Objective To investigate the prognostic outcome of open reduction and internal fixation(ORIF)with radial head resection in treatment of comminuted radial head fractures so as to discuss the corresponding operation options. Methods A series of 23 patients with Mason type Ⅲ or Ⅳ radial head fractures were enrolled in the study including 11 males and 12 females.Eight patients(three with Mason type Ⅲ and five with Mason type Ⅳ)underwent radial head resection and 15(seven with Mason type Ⅲ and eight with Mason type Ⅳ)received open reduction and internal fixation including nail and plate fixation in eight and screw and K-wire fixation in seven. The outcomes were assessed in regard of range of motion, radiographic findings, visual analogous scale(VAS)of pain, patient satisfaction degree, disabilities of the arm, shoulder and hand score system(DASH)and the Broberg & Morrey elbow performance score system. Results The follow-up period ranged from 12 to 65 months(mean 35 months),which showed fracture healing in all patients, with no traumatic arthritis happened. The mean range of flexion and extension arc at the elbow of ORIF group and resection group was 132°and 79.4°(P<0.01)and the patient satisfaction was 9.1 and 8.0(P <0.05)respectively.According to the Broberg & Money elbow performance score system, the score was average 93.9 in the ORIF group, with excellent result in nine patients, good in five and fair in one, while the score was average 80.6 in the resection group, with excellent result in two patients, good in two and fair in four. There was statistical difference between two groups(P <0.01).The DASH score of the ORIF and resection group was 2.9 and 18.1 respectively(P<0.05).Conclusions ORIF has better elbow motion, elbow function, daily living and satisfactory outcomes than radial head resection and is recommended for treatment of Mason type Ⅲ or Ⅳ radial head fractures for repair of the radial head as far as possible.

Objective To study the preventive effects of vitamin K2 on tumor recurrence in patients with hepatocellalar carcinoma (HCC) after radical resection. Methods The clinical data of 50 patients with HCC who received radical resection from March 2006 to March 2007 in No. 181 Hospital of PLA were analyzed retrospec-tively. All the patients were divided into 2 groups according to the random number table. Twenty-six patients in vitamin K2 group were administered with menatetrenone (45 mg per day), and the rest 24 pateints were in the control group. The accumulative and tumor-free survival rates, differences between the 2 groups, multivariate factors for prognosis were analyzed by Kaplan-Meier curve, Log-rank test and Cox regression model, respectively. Results During a period of 36 month follow-up, 10 patients died and 28 had tunor recurrence. The 1-, 2-, 3-year accumulative survival rates were 96%, 92% and 83% in vitamin K2 group, and 96%, 82% and 63% in control group (χ2 = 3.61, P > 0.05). The 1-, 2-, 3-year tumor-free survival rates were 92%, 60% and 38% in vitamin K2 group, and 75%, 42% and 12% in control group, with significant difference between the 2 groups (χ2 =5.61, P <0.05). Univariate and multivariate Cox proportional hazard analysis showed that without taking menate-trenone, the preoperative level of alpha fetoprotein (AFP) ≥800 μg/L and vascular invasion were the indepen-dent risk factors for tumor recurrence. Conclusions Vitamin K2 has a suppressive effect on tumor recurrence of HCC, while patients with AFP≥800 μg/L before operation or with vascular invasion have poor prognosis.

Objective:To study the effect of portal vein occlusion on intestinal mucosal barrier in rats and the protection of ulinastatin to the injury,to present the experimental data for the clinical surgery.Methods:70 Sprague-Dawley rats were randomly divided into controlled group (n=10),operation group (n=30) and operation+medication group (n=30).The portal vein were occlused 40 min in the operation groups and operation+medication groups.2ml blood from portal vein, lymph nodes around appendix,1cm small intestine wall were taken for endotoxin levels, bacterial translocation and pathiology examinations in the all rats 280 mins after operation. The mocusal barrier and microscopic structure of intestine were observed. Results:Compared between the control group and the operation group, endotoxin levels, bacterial translocation rates rise greatly and gut structure change obviously in the latter.Compared between the operation group and operation+medication group, the former changes is also obvious. Conclusion:The occlusion of portal vein can leads the decrease of intestine mocusal barrier and the increase of its permeability.Ulinastatin has a good protective effect on the damages above.

Compared with C3 plant,C4 plant had evident growth advantage, higher rates of water and nutrition using, and higher bio-yield than C3 plant,Sugarcane was one of the typical C4 plants.DNA was extracted from sugarcane leaves, primers were designed by the cDNA sequence of PPDK gene from GenBank. Then DNA was amplified by LA-PCR(Long Acute PCR) method, ligated into pMD18-T vectors, and transformed into E.coli.JM109.Full-length PPDK gene sequence of sugarcane was obtained, the sequence was 13.5kb in length. For convenient of the next experiment, two digest sites(XhoI and NotI) were introduced into primers, the full-length PPDK gene was splitted to two parts, and was ligated into pMD18-T simple vectors, respectively.Whole PPDK gene clone of sugarcane was finished. The strains were storaged in the lab.

Objective To evaluate the clinical value of applying special calcaneal anatomy plate to treat serious comminuted fracture of calcaneus, with consideration of its frequent complications and difficulties in treatment. Methods From January 1999 to august 2001, we applied calcaneal anatomy plates to treat 14 cases of serious crumbled fracture of calcaneus. The vein pump was used to decrease the swelling before and after operation. Anti mycete measures were taken. During operation, we paid attention to the reduction of articular face, Bohler angle and critical angle, and made secure internal fixation with calcaneal anatomy plate and cancellous bone screw. Results 14 cases were followed up for 23 months averagely. 85.7% of the patients were satisfactory with the remission of subjective symptoms, while the rate of excellent foot outline was 92.9% . The rate of excellent joint motion of foot and ankle was 71.4% , and the rate of excellent Bohler angle was 78.6% . The overall excellent and good rate was 98.3% . Conclusion Applying the calcaneal anatomy plate to treat serious crumbled fracture of calcaneus and paying attention to good reduction, can acquire secure internal fixation, and permit patients to exercise earlier after operation, and can greatly decrease complications of calcaneal fracture.

Objective To observe the effect of arthroscopic meniscectomy for symptomatic discoid lateral meniscus and the importance of early treatment. Methods Arthroscopic meniscectomy was performed on 29 patients (31 knees) with symptomatic discoid lateral meniscus. Partial meniscectomy was performed on 27 knees, subtotal meniscectomy on 4 knees with tear on the edge of meniscus. Results The operations on all 31 knees were successful without complications. The mean Lysholm score was 95.6 (87-100) during the follow-up, which was significantly higher than that before the operation (65.7, range 36-78; P
