2.Recombinant Human Platelet-derived Growth Factor Enhances Repair of Cutaneous Full-thickness Excision by Increasing the Phosphorylation of Extracellular Signal-regulated Kinase in Diabetic Rat

Objective To investigate the possible signaling mechanisms by which recombinant human platelet-derived growth factor (rhPDGF) accelerated healing of cutaneous wound in diabetic rats. Methods Four full-thickness skin wounds were incised in the back of 26 male Wistar diabetic rats. The wounded rats were divided into 3 groups (7 or 8 rats each group). One group without treatment was used as a control, and the other 2 groups were treated with rhPDGF at a dose of 7.0 μg/cm2 wound or vehicle ( DMSO/0.9%NaCl, vol/vol 1:1) from 1 to 14 days. The wound healing was evaluated by the measurements of the wound volume and area. Immunofluorescent and immunohistochemical staining were used to examine the phosphorylation of extracellular signal-regulated kinase 1/2(ERK1/2) and the expression of proliferative cell nuclear antigen (PCNA), respectively. Results Granulation tissue appeared in the bed of wound after injury. The number of blood capillary buds and fibroblasts was greater in the rhPDGF-treated group than that in the other 2 groups. A lot of inflammatory cells infiltration and collagen deposition were observed in the wound. The wound-volume in the rhPDGF-treated group was smaller than that in control group ( P < 0.05). The reepithelialization rate in rhPDGF-treated group was higher than that in the other 2 groups at 7 days after injury ( P < 0.05). The expression of PCNA in reparative cells was higher in rhPDGF-treated group than in control group or vehicle-treated group at 3,7 days after injury( P < 0.05). The phosphorylation of ERK1/2 was stronger in rhPDGF-treated group than that in control group or vehicle group at 7 and 14 days after injury( P < 0.05). Conclusion These results suggest that rhPDGF accelerates wound healing and improves healing quality by increasing the phosphorylation of ERK1/2.
3.Role of extracellular signal-regulated kinase 1/2 signal pathway in platelet-derived growth factor promoting the healing of cutaneous full-thickness incisional wounds in diabetic rats

Objective To investigate the possible signal transduction pathway during the course of recombinant human platelet derived growth factor (rhPDGF) promoting cutaneous incisional wound healing in diabetic rats. Methods Four full thickness skin wounds were incised in the back of 26 Wistar rats with diabetes mellitus that were randomly divided into two groups, Group A (diabetic rats control) and Group B (treated with rhPDGF at 7.0 ?g/cm2 wound). The granulation, collagen sedimentation, the reepithelialization rate of rhPDGF as well as the inflammatory cell filtration were observed 3,7 and 14 days after wound. Immunofluorescence staining was used to observe the expression of ERKs either around the wound and in the wound center and immunohistochemical method applied to observe the changes of c-fos, proliferation cell nuclear antigen (PCNA) and focal adhesion kinase (FAK). Results The histological investigation showed a lot of granule tissues in the bed of wound, a large of inflammatory cells infiltration and collagen deposition, active growth of granule tissues and significant wound contraction. The number of blood capillary buds and fibroblasts in the Group B were more than that in the Group A. The immunohistochemistry showed that the expressions of ERK1 and c-fos increased significantly three days, strengthened seven days and went weak 14 days after rhPDGF application. In the Group B, at every time point, the expressions of PCNA and FAK were more significant than those in the Group A. Conclusions ERK1/2 signal pathway exerts function in rhPDGF accelerating wound healing of rats with diabetes mellitus and plays an important role in proliferation and migration of reparative cells.
4.Analysis on present situation of dental instruments cleaning and disinfection in Hainan province

Objective To analyze the present situation of the dental instrument cleaning and disinfection in Hainan province and to explore the management mode of dental instrument cleaning and disinfection which is suitable for the provincial situation .Meth-ods By adopting the stratified random sampling method according to the hospital grades ,24 hospitals in Hainan province were per-formed the questionnaire survey on the cleaning mode of oral instruments ,layout of cleaning and disinfection room ,cleaning method and facilities ,protection measures and training of cleaning staff .Results 14 hospitals (58 .33% ) had the sterilization and supply center for conducting the centralized processing on the dental instruments .The tertiary hospitals and the second-grade hospitals had the independent cleaning and disinfection rooms with the rational layout and professional cleaning staff ;the safeguard facilities had the application in place ,the training of the related cleaning and disinfection work and the cleaning process conformed the require-ment of the standards .Among 10 first-grade and below hospitals ,only 1 hospital(10 .00% ) had the rational layout of cleaning and disinfection rooms ;3 hospitals(30 .00% ) had the professional cleaning staff ;the related training of the cleaning staff was not basi-cally carried out and the safeguard was not in place ,most of the cleaning and disinfection instruments and the cleaning process were not in accordance with the requirements .The qualification rates of instruments cleaning and disinfection in different grades of hospi-tals by the ATP bioluminescence assay were 100 .00% ,90 .00% and 80 .00% .Conclusion The existing problems are general and prominent in the hospitals of the first-grade and below .It is suggested that the regionalized disinfection and supply management mode is implemented for maximally realizing the optimized resource configuration in the disinfection and supply center .
5.Association between genetic variation in PAF-AH V279F and coronary artery disease

Objective To explore the relationship between genetic variation in PAF-AH V279F and coronary heart disease among Han population in Beijing.Methods A case-control study was held which enrolled 124 patients with coronary heart disease and 103 normal subjects.The genotype of PAF-AH V279F was determined with allele-specific polymerase chain reaction(AS-PCR)method. Results The highest frequency of PAF-AH V279F genetic variation was VV genotype(92.2%),the next was VF genotype(5.8%)and the lowest was FF genotype(2.0%)among the studied Han population in Beijing.In the coronary heart disease group the frequency of 279 V→F carriers was significantly higher than in the control group(19.3% vs.7.8%,P<0.05)and F allele frequency was also higher(12.1% vs.4.9%,P<0.01).Among the coronary heart disease group,the V279F variation frequency and the F allele frequency were significantly higher in patients with myocardial infarction than in those without myocardial infarction(27.3% vs.13.0%,17.3% vs.8.0%,both P<0.05).In multiple logistic regression analysis,the odds ratio(OR)of V279F genetic variation for coronary heart disease was 1.919(95% CI:1.448-2.544,P=0.033).Conclusions The PAF-AH V279F genetic variation may be a novel genetic marker for high risk of coronary heart disease.
6.Expression of alpha-smooth muscle actin in scar tissue and its relationship with apoptosis

AIM: To examine the expression of alpha-smooth muscle actin in scar tissue, and observe the phenomenon of apoptosis and its involvement in the process of pathological scarring and the presence of myofibroblasts or absence of cell in the dermis. To investigate the potential role of reparative cell apoptosis in hyperplastic scar formation. METHODS: The samples of scar were obtained from post-burn patients undergoing plastic operation in our burn unit recently, and the samples of control came from skin donor site of the same patient correspondingly. TUNEL assays were performed to evaluate the number of apoptotic cells in scar versus normal skin. In situ hybridization and immunohistochemistry staining technique were employed to determine the expression of different dermis cells markers in scar tissue and normal skin. RESULTS: There existed evident difference in apoptotic cells in the dermis between scars tissue and normal human skin. The expression positive cells were much more in hyperplastic scars than that in normal human skin; the apoptotic cells of proliferative stage were slight more than that of mature stage. However, in proliferative stage, the number of apoptotic cells was higher for the combination of hyperplastic scar than normally healed flat scars. But in mature stage, no obviously difference was detected between hyperplastic scar and normally healed flat scar. The monoclonal anti-? smooth muscle actin (ASMA) expression was significantly stronger in proliferative stage than that of mature stage. CONCLUSIONS: With reconstitution of dermal tissue, myofibroblasts containing alpha-SM actin disappear under normal wound healing, probably as a result of apoptosis. The myofibroblast play a critical role in wound closure and in the pathologic sequelae of healing.
7.The use of prealbumin as index to compare sensitivities and specificities of Nutritional Risk Screening 2002,Subjective Global Assessment, Mini Nutritional Assessment: a prospective multicenter clinical studies

ObjectiveTo explore sensitivities and specificities of Nutritional Risk Screening 2002 ( NRS 2002),Subjective Global Assessment (SGA),and Mini Nutritional Assessment (MNA) in the detection of blood prealbumin level.MethodsTotal 126 patients were consecutively enrolled from the oncology departments of three hospitals from November,2010 to March,2011.Nutritional screenings or assessments were performed on the next morning of hospital admission by nutritionists and medical oncologists using NRS 2002,SGA,and MNA.Prealbumin ( < 180 g/L) was used as the reference standard of malnutrition (undernutrition) for assessing the sensitivities/specificities of these tools.ResultsAll 126 patients completed nutritional screening or assessment using the above three tools.The prevalence of malnutrition (undernutrition) determined by NRS 2002,SGA and MNA was 67.0%,46.2%,and 64.8%,respectively.With prealbumin in relation to clinical outcome as the reference standard,the sensitivities ofNRS 2002,SGA,and MNA were 78.2%,58.2% and 89.1%,and the specificities were 50.0%,72.2%,and 27.8%,respectively.ConclusionsAmong these three tolls,NRS 2002 detects the nutrition risks,whereas SGA and MNA detect malnutrition.Since prealbumin is not an appropriate reference standard,it can not be used for the comparison among the three tools.Nevertheless,NRS 2002 reflects the clinical outcomes,and therefore is suitable for inpatients.
8.Clients' evaluation on ethical behavior of their counselors or psychotherapists

Objective:To explore the clients' assessment for the ethical behavior of the counselors and psychotherapists.Methods:The main method used in this evaluation was questionnaire,and the self-made questionnaire of the ethical problems in psychotherapy and counseling(for the clients)were used as evaluation tools.Totally 1100 clients,from 12 cities in China,were investigated with 807 questionnaires returned.Results:84.5% of the clients were confident in the treatments,and 89.2% of them gave positive assessment for the ability of their counselors and psychotherapists.15.9% of clients said the counselors and psychotherapists showed impatience or talked to them about their stress sometimes.Besides,59.1% knew about the characters,names and charge of the psychotherapies and counseling,and 53.4% knew the background of the counselors and psychotherapists.91.3% of clients thought the privacy and treatment could be kept secret.7% of clients had intimate relationship with their counselors or psychotherapists,1.5% had sexual relations,and 18.3% made friends in lives.79% of clients could receive timely psychotherapies and counseling.Conclusion:From the assessment of the clients,the vast majorities of the counselors and psychotherapists do well in work ability,confidentiality and time-setting,but not so well in informed-consent and dual relations,which need to be paid attention to and improved.
9.Risk analysis of malignant tumor among medical diagnostic X-ray workers during 1997-2011 in Jiangsu province

Objective To continue following up the cohort members in Jiangsu province who were the largest subgroup in Chinese medical X-ray workers cohort subgroups on radiation carcinogenesis and analysis the cancer risk of medical X-ray workers which might be induced by chronic low-dose ionizing radiation between 1997 and 2011.Methods A retrospective cohort study was utilized to accumulate data.Epicure was applied to compute cumulative person years (with module of Datab) and relative risk (including 95% CI) of malignant tumor on exposed group(with module of AMFIT) while the factors such as sex and attained-age were adjusted in the cohort members.Results The 7 708 follow-up members included 3 668 medical X-ray workers.The cumulative person-year in the cohort members was 94 651.2 (1997-2011) and its lost rate was 5.79%.During the follow-up period,441 cases had became cancer patients.The relative risk of all cancers was 1.44 (95% CI:1.18-1.75),so was the solid cancers.Significant elevated risks of cancer were found on lung (RR =1.95,95% CI:1.30-2.92),esophagus (RR =3.04,95% CI:1.20-7.70),lymphoma(RR =2.24,95% CI:0.67-7.48),leukemia(RR =1.57,95 % CI:0.33-7.39),throat (RR =2.47) and thyroid (RR =2.80,95 % CI:0.24-32.26) among medical X-ray workers.However,The RR of malignant tumors might not be associated with occupational exposure to ionizing radiation if we considered the lost factors (RR =1.2,95% CI:0.94-1.46)Conclusions The RR of malignant tumors such as the cancer on lung,esophagus,thyroid among the medical X-ray workers in Jiangsu province were enhanced which might be associated with occupational exposure to ionizing radiation.
10.Relative bioavailablity of cefaclor effervescent tabletsin human volunteers

Aim To study relative bioavailablity of cefaclor effervescent tablets in healthy volunteers. Methods According to the crossover design, A volunteers were each orally given a single does of the 0.75 g cefaclor effervescent tablets and cefaclor capsules with an interval of 5 days between the two formulations.The plasma concentrations of the drug were determined by RP-HPLC.Pharmacokinetic parameters were obtained by ATPK programe,and calculated on the basis of open single compartment model.Results After a single oral dose, the peak levels in plasma averaged Cmax(31.27?5.81)?g?ml-1 and(30.56?5.25) ?g?ml-1 at (0.58?0.12)h and(0.73?0.17)h and AUC0~4(35.48?4.65) ?g?h?ml-1 and (35.89?2.90) ?g?h?ml-1 for tablet and capsule,respectively. Conclusion The result shows that two formulations are bioequivalence.
