Objective To probe the application of superimposed flaps in hand surgery. Methods Form March 1998 to December 2003, we repaired 35 cases of skin defects with non trunk blood flaps and neurocutaneous vascular flaps. The area of flaps ranged from 1cm?1cm to 6cm?8cm. Results After operation, the flaps in 33 cases survived, but died in 2 cases due to unsuitable design. The follow ups ranging from 6 to 12 months showed that the quality and color of the survived flaps were similar to those of the normal tissue around. No swelling was found. Conclusion The superimposed flaps have reliable blood supply so that they are an ideal means to repair skin defects of hand.

Objective To investigate the relationship between helicobacter pylori(Hp) infection and Henoch-Schonlein purpura(HSP) in children and find out the etiological treatment for HSP.Methods Positive rate of Hp-IgG between normal children and children with HSP were compared.Infection of Hp was detected by rapid urease test,and relationship between infection of Hp and clinical symptom in Henoch purpura was analyzed.Children with HSP were randomly divided into two groups and were respectively treated with Hp eradication therapy together with conventional therapy and conventional therapy only.The recurrence rates were compared.Results The infection rate in 150 children with HSP,especially the 90 children with Henoch purpura was significantly higher than that in normal children,and it was in positive correlation with abdominal symptom.Hp infection with triplex therapy for 2 weeks in HSP could reduce the recurrence of HSP.Conclusions Hp infection may be one of the reasons of HSP in children,and is related with Henoch purpura.Eradication of Hp can reduce recurrence of HSP.

L9 (3(4)) orthogonal experiment was used to design the extraction technology of compound Clematidis Radix spray. Weight coefficients of active ingredients and dry extract rate were solved by information entropy. Support vector machine (SVM) was established and the model parameters were optimized through the genetic algorithm. Grid search algorithm was used for optimization of extraction technology of Clematidis Radix spray. The optimal extraction technology was to extract Clematidis Radix spray in water with 6 times the weight of herbal medicine for 3 times, with 2 h once. Bias of value between real and predicted by SVM was 1.23%. SVM was compared with traditional intuitive analysis of orthogonal design. It indicates that the new method used to optimize the extraction parameters of compound Clematidis Radix spray is more accurate and reliable.

Objective: To observe the clinical efficacy of electroacupuncture (EA) plus Tanbo-plucking the trigger points for scapulohumeral periarthritis (SP). Methods:A total of 80 patients with SP were randomized into an observation group and an EA group by the random number table, with 40 cases in each group. The EA group was treated with EA therapy, and the observation group was treated with EA therapy plus Tanbo-plucking the trigger points. After treatment, the visual analog scale (VAS) and Melle scores of the two groups were compared to evaluate the improvement of shoulder pain and functional activity, and meanwhile the clinical efficacy was observed. Results: After treatment, the total effective rate of the observation group was 95.0％ and the cure and markedly effective rate was 72.5％. The total effective rate of the EA group was 87.5％ and the cure and markedly effective rate was 42.5％. There was no significant difference in the total effective rate between the two groups (P>0.05). The cure and markedly effective rate of the observation group was higher than that of the EA group, and the difference between the two groups was statistically significant (P<0.05). After treatment, the intra-group differences in VAS and Melle scores of both groups were statistically significant (bothP<0.001). The inter-group differences in the changes of the VAS and Melle scores after treatment were statistically significant (bothP<0.001). Conclusion: EA plus Tanbo-plucking the trigger points has a better curative effect than EA therapy alone in the treatment of SP.

ObjectiveToinvestigatetheinflammatoryresponseandapoptosisof peripheral blood mononuclear cells(PBMCs) and their regulation by triptolide(TP) in IgA nephropathy(IgAN) patients.MethodsBlood samples were collected from 29 IgAN patients and 16 healthy individuals.TNF-α and IL-6 concentrations were measured by ELISA and NO concentration by Griess reagent in the plasma of samples.PBMCs were isolated from IgAN patients and cultured in vitro,and subsequently activated by PHA(10 mg/L).The cytotoxicity of different TP concentrations was assayed by MTT and two non-toxic concentrations (12.5 μg/L or 25.0 μg/L)were selected for treatment.TNF-α,IL-6 and NO concentrations were measured in the culture media collected from PBMCs cultures activated by PHA (10 mg/L) and treated with TP (12.5 μg/L or 25.0 μg/L).The PHA-activated,TP-treated cells apoptotic rate was analyzed by FACS using Annexin V-FITC staining.The expression of Bcl-2,Bax,caspase-9 and caspase-3 were detected by RT-PCR and Western blotting from lyses of PHA-activated with or without TP-treated cells.ResultsThe serum concentrations of TNF-α[(131.57±50.61) ng/L vs(30.24±18.93) ng/L,P＜0.01],IL-6[(76.36±25.21) ng/L vs(35.08±16.59) ng/L,P＜0.01] and NO[(46.36±12.93) μmol/Lvs (26.61 ±10.87) μmol/L,P＜0.01] were significantly increased in IgAN patients compared to healthy individuals.PBMCs viability in culture decreased after TP treatment in a dose-dependent manner.TP also inhibited TNF-α,IL-6 and NO levels in the media of PHA-activated PBMCs in culture and induced PBMCs apoptosis.The expression of Bcl-2 decreased markedly and Bax,caspase-9 and caspase-3 increased significantly after TP treatment (all P＜0.05).Conclusions The PBMCs from IgAN patients are in a highly activated state,and have a high apoptotic rate.TP treatment induces benificial effects in IgAN patients by inhibiting the activation of PBMCs by activating pro-apoptotic pathway.

OBJECTIVETo explore the linkage of the polymorphism at D17S1878 site with susceptible gene of essential hypertension.

METHODSForty-five pedigrees from the high prevalence region of the hypertension were collected. The polymorphism of D17S1878 site was genotyped with genetic analyzer and gene-scan software. Discordant sib pair analysis and affected sib pair analysis were used in linkage analysis.

RESULTSThere were significant differences in the age, male, alcohol-consuming, over-salt intake, average systolic pressure, average diastolic pressure, waist-to-hip-ratio, total cholesterol, high-density lipoprotein, and low density lipoprotein between the hypertensive sib and the normotensive sib (P < 0.05). There were eleven alleles at D17S1878 site, and the allele frequency was significantly different between the hypertensive and normotensive sibs (P < 0.05). Forty-three pedigrees were analyzed with affected sib pair analysis (t = 3.05, P < 0.05).

CONCLUSIONThe polymorphism of D17S1878 may be linked with susceptible genes of essential hypertension.

OBJECTIVETo investigate the clinical features, prevention and treatment of respiratory distress syndrome (RDS) in neonates of different gestational ages (GA) in the tertiary hospitals in Northwest China.

METHODSA total of 440 neonates diagnosed with RDS between January and December, 2011 in 12 tertiary hospitals in Northwest China were enrolled and classified into three groups: early preterm (GA<34 weeks; n=247), late preterm (GA 34-36(+6) weeks; n=131) and full-term (GA≥37 weeks; n=62). The clinical data, including perinatal factors, prevention and treatment, complications and prognosis, were comparatively analyzed among the three groups.

RESULTSThe rate of multiple births in the early preterm group was higher than the other two groups. The two preterm groups showed a higher incidence of premature rupture of membranes than the full-term group. The full-term group had a higher rate of cesarean section without contractions than the two preterm groups. The early preterm group had the highest application rate of antenatal steroids. Compared with the other two groups, the early preterm group had a higher application rate of pulmonary surfactants (PS) and an earlier time of first application of PS. The full-term group had a lower resuscitation rate than the two preterm groups. The early preterm group showed a higher incidence of patent ductus arteriosus and intracranial hemorrhage than the other two groups. The cure rate of RDS (78.2%) was the highest in the full-term group, followed by the late preterm group (58.6%) and the early preterm group (42.9%).

CONCLUSIONSRDS infants of different GA in Northwest China have significant differences in perinatal factors, antenatal prevention, PS treatment, complications and prognosis.
