Traditional Chinese medicine (TCM)has certain advantages in the treatment of chronic liver diseases,especially chronic inflam-matory injury and liver fibrosis.However,the studies on medicinal components and action mechanism of TCM prescriptions are confronted with a great challenge.The computational pharmacology methods used in recent years,mainly including virtual screening technique,omics technologies,and network pharmacology approaches,as well as their application in TCM research,are presented;given the previous re-search work,the progress in research on active ingredients and action mechanism of TCM prescriptions using computational pharmacology methods is reviewed.It is thought that computational pharmacology techniques combined with conventional molecular biological and pharma-cological methods are an important research strategy for the material basis and action mechanism of TCMprescriptions for treating chronic liv-er diseases.

Objective To summarize the clincal effect of open reduction and internal fixation with locking-proximal-humerus-plate(LPHP)to treat proximal humeral fractures.Methods A total of 19 eases with proximal humeral fractures were treated with LPHP from August 2003 to February 2006 in this study.Their mean age was 58.6 years old.According to Neer classification,two cases were with two- part fractures,nine with three-part fractures and eight with four-part fractures including seven cases with osteoporosis.Redaction and fixation was done via deltopeetoral-groove approach.Bone graft was applied for five eases.Results The follow-up period was 3 to 32 months(mean 17 months).All cases were healed and the union period of 3-5 months.According to the Near shoulder score,the results showed that ten cases were excellent,6 cases good,with excellent rate of 84%.Conclusion The LPHP method is suitable for osteoporosis and comminuted proximal humeral fractures and enhances the treatment effect.

Objective To explore the safety and effectiveness of transurethral resection of the prostate (TURP) for large benign prostatic hyperplasia.Methods A total of 56 cases of patients with large benign prostatic hyperplasia were analyzed retrospectively.Results The weight of prostate tissue resected with TURP was 60.2 g (48-122 g).The resection rate was 60％,operation time 113.4 min (70-180 min).The lower urinary tract symptoms (LUTS) after TURP was improved significantly.The international prostate symptom scores (IPSS) of 38 cases were decreased from (25.6±1.5) before operation to (6.0± 2.0) after operation.The maximum urine flow rate were increased from (5.0±1.9) ml/s to (18.0±4.5) ml/s.The mean residual urine volume of 22 cases were reduced from 95 ml (40-250 ml) to 10 ml (0-60 ml).Conclusions Skilled TURP technology for large benign prostatic hyperplasia is a safe treatment with good effect and low complications rates.

Objective To investigate the correlations of ATP-binding cassette transporter A1 (ABCA1) protein expression in renal interstitium with foam cells formation,tubulointerstitial lesion and serum cholesterol in glomerulonephropathy.Methods Five patients with Alport syndrome,28 patients with membranous proliferative glomerular nephritis (MPGN),35 patients with focal segmental glomerulosclerosis (FSGS),36 patients with idiopathic membranous nephropathy (IMN) and 34 patients with IgA nephropathy (IgAN) were enrolled in the study.Expression of ABCA1 protein was detected by immunohistochemical method.The relative area of foam cells in renal interstitium was semi-quantified and the correlations of ABCA1 protein expression in renal interstitium (except for renal tubules) with the relative area of foam cells,integration of tubulointerstitial lesion (TIL) and serum cholesterol were examined using t test and linear correlation analysis.Results In renal interstitium,monocyte-macrophages,foam cells and renal tubular epithelial cells expressed ABCA1 protein.ABCA1 protein expression in renal interstitium with foam cells infiltration was significantly higher as compared to that without foam cells infiltration (P＜ 0.05),but there was no correlation between ABCA1 expression and foam cells area (P＞0.05).In MPGN,FSGS,IMN patients,there was a positive correlation between ABCA1 protein expression in renal interstitium and integration of TIL (P＜0.05).There was a positive correlation between serum cholesterol level and ABCA1 protein expression in renal interstitium (P＜0.05).Conclusions In glomerulonephropathy,cholesterol may promote the ABCA1 expression in renal interstitium.ABCA1 may have double-effects on transportation of cholesterol.ABCA1 may take part in the process of tubulointerstitial lesion.

Objective To assess the safety and efficacy of the local nerve block anesthesia procedure in daysurgery inguinal herniorrhaphy practice.Methods Fifty-eight inguinal herniorrhaphy were performed under the local nerve block anesthesia in the form of day surgery between April 2014 and April 2015 other 58 inguinal hemiorrhaphy were performed under general anesthesia.Time of operation,hospitalization stay,complications and costs were evaluated.Results The average operating time of the day-surgery group was (47 ± 12) min.All operations were completed successfully.Fifty-six patients were discharged in 24 hours and 2 patients were discharged in 48 hours after the surgery.The average costs were 8437 yuan.The rate of postoperative complications was approximately 6.6％ (4/58),including hematoma(3 cases),urinary relention(1 case),and chronic pain (1 case).The VAS scores of 6 hours after the operation were (2.4 ± 0.8).Conclusions It suggests that preperitoneal hemiorrhaphy under the local nerve block anesthesia in the form of day-surgery was safe and effective.Also,it could significantly shorten the time of hospitalization and reduce medical costs.

The distinctive LT siRNAs were designed according to the LT sequence .During the process of cultivation ,siRNA targeting the LT gene ,non‐specific control siRNA ,negative control siRNA and culture medium were added into siRNA group (siRNA‐LT1 group , siRNA‐LT2 group) ,siRNA‐coa3 group ,siRNA‐NC group and blank control group ,respectively ,and three times in each group (1 nmol each time) .After siRNA added at the first time ,bacteria was collected in 45 min (A) ,90 min (B) and 135 min (C) time points .The expression of mRNA in three time points (A ,B and C) were detected by real‐time fluorescence quantitative PCR .The protein level of LT in siRNA‐LT1 group ,siRNA‐LT2 group and blank control group were detected by Western blot in three time points .Results The results of real‐time fluorescence quantitative PCR showed that inhibition of siRNA‐LT1 on the expression of LT mRNA at the three time points(A ,B and C)were 70 .9% ,70 .1% ,72 .5% respectively ,and inhibition of siRNA‐LT2 on the ex‐pression of LT mRNA at the three time points(A ,B and C)were 70 .1% ,69 .2% and 70 .5% respectively .In the three time points (A ,B and C)the inhibition rate of the expression of LT mRNA in siRNA‐LT1 group and siRNA‐LT2 group were statistically lower than that in the siRNA‐NC group ,siRNA‐coa3 group and blank control group (P<0 .05) .The results of Western blot showed that in siRNA‐LT1 group the inhibitory rate of expression of LT protein in the three time points were 43 .1% ,18 .4% and 5 .0% ,re‐spectively ;in the siRNA‐LT2 group were 38 .2% ,15 .4% and 30 .1% ,respectively .Conclusion The specific siRNA could inhibit the expression of LT gene in vitro .

Objective To analyze MRI features of breast intraductal papilloma,to improve the acquaintance of this disease.Methods The preoperative MRI images (T2 WI-SPAIR,DWI,ADC,TIC and MIP)of eight patients with breast intraductal papilloma confirmed by surgery were reviewed retrospectively,which were compared with the postoperative pathology.The morphology,distribution, enhanced methods and signal characteristics of the lesion were summarized.Results Of 8 cases,the lession was dormant,and MRI revealed three patterns:the first type was occult papilloma,which presented distention of ducts and no solid nodule in 3 cases (37.5%);the second type showed tiny nodules in bar-like dilatation of ducts in 3 cases (37.5%);and the third type presented small round nodules scattering distribued in terminal ductules in 2 cases (25%).The maximum diameter of nodular lesions was 0.4-1.0 cm(average 0.7 cm).The lesions on T2 WI-SPAIR showed equal signal intensity or mixed signals with isointensity/hypointensity,and dilatation catheter.enhancement. The ADC and TIC value were various,therefore,diagnosis of the tumor should be combined with morphology and associated symptoms. Conclusion At early stage,the occult focus of breast intraductal papilloma can be sensitively detected and localized on MRI.It reveals that MRI has advantage in showing morphology of lesion and functional imaging,which is of important value in clinical diagnosis and early treatment.

Objective:To express and purify recombinant human collagen type Ⅲ and evaluate its properties.Methods:The recombinant genetic engineering strain pET30a(+)-1880/pACYCDuet-hy726/bL21(DE3) was constructed to stably co-express recombinant human type Ⅲ collagen (rhCol) and prolyl hydroxylase. rhCol was prepared and purified by E. coli high-density fermentation, salting out and column chromatography protein purification technology. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis was used to determine the purity of rhCol. The N-terminal amino acid sequences of rhCol were determined by automatic protein polypeptide sequencing instrument. The hydroxyproline content of rhCol was determined by ultraviolet spectrophotometry. The cellular compatibility of rhCol was evaluated by MTT assay. Results:The final wet weight of high-density fermentation was about 200 g/L. The expression level was about 3 g/L. The purity of rhCol by affinity chromatography was over 95%. The results showed that the hydroxyproline content of rhCol was 11.44%, and the rhCol products have good water solubility and cell compatibility.Conclusions:RhCol can be widely applied to the field of skin care and biomedicine as an excellent biological material.

BACKGROUND: Flatfoot is a commonly seen disease in foot and ankle surgery, and stage II adult acquired flatfoot is mostly seen in clinic, so this stage is a key to treatment. However, medial column instability occurs in stage II adult acquired flatfoot, which is an important cause for arch collapse. Medial column stabilization can correct the deformity to great extent, but there is a lack of biomechanical study to assess the effect of medial column stabilization on the whole foot. OBJECTIVE: To investigate the biomechanical effects of medial column stabilization on stage II adult acquired flatfoot. METHODS: A three-dimensional finite element model of stage IIa and IIb adult acquired flatfoot was established. Geomagic software, Solidwork software and Abaqus software were used to simulate medial column stabilization operation (naviculocuniform joint fusion, tarsometatarsal joint fusion, and both fusion). The maximum pressure of plantar soft tissue, medial column bone and medial ligaments was compared before and after simulated single-foot weight loading. Meanwhile, the related parameters were measured to carry out a comprehensive comparison. RESULTS AND CONCLUSION: (1) The maximum plantar stress was located under the first metatarsal head after the simulated medial column stabilization operation. The maximum plantar stress increased significantly after the medial column stabilization in stage IIa flatfoot model, but did not change significantly after the medial column stabilization in stage IIb model. (2) After medial column fusion, the stress of the corresponding joint was reduced, but increased for the other joints of the first metatarsal column. (3) The stress of medial ligament and plantar fascia was not alleviated after medial column fusion. (4) These results indicate that simple medial column stabilization surgery cannot reduce the pressure of medial column of flatfoot in stage II acquired flatfoot adults. It can only be used as a combined surgery to stabilize joints with excessive motion and correct the deformity of supination of forefoot.
