Background and purpose:The curative effect with radiotherapy or chemotherapy alone for the patients with unresectable stage Ⅲ non-small-cell lung cancer was poor. The 5-year survival was only 5-10 percent. Concurrent chemoradiotherapy could achieve better local control and overall survival of those patients and it had been reported that the 5-year survival was improved to 15.8 percent, but the toxicity were much more severe at the same time. We prospectively evaluated the efficacy and toxicity of concurrent chemoradiotherapy with low dose weekly paclitaxel for unresectable stage Ⅲ non-small-cell lung cancer and tried to make the regime more tolerable without the deterioration of treatment response. Methods:Forty-eight patients with unresectable stage Ⅲ non-small-cell lung cancer were randomized into low dose weekly paclitaxel group and control group.Both groups were treated by the standard fractionation schedule. All patients were irradiated 2.0 Gy/per fraction,five fractions a week,the total radiation dose was 60-64 Gy for tumor. Patients in the low dose weekly paclitaxel group received chemotherapy with 45 mg/m 2 of paclitaxel on every Monday; the patients in control group received 50 mg/m2 of cisplatin on days 2-4 and day 23-25, and 135 mg/m 2 of paclitaxel on days 1 and 22 concomitant with the radiotherapy.Results:The CR (complete response) rates of low dose weekly paclitaxel and control group were 21% and 13% respectively(P0.05).Conclusions:The patients treated by low-dose weekly paclitaxel group showed that both survival fraction and the period of local tumor control were higher than in control group. Additionally, low dose weekly paclitaxel concomitant with radiotherapy was well tolerated and were not statistically different from control group in terms of toxicities.

Objective:To examine the relationships between acute stress disorder (ASD) symptoms,posttraumatic stress disorder (PTSD) symptoms and depressive symptoms in children survivors following the Lushan earthquake in China.Methods:One hundred ninety-seven children aged 1 to 14years (93 male and 104 female) were followed up at two and six weeks after Lushan earthquake.The ASDS,UCLA PTSD Reaction Index and DASS were used to measure the ASD,PTSD and depressive symptoms.Results:The average scores of ASDS and the depression subscale of DASS at two weeks after earthquake were(36.5 ± 10.9) and (3.3 ± 2.8),respectively.The average scores of UCLA PTSD Reaction Index and the depression subscale of DASS-21 at six weeks after earthquake were (18.5 ± 12.6) and (3.3 ± 3.5),respectively.The rates of ASD and PTSD were 28.6％ and 8.2％,respectively.The cross-lagged structural equation analysis with latent variables indicated that the ASD symptoms at two weeks after earthquake positively predicted PTSD symptoms (β =0.59,p ＜ 0.001) and depressive symptoms (β =0.29,p ＜0.001) at six weeks after earthquake.But there was no significant association between depressive symptoms at two weeks and PTSD symptoms at six weeks after earthquake.Conclusion:ASD symptoms in the early time postearthquake could predict subsequent PTSD and depressive symptoms,while depressive symptoms tend to remain stable within six weeks after earthquake.

Objective To explore the association between the genetic polymorphism of hypoxia inducible factor 1 α (HIF-1α) G1790A and the radiosensitivity of nasopharyngeal carcinoma.Methods A total of 189 patients with nasopharyngeal carcinoma treated with radical radiotherapy were followed-up for 3 years.The patients were divided into cured group with 135 cases and recurrence group with 54 cases by clinical follow-up results.PCR-RFLP was used to determine the mononucleotide genotypes of HIF-1α G1790A.Results The observed genotype frequencies of HIF-1α gene 1790 (G→A) for GG, GA and AA were 70.04% , 20.74% , 2.22% in cured group and 59.26% , 38.89% , 1.85% in recurrence group, respectively.The allele frequencies for G and A were 87.4% , 13.9% in cured groups and 78.7% ,21.3% in recurrence group, respectively, without significant difference in distribution of allele frequencies between the two groups(x2 =6.919, P =0.077).Conclusions The genetic polymorphisms of HIF-1α G1790A might be related with the radiosensitivity of nasopharyngeal carcinoma.

Objective: The aim of this study was to evaluate whether there is an association between Parkinson’s disease (PD) and colorectal cancer in Taiwan. Methods: This was a case-control study using claim data of the Taiwan National Health Insurance Program. There were 64,619 subjects aged 20-84 with newly diagnosed colorectal cancer as cases and 64,619 randomly selected subjects without colorectal cancer as controls from 2005 to 2011. Both cases and controls were matched by sex, age, comorbidities, and index year of diagnosing colorectal cancer. Subjects who were diagnosed with PD within 5 years of diagnosing colorectal cancer were excluded. The multivariable logistic regression model was used to estimate the adjusted odds ratio (OR) and 95% confidence interval (95% CI) for risk of colorectal cancer associated with PD. Results: After adjusting for confounding factors, the multivariable logistic regression analysis revealed that the adjusted OR of colorectal cancer was 0.69 for subjects with PD more than 5 years before index date (95% CI 0.59, 0.81), as compared with subjects without PD. Conclusion: PD is associated with reduced odds of colorectal cancer. Further research is needed to elucidate the mechanisms underlying our findings.

ZJ-XD-I multifunctional internal sterilization machine for medical instrumentation is applied in medical instrumentations with circulatory loop such as anesthetic machine and respirator. The machine can generate ozone and hydrogen peroxide, and then the two gases are mixed and introduced into the internal loop to eliminate adnexed hydrogen peroxide such as the virus and germ, and thus the nosocomial infection due to repeated uses of medical instrumentations. The machine is composed of the gas compression component, atomization unit, gas extraction component, control circuit and the filtrating unit.

Objective To observe the rate-adaptive response of dual sensor pacemakers integrated with activity sensor and minute ventilation sensor.Methods Fifty patients with chronotrepic incompetence were implanted with pacemakers of integrated sensor.The patients with pacemakers implanted were arranged to take upstairs test,downstairs test and tapping test under the monitor of integrated sensor,minute ventilation sensor and activity sensor respectirely.The rate-adaptive responses were tracked down every 15 seconds with remote heart rate monitor.The results were compared with the control group patients with normal sinal chronotropic function(n=15).Results In activity sensor group,pacing rates were significantly higher than the normal control group in downstairs test and tapping test.In minute ventilation sensor group,pacing rates increasement was significantly slower in the beginning of upstairs test.Pacing rates of integrated group were similar to the normal control group in upstairs and downstairs tests.Conclusion The rate-adaptive pacing response of dual sensor integrated with activity sensor and minute ventilation sensor,which is mostly close to the normal chronotropic response,better than that of single sensor.

Background: The present study aimed to digitally evaluate the risk of overwork-related adverse effects (OrAEs) among employees from various occupational categories in Taiwan. Methods: Anonymous data of employees from seven companies/factories providing occupational health services were analyzed. The studied population comprised 5505 employees, and the data analyzed included employment duration, working hours, shift work schedules, and health checkup results. The risk for OrAEs was assessed by an index, Karo index (0-4, the larger the value, the higher the risk for OrAEs) obtained using a risk matrix made up of cardiocerebral and occupational risk factors. Karo index values of 3 and 4 were categorized as at high risk for OrAEs (h-OrAEs). Results: The 5505 employees had an average employment duration of 8.5 years and a mean age of 39.4 years. The prevalence rates for h-OrAEs of the seven companies/factories ranged from 3.9% to 34.2%. There were significant differences in prevalence rates for h-OrAEs between employees of retail stores and high-tech manufacturing factories. Multivariate analysis results indicated that workers of high-tech manufacturing factories had significantly higher risk for h-OrAEs compared with retail store workers. Conclusion In terms of satisfying health risk management and legal requirements in Taiwan, the newly issued Karo index, which covers a wide range of occupational risk factors, can serve as an assessment and a warning tool for managing the risk of OrAEs in workplaces. To reduce risks for h-OrAEs, active and prudent control of cerebrocardiovascular risks and working hours is recommended.
