1.The foci and problems in diagnosis and treatment of gastric cancer

China has high prevalence of gastric cancer, with the morbidity and mortality leading the list of malignancy. The conditions of diagnosis and treatment of gastric cancer in China fail to meet the need of a large number of patients because of restriction in re-gional development. Therefore, we are facing a very serious situation of fighting against gastric cancer. Multidiscipline, individualiza-tion, and standardization are the development tendency of diagnosis and treatment of gastric cancer. Numerous topics about gastric cancer are currently controversial and solutions to these problems depend on the development of evidence-based medicine. This re-view summarized the recent progress in the clinical methods used for stomach cancer, laparoscopic surgical techniques, diagnosis and treatment of early gastric cancer, conversion therapy of advanced gastric cancer, therapeutic strategy of esophageal-gastric junction cancer, enhanced recovery after surgery, and translational research and clinical trials of gastric cancer. Moreover, the foci and prob-lems of the diagnosis and treatment of gastric cancer were discussed to provide reference for further studies.
2.Free superficial iliac circumflex artery skin flap: the clinical application and management of donor site defects.

OBJECTIVETo investigate the clinical application of free superficial iliac circumflex artery skin flaps, as well as the management of donor site defects.
METHODS17 free superficial iliac circumflex artery skin flaps were applied for the traumatic defects or deformities on face, neck, foot, hand, ankle and lower leg, respectively. The donor site defects were closed directly or covered by paraumbilical island flaps.
RESULTSThe 17 flap size ranged from 5 cm x 3 cm to 19 cm x 14 cm. 16 flaps survived completely except 1 flap with partial necrosis, which was closed by free skin graft. The donor site defects were closed directly in 10 cases, and covered by paraumbilical island flaps in 7 flaps without no flap necrosis. The abdomen had a good appearance.
CONCLUSIONSGood appearance can be achieved with free superficial iliac circumflex artery skin flaps for the defects on face, neck, foot, hand, ankle and lower leg. Paraumbilical island flap can be used for the donor site defects.

4.Preoperative chemoradiation therapy for upper gastric cancer:a retrospective study for its efficacy and safety

Objective:To investigate the efficacy, safety, and overall survival of advanced upper gastric cancer patients who received preoperative chemoradiation therapy. Methods:A total of 62 patients who received preoperative chemotherapy or chemoradiation therapy in the Department of Gastrointestinal Surgery of Beijing Cancer Hospital&Institute were retrospectively observed to determine the efficacy and safety and to perform survival analysis of preoperative chemoradiation therapy. Results:Results of the postoperative pathology showed that the number of patients with T4 and N3 stages was significantly lower in the preoperative chemoradiation therapy group than in the preoperative chemotherapy group (P<0.05). In addition, the differences between the two groups in terms of safety and toxicity were not significant (P≥0.05). Analysis also showed that the differences between the two groups in terms of survival were not significant (P≥0.05). Conclusion:Patients with advanced upper gastric cancer can gain a potential survival advantage from preoperative chemoradiation therapy. Compared with preoperative chemotherapy, preoperative chemoradiation therapy was performed without increased risk of toxicity and insecurity. Preoperative chemoradiation therapy can also improve the local control ratio, especial y the control ratio of lymphatic metastasis. However, the final results of survival analysis depend on long-term follow-up of patients.
5.Advances of study on the formulae based on a correlation and linear regression between the potassium concentration in the vitreous humor and the post-mortem interval and the factors which modifies the relationship.

In this paper we reviewed the formulae and analyzed the factors which modified the relationship and estimate the time of death as precise as possible by potassium concentration in vitreous humor. The extra factors including numbers of the cases, cause of death, different eye of both, the urea or/and creatinine in vitreous humor, environmental temperature, sampling techniques, experimental and analytical method were also mentioned.

6.Multidisciplinary treatment for a patient with locally advanced esophagogastric junc-tion cancer

Since the biological characteristics of esophageal gastric junction (EGJ) cancer are different from those of gastric cancer and esophageal cancer, the choice of initial treatment is particularly important. This article introduces a case of locally advanced EGJ can-cer with single metastasis factor treated with preoperative radiochemotherapy in the Beijing Cancer Hospital. Through the multidisci-plinary team, we aim to achieve a better prognosis for this patient and propose new treatment practices for EGJ cancer.

8.Graft heaIing and functionaI recovery after anterior cruciate Iigament reconstruction:a case study of rehabiIitation training

BACKGROUND: Anterior cruciate ligament (ACL) injury directly impacts the individual's lower limb function. It is very important for althelets whether rehabilitation training after ACL reconstruction is reasonable, effective and can restore the normal function of the knee joint.OBJECTIVE: To explore the effect of rehabilitation training on graft healing and knee joint function recovery after ACL reconstruction. METHODS: An elite rugby player was enrolled and subjected to phased rehabilitation training after ACL reconstruction. MRI examination was done before and after rehabilitation training. Then, the following indicators were tested, including joint flexion of knee, leg circumference degrees, isokinetic muscle strength of the lower limb, feedback test, the test of YBT, the Balance Error Scoring System, feedforward test, static balance test on one foot, dynamic balance test on the feet, Lysholm knee score, anaerobic work of the upper extremity, torso strength, to analyze the graft healing, body shape, the knee joint function and physical quality improvement after ACL reconstruction. RESULTS AND CONCLUSION: After 10 months of phased rehabilitation training, the graft healed well; the knee flexion degree was back to normal; knee joint swelling basically eliminated; the muscle strength of the extensor flexor of the knee joint was back to over 95% of the normal level; the function of the injured knee joint was good and returned to a higher level; the Lysholm knee score was 85 points; the anaerobic work of the upper limbs and trunk strength reached to 120% of the normal level; and the quality of the body was greatly improved. To conclude, a great improvement in graft healing, knee joint function, recovery of muscle strength and physical qualities has been achieved in athletes who receive rehabilitation training after ACL reconstruction.
9.Analysis on misdiagnosis of 271 patients with rectal cancer

Objective To investigate the common reasons for the misdiagnosis of rectal cancer. Methods A retrospective study was performed in 568 cases of rectal cancer in the Changhai Hospital from January 2007 to December 2008. Age at diagnosis, gender distribution, symptom, delay in diagnosis, TNM stage, and grade of differentiation were recorded and analyzed. The importance of digital examination and colonoscopy were addressed. Results Two hundred and seventy-one(47.7%) out of 568 patients were misdiagnosed for iatrogenic reasons. Rectal cancer patients who presented hematochezia were more likely to be misdiagnosed. There were 110 cases of stage Ⅲ(40.6%) and 68 cases of stage Ⅳ(12.5%) in patients who were misdiagnosed, which was significantly higher than those who were diagnosed correctly(P<0.05). Patients under 40 years old were more likely to be misdiagnosed, and their correct diagnosis was often delayed longer and the tumors were in more advanced stage as compared to the older groups(P<0.05). Conclusions The misdiagnosis rate of rectal cancer is high. Tumor stage of patients misdiagnosed is significantly more advanced than those who are correctly diagnosed. Digital examination and colonoscopy should be emphasized, especially for patients under the age of 40.
