1.Efficacy at early stage after FS-LASIK with accelerated corneal collagen cross-linking

· AIM:To evaluate the safety and efficacy at early stage after femtosecond laser-assisted excimer laser in situ keratomileusis (FS-LASIK) combined with collagen cross-linking.· METHODS:Totally 30 myopia patients (60 eyes) received FS-LASIK combined with collagen cross-linking from July 2016 to August 2017 in our hospital were tested the uncorrected visual acuity (UCVA),Ks,Kf,surface regular index (SRI),surface asymmetry index (SAI) by corneal topography (TMS) preoperatively,1,3mo after operation.The corneal compensated intraocular pressure (IOPcc),Goldmann correlated IOP value (IOPg),corneal resistance factor (CRF),corneal hysteresis (CH) tested by ocular response analyzer (ORA) were also observed.Ziemer Galilei Dual Scheimpflug Analyzer detected the anterior diopter and the corneal thickness.Endothelial cell density were examined.· RESULTS:The postoperative UCVA remained beyond 0.8 including 26 eyes (43%) of 1d,55 eyes (92%) of 1mo,50 eyes (83%) of 3mo.IOPcc,IOPg,CRF,CH at postoperative 1mo significantly decreased comparing with preoperative (P<0.01).IOPcc,IOPg,CRF,CH at postoperative 3mo did not change significantly comparing with 1mo (P>0.05).The corneal topographic map parameters of Ks and Kf significantly decreased at postoperative 1mo (P < 0.01),while SRI and SAI increased significantly (P<0.01) on comparison of 1mo and preoperative.Ks of postoperative 3mo was significantly higher than that of 1 mo (P<0.05),while Kf did not change significantly (P> 0.05).There was a significant decrease in SRI and SAI between 3mo and 1mo after operation (P< 0.05).In Galileo analysis on comparison of 3mo and 1mo,Sim Ks,Sim Kf had 0.94D,0.95D growth and the front and back surface value of Kf increased more at 3mo postoperative (P<0.01),while Ks was no significant difference (P> 0.05).There was no statistically significant difference in corneal thickness between the central point and the thinnest point (P> 0.05).There was no significant endothelial cell loss throughout follow up (3059.95±247.87/mm2 vs 3052.87± 267.71/mm2;t=0.279,P>0.05).· CONCLUSION:Refractive corneal surgery combined with collagen cross linking for high degree,thin cornea,uneven thickness of the cornea,topographic anomalies except for the patients with keratoconus,play a role in stabilizing corneal shape and increasing corneal biomechanics early.
2.Research advances on the chemical components and pharmacological activities of Zanthoxyli Radix

Zanthoxyli Radix is a traditional Chinese medicine. It can be used for the treatment of wind-cold-dampness arthralgia, muscle and bone pain, fall fracture, hernia, sore throat, toothache and other diseases. Due to possessing many excellent and mild pharmacological properties, there are lots of reports about Zanthoxyli Radix worldwide. At present, more than 100 bioactive components have been extracted and purified from Zanthoxyli Radix. Nitidine chloride (NC), one of the most important alkaloids in Zanthoxyli Radix, has the activities of anti-tumor, anti-inflammation, anti-bacteria, etc. In this review, we summarize the chemical components of Zanthoxyli Radix, pharmacological activity and mechanism of action of NC to provide references for further research and utilization of Zanthoxyli Radix.
3.Anti-tumor mechanism of norcantharidin for the implanted tumors of human gallbladder carcinoma in nude mice in vivo.

OBJECTIVETo explore the anti-tumor mechanism of norcantharidin (NCTD) for the implanted tumors of human gallbladder carcinoma in nude mice in vivo.
METHODSAnimal model of implanted tumors of human gallbladder carcinoma in nude mice was established. Mice were randomly divided into control, 5-FU, NCTD and NCTD + 5-FU groups and were taken different treatment. The expressions of PCNA, Ki-67, cyclin D1, p27, Bcl-2, Bax, Survivin, nm23/nm23-H1, MMP2 and TIMP2 proteins or genes in each tissue section of every group were determined by immunohistochemistry and RT-PCR.
RESULTS(1) On proliferation-related gene proteins, the expression of PCNA, Ki-67, cyclin D1 was significantly decreased, with significantly increased expression of p27 protein, in paraffin sections of NCTD group when compared with control group (P < 0.05); The expression of PCNA mRNA, cyclin D1 mRNA was decreased, with significantly increased expression of p27 mRNA in NCTD group. (2) On apoptosis-related gene proteins, the expression of Bcl-2 was significantly decreased in paraffin sections of NCTD group when compared with control group (P < 0.05); The expression of Bcl-2 mRNA, Survivin mRNA was significantly decreased, with significantly increased expression of Bax mRNA in NCTD group. (3) There was significant difference on invasion around tumor and lung metastasis in NCTD group when compared with control group (P < 0.01). On metastasis-related gene proteins, the expression of nm23 and TIMP2 was significantly increased, with significantly decreased expression of MMP2 in paraffin sections of NCTD group when compared with control group (P < 0.05); The expression of nm23-H1 mRNA, TIMP2 mRNA was significantly increased, with significantly decreased expression of MMP2 mRNA in NCTD group.
CONCLUSIONSThe anti-tumor mechanism of NCTD for human gallbladder carcinoma in nude mice might correlated with inhibition of cell proliferation, blockage of cell cycle, induction of cell apoptosis, reducing of cell motility and invasive capability, alteration of the expression of proliferation-, apoptosis- and metastasis-related gene proteins such as PCNA, Ki-67, cyclin D1, p27, Bcl-2, Bax, Survivin, nm23, MMP2 and TIMP2.

4.Comparison of IOP and corneal biomechanical changes after SMILE and FS-LASIK

AIM:To evaluate the influence on corneal biomechanics after small incision lenticule extraction (SMILE) and Femto LDV laser flapping in laser in situ keratomileusis(FS-LASIK).METHODS:Preoperatively,1mo after surgery,101 patients (202 eyes) with myopia in each group respectively by SMILE and FS-LASIK were examined by ocular response analyzer (ORA).Parameters were compared between postoperative (1 mo) and preoperative.Comparisons included the corneal thickness and corneal compensated intraocular pressure (Iopcc),Goldmann correlated lOP value(Iopg),corneal resistance factor (CRF),corneal hysteresis (CH).RESULTS:SMILE group:IOPcc,IOPg (1mo postoperatively,13.84 ± 2.22,10.81 ± 2.52mmHg) significantly changed comparing with preoperative (16.15±2.90,15.95±3.08mmHg;t=-13.58,-32.91;both P<0.01).CRF and CH (1 mo postoperatively,7.52±1.41,8.66±1.19mmHg) significantly changed comparing with preoperative (10.72± 1.61,10.60± 1.43;t=-41.21,-24.03;both P<0.01).The difference were 3.19±1.10,1.93± 1.14mmHg respectively.FS-LASIK group:Iopcc and Iopg (1mo postoperatively,13.99 ± 2.33,10.10 ± 2.55mmHg) significantly changed comparing with preoperative (15.88±3.29,14.86±3.34mmHg;t=-10.09、-23.00;both P<0.01).CRF,CH (1mo postoperaturely,6.68 ± 1.14,7.90 ± 1.27mmHg) significantly changed comparing with preoperative (9.93 ± 1.85,9.98 ± 1.60mmHg;t=-24.84,-18.90;both P<0.01).The difference were 3.25±1.86,2.08±1.57mmHg respectively.Changes of CRF and CH were significantly different between two groups (t=-0.351,-1.081;both P<0.01).CONCLUSION:The postoperative corneal biomechanics and lOP measurements are lower than preoperative.SMILE are lower than FS-lasik in changes of CRF,CH.
5.May chronic childhood constipation cause oxidative stress and potential free radical damage to children?

OBJECTIVETo investigate whether chronic childhood constipation (CCC) may cause oxidative stress and potential free radical damage to children, and to explore the mechanisms by which CCC may cause oxidative stress and potential free radical damage to chronic constipation patients (CCPs).
METHODSSixty CCPs and sixty healthy child volunteers (HCVs) whose ages, gender and others were matched for the CCPs were enrolled in a randomized controlled study, in which levels of vitamin C (VC) and vitamin E (VE) in plasma as well as activities of superoxide dismutase (SOD) and catalase (CAT) in erythrocytes were determined by spectrophotometric analytical methods.
RESULTSCompared with average values of the above biochemical parameters in the HCVs group, the average values of VC and VE in plasma as well as those of SOD and CAT in erythrocytes in the CCPs group were significantly decreased (P < 0.0001). Linear regression and bivariate correlation analysis showed that with prolonged course of the CCPs, the levels of VC and VE in plasma as well as the activities of SOD and CAT in erythrocytes in the CCPs were decreased gradually (P < 0.0001).
CONCLUSIONThe findings in the present study suggest that chronic childhood constipation causes oxidative stress and potential free radical damage to children with chronic constipation.

6.Anti-apoptosis effect of VEGF on the human chronic myelocytic leukemia cell line K562.

To explore the effects of vascular endothelial growth factor (VEGF) on the mechanisms of CML pathogenesis, the effect of VEGF on K562 cell apoptosis induced by As(2)O(3) was analyzed through morphologic observation, DNA fragmentation agarose gel electrophoresis and DNA ploidy flow cytometry analysis, and the effect of VEGF on the expression of bcl-X(L), Bax and caspase-3 in K562 cells was determined by Western blot, meanwhile the expression difference between bcl-X(L) and Bax mRNA in above conditions was detected by RT-PCR. The results showed that after VEGF added, the apoptosis of K562 cells reduced, however, there was no significant changes in cell cycle distribution (P > 0.05). At the same time, following the increasing of the concentration of VEGF, expression of mRNA and protein of bcl-X(L) was up-regulated and the expression of Bax protein was down-regulated in K562 cells, and the activation of pro-caspase-3 into caspase-3 was inhibited or reduced. It is concluded that VEGF may suppress the apoptosis of K562 cells through its influence on the bcl-X(L)/Bax expression ratio in K562 cells.

7.The in vitro effect of norcantharidin on proliferation and invasion of human gallbladder carcinoma GBC-SD cells and its mechanism.

OBJECTIVETo study the effect and mechanism of action of norcantharidin on proliferation and invasion of GBC-SD cells.
METHODSGBC-SD cells of human gallbladder carcinoma were cultured by cell culture technique. The tetrazolium-based colorimetric assay was used to evaluate cell growth. The Matrigel experiment and the crossing-river test were used to examine the invasiveness of GBC-SD cells. Expression of MMP(2), TIMP(2), PCNA and Ki-67 proteins of GBC-SD cells was determined by streptavidin-biotin complex method.
RESULTSNorcantharidin inhibited the growth and proliferation of GBC-SD cells in a dose and time dependent manner, with an IC(50) value of 56.18 micro g/ml at 48 h. The Matrigel experiment showed that norcantharidin began to inhibit the in vitro invasion of GBC-SD cells at the concentration of 5 micro g/ml. At 40 micro g/ml, the invasive action of GBC-SD cells was inhibited completely and their crossing-river time was prolonged significantly. After treatment with norcantharidin, the expression of PCNA, Ki-67, MMP(2) was significantly decreased. With the increase in TIMP(2) expression, the MMP(2) to TIMP(2) ratio was decreased significantly (P < 0.05).
CONCLUSIONNorcantharidin inhibits the in vitro proliferation and growth of human gallbladder carcinoma cells at relatively low concentrations by inhibiting PCNA and Ki-67 expression. Its anti-invasive activity may be the results of decrease in MMP(2) to TIMP(2) ratio and reduced cell motility.

8.Allogeneic hematopoietic cell transplantation for malignant hematological diseases in patients older than 50 years of age

Objective To investigate the efficiency and safety of allogeneic hematopoietic cell transplantation for malignant hematological diseases in patients older than 50 yeas of age. Methods From May 2002 to January 2010, 35 patients (> 50 years) with malignant hematological diseases received allogeneic hematopoietic cell transplantation. In 35 patients, 18 patients were conditioned with non-myeloablative regimen and 17 patients with myeloablative regimen. The outcome,engraftment and prognosis of allogeneic hematopoietic cell transplantation were analyzed. Results The hematopoetic reconstitution was achieved in 32 of 35 patients. The median time of granulocyte count exceeding 0. 5 × 109/L was 12 days and the that of platelet count exceeding 20 × 109/L was 17days. The cumulative incidence of aGVHD was 48. 6 %, and 37. 9 % patients developed cGVHD.The estimate probability of cumulative survival at 5 years was 48. 5 %, The estimate probability of cumulative mortality rate was 51.5 %, and the estimated transplant-related mortality was 22. 9 %.The relapse rate was 11.4 %. There was significant difference except for the incidence of cGVHD.Conclusion Allogeneic hematopoietic cell transplantation may be appropriate for older patients with malignant hematological diseases.
9.The crush syndrome patients combined with kidney failure after Wenchuan earthquake.

OBJECTIVETo retrospectively analysis the treatment characteristics of the systemic situation in patients with crush syndrome after Wenchuan earthquake happened in May 12th, 2008.
METHODSForty-nine patients with crush syndrome and subsequent acute renal failure (ARF) due to the earthquake were treated in West China Hospital. All of patients had been rescued from buildings that collapsed in Wenchuan earthquake. The major associated injuries were in the low extremities and upper extremities. 49 patients developed ARF with increased concentrations of serum creatinine (mean 64 022 U/L) had underwent haemodialysis. Hyperkalaemia was seen in 9 patients and four of them underwent haemodialysis. 49 patients were administered hemodialysis.
RESULTSNo patient died. All patients who suffered from the ARF were weaned from hemodialysis after admitted 7 to 35 days. Forty-five extremities underwent amputations and 52 extremities had fasciotomy.
CONCLUSIONSCrush syndrome requires urgent recognition and prompt surgical treatment with simultaneous measures to control hyperkalemia and ARF. The authors believe that immediate intensive care therapy and multi-subjective coordination would have improved the survival rate.

10.Changes of serum galectin-3 and pentraxin-3 levels in CHF patients and their correlation with progno-sis

Objective :To explore changes of serum levels of galectin-3 and pentraxin-3 (PTX-3) in patients with chro-nic heart failure (CHF) and their correlation with prognosis .Methods : A total of 150 CHF patients (CHF group) treated in our hospital and 150 healthy volunteers undergoing physical examination simultaneously (healthy group ) from Jan 2015 to Dec 2015 were enrolled .Serum levels of galectin-3 ,PTX-3 and N-terminal pro brain natriuretic peptide (NT-proBNP) ,left atrial diameter (LAD) ,left ventricular end-diastolic dimension (LVEDd) and left ven-tricular mass index (LVMI) were compared between two groups at enrollment .According to one-year follow-up out-come ,CHF group (12 cases lost ,another 138 cases) was divided into poor prognosis group (n=36) and good prog-nosis group (n=102).Spearman linear correlation analysis was used to analyze correlation among serum galectin-3 , PTX-3 levels and poor prognosis of CHF patients .Results :Compared with healthy group ,there were significant rise in serum levels of galectin-3 [ (2-23 ± 0-25) ng/ml vs .(16-61 ± 1-48) ng/ml] ,PTX-3 [ (1-28 ± 0-54) μg/L vs. (3-58 ± 0-52) μg/L] ,NT-proBNP [(223-23 ± 76-28) pg/ml vs.(952-75 ± 85-43) pg/ml] ,LAD ,LVEDd and LV-MI in CHF group , P=0-001 all.Compared with good prognosis group at enrollment ,there were significant rise in serum levels of galectin-3 [ (18-52 ± 1-91) ng/ml vs.(24-63 ± 2-26) ng/ml] and PTX-3 [ (2-65 ± 0-74) μg/L vs. (3-95 ± 1-05) μg/L] in poor prognosis group , P=0-001 both .Spearman linear correlation analysis indicated that serum galectin-3 and PTX-3 levels were significant positively correlated with major adverse cardiovascular events in CHF patients ( r=0-608 ,0-558 , P=0-001 both).Conclusion :Serum levels of galectin-3 ,PTX-3 and NT-proBNP levels are significant rise and closely related to prognosis of CHF patients ,can be used as auxiliary indexes predicting prognosis and may provide the basis for formulation of target therapy .
