1.Clinical analysis of 56 cases of recurrence thyroid carcinoma

Objective To investigate the risk for recurrent thyroid carcinoma.Methods Clinical data of 56 cases of recurrent thyroid carcinoma were retrospectively analyzed and summarized possible causes of relapse.Results Student's test revealed that age,initial operative approach,pathological types,postoperative medication and radionuclide therapy were associated with recurrence (P < 0.05),but gender was not associated with recurrence (P =0.240),and resection of single gland was associated with recurrence (P =0.006),but resection of both gland was not so (P =0.153).Multivariate analysis indicated that initial operative approach was the dependent factors for recurrence(P < 0.05).Conclusion Some patients that prone to recurrence should be strengthened regular follow-up.Standard treatment should be used for The recurrences diagnosed.
2.Utility Analysis of the Useful Methods for HLA-B Typing

Objective To evaluate the practical value of Serology, PCR-SSP and PCR-SSOP for HLA-B typing. Methods A total of 30 samples, the blood of patients and donors waiting kidney transplantation, were used in the study. HLA-B typing was performed by one-step monoclonal antibody typing, micro PCR-SSP typing and PCR-SSOP reverse hybridisation. Results All samples were successfully typed by three methods. The difference between serological and PCR-SSP typing was 13%. The difference between PCR-SSOP and PCR-SSP typing was 3%. Conclusion Serology is high discrepancy rate and low-resolution, but cheap, simple and rapid. PCR-SSP and PCR-SSOP typing are high specific and accuracy. SSP is suitable for several samples, and SSOP is for lots of samples simultaneously although it needed long time.
3.Cross-match test of the donor-recipient of organ transplantation by ELISA

Objective To investigate the significance of enzyme-linked immunosorbent assay (ELISA) in cross-match test of donor-recipient before organ transplantation.Methods HLA glycoproteins were prepared by solubilizing the lymphocytes of donor with a non-ionic detergent, and bond to the monoclonal antibody specific for HLA class Ⅰ or Ⅱ immobilized in the ELISA tray. ELISA with addition of recipients' serum and complement-dependent cytotoxicity (CDC) were compared.Results Cross-match test was performed by two methods for lymphocytes of 40 donors and sera of 72 recipients. All samples were successfully tested. The results of one pair of donor-recipient cross-match test by two methods were different.Conclusion ELISA for cross-match test is simple, convenient and time-saving, but more sensitive and specific than CDC.
4.Effects of platelet-rich fibrin on expression of BMP-6 during mandibular distraction osteogenesis

Objective To investigate the effects of platelet-rich fibrin (PRF) on BMP-6 expression during mandibular distraction osteogenesis. Methods Twenty-five mature rabbits were randomly divided into five groups. Mandibular osteoto?mies were performed corticectomy in bilateral mandibles of rabbits. PRF was implanted in the one side of mandibles, which was used as experimental group. Another side of madibular was used as control group. Rabbits were sacrificed and the dis?tracted calluses were harvested and processed for HE and BMP-6 immunohistochemistry staining at 1, 3, 7, 14 and 28 days after the end of distraction, respectively. The expressions of BMP-6 in the distracted calluses were analyzed by cell digital imaging software. Results The regenerated bone was found in the distraction gap after mandibular lengthening. Expres?sions of BMP-6 were co-localized in cytoplasm of osteoblasts and newly embedded osteocytes. Compared with control group, the positive cell rate and positive area percentage of BMP-6 were significantly higher on the 1st, 3th and 7th day after the distraction in experimental group (P<0.05). There were no significant differences in positive cell rate and positive area per?centage of BMP-6 at 14-day and 28-day after the distraction between experimental group and control group. Conclusion PRF can accelerate bone formation of mandibular distraction osteogenesis in rabbits. BMP-6 may play important role at the early stage of mandibular distraction.
5.Arterial-bypass-supported endovascular thoracic aortic repair

Objective To study the feasibility and efficacy of endovascular thoracic aortic repair supported by preliminary arterial bypass. Methods From June 2003 to Jan. 2005, eight patients underwent endovascular repair (EVR) combined with supportive arterial reconstruction in the thoracic aorta. Five patients had Debakey type Ⅲ aortic dissection aneurysm (ADA), and the remaining 3 had descending thoracic aortic aneurysm (DTAA), aortic arch aneurysm and aortic arch pseudoaneurysm respectively. The proximal landing zone was measured less than 15 mm in all instances by contrast-enhanced CT scan and digital subtraction angiography. The preliminary right-left axillary bypass was performed in an ADA case in whom the distance from the primary entry site to the origin of the left common carotid artery was longer than 15mm,and the right- left carotid and left carotid-subclavian bypass in the remaining 7 cases. EVR was conducted 1 week after the bypass. Results All procedures were performed successfully. However, the DTAA patient died of the hemispheric cerebral infarction and subsequent multiple system organ failure, albeit the uneventful recovery from the prior cervical reconstruction. Type Ⅱ endoleak occurred at the completion of EVR in the patient receiving the axillary bypass, but disappeared on 3-month CT. No neurological deficits or limb ischemia developed perioperatively or during the follow-up raging from 3 to 22 months, and complete thrombosis of the thoracic aortic false lumen or of the aneurysm/pseudoaneurysm was revealed on CT at 3 months in the 7 patients. Conclusion The adjunctive surgical bypass appeared to be feasible and effective in creating extra proximal landing zone for endovascular thoracic aortic repair, and thus broaden its application.
6.Transport of melanocytes by chitosan membrane: an animal experiment

Objective To study the feasibility of using chitosan membrane to carry and transport melanocytes, in order to refine the technique for melanocyte transplantation with chitosan membrane. Methods Melanocytes were inoculated onto chitosan membrane and cultured for a period of time, then, electron microscopy,MTT assay and NaOH assay were carried out to estimate the adherence, growth and melanogenesis of the melanocytes. Skin wound surface was prepared in 12 nude mice, which were equally divided into 3 groups, test group inoculated with melanocytes on chitosan membrane, negative control group I treated with chitosan membrane without melanocytes, and negative control group II directly dressed immediately after the preparation of wound surface. On day 10 and 20 after the transplantation, confocal laser microscopy and immunohistochemistry were performed to observe the migration of melanocytes into the skin wound surface. Results Scanning electron microscopy and inverted microscopy showed that melanocytes were evenly distributed on and adhered well to the underlying chitosan membrane. As the growth curve of melanocytes demonstrated, chitosan membrane could support the normal growth of melanocytes, and no significant difference was observed in the synthesized melanin content between melanocytes cultured on the chitosan membrane and those in culture disks (0.087 ± 0.027 vs. 0.101 ± 0.036, t = 0.79, P > 0.05). Melanocytes were seen at the transplantation sites by confocal laser microscopy, and biopsy specimens from the transplantation sites stained positive for antimelan-A monoclonal antibody. Conclusions Melanocytes can adhere to and grow on the chitosan membrane,which can facilitate the migration of melanocytes to the transplantation sites in animals with the maintenance of biological activity of melanocytes.
7.Evaluation of clinical-diffusion mismatch in intra-arterial thrombolysis treatment for acute middle cerebral artery occlusion

Objective To evaluate the function of clinical-diffusion mismatch (CDM) in intraarterial thrombolysis treatment for acute middle cerebral artery occlusion (MCAO) and to evaluate specialty of CDM in predicting the putative penumbra.Methods All 106 acute MCAO patients within 6 hours after onset and examined by magnetic resonance angiography (MRA) were assigned into two groups:the intraartery thrombolysis group (n =36) and without thrombolysis group ( n =70).Both groups were subdivided into CDM sub-group and non-CDM sub-group based on the criteria of CDM:National Institutes of Health Stroke Scale (NIHSS) score ≥8 and ischemic volume on DWI ≤25 ml upon admission.NIHSS scores at the day 30 and 3 month after onset,and infarct volumes on T2 weighted imaging (T2 WI)at day 14 after onset were analyzed and compared between each sub-groups.Results The NIHSS scores in the CDM sub-group at both day 30 and the 3 month were significantly lower than the scores in the non-CDM sub-group among the patients having thrombolysis(3.20 ± 2.40 vs 6.76 ± 4.00,t =- 3.330,P =0.002 ; 2.20 ± 1.70 vs 6.05 ± 4.06,t =3.895,P =0.001 ),but not among the non-thrombolysis patients (5.22 ± 2.95 vs 5.66 ± 3.21,t =- 1.756,P =0.084 ;4.34 ± 2.53 vs 5.34 ± 3.42,t =1.234,P =0.353 ).Among the patients having CDM,the thrombolysis group resulted significant lower NIHSS scores at both day 30 and 3 month follow-up than non-thrombolysis group did (3.20 ±2.40 vs 5.22 ±2.95,t =- 2.210,P =0.034;2.20 ± 1.70 vs 4.34 ± 2.53,t =-3.128,P =0.003 ).However,among the patients of non-CDM,there was no difference in the NIHSS score between the thrombolysis group and the non-thrombolysis group at day 30 and 3 month (6.76±4.00 vs 5.66±3.21,t=1.209,P=0.231;6.05 ± 4.06 vs 5.34 ± 3.42,t =1.234,P=0.460).Among the CDM patients,the T2 WI infarct volume in the thrombolysis group was significantly smaller than that in the non-thrombolysis at day 14 onset ( ( 6.29 ± 4.41 ) ml vs ( 60.25 ± 49.23 ) ml,Z =- 4.848,P =0.001 ).Conclusion CDM may predict the putative penumbra with high specificity and can be applied in the therapy of intra-artery thrombolytic for acute MCAO patients.
8.Comparisons of clinical and imaging characteristics in acute thrombotic and embolic middle cerebral artery occlusion A retrospective diffusion-weighted imaging-based study

Objective To investigate the clinical and imaging differences in acute thrombotic and embolic middle cerebral artery occlusion.Methods The cerebral infarction patients with acute middle cerebral artery trtmk occlusion confirmed by diffusion-weighted imaging (DWI) and magnetic resonance angiography (MRA) within 24 hours of onset were divided into large artery atherosclerosis (LAA) group and the cardioembolism (CE) group according to the TOAST classification criteria.Under the circumstances of not receiving thrombolytic therapy,the infarct volume on DWI,morphological characteristics and the changes of the National Institutes of Health Stroke Scale (NIHSS) scores both at admission and day 14 were compared.Results A total of 102 cerebral infarction patients with acute middle cerebral artery trunk occlusion were included.The age of the CE group was significantly older than that of the LAA group (67.60 ± 9.62 years vs.62.57 ± 10.18 years,P =0.017),and more patients with coronary heart disease (27.27% vs.2.90%,P=0.001 ).The infarct volume (31.96 ±39.20 ml vs.65.66 ±84.74 ml,P =0.005),the NIHSS score at admission (6.42 ± 3.38 vs.11.67 ±8.50,P=0.007),and the improvement of the disease (i.e.the difference of NIHSS scores between admission and day 14) (1.55 ± 6.43 vs.5.75 ± 9.28,P =0.027) in LAA group were significantly lower than those in the CE group.However,there was no significant difference in the NIHSS score between the 2 groups at day 14 (4.87 ± 6.61 vs.5.97 ± 3.60,P =0.324).The infarct volume was significantly correlated with the NIHSS scores at day 14 (CE group:r=0.625,P=0.001; LAA group:r=0.295,P=0.014).The LAA group was mostly the multiple lesions (71.01%),and the CE group was mostly the single lesions (54.55%) (P =0.016).Conclusion There were differences in morphology of cerebral lesion between the LAA and CE groups.In the early stage after onset,CE caused middle cerebral artery trunk occlusion was more serious and had larger infarct volume,but some patients could naturally significantly improve within a short time after onset.

Aim To construct the eukaryotic expression vectors of short hairpin RNA targeting survivin and observe its effect on biologic behavior of A549 cells and sensitivity of A549 cells to paclitaxel.Methods The DNA fragment targeting human survivin was inserted into the plasmid,and the recombinant plasmid was constructed.The recombinant plasmids cells were transfected into A549 cells by FuGENE transfection reagent.The expression levels of survivin gene were detected with RT-PCR and Western blot before and after transfection,respectively.Cell apoptosis was detected by TUNEL method.The sensitivity of A549 cells to paclitaxel was detected by MTT after transfection.Results The recombinant plasmid was successfully constructed.RNAi group cells showed lower expression of survivin than control group.The apoptosis rate of A549 cells increased after transfection.The IC50 of paclitaxel inhibiting A549 cells was 11.9 fold before transfection compared with those after transfection.There was significant difference between the two groups(P

Objective To investigate the histologic characteristics of small -size prostatic hyperplasia (SBPH)and the effect on low urinary tract symptoms (LUTS).Methods 154 patients with SBPH who received transurethral resection of prostate (TURP) were enrolled,preoperative examinations included the obtainment of International Prostate Symptom Score(IPSS),evaluation of the quality of life(QOL),cases of urinary retention,ultra-sonography,urodynamic examination,and cystoscopy,postoperative pathology specimen analysis was conducted.The histologic characteristics of small -size prostatic hyperplasia(the histology prostatis and the grade of inflammation) and the effect on LUTS were analyzed.Results A total of 141 cases (91.56%)were diagnosed as SBPH with histo-logical prostatitis,13 cases(8.44%)in grade 0,18 cases(11.69%)in grade 1,76 cases(49.35%)in grade 2 and 47 cases(30.52%)in grade 3.There were statistically significant differences among ages,IPSS,QOL,Qmax,cases of urinary retention between the pure SBPH group and SBPH with HP group(t/χ2 =2.610,6.607,4.925,2.673, 4.421,all P <0.05),while there was no statistically significant difference in PV(t =1.652,P >0.05).Conclusion The main pathologic type is stroma hyperplasia(fibrous and smooth muscle)of SBPH,SBPH is mostly complicated with histological prostatitis,which are mostly moderate and severe.The severity of LUTS is positively correlated with the grade of inflammation.
