Objective]The article summarizes professor Fu Ping clinical evidence-based medicine in the experience of vicarious menstruation. [Methods] From disease name,pathogenesis,therapy and so on,summarize the experience and thoughts of treating,and for a case to analyze treatment characteristics and therapeutic effect. [Results]Fu Ping holds the view that the reason for the occurrence of vicarious menstruation is liver fire invading lung,lung or kidney Yin deficiency,qi stagnation blood stasis or liver-yang hyperactivity and spleen deficienty; The location of disease is in the lung,closely related with liver and kidney.The main therapy is to nourish Yin and moisten lung or clear liver and decrease internal heat. For the syndrome of liver-yang hyperactivity and spleen deficiently,Fu Ping using method of breeding earth and suppressing wood to treat is medicable.[Conclusion]Professor Fu Ping's experience in treating vicarious menstruation is worthy of study.

Objective To investigate the association of α-synaptophysin A53T gene polymorphism with parkinson's disease(PD) in Chinese people.Methods The conventional polymerase chain reaction was used to detect the α-synaptophysin A53T gene polymorphism in 224 sporadic PD patients(PD group)and 154 healthy individuals(control group).According to the Hoehn-Yahr(H-Y) classification standard, PD patients were divided into H-Y ≥ 3 group(n=172) and H-Y ≤ 2.5 group(n=52).Each genotype and alleles frequencies as well as the A53T gene expression and their relation to the severity of parkinson's disease were analyzed with Chi-square test of SPSS19.0.Results The frequency of the A53T genotype of A/A were 40(17.9％) and 10(6.5％) (x2 =10.267, P=0.001, OR=3.13,95％ CI =1.514-6.473) in the PD group and control group, respectively.The frequency of the allele A was 160(35.7％) and 70(22.7％) (x2 =14.543, P=0.000, OR=1.889,95％ CI =1.359-2.625) in the PD group and control group,respectively.The frequency of the A53T genotype of A/A was 30(17.4％) and l0 (19.2％) in the H-Y ≥ 3 group and H-Y ≤ 2.5 group, respectively, with significant difference (P=0.003,0.007) as compared to the control group.The frequency of the allele A was 122(35.5％) and 38(36.5％) in the the H-Y ≥ 3 group and H-Y ≤ 2.5 group,respectively, with significant difference (P=0.000,0.006) as compared to the control group.There was no statistically significant difference of the A53T genotype of A/A and the allele A between the H-Y≥3 group and H-Y≤2.5 group (P=0.768,0.841).Conclusion The A53T gene polymorphism is the risk factor of Parkinson's disease in Chinese people, but it isn't correlated to stage of sporadic parkinson' s disease.

目的观察壮腰活血汤治疗腰椎间盘突出症的疗效。方法将198例腰椎间盘突出症患者随机分为治疗组和对照组,治疗组内服壮腰活血汤配合腰椎牵引、超短波等治疗;对照组采取与治疗组相同的物理治疗方法。治疗2个疗程和疗程结束2个月后进行疗效评定。结果治疗组的有效率明显高于对照组(P<0.05—0.01)。结论壮腰活血汤治疗腰椎间盘突出症疗效确切,并且远期疗效更优。

Objective To observe the efficacy of intravitreal injection of ranibizumab (IVR) for different patterns of optical coherence tomography (OCT) of diabetic macular edema and the relationship between integrity of ellipsoidal zone and visual acuity outcomes.Methods Eighty-five IVR treated eyes were enrolled.The examination of BCVA was according to Early Treatment Diabetic Retinopathy Study,and the results were recorded as logarithm of the minimum angle of resolution (logMAR).Frequency-domain OCT was used to measure the central foveal thickness (CFT) and the integrity of ellipsoidal zone.All eyes were classified as diffuse macular edema (DRT group,31 eyes),cystoid macular edema (CME group,29 eyes),and serous retinal detachment (SRD group,25 eyes).All the patients were treated with intravitreal injection of 0.05 ml (0.5 mg) ranibizumab.The mean follow-up time was (9.21+3.56) months after IVR treatment.The changes of BCVA and CFT in 3 groups were compared and analyzed after 3,6 and 12 months.According to visual acuity at different ranges,the relationship between integrity of ellipsoidal zone and BCVA was analyzed.Results Compared with the average logMAR BCVA before treatment,except for 12 months after treatment in group SRD (t=2.104,P=0.053),the average logMAR BCVA after IVR at 3 months,6 months and 12 months improved in DRT group (t=7.847,6.771,6.426;P=0.000,0.000,0.000),CME group (t=8.560,6.680,5.082;P=0.000,0.000,0.000) and SRD group (t=5.161,3.968,2.104;P=0.000,0.001,0.053).The average logMAR BCVA of the DRT group was lesser than that in CME and SRD group after 12 months treatment (t=-2.043,-3.434;P=0.030,0.001).The average CFT after IVR at 3 months,6 months and 12 months reduced significantly in DRT group (t=12.746,10.687,9.425;P=0.000,0.000,0.000),CME group (t=13.400,11.460,10.169;P=0.000,0.000,0.000),and SRD group (t=1 1.755,10.100,9.173;P=0.000,0.000,0.000).After 12 months of treatment,the average CFT of the SRD group was thicker than that in DRT group and CME group (t=-3.251,-1.227;P=0.003,0.025);there was significant difference in the integrity of ellipsoidal zone among 3 groups (x2=1.267,P=0.531).The results showed that there were significant differences in the integrity of ellipsoidal zone with different ranges of BCVA before and after 12 months treatment (x2=20.145,41.035;P=0.000,0.000).Conclusions IVR could significantly improve the visual acuity of different patterns of DME,reduced the CFT,and had the best efficacy in the DRT group.There was relationship between the integrity of ellipsoidal zone and the visual acuity outcomes.

Objective To investigate the role of glucagon peptide-1(GLP-1) in blood pressure variability (BPV) in diabetic hypertensive patients. Methods A total of 120 diabetic hypertensive patients were included in this study. GLP-1 was used for the treatment. The values of body mass index (BMI), fasting blood glucose, glycosylated hemoglobin, creatine and 24-hour ambulatory blood pressure (BP) monitoring were detected before and after treatment with GLP-1 for six months. The clinical indices were compared before and after treatment. Results The values of fasting blood glucose (mmol/L:7.12± 0.64 vs 9.19±2.78), glycosylated hemoglobin (%:7.00±0.14 vs 8.28±1.32) and BMI (kg/m2:19.30±3.24 vs 24.5±4.53 ) were de-creased to the normal level after GLP-1 treatment for six months (P<0.05). There were significant decrease in the 24-h sys-tolic BP (mmHg:135.02 ± 16.57 vs 139.52 ± 15.60), daytime systolic BP (mmHg:132.5 ± 14.6 vs 136.44 ± 14.24), 24-h systolic BP variability (mmHg:12.20 ± 1.44 vs 12.73 ± 1.66), daytime systolic BP variability (mmHg:11.11 ± 1.48 vs 11.74 ± 1.52 ), and daytime diastolic BP variability (mmHg:7.03±1.42 vs 7.43±1.45) after GLP-1 treatment for six months (P<0.05). Conclu-sion GLP-1 can decrease BPV in diabetic hypertensive patients.

OBJECTIVE To investigate the role of TLR4 signaling pathway in secretory otitis media of rats.METHODS Rats SOM models were made by infusing FS and LPS to the middle otocysts. The rats were killed separately at the 3rd, 5th, 7th, 9th,11th and13th day(5 rats each time). And then the expressions of TLR4, iNOS, IL-8mRNA of middle ear mucosae were detected. NF-?B was also detected through the method of immunohistochemistry. RESULTS The expressions of TLR4mRNA, iNOS and IL-8mRNA in middle ear mucosae of SOM group were significantly higher than that of control group. Immunohistochemistry method showed that the activation of NF-?B in middle ear mucosae was obviously increased in SOM. CONCLUSION The expression of TLR4 was obviously increased in SOM and it activates the NF-?B, promotes the expression of downstream IL-8. That maybe the mechanism which cause SOM.

Objective: To evaluate the incidence, clinical features, diagnosis and treatment of primary ex-tranodal non-Hodgkin' s lymphoma (PE-NHL). Methods: The data of 110 patients diagnosed as PE-NHL be-tween January 2001 and May 2008 were reviewed. Results: These PE-NHL patients counted 60.11% of the 183 malignant lymphoma patients at the same period. The primary sites affected were the gastrointestinal tract 21.82% (24/110), Waldeyer ring 10.91% (12/110), nasal cavity 9.10% (10/110), soft tissue 9.10% (10/110), mediastinum 7.27% (8/110) and other unusual sites 41.82% (46/110). Symptoms and signs of PE-NHL were not specific, and 77.27% of these cases had a swelling organ or lump of the primary organ affected. Ac-cording to the International Prognosis Index (IPI), the percentage of patients in low, intermediate, and high group was 41.11%, 44.44% and 14.44%, respectively. Immunophenotype was assayed in 93 cases. The per-centage of B-cell lymphoma was 69.90% while that of T-cell lymphoma was 30.10%. For those 95 cases treat-ed, the effective rate including complete remission (61.05%) and part remission (16.84%) was 77.89%, the median survival was 30 months, and the 5-year overall survival (OS) was 27%. While, for patients with prima-ry gastrointestinal non-Hodgkin' s lymphomas (PGIL), the complete remission rate, part remission rate and the effective rate was 65.21%, 17.39% and 82.80%, respectively. The median survival was 24 months, and the 5-year overall survival (OS) was 30%. Conclusion: PE-NHL is more common than nodal lymphoma. The symptoms and signs of PE-NHL of different sites are quite different. To improve the curative strategies of PE-NHL, it is important to make an allround understanding of PE-NHL and follow reasonable mode of diagno-sis and therapy.
