/or hypertension, the levels of serum TSH, TgAb, and TPoAb, and ultrasound features of TN are related to MN, and some of these factors may be the risk factors for MN.
