OBJECTIVETo evaluate the clinical results of the medial patellofemoral ligament (MPFL) reconstruction combined with the lateral retinacular release for the treatment of recurrent patellar dislocation.
METHODSFrom March 2011 to June 2013, 15 patients with recurrent patellar dislocation underwent arthroscopic MPFL reconstruction combined with the lateral retinacular release. The graft was autogenous semitendinosus and semimembranosus tendon. There were 5 males and 10 females with an average age of 19.4 years old (ranged,14 to 32 years old). The patients suffered recurrent patellar dislocation at least twice preoperatively. Preoperative conventional X-ray, CT, and MR examination were used to analyze the causes of the patellofemoral joint and MPFL injury. Preoperative Lysholm score was 69.85 ± 11.52. During operation, the arthroscopic examination was performed to evaluate the patellofemoral alignment and patellar tracking.
RESULTSAll the patients were followed up for an average of 27.6 months (ranged,12 to 36 months) with no recurrent dislocation and sub-dislocation. All the patients showed negative apprehension test at straight and 30 ° flexions of knee. The range of motion of knee returned to normal level at 12 months after operation. There were no patients with subjective discomfort of knee. Postoperative Lysholm score was improved to 92.60 ± 5.75.
CONCLUSIONThe technique of arthroscopic MPFL reconstruction combined with the lateral retinacular release is an effective surgical procedure for the treatment of recurrent patellar dislocation, which can relieve the symptom of knee and improve the patella stability and knee function.

AIM To study the pharmacokinetics of 20(R)-ginsenoside Rg3 (GRg3) in human. METHODS High-performance liquid chromatography-ultraviolet detection method was used in this study. RESULTS The pharmacokinetics of GRg3 in 14 healthy volunteers were investigated. After a single oral dose of 3.2 mg.kg-1 in 8 male volunteers, the plasma concentration-time course fitted well to a two-compartment open model, with the following pharmacokinetic parameters: Tmax (0.66±0.10) h, Cmax (16±6) ng.mL-1, T1/2α (0.46±0.12) h, T1/2β (4.9±1.1) h, T1/2(Ka) (0.28±0.04) h, AUC0-∞ (77±26) ng.mL-1.h. No kinetic analysis was made after an oral dose of 0.8 mg.kg-1 GRg3 in 6 other volunteers because of the low concentration, but a good correlation between Cmax and dosage of the two groups was found. CONCLUSION The absorption of GRg3 was rapid in man, and its elimination was rapid after oral administration of ginsenoside Rg3. The pharmacokinetic results shows that it exhibited first order kinetic characteristics.

OBJECTIVE To explore the latency characteristics of speech evoked auditory brainstem response(speech-ABR)from healthy Chinese adults by different stimulation intensities and its significance in neural encoding of speech in the brainstem. METHODS ABRs to speech syllables of 32 subjects were recorded in four stimulus intensities varied from 20 to 80 dB SPL by 20 dB steps, under 11.1/s stimulus rate. A set of featured waves were determined under these intensities, and then the characteristics of their latencies were analyzed. RESULTS A series of featured waves were observed starting from onset response(including V and A waves), transitional C wave, then following frequency-following responses(FFRs), and the offset O wave. As the reduction of stimulus intensity, the latencies of speech-ABR delayed significantly(P

Objective:To compare the time domain and the frequency domain of speech-evoked auditory brainstem response measured by stimulation of left and right ears, and to explore the difference and possible reasons of neural coding for speech from different ears in auditory brainstem.Method:Speech-ABRs to syllable /da/ of 31 healthy adults were recorded. Statistical analysis was performed on time-domain parameters, such as latencies and amplitudes of featured peaks, and frequency-domain ones, such as amplitudes of the fundamental frequency and the first formant of speech-ABRs ranging from 20-50 ms. A scoring criterion to grade the appearance of featured waves was proposed for waveform evaluation.Result:There were no significant difference for the latencies of binaural featured peaks and amplitudes of feature peaks(except peaks A and O). The waveform scores of right ear were greater than that of left ear. The amplitudes of fundamental frequency of binaural waves were both greater than that of the first formant. There was no significant difference of amplitudes of fundamental frequency and the first formant between two ears. Conclusion:The origins and distributions of speech-ABR are essentially symmetrical in brainstem in contrast with the hemisphere asymmetry of speech.

Objective To investigate the efficacy of laboratory tests in the renal damage early diagnosis of children with Henoch-Schoalein purpura (HSP) and clinical effect of early intervention. Methods For the 143 HSP patients with normal repeated urine routine test findings, renal function biomarkers including urinary proteins ( immunoglobulin G (IgG),micro-albumin ( MA ),transferrin (TRF),a1 -microglobulin ( α1 -MG),β2-Microglobulin (β2-MG) ) and urinary enzymes ( N-acetyl-beta-D-glucosaminidase ( NAG ),γ-glutamyltransferase (y-GT) ) were detected to investigate the details of renal function changes. One hundred and thirty-one HSP patients, who had abnormal laboratory test findings of renal function biomarkers mentioned above, were randomly divided into control group ( n =65 ) and intervention group ( n =66 ),and both groups received comprehensive treatment including cimetidine, loratadine and calcium agents.However,66 patients in intervention group received low-dose heparin via micropump-based continuous intravenous infusion and regular oral diammonium glycyrrhizinate treatment.Sixty-five patients were enrolled in control group, without further treatment. Results Among the 143 patients with normal urine routine examination,131 cases (91.61% ) had abnormal findings of renal function biomarkers. After therapy either for 2 months or 4 months, urine protein and urine enzymes were lower than before treatment, and the difference was significant (P < 0.01 ).In the control group only β2-MG,NAG,γ-GT3 indexes significantly lowered at the end of 2 months ( P <0.01 ),and all parameters were significantly decreased at the end of 4 months ( P <0.01 ).Furthermore,Intervention group had lower levels of renal function biomarkers at the end of 2 months or 4 months,as compared with the control group, showing significant difference ( P <0.05 or P <0.01 ).Urinary IgG,MA,TRF,NAG recovered rapidly in the intervention group after 4 months and almost returned to the normal, but urinary α1-MG,β2-MG,γ-GT recovered slowly and still remained abnormal after 4 months due to the varying severity. After treatment for 4 months, the rate of urine testing abnormalities was higher in the control group than in the intervention group (36.92% vs 6.10% ),and the difference was significant (P <0.05).Conclusion Combined detection of renal function biomarkers is helpful for early diagnosis of renal damage in HSP patients.Early intervention with heparin and diammonium glycyrrhizinate can prevent kidney damage, delay disease progress.Early diagnosis and early intervention should be emphasized for the treatment strategy of the renal damage of children with HSP.

Background Intermittent exotropia is a type of strabismus that between latent extropia and manifest extropia. The assessment of fusional convergence/divergence is important for understanding control ability of exodeviation in children with intermittent exotropia. Objective This study was to analyze the correlations between fusional convergence/divergence and control ability of exodeviation in children with intermittent exotropia. Methods Sixty-three children with intermittent exotropia were recruited in Beijing Tongren Eye Centre from July 2013 to February 2014 under the informed consent of children and their parents.Angle of deviation was measured by wearing prism and covering method alternately. The control ability of exodeviation was evaluated and scored by the Revised Newcastle Control Score (RNCS),and fusional convergence and divergence were measured with 1 Δ-40Δ horizonal prisms and regulating targets. The correlations between the measured parameters of fusional convergence/divergence and control scores of exotropia were analyzed by Spearman rank correlation analysis. Results The mean diopter of the right and left eyes was (-1.95 ± 1.63)D and (-2.01 ± 1.73)D,respectively,and the mean deviation angle for distantly and near was (36.67 ± 15.69) Δ and (38.25 ± 14.83) Δ,respectively, without significant differences between them (diopter:t =-0.13,P>0.05;deviation angle:t =-0.57,P>0.05).Considerably negative correlations were found between the breakpoints of fusional convergence for distant or near and control scores of exodeviation (rs =-0.41,P=0.03;rs =-0.56,P<0.01).No significant correlations were found between the breakpoints of fusional divergence for distantly or near and control scores of exodeviation (rs =0.05,P =0.78;rs =0.04,P <0.75).In addtion, there was no significant correlation between fusional recovery level and control scores (both at P > 0.05).Conclusions Breakpoints of fusional convergence may be useful in grading the severity of intermittent exotropia in children, and it is probably one of the surgical indications of intermittent exotropia.
