Objective To discuss the clinical significance of vancomycin plasma concentration monitoring and its relationship with adverse reactions in treatment of elderly patients. Methods The clinical records of 32 patients aged over 70 years admitted in the Emergency Medical Department of Sichuan Provincial People's Hospital were collected. Based on the diagnosis and laboratory examinations on admission, the patients were treated with cefoperazone+sulbactam or moxifloxacin, while in patients with severe infections, intravenous drip of carbapenem antibacterial drugs were given. After treatment for 3-5 days, when no effective results were obtained, according to the pathogenic results achieved from cultures of blood, sputum, secretions, catheter, etc, the corresponding treatment was given. If the infection was caused by positive bacteria mainly methicillin-resistant Staphylococcus aureus (MRSA) sensitive to vancomycin, the original antibiotic was replaced by vancomycin or vancomycin combined with other antibiotic; intravenous drip of vancomycin 1 g in 250 mL normal saline was given, once in 12 hours, with a speed of 10 mg/min or not over 15 mg/min. The function of liver and kidney, and auditory impairment were observed, and the correlation between vancomycin plasma concentration and adverse reactions was analyzed. Results The total incidence of adverse reactions in elderly patients with different serum vancomycin trough concentrations was 37.50%(12/32) in which the highest incidence was kidney damage 18.75%(6/32) followed by the hearing loss 9.38%(3/32) and liver damage 9.38%(3/32). There were no statistical significant differences in the rates of adverse reactions among the different vancomycin trough concentrations (<10, 10-20,>20 mg/L) and [37.50%(6/16), 38.46%(5/13), 33.33%(1/3), all P>0.05], that meant along with the increase of vancomycin concentration, no tendency of increment of adverse reactions was seen. Of the 32 cases, there were 13 treated by vancomycin combined with other antibiotic, the combination rate being 40.63%. There were 2 cases of hepatic impairment (11.83%), 1 case of renal impairment (7.69%) and none hearing damage in patients treated with combination of antibiotic therapy. Therefore no correlation was concluded between the occurrence of adverse reactions and the combination therapy ( r=0.15, P>0.05). Conclusions In elderly patients over the age of 70 years, there is a higher incidence of kidney damage in the application of vancomycin. Thus, the monitoring of vancomycin plasma concentration in elderly patients has instructive significance in its clinical use.

Cyclic amp receptor protein (CRP) is a global transcriptional factor in many prokaryotes, capable of governing nearly half of the total genes in Escherichia coli. Through the method of error-prone PCR or DNA shuffling, we can first obtain CRP mutant library and then get the expected cell phenotype with enhanced resistance. In this article, we reviewed the following desired phenotype: enhanced tolerance towards oxidative stress, improved osmotolerance, enhanced organic solvent (toluene) tolerance, improved acetate tolerance of E. coli fermentation and improved ethanol tolerance during bio-ethanol production. We then concluded that CRP can also be applied in other host cells to get desired phenotypes. Last, we predicted potential applications of mutant CRP transcriptional factor.

Objective To investigate whether the tyrosine phosphatase SHP-2 participate in the cell migration induced by IL-1β.Methods Two different reorganized plasmids transfected into the MCF-7,and achieved two different cell lines:SHP-2-GFP-MCF-7 cell lines.SHP-2C＞S-GFP-MCF-7 cell lines.The cell migration was detected by a fluorescence microscope and the expression of E-eadhefin and MMP-9 wee detected by western blotting analysis and RT-PCR.Results The SHP-2-GFP-MCF-7 cell lines showed a higher cell migration ability,and the expression of E-cadherin in the SHP-2-GFP-MCF-7 cell lines was especially lower,neady undetectable.But in this group the expression of MMP-9 was the highest.Conclusion The tyrosine phosphatase SHP-2 participate in the cell migration induced by IL-1β,and maybe produce a marked effect by reinforcing the expression of MMP-9 and reducing the expression of E-cadherin.

Objective To research changes of the terminal height,the level of growth hormone,sexual development and record of formal schooling in Turner syndrome(TS).Methods In 213 cases TS were karyotyped,57 cases growth hormone exciting test were carried out,41 cases sexual development and record of formal schooling were followed up.Results 213 TS were karyotyped in 4 groups:Ⅰ,45,X,78 cases;Ⅱ,mosaicism,60 cases;Ⅲ,with various aberrations of structure of X chromosome,69 cases;Ⅳ,6 cases with Y chromosome.Average terminal height was(139 1?8 3)cm.Of 57 growth hormone exciting tests,growth hormone complete deficiency 16 cases,partly deficiency 30 cases and normal 11 cases.In the follow-up survey of 41 cases TS,record of formal schooling were mainly junior middle school,technological school and secondary speciality school.17 cases had differential degree sexual development and 24 cases had no.Conclusions The terminal heights of TS are markedly lower than normal,growth hormone is deficiency,learning ability decline,hypoplasia of sex gland.

Objective To detect the proliferation of neural stem cells (NSC) and analyze the effects of taurine on NSC proliferation in rats with fetal growth restriction (FGR).Methods For in vivo experiment,pregnant rats were randomly divided into three groups:control group (n=5),FGR group (n=7) and taurine group (n=6) [treated with taurine,300 mg/(kg · d)].Rats in the latter two groups were fed with 40％ of food intake in control group throughout pregnancy to establish the FGR model.Fatty acid binding protein-7 (FABP-7) expression in brains was detected by immunohistochemistry.For in vitro experiment,pregnant rats were divided into two groups including control group and FGR group.FGR modeling was established in the same way as above.On the 14.5 day of pregnancy,NSC were isolated and cultured for five days to form neurospheres with different concentrations of taurine (control,0,5,10,15,20,30,40 mmol/L).Immunofluorescence staining was used to detect the expression of NSC-specific marker FABP-7.CCK8 test was used to detect the cell proliferating index.Neurospheres were collected at the end of culture and disaggregated to get the total number of viable cells.One-way ANOVA,Kruskal-Wallis Test,SNK-test and Tamhane's test were used for statistical analysis.Results Compared with the control group (18 012±3 016),the number of FABP-7-positive cells in FGR model group (10 173±1 728) was decreased (P＜0.05),but increased in taurine group (19 518±3 201) (F=200.8,P＜0.05).Free-floating neurospheres could be formed in both control and FGR groups and the cells within neurospheres were mostly FABP-7-positive and were then considered as NSC.The proliferation indexes in control,FGR,FGR+5 mmol/L taurine,FGR+10 mmol/L taurine,FGR+15 mmol/L taurine,FGR+20 mmol/L taurine,FGR+30 mmol/L taurine,FGR+40 mmol/L taurine groups were 1,0.80±0.07,0.92±0.11,1.33±0.24,1.20±0.15,1.11 ±0.14,0.80±0.12 and 0.82±0.08,respectively.The proliferation indexes in FGR groups treated with 10,15 or 20 mmol/L of taurine were significantly higher than that of FGR group without taurine interference (P＜0.05).The total cell numbers in the eight groups were (24.5 ± 0.6)× 105,(20.23 ± 1.09)× 105,(22.80 ±0.86) × 105,(27.53± 1.22)× 10s,(25.30±1.05)× 105,(22.70±0.72)× 105,(21.80± 1.80)× 105 and (18.06 ± 0.90)× 105,respectively.Compared with the FGR group without taurine interference,those treated with 5,10,15 or 20 mmol/L of taurine showed higher total cell numbers (all P＜0.05 or ＜0.01),while that treated with 40 mmol/L oftaurine was lower.Conclusions The number and proliferation of NSC in FGR fetal rat brains is significantly decreased,however,these could be reversed by antenatal supplementation of taurine.The best effect is shown when l0 mmol/L of taurine is given.

Objective To observe the application effect of mind mapping in the nursing teaching of higher vocational nursing students. Methods A total of 120 nursing students from 2014 four vocational nursing classes. First class and second class were observation group (60 cases), third classes and fourth classes were control group (60 cases). Nursing students in observation group unfold nursing teaching with mind mapping teaching method. Nursing students in control group unfold nursing teaching with traditional teaching method. The test results and teacher satisfaction with nursing students at the end of the period were assessed. And the evaluation of nursing students on mind mapping teaching method was compared. Results The scores of the instruments and materials, the performance of the operation steps, the technique and the achievement scores were (9.85 ± 0.40), (64.67 ± 1.64), (17.37 ± 1.25) points in the observation group and (9.22±0.64), (58.68±2.21), (14.18±2.23) points in the control group, the differences were significant(t =-8.440,-18.539,-10.881, P<0.05). The satisfaction degree of the control group was 78.3%(47/60) and the satisfaction degree of the observation group was 95.0%(57/60), and there was significant difference (χ2=5.550, P<0.05). Nursing students in observation group of mind mapping teaching hold a higher rating (t =11.32-5.25, P<0.01). Conclusions Mind mapping teaching method allows nursing students quickly master the nursing skills, improve teaching effectiveness, improve teaching nursing students′satisfaction, enhance clinical thinking and critical thinking skills.

BACKGROUND: Rejection is the main cause of the failure in small bowel transplant. Chemotatic factor RANTES and receptor mediated cellular immunity are very important in acute rejection.OBJECTIVE: To explore the immunosuppressive effect of early adopting chemokine receptor antagonist, Met-RANTES after small bowel transplant on acute allograft rejection and its coordinative effect with Tacrolimus (FK506).DESIGN: Randomized complete-block design, controlled animal experiment.SETTING: Department of General Surgery, the 451 Hospital of Chinese PLA; Laboratory of Department of Gastrointestinal Surgery, Xijing Hospital, Fourth Military Medical University of Chinese PLA; Electronic Microscope Center, School of Basic Medicine, Fourth Military Medical University of Chinese PLA.MATERIALS: This study was carried out in the Laboratory of Department of Gastrointestinal Surgery, Xijing Hospital,Fourth Military Medical University of Chinese PLA from September 2003 to March 2005. Totally 192 animals including 96 SD rats (donors) and 96 Wistar rats (recipients) were involved in this study. Heterotopic segmental small bowel transplantation was performed.METHODS: The transplant rats were divided into 4 groups averagely by the randomized complete block design: control group (allogeneic small bowel transplant untreated group), Met-RANTES group(200 μg/d, 0-7 days, i.p.), FK506 group [0.5 mg/(kg·d) ,0-7 days,i.p.], Met-RANTES + FK506 group [Met-RANTES, 200 μg/d,0-7 days,i.p.+ FK506 0.5 mg/(kg ·d),0-7 days, i.p.]. Rats in the latter 3 groups were intraperitoneally administrated after transplant within 7 days successively.Rats in the control group were not given any treatments before and after transplant. Postoperatively, gross status,survival time and immunocyte infiltration were observed. Pathological examination was conducted in 6 rats of each group on postoperative days 3, 5 and 7. Fluorescent staining and successive quantitative measurement were conducted to detect the expressions of intragraft RANTES, CD4+, CD8+ and CD25+ T lymphocyte. Survival duration of the rest 6 rats of each group was observed for 5 weeks.MAIN OUTCOME MEASURES: ① Survival time of rats in each group following transplant. ② Pathological changes of small bowel intragraft of rats in each group. ③ RANTES and T lymphocyte expressions of rats in each group.RESULTS: Following transplantation, 96 Wistar rats (recipient) were all involved in the final analysis. ①Compared with control group, the survival time of rats in Met-RANTES group, FK506 group, Met-RANTES + FK506 group was significantly longer (P ＜ 0.01). In addition, rats in Met-RANTES + FK506 group survived the longest. There were significant differences in survival rate as compared with Met-RANTES group and FK506 group (P ＜ 0.01). ②All rats in the control group died of acute rejection and infection. Histopathologic examination showed mild, moderate and severe rejection on the postoperative days 3,5 and 7, respectively. No obvious rejection was found in the rats in the Met-RANTES group, FK56 group and Met-RANTES+FK506 group on the postoperative days 3,5 and 7. ③Postoperatively, intragraft RANTES expression of rats was significantly higher in each time period in control group than in the other 3 groups (P ＜ 0.01), and its dynamic change was positively correlated with the process of acute rejection; The expression of intragraft RANTES, CD4+, CD8+ and CD25+ T lymphocytes of rats was significantly lower, respectively, in the Met-RANTES group and Met-RANTES+FK56 group than in the control group (P ＜ 0.01).CONCLUSION: Met-RANTES may obviously suppress acute allograft rejection in small bowel transplant, effectively protect the function of grafts, and significantly prolong the survival time of the recipients. In addition, Met-RANTES may enhance the immunosuppressive function of small dose of FK506[0.5 mg/(kg · d)].

Background and purpose:Smac/DIABLO was the only apoptosis-related protein that could inhibit IAPs directly and simultaneously.The four amino-residual AVPI(Ala-Val-Pro-lie)in its N-terminal was the very important domain that could stimulate apoptosis.This study investigated the effect of synthetic Smac peptide (SmacN7) on chemotherapy sensitivity of bladder cancer cells.Methods:SmacN7 penetratin peptide was synthesized and delivered into T24 cells.MTT assay was adopted to evaluate the viability of T24 cells induced by low-dosage of MMC. Flow cytometry was applied to analyze the proportion of apoptosis and Western blot was used to detect the expression of XIAP and caspase-3;The activity of caspase-3 was measured and the effect of SmacN7 combined with MMC on T24 cell lines was also determined.Results:SmacN7 penetratin peptide could successfully interact with endogenous XIAP and increase the proportions of apoptosis of T24 cell lines induced by low-dosage of MMC in a dose-and time- dependent manner.An obvious down-regulation of XIAP expression and up-regulation of caspase-3 was identified by Western blot.The activity of caspase-3 in experimental group was significantly increased as compared with that in the control group;Combining the treatment with SmacN7 penetratin peptide,the viability of T24 cells decreased to 55% and 72.7% in 24 hrs and 48 hrs respectively.Conclusion:SmacN7 penetratin peptide could act as a cell-permeable IAP inhibitor,inhibit the proliferation,induce apoptosis and enhance the chemo-sensitivity of bladder cancer cells to MMC. When combined with chemotherapy,it may be a very promising strategy for bladder cancer therapy.
