1.Protective Effect of Astragalus Membranaceus on Ischemic Acute Renal Failure

Objective: To investigate the effect of astragalus on the serum levels of hepatocyte growth factor (HGF), renal function and the damage of renal morphology in ischemic acute renal failure (IARF). To study the protective mechanism of astragalus in IARF. Methods: The rats were pretreated with astragalus or vehicle for 3 days. IARF was induced by left renal artery clipping for 60 min, after the right kidney was removed. Biochemical parameters for serum HGF,blood urea nitrogen(BUN),creatinine(Cr) were measured and histological analyses of the kidneys were performed by tubular grading. Results: There have no significant difference between the serum HGF levels of I/R controls and the levels observed in sham-operated group (P > 0.05). Serum HGF levels in astragalus treated group was significantly increased than those of I/R control group(P < 0.01) .After 4 h of I/R, serum levels of BUN and Cr were increased in I/ R control rats compare with Sham-operated rats(P < 0.01). Astragalus treatment reduced the levels elevation(P < 0.05). Simultaneously,. I/R-induced severe damage in renal morphology were significantly ameliorated by astragalus treatment(P < 0.01). Conclusions: Astragalus pretreatment can alleviate the damage of renal function in IARF. It is may be one of the mechanisms of astragalus for protecting renal tissue in IARF that astragalus can induce the expression of HGF.
2.Spatial distribution pattern of rodent community and the risk of disease transmission in Xiji County of Ningxia

Objective To analyze the spatial distribution patterns of rodent community and the risk factors of disease transmission in natural foci of Xiji County,Ningxia,in order to provide a scientific basis for disease control and prevention.Methods Rodents were captured at different habitats in different latitudes which were selected with a stratified sampling method in 2012-2013.Capture rate of different rodent species and spatial distribution patterns of the animal community were analyzed.Antigen and antibody of hantavirus were detected in lung tissue and blood of the rodents with immunofluorescence and enzyme-linked immunosorbent assay (ELISA),respectively.Serum F1 antibody of Yersinia pestis (Y.pestis) was detected with indirect hemagglutination test and Y.pestis from liver and spleen tissue was cultured in vitro.Results Fourteen species of rodents were captured,belonging to 2 orders,7 families and 9 genera.Among them,ground squirrels and wood mouse were dominant species,accounted for 49.56% (112/226) and 34.51% (78/226) of the community,respectively.Infections of hantavirus and Y.pestis were not found in the rodent's community.Conclusion With the improvement of ecological environment in Xiji County,the spatial distribution patterns of rodent community is changing; the risk of zoonotic plague is reduced,but the risk of hemorrhagic fever with renal syndrome is being.
3.Comparison of the Efficacy of Single-incision and Two-incision Posterior Chamber Phakic Intraocularlens Implantation in the Correction of Extreme Myopia

Objective:To observe and compare the clinical effect of single-incision and two-incision posterior chamber phakic intraocularlens implantation in the correction of extreme myopia.Methods:40 cases(80 eyes) of patients with extreme myopia underwent conventional two-incision posterior chamber phakic intraocularlens implantation in our hospital from January 2011 to June 2013 were selected as the control group and 40 cases(80 eyes) underwent single-incision posterior chamber phakic intraocularlens implantation from July 2013 to January 2016 were selected as observation group.The open hole before and after operation,best corrected visual acuity of vision,operation safety,effectiveness index,diopter (column mirror,equivalent ball for eyeglasses),intraocular pressure,ECD,anterior chamber depth and occurrence of complications were compared between two groups.Results:The uncorrected visual acuity (UCVA) of both groups at 6 months after operation were significantly higher than those before operation (P<0.05),which was significantly higher in the observation group than that of the control group (P<0.05);The operation safety and effectiveness index of observation group were significantly higher than those of the control group (P<0.05);The cylinder and spherical equivalent degree of both groups at 6 months after operation were significantly better than those before operation(P<0.05),which was significantly better in the observation group than that of the control group(P<0.05).The corneal endothelial cell counts(ECL) of control group after operation was significantly lower than that before operation (P<0.05),and no significant difference was found in the ECL of observation group before and after operation (P>0.05).The ECL of observation group after operation was significantly higher than that of the control group (P<0.05);The incidence rate of complication in observation group was significantly lower than that of the control group (P<0.05).Conclusion:Compared with two-incision operation,the single-incision posterior chamber phakic intraocularlens implantation had remarkable clinical effect in the correction of extreme myopia with higher security.
4.Difference in radiotherapy dose caused by different ways of adding bolus

Objective To compare the difference in radiotherapy dose caused by different ways of adding bolus.Methods A total of 20 patients who needed to receive postmastectomy chest wall irradiation from October to December on 2014 were selected.Each patient underwent two CT scans;CT-1 was to perform CT scan directly without bolus, and CT-2 was to perform CT scan after adding bolus to the body surface.An equivalent bolus was added for CT-1 in the radiotherapy planning system, and Plan-1, which met the clinical requirements, was performed.Then Plan-1 was put on CT-2 through image fusion and plan verification to develop Plan-2, which was to develop plans with equivalent boluses at other times and perform radiotherapy with a bolus added to the surface of the body.At last, CT-2 was used to perform radiotherapy Plan-3, which met the clinical requirements.The paired t-test was used for comparison of clinical data between any two plans with SPSS 19.0.Results The V20 of the whole lung, V20 of the diseased lung, V30 of the heart, and Dmax of the healthy breast showed no significant differences across the three plans (P=0.074-0.871).The V50 , V55 , conformity index, and homogeneity index of the planning target showed significant differences across the three plans, and the total number of monitor units showed a significant difference between Plan-1 and Plan-2(P=0.002-0.049).The dose distribution in the target volume and the number of monitor units in each radiation field also showed significant differences.Conclusions When the equivalent bolus is added to the body surface before CT scan, such a plan can accurately reflect the dose distribution of the planning target and the dose to organs at risk.
5.Clinical significance and prognostic value of C-reactive protein/albumin ratio in gastric cancer

Purpose:
This study was aimed to evaluate the clinical significance and prognostic value of CRP/albumin ratio (CAR) in patients with gastric cancer.
Methods:
The data of 205 gastric cancer patients who underwent surgery was analyzed retrospectively. The association of CAR with the clinical features and prognostic value in gastric cancer was analyzed. The data of this study was combined with previous studies to further determine the prognostic value of CAR in patients with gastric cancer using a metaanalysis method.
Results:
Cox analysis revealed that preoperative CAR was an independent prognosis indicator in patients with gastric cancer. High expression of CAR indicated a shorter survival time than in those with lower expression. CAR has a higher prognostic value in the 1-, 3-, and 5-year overall survival in patients with gastric cancer. CAR showed significant difference regarding the gastric cancer patients’ age, M stage, and clinical stage. The discriminate value of CAR in M stage of gastric cancer was high (area under the curve, 0.809). A meta-analysis combining previous data and our data showed that preoperative CAR demonstrated a significant association with the overall survival of patients with gastric cancer.
Conclusion
This study demonstrated that preoperative CAR could serve as an important prognostic indicator in patients with gastric cancer.

7.Alkaloids from Macleaya cordata and their cytotoxicity assay.

OBJECTIVETo study the alkaloids of Macleaya cordata and their anti-tumor activities.
METHODAlcohol and liquid-liquid extraction were used methods were used to extract the alkaloids constituents, and silica gel, reverse-phase octadecylsilyl (ODS), sephadex LH-20 chromatographic methods and HPLC were applied to isolate and purify compounds. MS, NMR spectroscopic methods were used to determine their structures. Furthermore, the cytotoxicity of these chemical components for MCF-7 and SF-268 cell lines was measured by MTT method.
RESULTTwelve alkaloids were isolated from the fruits of M. cordata, and their structures were identified as: maclekarpine E (1), 6-acetonyldihyrochelerythrine (2), cavidilinine (3), 6-acetonyldihyrosanguinnarine (4), O-methylzanthoxyline (5), 6-methoxy-dihydrosanguinarine (6), spallidamine (7), 6-hydroxyldihydrochelerythrine (8), arnotianamida (9), dihydrosanguinarine (10), protopine (11), and cryptopine (12).
CONCLUSIONCompounds 1, 3, 7-9 were isolated from M. cordata for the first time, and compound 5 is a new natural product. The results of cytotoxic assay indicated that compound 6 showed strong cytotoxicity against MCF-7 and SF-268 cell lines with IC50 values of 0.61 μmol · L(-1) and 0.54 μmol · L(-1), respectively.

9.An experimental study of the inhibition of tamoxifen on rat model of benign prostatic hyperplasia.

OBJECTIVESTo study the inhibitory mechanism of tamoxifen on benign prostatic hyperplasia.
METHODSThe Wistar male adult rats were injected into muscle with testosterone propionate 4-6 mg/kg, simultaneously were irrigated into stomach with tamoxifen citrate 0.21 mg/kg. The partly rats of each group were decapitated at 7, 15 and 30 days, then their index numbers of prostate were calculated, and the structural changes of the prostatic histocyte were observed in the light microscopy and scan electronic microscopy.
RESULTSAt the 7th, 15th, and 30th day, the index numbers of prostate of those rats which had been injected only with testosterone propionate were higher than that of the control group and the irrigating group(P < 0.05). The hyperplasia of the prostatic epithelial cells and the ground substance of those rats, which had been irrigated with tamoxifen citrate, had not happened in light microscopy and the scan electronic microscopy.
CONCLUSIONSTamoxifen could block the effect of the estrogen, which could suppress the prostatic hyperplasia. This study could provide the experimental evidences for using Tamoxifen to treat the human benign prostatic hyperplasia.

10.Hypoglossal neurilemmoma: current experience in diagnosis and treatment.

OBJECTIVETo improve the diagnosis and treatment of hypoglossal neurilemmoma (HGN).
METHODSThe data of 10 patients with HGN were retrospectively reviewed, 5 of 10 patients (early group) had been reported previously. A comparison of treatment strategy between early and latter group was made.
RESULTSTypical hemiatrophy of the tongue presented in all 10 patients, hypoglossal canal could be showed in the bone window of CT, which, however, might not confirm the existence of tumor if it is very small. MRI was the optical choice for diagnosis. Among the early 5 cases with HGN which were all of dumbbell type, the intracranial part and the extracranial part of the tumor in 2 cases were removed by stages, 3 cases were operated via the far-lateral approach, of which 1 was via trans-condylar approach and 2 were via trans-supracondylar approach; the 5 tumors were removed subtotally except 1 totally; CSF leakage and intra-cranial infection after operation occurred in 1 case; the rating of Karnofsky Prognosis Scale was good in 4 cases and dead in 1 case. However, among the latter 5 cases, 4 cases including 2 of dumbbell type, 1 of intra-cranial type and 1 of intracanal type were operated via the modified far-lateral approach, of which 1 was via trans-condylar approach and 3 were via trans-supracondylar approach; and the tumor of extracranial type in the last case was resected twice via transcervical approach, being removed subtotally in the first operation followed by total removal with the aid of neuronavigation and neuroendoscopy in the second operation when it failed to react to the treatment of gamma knife; the 5 tumors were removed totally except 1 subtotally.; all postoperative courses were uneventful.; follow up was performed successfully in 4 cases, the rating of Karnofsky Prognosis Scale was excellent in 3 cases and fair in 1 case. The outcomes of the latter 5 cases resected via modified approach were better than those of the early 5 cases.
CONCLUSIONHGN is extremely rare and it is difficult to treat it. To achieve a good operative outcome, it is very important to design the approach individually based on its location and size and operate mini-invasively and resect tumor totally as possible as can in the primary stage.
