Objective To explore the efficacy and safety of arterial embolectomy with Solitaire AB stent in treatment of acute intracranial large artery occlusion. Methods The clinical data of 11 patients with acute intracranial large artery occlusion receiving arterial embolectomy with Solitaire AB stent from March to December 2013 were analyzed retrospectively.Among them,5 cases were with simple middle cerebral artery occlusion,1 case combined with occlusion in the intracranial part of the internal carotid artery,4 cases had simple basilar artery occlusion, and 1 case had occlusion in the intracranial portion of the vertebral arteries extending to intracranial portion of the basilar artery.Revascularization and postoperative hemorrhage were analyzed, and nosocomial as well as clinical outcomes after 3 months were evaluated. Results Ten patients showed successful revascularization,of which 8 cases reached the standard of perfect revascularization. After embolectomy, there was obvious stenosis in offending vessel of 4 cases, who were given balloon dilatation then.In 3 cases, the vessels were still narrow,so Solitaire AB stent was placed in the stenosis area to correct stenosis with the residual stenosis rate of less than 40%.Thus electrolytic interruption was given. There was no symptomatic intracranial hemorrhage.Three months after the operation,5 cases recovered, among whom 2 cases had obvious curative effect instantly,1 case had complete remissions in 1 week, and 1 case remained hemiplegia.Five cases died, and 4 cases discharged from the hospital because their family gave up the subsequent treatment. Conclusions Arterial embolectomy with Solitaire AB stent to treat patients with acute intracranial large artery occlusion has a relatively higher revascularization rate. The clinical outcome after 3 months can be significantly improved. Thus,it provides a safe and effective endovascular treatment for patients with acute intracranial large artery occlusion.

Objective To construct a large animal model of ischemic heart disease induced by acute myocardial infarction (AMI) . Methods Male Diannan Mini-pigs were selected (n=8), weight 20 ± 4 Kg. After general anesthesia and endotracheal intubation, blood pressure and electrocardiograph were monitored. Parasternal incision was taken and thoracotomyv was performed at 4th-to-5th intercostalgap. Pericardium was opened and left anterior descending artery was ligated. Echocardiography was performed at baseline, postoperative 28 and 60 day. Euthanasia and heart explanted was conducted at 60 day of experimental pig. After gross examination of the heart, the histological examination was used to assess myocardial infarction by using H&E staining. Results All of the pigs were presented with ventricular arrhythmias in varying degrees after ligation of the left anterior descending artery, of whom 2 died of refractory ventricular fibrillation, the mortality was 25%. Echocardiography assessment showed the loss left ventricular function, the thinning of left ventricular apical and the enlargement of left ventricule. Histological examination revealed that myocardial fibrosis and fibrous scar were formed in myocardial infarction area. Conclusion The ligation of left anterior descending artery through thoracotomy under direct vision is a reliable strategy for construction of AMI porcine model and the experimental porcine can develop ischemic cardiac dysfunction progressively.

Objective To investigate the effect of different duration skeletal muscle denervation on acetylcholine receptor activity in rats. Methods Fourteen Balb/c mice weighing 18-22 g were used in this study. The denervation model was established by excising sciatic nerve. Two rats were chosen before(T0 ) and at days 1,4,7,14,21 and 28 after excising sciatic nerve (T1~6),and flexor digitorum brevis of the hindfoot was acutely isolated Skeletal muscle cells were isolated ( five cells in each rat),the acetylcholine currents were recorded using whole-cell patch-clamo technique.Extracellular fluid containing 30 μmol/L acetylcholine was first applied to skeletal muscle cells for 10 s,acetylcholine currents (11)were recorded, then the ceils were washed out using extracellularfluid.Skeletal muscle cells were balanced using extracellular fluid containing 0,0.1,1,10,30,100,1000,3000,or 10 000 nmol/L atracurium for 3 min respectively, then perfused using extracellular fluid containing 30 μmol/L acetylcholine and differents concentrations of atracurium mentioned above for 10 s respectively, and acetylcholine currents were recorded, then the cells were washed out,and 30 μmol/L acetylcholine was perfused again and currents(I2 ) were recorded. The mean value of I1 and I2 was taken as control current, and inhibitory percentage of control current was calculated, and the inhibition concentrations for the half-maximal response (IC50) of atracurium were determined by nonlinear regression analysis. Results Compared with T0,IC50 significantly increased at T1~6 ( P < 0.05).IC50 was increased gradually at T1~3 ( P < 0.05).Compared with T3,IC50 was decreased at T4~6 ( P < 0.05).IC50 was decreased gradually at T4~6 ( P < 0.05 ).Conclusion Skeletal muscle denervation can inhibite acetylcholine receptor activity, which is relate to the denervation time.
