1.Research on Ancient Books Protection System and Standard for Special Library of Traditional Chinese Medicine

This paper put forward a construction mode of ancient books protection system for special library of traditional Chinese medicine based on the survey and analysis of ancient books protecting status. With the working experience of protecting ancient books in our library, we studied the technical system and organization system for ancient books protection, listed out the standard of technical system and content of organization system, and formed an ancient books protection system for special library of traditional Chinese medicine. This system included scientific technology, advanced methods, effective management, orderly operation and right-hand protection, which can be widely applied in our industry to promote benign and sustainable development of work in ancient books protection.
2.Studies of treatment strategy and prognosis on acute myeloid leukemia with chromosome 8 and 21 translocation.

OBJECTIVETo investigate the relationship between the biological features and the treatment efficacy and prognosis in acute myeloid leukemia subtype M2 (AML-M2) patients with chromosome 8 and 21 translocation.
METHODSBy using Cox regression model and Kaplan-Meier analyses, prognostic factors in 54 cases of de novo adult AML with t(8;21) in our institute from 1990 to 2003 were retrospectively analyzed.
RESULTThe complete remission (CR) rates were 81.9% for all M2 patients, 82.4% for patients with normal karyotype, 88.5% for patients with t(8;21) [P > 0.05 for normal karyotype vs t(8;21)], 100.0% for 28 patients with t(8;21) alone and 75.0% for 24 patients with additional chromosome abnormalities (P < 0.01). The actuarial 3 year overall survival(OS) was 26% for M2 patients with normal karyotype, 25% for patients with t(8;21) [P > 0.05 for normal karyotype vs t(8;21)], in whole t(8;21) group, 46.4% for patients with t(8;21) alone and 0% for patients with additional chromosome abnormalities (P < 0.01). Multivariate analysis of prognostic factors showed that chromosome abnormalities besides t(8;21) was the only factor affecting CR, disease-free survival (DFS) and OS. DFS of allogeneic hematopoietic stem cell transplantation (HSCT) and intermediate-dose cytarabine/high dose cytarabine (IDAC) groups were better than the group received routine dose cytarabine as postremission therapy (P < 0.01).
CONCLUSIONAML with t(8;21) is not a single defined AML subset, and patients with additional chromosome abnormalities have a worse prognosis. HSCT and IDAC could improve the outcome. HSCT is the best choice for patients with high risks, especially with additional chromosome abnormalities.

3.Four-color flow cytometric immunophenotypic features of blasts in myelodysplastic syndromes.

This study was aimed at exploring the immunophenotypic features of blasts in patients with myelodysplastic syndromes (MDS). Four-color flow cytometry using conventional and secondary gating strategies was used to immunophenotype blasts and the CD34 positive cells in bone marrow nucleated cells of 29 patients with MDS. The results showed: (1) with progression of MDS from RA/RAS, RAEB to RAEB-T, the proportion of CD34(+) cells were gradually increased from 8.0%, 46.4% to 76.8% (P < 0.05); (2) using CD45 vs SSC gating strategy, with the transformation of RA/RAS, RAEB to RAEB-T, the expression of HLA-DR, CD13, CD33, CD117 were also gradually increased (P < 0.05), and the expression of CD15 was gradually decreased (P < 0.05); (3) using CD45 vs CD34 gating strategy, the expression of HLA-DR, CD13, CD33, CD117 on blasts were higher by secondary gating method than those by conventional gating (P < 0.05). However, there were no significant difference (P > 0.05) in the expression of above-mentioned antigens on CD34(+) cells among different MDS subtypes. It is concluded that conventional gating method can reflect MDS progression from RA/RAS, RAEB to RAEB-T, and secondary gating strategy may accurately reflect the biological features of blasts in MDS. Abnormal expression of CD34 is related to the immaturity level and heterogeneity of blast cells, which is beneficial to the diagnosis of clinically suspected MDS incapable of diagnosing with morphology and cytogenetics.

4.Determination of loratadine in human plasma by HPLC with fluorescence detector and study on its bioavailability.

AIMTo establish an HPLC-fluorescence method for determination of loratadine in human plasma and evaluate its relative bioavailability.
METHODSAn Alltech-C18 column and a mobile phase of acetonitrile-water-glacial acetic acid-triethylamine (90:100:6:0.15) were used. The fluorescence detector was set at Ex 274 nm, Em 450 nm. The flow rate was 1 mL.min-1.
RESULTSThe calibration curve was linear over a concentration range of 0.2-30 micrograms.L-1. The limit of quantification was 0.2 microgram.L-1. The average method recoveries varied from 96% to 98%. The results showed AUC, Tmax, Cmax and T1/2 beta between the testing tablets, testing capsules and reference tablets had no significant difference (P > 0.05). Relative bioavailabilities were 107% +/- 17% and 100% +/- 14% respectively.
CONCLUSIONThe three formulations were bioequivalent.

5.Expression of beta-endorphin and micro-opioid receptor during wound healing process in rat with deep partial-thickness scald.

OBJECTIVETo observe the expression of beta-endorphin and micro-opioid receptor (MOR) during wound healing process in rat with deep partial-thickness scald.
METHODSThirty-six Wistar rats were randomly divided into control( n = 6, without treatment) , and scald ( n = 30, with 5% TBSA deep-partial thickness scald) groups. Skin specimens from wound were harvested immediately after scald and on 3, 7, 14, 21 post-scald days( PSD) for the determination of 1-endorphin and MOR expression with immunofluorescent staining.
RESULTSbeta-endorphin and MOR were mainly distributed in nerve terminal at the border of dermis and epidermis , keratinocyte in some epidermis , in the fibroblast in dermis , with a weak expression The expression of beta-endorphin peaked in whole layer of skin on 3 PSD( 196 +/-16, P <0. 01) ,while that of MOR was concentrated in keratinocytes in the basal layer and the basement membrane. The expression of MOR was strengthened on 7 PSD with disarrangement of collagen , and it peaked on 14 PSD (306 +/- 23, P < 0.01) with epithelization in some wounds. There was still strong expression of beta-endorphin on 7 and 14 PSD. Complete epithelization was observed in scald group on 21 PSD, with nerve terminal approaching the boundary between the dermis and epidermis, and collagen began to arrange in good order. The expression of P-endorphin in scald group (31 +/-24)was similar to that in control group(30 +/- 18) on 21 PSD, but the expression of MOR (56 +/- 16) was still higher than that in control group (28 +/- 15 ).
CONCLUSIONThe expression of MOR and P-endorphin exhibits chronobiological nature during the process of wound healing,

6.Bone marrow necrosis as an initial manifestation of Philadelphia chromosome and myeloid antigens positive B acute lymphoblastic leukemia--a case report.

Many diseases cause bone marrow necrosis (BMN), especially lymphocytic leukemia. To explore the complexity of the pathogenesis and pathology of BMN and understand the multiplicity of clinical features, a case of Philadelphia chromosome positive (Ph+) B acute lymphoblastic leukemia (ALL) expressing myeloid antigens was reported. The results indicated that the clinical features of this case were complicated and multiplex, the diagnosis was confirmed by using bone marrow smear and biopsy, immunophenotype analysis, conventional cytogenetics and fluorescence in situ hybridization (FISH), the prognosis of patients improved by active treatment for primary disease. In conclusion, the Ph+ B ALL expressing myeloid antigen with BMN is very rare, its diagnosis should be confirmed by using multiple methods, and the active treatments should be performed.

7.The value of virtual monoenergetic images and electron density map derived from dual-layer spectral detector CT in differentiating benign from malignant pulmonary ground glass nodules

Objective:To investigate the clinical value of virtual monoenergetic images (VMI) and electron density map (EDM) derived from the dual-layer spectral detector CT (DLCT) in the differential diagnosis of benign and malignant pulmonary ground glass nodules (GGN).Methods:From July 2019 to August 2020, a total of 65 patients with lung GGN (27 benign GGNs and 38 malignant GGNs) confirmed by pathology were retrospectively enrolled in Gulou Clinical Medical College of Nanjing Medical University. All the patients underwent DLCT plain scanning within two weeks before the surgery. The conventional 120 kVp polyenergetic image (PI), EDM and 40-80 keV VMI were reconstructed. The differences of CT and electron density (ED) values between benign and malignant lesions on different images were compared by Mann-Whitney U test. Independent t-test was used to compare the lesion size and χ 2 test was used to analyze the CT features (including lesion location, shape, edge, internal structure, adjacent structure, nodule type) between benign and malignant lesions. Receiver operating characteristic (ROC) curve was used to analyze the efficacy of different energy spectrum quantitative parameters in the differential diagnosis of benign and malignant GGN. The statistically significant CT signs and energy spectrum quantitative parameters were analyzed by logistic regression analysis to find out the independent risk factors of malignant GGN, and then ROC curve analysis was performed for each independent risk factor alone or in combination. Results:There were significant differences in lesion shape, spiculation, lobulation, location and size between benign and malignant groups ( P<0.05). The CT value of pulmonary GGN in PI, 40-80 keV VMI and the ED value in EDM were statistically different between benign and malignant lesions ( P<0.05). The area under ROC curve (AUC) were 0.680, 0.682, 0.683, 0.686, 0.694, 0.676 and 0.722, respectively, among which the ED value had the highest AUC. Binary logistic regression analysis was carried out with GGN shape, spiculation, lobulation, location, size, ED value and CT value in PI, 40-80 keV VMI as independent variables, and malignant GGN as dependent variables. The results showed that ED value (OR=1.045, 95%CI 1.001-1.090, P=0.044), lesion size (OR=1.582, 95%CI 1.159-2.158, P=0.004), spiculation sign (OR=11.352, 95%CI 2.379-54.172, P=0.002) were independent risk factors for malignant GGN. ROC curve analysis showed the AUC of ED value, lesion size, spiculation sign and combination of the three for differential diagnosis of benign and malignant GGN were 0.722, 0.772, 0.698 and 0.885. The AUC for the combined parameters was the largest, with sensitivity of 92.1% and specificity of 74.1%. Conclusion:The diagnostic efficacy of EDM is higher than that of other VMI in the differential diagnosis of pulmonary GGN by DLCT images; The efficacy is further improved when EDM is combined with lesion size and spiculation sign for comprehensive diagnosis.
8.Establishment of Education System of Professionals in the Fields of Sports and Exercises Rehabilitation in China

Objective:To analyze the policy development background of personnel education in the fields of sports and exercises rehabilitation, discuss the educational objectives, disciplinary knowledge system and curricula principles, and specialties in China, to construct the education system of sports and exercises rehabilitation specialty in China. Methods:In light of policy recommendations of the World Report on Disability and the documents of Rehabilitation in Health Service System by World Health Orgnization (WHO), the theoretical framework of International Classification of Functioning, Disability and Health (ICF) had been adopted in the policy analysis and development of education system of speciality of sports and exercises rehabilitation in China at undergraduate level. Results:Rehabilitation is an important part of the national health service system and one of the important approaches to achieve the United Nations 2030 Sustainable Development Goal III: Universal Health Coverage. The education of professionals in the fields of sports and exercises rehabilitation is an important guarantee for the development and improvement of the rehabilitation service system. The educational goals of professionas tailored to the needs of health system for the coverage of total population and life-span experiences. Professionals in the fields of sports and exercises rehabilitation should learn knowledge and skills of rehabilitation sciences, which is integration of sport sciences and health sciences. They will work in a "sports and medicine integration" model in the health service system. The speciality of sports and exercises rehabilitation should be established on the basis of a comprehensive interdisciplinary and cross sectors principles guided by ICF theory, such as physical education, biology, psychology, sociology and environment sciences. The specialized knowledge system of sports and exercises rehabilitation can be divided into three levels: fundamental courses for liberal education, specific courses for professional education, and specialized training for speciality. The professional education tailored to the applied disciplines; rehabilitation skills and practices tailored to rehabilitation specialties. In the way of ICF, the principles of courses had been developed in the fields of sports and exercises rehabilitation: developing the health-related general education courses that matched with the international standards; strengthening and developing the professional foundation courses of sports and exercises rehabilitation in the view of health sciences and rehabilitation sciences, according to the laws of rehabilitation science and sport sciences; developing career-related skills based on the requirements of the rehabilitation service industry. Conclusion:In light of the ICF theory, according to the requirements of WHO Rehabilitation of Health Service System and the action suggestions of WHO World Report on Disability, the educational goals, theoretical frameworks, approaches, disciplinary knowledges and courses of sports and exercises rehabilitation had been developed. In order to further promote the professional development of sports and exercises rehabilitation, this paper provided a theoretical foundation and scientific basis for improving the scientific and standardized level of sports and exercises rehabilitation.
9.Integrity evaluation of resected mesentery specimen after total mesorectal excision by methylene blue perfusion via superior rectal artery.

OBJECTIVETo evaluate the integrity of the resected mesentery specimen after total mesorectal excision (TME) for low rectal cancer using methylene blue perfusion via the superior rectal artery.
METHODSTwenty patients with low rectal cancer were randomly divided into the methylene blue group (n=10) and the control group (n=10). All the patients received TME and macroscopic examination of the mesorectal surface was performed to evaluate the quality of the surgical specimen. The methylene blue was injected into the specimen postoperatively via superior rectal artery.
RESULTSThe mesorectal surface of all the specimens was intact on macroscopic examination. However, after methylene blue perfusion, 2 specimens were found to be incomplete. The number of lymph nodes in the methylene blue group were significantly larger (17.3+/-2.4 vs 12.4+/-5.4, P=0.016).
CONCLUSIONSIntegrity evaluation of TME specimen is necessary. Methylene blue perfusion is a convenient and effective method to identify subtle incompleteness of specimen and can improve the detection of lymph node.

10.Effective inhibition of hTERT expression by RNA interference on the radiosensitivity of human laryngeal cancer Hep-2 cell line.

OBJECTIVETo construct an eukaryotic expression vector of human telomerase reverse transcriptase (hTERT) gene specific shRNA, and investigate the effect of pshRNA-hTERT combined with gamma-irradiation on cell survival and telomerase activity.
METHODSAccording to the coding sequence of hTERT mRNA, the target of RNAi was designed, and recombinant expression plasmid pshRNA-hTERT was constructed. The vector was transfected into Hep-2 cells. The radiosensitivity of Hep-2 cells was determined by clonogenic assay. Telomeric repeat amplification protocol (TRAP-PCR-ELISA) was used to observe the telomerase activity in each group. Results Recombinant expression vector pshRNA-hTERT was successfully transfected into Hep-2 cells. The hTERT expression inhibition rate reached 60. 8%. pshRNA-hTERT not only inhibited telomerase activity of Hep-2, but also inhibited the raise of telomerase activity induced by gamma-irradiation. Exposure of Hep-2 cells to pshRNA-hTERT for 24 hrs before irradiation resulted in a decrease in mean surviving fraction of Hep-2 cells compared with cells of group with irradiation alone (67. 7% vs 85. 7%, P <0. 05) .
CONCLUSIONRNAi showed a significant inhibitory effect to the expression of hTERT. The results indicate that pshRNA-hTERT can effectively inhibit telomerase activity of Hep-2 cells treated or untreated with 2 Gy gamma-irradiation and significantly enhance the radiosensitivity of Hep-2 cells in vitro. The role of radiosensitization of pshRNA-hTERT may be related with the inhibition of telomerase activity.
